    Problems related to environmental damage is a hottopic discussed today. Aside from being concerned with thebasic needs of all humanity, environmental degradationgoes hand in hand with economic and technologicaladvancements which are expected to lead to humanbenefit and prosperity.     Spinal Extraosseous Extradural Ewing’s Sarcoma: A Systematic Review

    Objective:&nbsp;&nbsp;The objective of this systematic review was to state and analyze all the case reports published on extraskeletal extradural Ewing’s Sarcoma to date. Material and Methods:&nbsp;&nbsp;We searched PubMed, MEDLINE, EMBASE, and Google Scholar using the following search term, including Boolean operators AND and OR. We only included published&nbsp;case reports and series about Ewing's Sarcoma that were Extra-skeletal and Extradural. We excluded the intradural EWS cases and those that did not have both components, i.e. extraskeletal and extradural. Conference articles, commentaries, and unpublished articles were also not included. A PRISMA flowchart was also formed. Results:&nbsp;&nbsp;After the first case of EES in 1969, around 40 case reports and series have been reported, which sheds light on the rarity of this disease. Age of presentation varies from as early as two months, with the oldest report being 49 years of age, and most cases presenting in adolescents and young adults. Spinal Epidural extraskeletal Ewing's sarcoma has been more frequently reported in males. The level of occurrence of the tumor also varies among cases, with cervical, thoracic and lumbar regions being more frequently affected than sacral regions. Conclusion:&nbsp;&nbsp;Extraskeletal extradural ES is an exceedingly rare anomaly. Multicentric collaborative work is the need of the hour for such orphan illnesses, to optimize treatment paradigms as a single-center is exceedingly unlikely to amass cases

    Extraosseous extradural ewing sarcoma of the thoracic spine: case report and literature review

    Background: As part of the ‘small round-cell tumor’ family, Ewing’s sarcoma (ES) is a malignant mesenchymal tumor classified as a primitive neuroectodermal tumor (PNET). Within the PNETs, spinal extraosseous extradural lesions are exceedingly rare. Case Description: A 19-year-old male presented with a one-month history of back pain and paraparesis with loss of urinary control. The MRI revealed an intradural, extramedullary mass at the T3-T4 level. Intraoperatively, the tumor was solely extradural, without evidence of local invasion; it was entirely removed. Due to the high risk of metastasis, the patient was subsequently treated with chemotherapy and radiation. In addition, similar studies on PNETs were reviewed. Conclusion: A 19-year-old male presented with a paraparesis attributed to an extraosseous extradural ES at the T3-T4 level. Following total gross resection, he was successfully managed with chemotherapy and radiation. The patient has been in remission for one month

    Gender bias and ‘doctor brides’. A social dilemma of medical students in Pakistan

    Objective: To assess gender bias, discrimination and bullying at medical schools, and to explore the phenomenon of ‘doctor brides’. Method: The multicentre survey was conducted at 14 medical education institutions across Pakistan from September 2020 to April 2021, and comprised medical students of either gender at both public-sector and private-sector institutions. The survey questions explored beliefs, experiences and knowledge related to common stereotypes and social issues in medical education, including female role models, work-life balance, gender roles, lack of support from family and faculty, and bullying. Association between gender with survey variables was explored. Data was analysed using SPSS 26. Thematic analysis was used to exploring knowledge around ‘doctor-brides’. Results: Of the 377 subjects, 245(65%) were females. The overall mean age was 21.4+/- 1.8 years. There were 211(53.8%) subjects aged 21-23 years, and 368(97.6%) were Muslims. Significantly more women than men were of the opinion that men are encouraged and are more likely to assume leadership roles (p=0.002). More women than males agreed that household chores and work had an impact on speciality choice (p&lt;0.001). Most sexual assault victims were women (p&lt;0.0001), but men generally faced more bullying and hostile behaviour (p=0.014). With regard to women being forced to quit medicine after marriage/childbirth by their in-laws/husbands or change their careers from clinical medicine to preclinical teaching, 99(26.25%) subjects knew first-hand of such cases, while 238(63.12%) had no such experience to share. Conclusion: Gender bias, discriminatory behaviour and bullying were found to be widely prevalent in medical schools across Pakistan. The general perception of ‘doctor brides’ needs to be revisited

