The effects of L-arginine in renal protection using urinary and serum biomarkers levels and histopathologic features: experimental model study of ischemia/reperfusion injury in rats under inhalatory anesthesia
Background: Nitric oxide (NO) inactivation occurs during renal ischaemia/reperfusion injury. This inactivation decreases endothelium-dependent and -independent arterial relaxation. Higher NO levels can improve endothelial dysfunction and arterial relaxation, thereby reducing the level of kidney injury. NO, synthesized from L-arginine by the enzyme nitric oxide synthase (NOS), seems to exerts a protective effect on the kidneys during tissue ischemia-reperfusion injury. The objective of this study was to evaluate the effect of L-arginine on the levels of injury biomarkers (pNGAL, uNGAL, KIM-1 e IL-18) for kidney function and histology in rats subjected ischaemiareperfusion injury. Methods: Thirty-two Wistar rats were randomised into the following 4 groups: Sham (S), laparotomy and right nephrectomy; Control (C), laparotomy and right nephrectomy and left kidney I/R; L-arginine control (LAC), laparotomy and right nephrectomy and L-arginine at a dose of 800 mg.kg-1 .dose-1 , 24 hours and 1 hour before surgery; and L-arginine (LA), laparotomy and right nephrectomy, left kidney I/R, and L-arginine at a dose of 800 mg.kg -1 .dose-1 , 24 hours and 1 hour before surgery. The pNGAL, uNGAL, KIM-1 e IL-18 and creatinine levels and kidney histopathology were analysed. The significance level was p S≈LAC. pNGAL was increased in all groups and showed a similar trend. uNGAL levels were higher in groups C and LAC than LA and had intermediate values in group S. The KIM-1 levels were increased in the S, LAC and LA groups, but the contrast analysis revealed no differences between the groups in terms of their trends. IL-18 levels were higher in LA than in the other groups (C, S and LAC). The C and LA groups, compared with the S and LAC groups, had higher damage on the left side. There was no histopathological difference between C and LA or between S and LAC. Conclusion: L-arginine does not exert a damaging or protective effect on kidneys undergoing I/R injury based on the evaluation of kidney functions, histopathology and pNGAL and KIM-1 levels.Justificativa: Durante lesão de isquemia e reperfusão renal ocorre a inativação de óxido nítrico (NO), essa inativação diminui o relaxamento arterial endotélio-dependente e independente. O aumento nos níveis de NO poderia melhorar a disfunção endotelial e o relaxamento arterial diminuindo a lesão renal. O NO, sintetizado a partir da L-arginina pela enzima óxido nítrico sintase (NOS), parece exercer um efeito protetor sobre os rins durante a lesão de isquemia/reperfusão (I/R). O objetivo do estudo foi avaliar o efeito da L-arginina nos níveis dos biomarcadores de lesão (NGALp, NGALu, KIM-1 e IL-18) na função e na histopatologia renal em ratos submetidos a lesão de I/R. Métodos: 32 ratos Wistar foram randomizados em 4 grupos: Sham (S): Laparotomia e nefrectomia direita. Controle (C): Laparotomia e nefrectomia direita, I/R em rim esquerdo. Controle L-arginina (CLA): Laparotomia e nefrectomia direita, L-arginina na dose de 800mg.kg-1.dose-1, 24 e 1 hora antes. L-arginina (LA): Laparotomia e nefrectomia direita, I/R em rim esquerdo, L-arginina na dose de 800mg.kg-1.dose-1, 24 e 1 hora antes. Foram analisados os níveis de NGALp, NGALu, KIM-1 e IL-18, creatinina e histopatologia renal. Nível de significância: pS≈CLA. A NGALp, aumenta em todos os grupos com evolução semelhante. A NGALu foi maior no grupo C e CLA em relação ao grupo LA e apresentou valores intermediários no grupo S. Os níveis de KIM-1 aumentam nos grupos S, CLA e LA, porém na análise de contrastes não há diferença na evolução dos grupos. A IL-18 apresentou valores maiores no grupo LA comparado aos demais grupos (C, S e CLA). Os grupos C e LA apresentaram grau de lesão no lado esquerdo maior que os grupos S e CLA. Não existe diferença histopatológica entre C e LA e nem entre S e CLA. Uso de realidad virtual en el manejo del dolor y la ansiedad durante la inducción anestésica en niños y adolescentes en el centro quirúrgico
Introdução: A ansiedade pré-operatória é um ponto importante e merece bastante atenção pois acomete aproximadamente 50 a 70% das crianças submetidas a procedimentos em centro cirúrgico. A ansiedade extrema pode se manifestar de forma multifatorial tanto com sintomas físicos como tremores, sudorese, choro, agitação, brigas e comportamento de fuga, como também pode desencadear desfechos emocionalmente traumáticos nas crianças levando a alterações psicológicas e comportamentais como: distúrbio do sono, incluindo pesadelos, enurese noturna, distúrbios da alimentação, ansiedade de separação dos pais, delirium e hiperalgesia pós-operatória.
