Immediate Relief of Cyanosis Following Surgical Removal of a Large Pulmonary Arteriovenous Malformation: A Case Report
Background: Pulmonary arteriovenous malformation (PAVM) is a rare structurally abnormal communication between the pulmonary arterial and pulmonary venous network, creating a pathologic intrapulmonary right-to-left shunt. Methods: A four-year-old boy presented with the history of chest pain, cyanosis, respiratory distress, cough and hemoptysis for two years. He was diagnosed to have a congenital pulmonary arterio-venous malformation occupying the posterior segment of the right upper lobe. The data and photos were collected from hospital records with the consent from the parents of the patient. Ethical aspects were discussed by our departmental review board. Results: CT Angiogram revealed that a large vascular mass in the posterior segment of right upper lobe. The lesion was supplied by branches from pulmonary arterial system as well as descending thoracic aorta. A joint team of cardiac and thoracic surgeons opened the chest by a posterolateral incision along the right 4th intercostal space. A large AVM mass, measuring approximately 10 cm x 6 cm x 5 cm, was seen occupying the lower part of the upper lobe of right lung. Following ligation of the feeding vessels, the AVM had shrunk and the affected part of the upper right lobe could be removed with the help of a stapler gun. Immediately after resection, the saturation went up to 100%. He made an uneventful recovery and was discharged from the hospital on the 8th postoperative day. Histopathologic examination confirmed diagnosis of AVM. Conclusion: Surgical removal of the Pulmonary arteriovenous malformation may improve the oxygen saturation immediately and relieve cyanosis
Riwayat and dirayah in hadith studies: Muhammad Amim al-Ihsan’s methodology in Fiqh al-Sunan wa al-Athar as a case study / Muhammad Hosain
Identifying the method of writing is important to help the reader or researcher to understand what the author of a book presented in his or her writing. This methodology can offer a preliminary overview of the content of the book as well as to detect the author’s approach in his or her authorship. This research aimed to achieve several objectives, namely to highlight the biographical account of Muḥammad ‘Amīm al-Iḥsān as well as identifying his published and unpublished authorships, i.e books, edited books, commentary and manuscripts written by him. Similarly, the study also discussed his thought and academic contribution. The researcher also identified and meticulously analyzed Iḥsān’s exclusive dirāyah and riwāyah in his method of writing in his celebrated book of ḥadīth Fiqh al-Sunan wa al-Athār. In addition, the study showed his benefits and contribution to the development of the study of al-Ḥadīth in Bangladesh. In this study, the researcher had utilized two main research methods, i.e data collection and analysis. During data collection, the researcher had carried out library research and some interviews, with no questionnaires since the main attention of the study was on the texts. Meanwhile, in data analysis, the researcher had used the deductive and inductive methods. The findings of this work are especially helpful for local and international readers or researchers. This study discovered much information about Iḥsān’s missing books. In authoring this book, Iḥsān employed various methods. In the same manner, the study also discovered his contribution and benefits to the development of the Ḥadīth studies in Bangladesh

Headed stud shear connectors in solid slabs and in slabs with wide ribbed metal deck
This thesis summarizes the results of an experimental investigation of the behaviour of headed stud connectors in push-out specimens with headed studs embedded in solid slabs and in slabs with wide ribbed metal deck oriented parallel to the beam. The experimental investigation involved the testing of 104 push-out specimens and was conducted in three phases. The first phase involved a study of the effects of transverse stud spacing on the shear strength of headed studs in push-out specimens with solid slabs and those with wide ribbed metal decks. The objectives of the second phase were to conduct a parametric study of the behaviour of headed studs in push-out specimens with solid slabs and to propose new equations for predicting the ultimate stud capacity for this case. A similar study involving specimens with wide ribbed metal decks formed the third phase.
For specimens with 150 mm solid slabs, there is an increase in the shear capacity of headed studs when the transverse stud spacing is increased from 3 times the stud diameter to 4 times the stud diameter (d) beyond which the strength-transverse stud spacing curve forms a plateau. The percentage increase in stud shear capacity is higher when failure is concrete related than when shank shear of studs is the mode of failure. For specimens featuring 150 mm slabs with wide ribbed metal decks, the shear capacity of headed studs attains a maximum value when the transverse spacing is 3d and decreases when the transverse spacing is increased to 4d beyond which the strength-transverse stud spacing curve forms a plateau.