    Probiotics for the prevention of ventilator-associated pneumonia: an updated systematic review and meta-analysis of randomised controlled trials

    Background: Presently, there is conflicting evidence regarding the efficacy of probiotics in the prevention of ventilator-associated pneumonia (VAP). This meta-analysis was conducted to update current clinical evidence and evaluate the efficacy and safety of probiotics for the prevention of VAP. Methods: We searched three databases and two trial registers to retrieve randomised controlled trials (RCTs) comparing probiotics or synbiotics with placebo or standard treatment for the prevention of VAP in adult patients receiving mechanical ventilation in the intensive care unit (ICU). Results: Our meta-analysis included 18 RCTs involving 4893 patients. Our results showed that probiotics may reduce the incidence of VAP (RR 0.68, 95% CI: 0.55–0.84; low certainty). However, in our subgroup and sensitivity analyses, the effect was not significant in double-blind studies, and in studies with a low risk of bias in the randomisation process. Probiotics reduced the length of ICU stay (MD −2.22 days, 95% CI: −4.17 to −0.28; moderate certainty) and the duration of antibiotic use (MD −1.25 days, 95% CI −1.86 to −0.64; moderate certainty). Conclusions: Probiotics may reduce the incidence of VAP but due to the low quality of pooled evidence, the use of probiotics warrants caution. Further, large-scale, high-quality RCTs need to be conducted to provide conclusive evidence

INDONESIA : Problematika pembelajaran bahasa Arab dilembaga Perguruam tinggi belum mendapatkan tempat yang strategis khusunya perguruan tinggi agama islam.Dalam penerapan pembelajaran bahasa Arab perguruan tinggi agama islam belum menemukan model pengembangan pembelajaran bahasa Arab sebagai karakteristik perguruan tinggi agama islam.Melalui pemikiran Prof.Dr.Imam Suprayogo memberikan sebuah solusi dalam mengembangkan pembelajaran bahasa Arab yaitu dengan mendirikan sebuah program khusus pembelajaran bahasa Arab secara intensif yang diperkuat dengan didirikanya mahad. Dari keterangan tersebut,maka peneliti merumuskan masalah penelitianya yaitu:(1)Bagaimanakah pemikiran Prof.Dr.Imam Suprayogo dalam pengembangan bahasa Arab di Universitas Islam Maulana Malik Ibrahim Malang, (2)Bagaimanakah penerapan pemikiran Prof.Dr.Imam Suprayogo dalam pengembangan bahasa Arab di Universitas Islam Maulana Malik Ibrahim Malang.Adapun metode yang digunakan dalam penelitian ini adalah kualitatif deskripti. Berdasarkan hasil penelitian,dapat disimpulkan bahwa pemikiran Prof.Dr.Imam Suparyogo dalam mengembangkan Pembelajara bahasa Arab di Universitas Islam Negeri Maulana Malik Ibrahim Malang menjadikan bahasa Arab sebagai karakteristik perguruan tinggi agaa islam dengan harapan dapat membantu mahsiswa dalam mengkaji agama islam secara mendalam sehingga tujuan daripada pohon keilmuan Universitas Islam Maulana Malik Ibrahim Malang dapat terwujuad. Adapun penerapan yang beliau lakukan adalah mencari informasi bagimana pembelajaran bahasa Arab yang mudah dan cepat, mendirikan sebuah program intensif pembelajaran bahasa Arab serta secara langsung mengawasi proses pembelajaran bahasa Arab disetiap kelas,mendirikan mahad sebagai “biah al-lughowiyah (lingkungan bahasa” dan mengundang langsung pengarang buku al-Arobiyah baina Yadaika untuk mengajarkan kepada dosen Program Khusus pembelajaran Bahasa Arab. Dengan demikian, pemikiran Prof.Dr. Imam suprayogo membantu perguruan tinggi islam dalam mengembangakan segala potensinya sehingga memiliki nilai lebih dari Universitas yang lainnya.Saat ini, bangunan Universitas Islam Maulana Malik Ibrahim Malang berdiri dengan kokoh dan dipadu dengan konsep integrasi ilmu Agama dan Sains dalam pengembangan keilmuaanyan .Hal ini berdasarkan dengan hasil penelitain peneliti dengan menggunakan data dokumentasi, observasi,serta wawancara kepada pihak-pihak yang berhubungan dengan beliau. ENGLISH : The problem of Arabic language learning in the university do not get a strategic place yet especially in Islamic university. The applied of Arabic language learning in Islamic university has not found development of Arabic language learning model as the characteristic of Islamic university. Through Prof.Dr.Imam Suprayogo’s thought, it gives a solution on developing Arabic language learning by building a special program of Arabic language learning intensively which strengthened by building of Ma’had. From the explanation above, the researcher arranges the problem of the study as follow: (1) how is Prof.Dr.Imam Suprayogo’s thought on developing Arabic language in Maulana Malik Ibrahim State Islamic University of Malang? (2) How is Prof.Dr.Imam Suprayogo’s thought applied on Arabic language development in Maulana Malik Ibrahim State Islamic University of Malang? Moreover, the method that is used in this study is descriptive qualitative. Based on the result of this study, it can be concluded that Prof.Dr.Imam Suparyogo’s thought on developing Arabic language in Maulana Malik Ibrahim State Islamic University of Malang brings Arabic language as characteristic of Islamic university. It is expected to help the student on studying Islamic religion deeply so that the aim of science tree of Maulana Malik Ibrahim State Islamic University of Malang can be realized. Moreover, the applied which He did is looking for information about how Arabic language learning become easy and practical, creating intensive Arabic learning program and directly controlling the process of Arabic language learning in each class, building Ma’had as “biah al-lughowiyah (language environment) and inviting the author of al-Arobiyah baina Yadaika directly to teach the lecturer of intensive Arabic learning program. Hence, Prof. Dr. Imam suprayogo’s thought help Islamic university on developing all of potential aspect so that it has more value from other university.     Aggressive Versus Moderate Fluid Replacement for Acute Pancreatitis: An Updated Systematic Review and Meta‐Analysis

