Development of a virtual reality milling machine for knowledge learning and skill training
Current methods of training personnel on high cost machine tools involve the use of both classroom and hands on practical training. The practical training required the operation of costly equipment and the trainee has to be under close personnel supervision. The main aim of this project is to reduce the amount of practical training and its inherent cost, time, danger, personal injury risk and material requirements by utilising a virtual reality technology.
In this study, an investigation into the use of Virtual reality for training operators and students to use the Milling Machine was carried out. The investigation has been divided into two sections: first the development of Milling Machine in the 3D virtual environment, where the real machine was re-constructed in the virtual space.
This has been carried out by creating objects and assembling them together. The complete Milling machine was then properly modelled and rendered so it could be viewed from all viewpoints.
The second section was to add motion to the virtual world. The machine was made of functions as for the real machine. This was achieved by attaching Superscape Control Language (SCL) to the objects. The developed Milling machine allows the users to choose the material, speed and feed rate. Upon activation, the virtual machine will be simulated to carry out the machining process and instantaneous data on the machined part can be generated.
The results were satisfactory, the Milling Machine was modelled successfully and the machine was able to perform according to task set. Using the developed Virtual Model, the ability for training students and operators to use the Milling Machine has been achieved
Chromatographic analysis of Hb S for the diagnosis of various sickle cell disorders in Pakistan
Sickle cell disease remains a relatively obscure theme in research on haemoglobinopathies in Pakistan. Limited data is available regarding its prevalence in the country. The objective of our study was not only to estimate the frequency of different sickle cell diseases but also to provide quantitative estimation of haemoglobin S and other haemoglobin variants using an automated high-performance liquid chromatography (HPLC) system. For this Purpose, we retrospectively evaluated the results of HPLC performed on all Patients with suspected haemoglobinopathies during the years 2005 and 2006. Information derived from various sources was used to identify a particular genotype by analysing each sample containing Hb S with respect to haemoglobin, red cell indices and levels of various associated haemoglobin variants. Analysis of 15,699 samples identified 302 Patients with Hb S (1.92%). The genotypes identified included S beta(0)(46.7%), SS (19.2%), SA (11.6%), S beta(+) (8.6%) and SD (2.3%). Thirty-five cases could not be categorised and were labelled \u27unclassified\u27. Majority of the Patients (62.3%) were below the age of 18 years. Balochistan, which is the largest province based on the area, yielded the highest number of Patients (n=140). In the S beta(0) group, the mean haemoglobin and Hb S were lower in children compared to adults (p value of 0.001 and 0.016, respectively). We conclude that sickle cell disorders are prevalent in Pakistan to a significant extent, being concentrated in certain areas of the country. We present the first report of various haemoglobin S genotypes from our population. It is hoped that it will act as a database to characterise the same for our population

Outcomes in patients with severe COVID-19 disease treated with tocilizumab: a case-controlled study

BACKGROUND: COVID-19 is an ongoing threat to society. Patients who develop the most severe forms of the disease have high mortality. The interleukin-6 inhibitor tocilizumab has the potential to improve outcomes in these patients by preventing the development of cytokine release storm. AIMS: To evaluate the outcomes of patients with severe COVID-19 disease treated with the interleukin-6 inhibitor tocilizumab. METHODS: We conducted a retrospective, case-control, single-center study in patients with severe to critical COVID-19 disease treated with tocilizumab. Disease severity was defined based on the amount of oxygen supplementation required. The primary endpoint was the overall mortality. Secondary endpoints were mortality in non-intubated patients and mortality in intubated patients. RESULTS: A total of 193 patients were included in the study. Ninety-six patients received tocilizumab, while 97 served as the control group. The mean age was 60 years. Patients over 65 years represented 43% of the population. More patients in the tocilizumab group reported fever, cough and shortness of breath (83%, 80% and 96% vs. 73%, 69% and 71%, respectively). There was a non-statistically significant lower mortality in the treatment group (52% vs. 62.1%, P = 0.09). When excluding intubated patients, there was statistically significant lower mortality in patients treated with tocilizumab (6% vs. 27%, P = 0.024). Bacteremia was more common in the control group (24% vs. 13%, P = 0.43), while fungemia was similar for both (3% vs. 4%, P = 0.72). CONCLUSION: Our study showed a non-statistically significant lower mortality in patients with severe to critical COVID-19 disease who received tocilizumab. When intubated patients were excluded, the use of tocilizumab was associated with lower mortality

