CLINICAL APPLICATION OF AUTOLOGOUSPLATELET RICH PLASMA (P.R.P.) IN THEEXTRACTION OF THIRD IMPACTEDMANDIBULAR MOLAR
The impacted third molar surgery has various limits; one of these limit is the type of surgery applied, often demolitive for the patientswith long term consequeses in the post-op period. Aim of our study is to get a better healing of soft and hard tissues with theapplications of PRP in this type of surgery. 5 patients were included in the study with these requests: the impacted or semiimpacted third molar were on both side; the acceptance of PRP tecnique on one side (considered as the case) the extraction ofthe other impacted molar as the controll side to our case; both teeth were extracted on the same day, both sockets were closedby hermetically suturing the flap; on one socket it has been inserted the platelet gel on the other side nothing. Pre operativemesurements were: 1. probing depth of both the seventh (3.7-4.7); 2. ortopantomography (OPT). Post operative measurementsincluded: 1. probing depth two months after surgery; 2. OPT at one weeek, one month, two month.One week after surgery patiens were aked about the post-op through a questionary on the course of the week, in specific theywere asked to assess a score from one to three on swelling and pain of the two side. One week after a clinician who was out of thestudy (not the surgeon) evaluated the eventual bacterial sovrainfections, the dehicence of the flaps, the eventual collateral effectsgiven by the application of PRP giving a score from one to three to the type of healing. Periodontal healing was evaluated on bothside after 2 months after surgery in all the cases treated the initial P.D. was 2-3mms on both sides. It showed an improvement inthe sites treated with P.R.P. Swelling (perceived by the patients during the course of the fist week) was not reduced by theapplication of PRP gel, while there has been a reduction in the pain in comparison with the control side reffered by the patients.Clinical evaluation realized a week after the extractions showed a better healing on the PRP side vs the controll side (total score12 vs 8) where three patients from five realized a primary closure with no bacterial sovrainfection or dehiscence of the flap vs oneprimary closure on the controll side. Autologous platelet concentrates for bisphosphonate-related osteonecrosis of the jaw treatment and prevention : a systematic review of the literature
Purpose: Bisphosphonate related osteonecrosis of the jaw (BRONJ) is an adverse drug reaction consisting of progressive bone destruction in the maxillofacial region of patients under current or previous treatment with a bisphosphonate. Autologous platelet concentrates (APC) demonstrated to enhance bone and soft tissue healing in oral surgery procedures. The present systematic review aimed at evaluating if APC may improve treatment and prevention of BRONJ in patients under bisphosphonate therapy.
Methods: MEDLINE, Scopus and Cochrane databases were searched using terms like bisphosphonates, osteonecrosis, BRONJ, platelet concentrate, PRP, PRF, PRGF. No language, publication date and study design limitation was set. A hand search of the bibliographies of identified articles was also performed. The primary outcome was recurrence/onset of BRONJ after oral surgery procedures.
Results: Eighteen studies were included, reporting on 362 patients undergoing oral surgery in combination with APC. The adjunct of APC in BRONJ treatment significantly reduced osteonecrosis recurrence with respect to control. APC was associated with a lower BRONJ incidence after tooth extraction, though not significant. Heterogeneity was found regarding bisphosphonate type, clinical indication, treatment duration, triggering factors, study design, follow-up duration, type of APC, outcomes adopted to evaluate treatment success.
Conclusion: Though the results of this review must be cautiously interpreted, due to the low evidence level of the studies included, and the limited sample size, they are suggestive of possible benefits of APC when associated with surgical procedures for treatment or prevention of BRONJ. To confirm such indication, prospective comparative studies with a large sample size are urgently needed
Long-term (9-12 years) outcomes of titanium implants with an oxidized surface : a retrospective investigation on 209 implants
To retrospectively assess the long-term clinical and radiological results in a group of patients treated with Brånemark TiUnite implants supporting mostly single-tooth, and partial restorations.Material and Methods: The clinical records of 90 consecutive patients (mean age 55.9 years; range 21-82 years), treated with 209 Brånemark System MkIII or MkIV TiUnite® implants (72 maxillary/137 mandibular; 26 anterior inter-canine/183 posterior sites), were analyzed. Indication types were single tooth (n=21 implants), partial (n=180) and full arches (n=8). A delayed loading protocol was applied in 128 implants while 81 were immediately loaded. Cumulative survival rate and marginal bone remodeling were evaluated. Marginal bone level was evaluated by an independent radiologist from periapical radiographs taken at implant insertion and at long-term follow up. Plaque, probing pocket depth and peri-implant mucosa conditions were also assessed.Results: Mean follow up duration was 11.0 years (range 9.6-12.4 years): 181 implants (90.5%) reached at least ten years follow up, 100 implants eleven years, and 17 implants twelve years. Overall six implants failed in four patients (5 during the first year and 1 after two years) resulting in a 97.1% survival rate after 12 years. Mean bone levels at implant insertion and at the last follow up were -0.90±1.16 mm (mean±SD; n=169) and -1.49±0.95 mm (n=195), respectively. Mean marginal bone remodeling from implant insertion to the last follow up was -0.60±1.17 mm (n=168). At the last available follow up mean pocket depth was 1.65±0.84 mm. Peri-implant mucosa was normal for the majority (97%) of implants.Conclusion: This retrospective long-term study showed excellent survival rate of TiUnite® implants as well as favorable marginal bone response and soft tissue conditions
Des collines florentines à Tours: Antoine Juste et sa famille
Au cours de la première décennie du XVIe siècle, le sculpteur et scarpellino florentin Antoine Juste fait son apparition dans la production artistique française, assisté par son frère puîné Jean. Leur présence à Amboise puis à Tours est très appréciée dans le milieu de la Cour : des œuvres telles le tombeau de Thomas James à Dol (1507), le bas relief bronze de la bataille de Gênes qui ornait la porte monumentale du château de Gaillon (1508) et le tombeau de Louis XII et Anne de Bretagne, ont joué un rôle central dans le processus de création de la première Renaissance française.
Malgré cette centralité dans le panorama artistique du règne de Louis XII et de la première partie de celui de François Ier, leur formation en Italie n'a pas encore été étudiée, tout comme les raisons qui ont conduits les Juste à quitter leur Pays pour s'établir en France.
À l'aide d'une série de documents nouveaux issus des archives italiennes et par l'analyse du tombeau de Louis XII et Anne de Bretagne, cette communication place la formation des Juste dans le milieu des artistes –sculpteurs et scalpellini– agissant dans la deuxième moitié du XVe siècle entre Fiesole et Settignano, sous l'influence de Benedetto da Maiano et Andrea Ferrucci, permettant de préciser la chronologie et les modalités de leur arrivée à la Cour des rois de France. Ces réflexions nouvelles consentent de mieux apprécier l'apport stylistique qu'ils ont donné au développement de la première Renaissance mais également l'impact de la tradition française sur leur formation florentine.From 1506, the Florentine sculptor Antoine Juste was working at Amboise and then Tours for Louis XII and his entourage. His early education in Italy has, however, not yet been studied; nor have the reasons for his settling in France. With the help of a series of newly found documents, the present essay places his early training among those artists working between Fiesole and Settignano in the second half of the 15th century
