    Plasma thymulin and nerve growth factor levels in patients with primary open angle glaucoma and elevated intraocular pressure

    Background: The objective was to measure the plasma concentrations of thymulin and nerve growth factor (NGF) in a group of patients with primary open angle glaucoma (POAG) and compare them with age- and sex-matched normal controls. Methods: Twenty-eight patients newly diagnosed with POAG who were not undergoing treatment were compared with the same number of age- and sex-matched healthy controls. Blood samples were drawn into heparinized tubes and plasma samples were collected for the determination of the concentrations of thymulin and NGF, using specific enzyme-linked immunosorbent assay (ELISA). The Student's t test was used to perform the necessary statistical analysis of the results. Results: Seventeen women and 11 men were enrolled in each of the two groups (study and control), with a mean age of 63.7 (SD 10.3) years in the former and 63.3 (SD 9.6) years in the latter. There was a highly significant (p0.001) elevation in the thymulin levels in POAG patients compared with the control group. However, no significant difference was observed when comparing the plasma NGF levels. Conclusion: This is the first report to measure plasma thymulin levels in glaucoma patients. The significant results point the possible role of this immunomodulator in the pathogenesis of primary open angle glaucoma. The potential role of NGF seems to be less likely.     LASIK for high myopic astigmatism resulting from perforating ocular injury

    PURPOSE: To present a case of successful treatment of high myopic astigmatism after a perforating injury with significant corneal laceration and pupil ectopia using excimer LASIK. METHODS: A 30-year-old man presented for treatment of high myopic astigmatism in his right eye resulting from perforating ocular trauma, which occurred 8 years prior to presentation. The corneoscleral wound repair was performed with posterior chamber intraocular lens implantation. Uncorrected visual acuity (UCVA) in the right eye was counting fingers at 1 m, best corrected to 20-30 with -7.00 +5.00 × 110. Visual acuity in the left eye was 20-20 with -1.5 D. The patient was unable to tolerate spectacles and contact lenses because of asthenopia and high astigmatism, respectively. As an alternative, LASIK with the Allegretto Wave excimer laser was performed. RESULTS: Three months postoperatively, UCVA was 20-25 in the right eye and 20-15 in the left eye.     Recalcitrant Post-LASIK Mycobacterium chelonae Keratitis Eradicated after the Use of Fourth-Generation Fluoroquinolone

    Objective: To report a patient with Mycobacterium chelonae keratitis after LASIK and discuss therapeutic measures. Design: Interventional case report and literature review. Intervention: A healthy, 25-year-old man presented 6 weeks after LASIK with infectious keratitis in the left eye. Scrapings were obtained from the central stromal bed after lifting the flap and inoculated on culture media. Main Outcome Measures: Response to medical treatment. Results: Mycobacterium chelonae was identified from stromal bed scrapings. The protracted course of the infection necessitated surgical debridement and flap amputation with slow and suboptimal response to prolonged (14 weeks) treatment with topical amikacin 3.3percent, clarithromycin 1percent, and levofloxacin 0.5percent. The substitution of levofloxacin 0.5percent with gatifloxacin 0.3percent resulted in closure of the epithelial defect within 1 week and resolution of the infiltrates in 3 weeks. The combined regimen of gatifloxacin 0.3percent, amikacin 3.3percent, and clarithromycin 1percent was continued for a total of 4 months. The patient remains infection free 1 year after stopping all antibiotics. Conclusion: Treatment of post-LASIK nontuberculous mycobacteria remains a challenge.     Reliability and Validity of the Persian Version of the Mini-Balance Evaluation Systems Test in Patients with Stroke

    Background: Stroke can cause balance disorders, which often lead to falls and fall-related injuries. The Mini-Balance Evaluation Systems Test (Mini-BESTest) is a balance test that has been recently translated into Persian. The reliability and validity of the Persian version of Mini-BESTest have not been assessed in patients with stroke. Objectives: To assess the reliability and validity of the Persian version of the Mini-BESTest in patients with stroke. Methods: A cross-sectional study was designed. Thirty patients with stroke participated in this study. Patients were tested using the Mini-BESTest according to the Persian instructions, and two raters independently rated each patient�s performance. Each patient was matched with a healthy adult in the terms of age and gender. Healthy subjects were also tested for discriminative validity. Results: There was excellent correlation between two raters on the Persian version of the Mini-BESTest total scores (rPearson = 0.98, P < 0.001) and its sections (rPearson > 0.9). There was a significant difference between stroke patients and healthy subjects confirming the discriminative validity of the Persian version of the Mini-BESTest (19.4 ± 5.4 vs. 24.8 ± 2.3, P < 0.001). Limitations: We only assessed stroke patients, and the results may not be generalized to other patients with balance deficits. Conclusions: The Persian version of the Mini-BESTest is a reliable and valid tool for balance evaluation of stroke patients. © 2020, The Author(s)

    Artificial intelligence in liver cancer research: a scientometrics analysis of trends and topics

    Background and aimsWith the rapid growth of artificial intelligence (AI) applications in various fields, understanding its impact on liver cancer research is paramount. This scientometrics project aims to investigate publication trends and topics in AI-related publications in liver cancer.Materials and MethodsWe employed a search strategy to identify AI-related publications in liver cancer using Scopus database. We analyzed the number of publications, author affiliations, and journals that publish AI-related publications in liver cancer. Finally, the publications were grouped based on intended application.ResultsWe identified 3950 eligible publications (2695 articles, 366 reviews, and 889 other document types) from 1968 to August 3, 2023. There was a 12.7-fold increase in AI-related publications from 2013 to 2022. By comparison, the number of total publications on liver cancer increased by 1.7-fold. Our analysis revealed a significant shift in trends of AI-related publications on liver cancer in 2019. We also found a statistically significant consistent increase in numbers of AI-related publications over time (tau = 0.756, p < 0.0001). Eight (53%) of the top 15 journals with the most publications were radiology journals. The largest number of publications were from China (n=1156), the US (n=719), and Germany (n=236). The three most common publication categories were “medical image analysis for diagnosis” (37%), “diagnostic or prognostic biomarkers modeling & bioinformatics” (19%), and “genomic or molecular analysis” (18%).ConclusionOur study reveals increasing interest in AI for liver cancer research, evidenced by a 12.7-fold growth in related publications over the past decade.