Technological impact on the art of moviemaking: deploying new and convergent media to redefine a model for Pakistan’s cinema
This thesis examines the decline in Pakistani cinema during the last two decades. It examines the history of the cinema and exposes some possible, previously ignored, causes for that decline. This research led the author to ask “Can new and convergent media be helpful in reviving the Pakistani cinema?” The thesis introduces the ideas of established and emergent cinema, building on the work of Williams (1977) in discussing the ideas of dominant, residual and emergent culture. The exploration reveals two gaps in the film industry: first, the lack of training in the making of films; and, second, the change in possible production methods allowed by new and emergent technologies. The thesis addresses both of these gaps by suggesting new production paradigms which incorporate the new technology and by examining two scripts to develop methodologies for teaching. The scripts are produced into films as the practice section of the research. The first film, creative element 1, is developed using some of the new tehnologies, students as crew and the available resources of an educational establishment to test the methodologies that have been derived. The outcomes of the creative element 1 laid the foundation of the second film, creative element 2. It is shot on mobile phones and distributed from Pakistan through Vimeo with a negligible budget. The social networks helped to arrange equipment and locations and allowed extreme freedom to the filmmaker
Liberal theory and Islam: (re)imagining the interaction of religion, law, state and society in Muslim contexts
Within the global phenomenon of the (re)emergence of religion into issues of public debate, one of the most salient issues confronting contemporary Muslim societies is how to relate the legal and political heritage that developed in pre-modern Islamic polities to the political order of the modern states in which Muslims now live.
This study seeks to develop a framework for addressing this issue by drawing upon two sources. The first is an interpretative understanding of the history of Muslim contexts emphasising, in particular, the diversity of views about what Islam mandates that have always been a part of Muslim experience and the distinction between political and religio-legal authority that developed in practice in these environments. The second source is a variety of contemporary liberal theory which this study develops and calls ‘justice as discourse’.
The central argument is that liberal theory, and justice as discourse in particular, though it may have emerged in a different social and cultural milieu, can be normatively useful in Muslim contexts for relating, religion, law, state and society. It is argued first, that Muslim contexts are facing issues similar to those out of which liberal theory emerged. Additionally, it is argued that both Muslim contexts and liberal theory are dynamic and continually developing and that this shared dynamism means that there may be space for convergence of the two. Just as Muslim contexts have developed historically (and continue to develop today) the same is the case with the requisites of liberal theory and this may allow for liberal choices to be made in a manner that is not a renunciation of Muslim heritage
Perspectives on the status of the teaching profession in Pakistan: an investigation of trainee teachers’ reasons for choosing the teaching profession, the role of the teacher and problems faced by trainee teachers
This thesis mainly analyses the trainee teachers’ reasons for choosing the teaching profession, the problems faced by them, perceptions about the role of a teacher and views about the status of the teaching profession in Pakistan. This study considered both trainee teachers and teacher trainers from a Teacher Training Institute, to examine the reasons for choosing the teaching profession in Pakistan. To seek the answer to the research questions of this study, grounded theory procedures are used to explore the views of participants and to provide an in-depth understanding in a particular context. In this research study, the research data are collected from three groups of participants by conducting one-to-one interviews and focus group discussions.
On the basis of the views of research participants, four major research categories are established and under each research category different sub categories are formed. It is reflected in the findings of this study that trainee teachers face many problems and difficulties in the Teacher Training Institute. Data analysis indicates the teaching profession is often associated with low achievers and poor people. It is also considered a respected profession and trainee teachers have a desire to bring positive changes to Pakistani society by adopting the teaching profession. Furthermore, teachers have a significant role in Pakistani society as there are very high expectations of a teacher; and many good qualities, roles and responsibilities are associated with a teacher. The results of this study indicate there is a great influence from the social, cultural and religious norms of Pakistani society on the participants' perceptions and understanding about the different aspects of this study
a global survey-based study to assess diagnostic and treatment approaches in pneumonia managed in intensive care
Publisher Copyright: © The Author(s) 2024.Background: Pneumonia remains a significant global health concern, particularly among those requiring admission to the intensive care unit (ICU). Despite the availability of international guidelines, there remains heterogeneity in clinical management. The D-PRISM study aimed to develop a global overview of how pneumonias (i.e., community-acquired (CAP), hospital-acquired (HAP), and Ventilator-associated pneumonia (VAP)) are diagnosed and treated in the ICU and compare differences in clinical practice worldwide. Methods: The D-PRISM study was a multinational, survey-based investigation to assess the diagnosis and treatment of pneumonia in the ICU. A self-administered online questionnaire was distributed to intensive care clinicians from 72 countries between September to November 2022. The questionnaire included sections on professional profiles, current clinical practice in diagnosing and managing CAP, HAP, and VAP, and the availability of microbiology diagnostic tests. Multivariable analysis using multiple regression analysis was used to assess the relationship between reported antibiotic duration and organisational variables collected in the study. Results: A total of 1296 valid responses were collected from ICU clinicians, spread between low-and-middle income (LMIC) and high-income countries (HIC), with LMIC respondents comprising 51% of respondents. There is heterogeneity across the diagnostic processes, including clinical assessment, where 30% (389) did not consider radiological evidence essential to diagnose pneumonia, variable collection of microbiological samples, and use and practice in bronchoscopy. Microbiological diagnostics were least frequently available in low and lower-middle-income nation settings. Modal intended antibiotic treatment duration was 5–7 days for all types of pneumonia. Shorter durations of antibiotic treatment were associated with antimicrobial stewardship (AMS) programs, high national income status, and formal intensive care training. Conclusions: This study highlighted variations in clinical practice and diagnostic capabilities for pneumonia, particularly issues with access to diagnostic tools in LMICs were identified. There is a clear need for improved adherence to existing guidelines and standardized approaches to diagnosing and treating pneumonia in the ICU.