    ABSTRACT Background A conservative strategy is the primary modality of treatment for acute pancreatitis of which fluid replacement is an important component. Since the results regarding early aggressive versus moderate fluid replenishment for acute pancreatitis are inconsistent, we sought to compare outcomes between the two resuscitation strategies in our meta‐analysis. Methods We searched MEDLINE (PubMed), Embase, the Cochrane Library, and ClinicalTrials.gov for all available randomized controlled trials (RCTs) assessing outcomes for patients treated with aggressive fluid replacement compared to moderate fluid replacement. Our primary outcome was all‐cause mortality. Results Our meta‐analysis included 6 RCTs involving a total of 632 patients. Our results showed that aggressive fluid resuscitation increased the risk of all‐cause mortality as compared to moderate fluid replacement (RR 2.40, CI: 1.38–4.19). For all of our secondary outcomes which included the development of organ failure, severe pancreatitis, pancreatic necrosis, clinical improvement, development of SIRS, persistent SIRS, and length of hospital stay, the results indicate that there was no significant difference between the two groups. Conclusions Aggressive fluid resuscitation is associated with higher mortality as compared to moderate fluid replacement in patients with acute pancreatitis. RCTs with larger sample sizes are needed to provide greater statistical power and establish more definitive conclusions

    Efficacy and safety of topical ruxolitinib cream for the treatment of vitiligo: a systematic review and meta-analysis of randomized controlled trials

    Vitiligo is an autoimmune condition with an estimated prevalence of 0.5%–2% worldwide.1 Besides the visible cosmetic concerns associated with vitiligo, it is marked by psychological concerns such as anxiety and distress leading to reduced quality of life.1 The disease is known to be mediated by interferon-gamma (IFN-γ) which acts through the Janus kinase-signal transducer and activator of transcription (JAK–STAT) pathway to recruit CD8+ T cells, which in turn drive the cytotoxicity directed against melanocytes via detachment and apoptosis, leading to the characteristic white skin patches.1 Ruxolitinib acts as a JAK1 and JAK2 inhibitor to suppress the IFN-γ-mediated pathway and prevent melanocyte damage, allowing them to heal and re-pigment.2 Phase 3 trials of ruxolitinib have recently been published and demonstrate encouraging outcomes in vitiligo patients. This meta-analysis aims to systematically collate outcomes from the relatively limited data available and evaluate the efficacy and safety of ruxolitinib in vitiligo patients.</p