For specimens with solid slabs, there is an increase in the stud shear capacity with the increase in longitudinal stud spacing, up to a transition point, beyond which the strength-longitudinal stud spacing curve forms a
plateau. This transition point occurs at a longitudinal stud spacing of approximately 5d when the concrete compressive strength is approximately 25 MPa and at 4.5d when the compressive strength of concrete is over approximately 30 MPa. In general, the failure modes of specimens with closely spaced studs was concrete related. When the stud spacing was increased, the failure mode changed to shank shear of studs. The effect of concrete compressive strength on the shear capacity of studs was found to vary approximately in proportion to the square root of the increase in the compressive strength of concrete. The effect of transverse reinforcement is more pronounced for specimens with concrete related failure than those with shank shear failure of studs. A new equation proposed by the author for predicting the shear capacity of headed studs in solid slabs provides much better correlation to test results than those obtained using CSA and Eurocode 4 provisions. Unlike these code provisions, the proposed equation takes into account the effects of longitudinal and transverse stud spacing, and transverse reinforcement.
For the specimens with wide ribbed metal deck, the relationship between longitudinal stud spacing and stud capacity was nonlinear and the strength-longitudinal stud spacing curve did not attain a plateau within the range of longitudinal stud spacings considered. Within the range of the flute widths considered, the deck geometry does not appear to have any significant influence on the stud capacity for specimens with 150 mm slabs as well as for those with 103 mm slabs. The most common failure mode for the specimens with wide ribbed metal decks was concrete shear plane failure. A new equation proposed by the author for predicting the shear capacity of studs in wide ribbed metal deck provides better correlation to test results than those obtained using CSA and Eurocode provisions
Know thy neighbors: The status of cardiac surgery in the South Asian countries around India
Objectives: The South Asian neighboring countries of India include Bangladesh, Bhutan, Nepal, The Maldives, Pakistan and Sri Lanka. Interestingly all these countries possess either a land or a sea border with India and no border among themselves. These countries have historic, cultural, ethnic and genetic links with India. The paper describes the developmental history and current status of cardiac surgery in these countries.
Methods: Thorough search of the printed and electronic materials has been made. The authors visited all these countries and contacted the eminent surgeons personally or through mails. All the information is cross-checked and compiled. Record keeping is not well organized in most of these countries. Best information often came from unusual sources like Anesthetists’ society or the corporate houses.
Results: Four of these countries Bangladesh, Nepal, Pakistan and Sri Lanka have their cardiac surgical programs. Collectively they perform around 38000 cardiac operations a year which is a quarter of the cases performed in India. These countries are important sources of medical tourism in India which is worth 3 billion US$ of business annually.
Conclusion: When the number of operations per million populations is considered, Bangladesh and Nepal are lagging behind India where as Pakistan has a comparable figure. Sri Lanka with 265 cardiac operations/million populations has the best figures in the region. However when compared with the Western countries even the Lankan figures also look quite inadequate

Impact of In-class Breaks on Students’ Revitalization: An Observational Study in China and Bangladesh
Students are the receiver and the teachers are the providers of information at a classroom. The exchange of information is conducted mainly through delivering lectures although there are several other ways to do that. Generally in a longer class duration (more than 1 hour), breaks are quite essential for the students as well as for the lecturer to get revitalized. This study aims at examining the impact of in-class breaks on revitalizing or energizing the students in order to focus on the lectures effectively. The investigation was conducted on 527 Bachelor and Master students of three universities in China and three universities in Bangladesh on a random sampling basis. Those students participated at two different classes: one with frequent short breaks and other one with two long breaks where they were being observed. The results indicate that frequent short breaks can revitalize the students more than fewer long breaks. Further, it was revealed that the Chinese students were more punctual and efficient in time management than the Bangladeshi students. The author hopes that this study will be inductive to the university lecturers as well as educational policymakers for putting more motivational focus for students in order to make the lectures enjoyable and effective