Considerando que o nível de ansiedade e estresse podem afetar o nível de experiência de dor durante procedimentos médicos, várias intervenções têm sido propostas no intuito de distrair e entreter as crianças e, assim, consequentemente reduzir estes efeitos adversos. Atualmente as intervenções disponíveis para o manejo da ansiedade pré-operatória em pacientes pediátricos se enquadram em quatro categorias: o uso de sedativos pré-operatórios; permitir a presença dos pais durante a indução anestésica; oferecer programas de preparação ao paciente antes da cirurgia e intervenções de gerenciamento de dor ou ansiedade.
Diante desse cenário, o uso da Realidade Virtual (RV) surgiu como uma intervenção não farmacológica promissora para aliviar a ansiedade pré-operatória, reduzir a dor durante e após o procedimento melhorando a qualidade de vida do paciente. Visando a criação de um projeto inovador foi desenvolvido um jogo interativo com o nome “Projeto Treina Dragão” o qual integra, por meio do uso de óculos 3D, as realidades virtual e real de forma simultânea.
O jogo traz a história de um pequeno dragão o qual deverá ser “treinado” pelo paciente e passará ao longo de sua jornada por diversas etapas/fases as quais equivalem às sensações que a criança sentirá durante a indução anestésica em tempo real. Sendo assim, o objetivo deste trabalho, foi mensurar os benefícios desta ferramenta inédita a qual integra a realidade real vivenciada no centro cirúrgico durante a indução anestésica e a realidade virtual criada pelo jogo.
Objetivo: Implantar o método de gamificação da monitorização e indução anestésica com uso da Realidade Virtual através do jogo interativo “Projeto Treina Dragão” em crianças e adolescentes submetidos a procedimentos no centro cirúrgico.
Método: Os pacientes e cuidadores foram convidados a participar da pesquisa e após o aceite foi aplicado o Termo de Consentimento Livre e Esclarecido (TCLE) aos responsáveis de todos os pacientes aptos a participarem do projeto de pesquisa. Para aqueles pacientes com idade entre 12 e 14 anos foi aplicado, também, o Termo de Assentimento.
Foram incluídas no estudo crianças de 4 a 14 anos, que foram submetidas a procedimento com uso de sedação endovenosa ou anestesia geral em centro cirúrgico. Os pacientes foram divididos em dois grupos, um que teve indução anestésica feita com o auxílio da RV, e outro que teve a indução anestésica sem o uso da RV. A avaliação da dor durante o procedimento foi realizada em dois momentos distintos, monitorização e a indução anestésica. Foram aplicadas, pelo mesmo avaliador, as escalas de dor de acordo com cada faixa etária. Para crianças de 4 a 7 anos, a escala de faces revisada (FPS-R) e para crianças com idade maior ou igual a 8 anos foi aplicada a escala analógica visual de dor (EVA).
A avaliação da ansiedade também foi realizada em dois momentos distintos pré-operatória, realizada na sala pré-anestésica, e avaliação da ansiedade pós-operatória, realizada na sala de recuperação anestésica após a completa recuperação do paciente. Para mensurar a ansiedade, foi aplicada, pelo mesmo avaliador nos dois momentos, a escala observacional EAPY-m (Yale Preoperative Anxiety Scale Modified).
Resultados: Nesse estudo prospectivo randomizado examinamos os efeitos do uso do jogo de Realidade Virtual como intervenção não farmacológica para redução da dor pré-operatória; redução da ansiedade pré-operatória e da ansiedade pós-operatória. Vale salientar que não houve relato de quaisquer eventos adversos ou efeitos colaterais relacionados ao estudo em ambos os grupos.
Os resultados desta tese confirmaram que uso da RV foi associado à redução da ansiedade pós-operatória e foi associado à redução da percepção de dor no momento de indução anestésica no subgrupo de pacientes acima de 8 anos. No entanto, ao avaliar o uso da RV como intervenção para redução da dor pré-operatória para faixa etária de 4 a 7 anos e da ansiedade pré-operatória para todas as faixas etárias os resultados mostraram que apesar dos pacientes do grupo intervenção (com uso da RV) apresentarem menores médias para ambas as variáveis não houve significância estatística.
Conclusão: Até o momento este é o primeiro estudo o qual se utiliza de um jogo interativo com uso da Realidade Virtual em concomitância com a realidade vivenciada no Centro Cirúrgico pelo paciente. Os resultados obtidos enfatizam os benefícios do uso da RV e sugerem a capacidade de atenuar a ansiedade pós-operatória e modular a dor em pacientes pediátricos submetidos à procedimentos em Centro Cirúrgico. Acreditamos que seja necessário a realização de novos estudos complementares.Introduction: Preoperative anxiety is an important point and deserves close attention as it affects approximately 50 to 70% of children undergoing procedures in a surgical center. Extreme anxiety can manifest itself in a multifactorial way, with physical symptoms such as tremors, sweating, crying, agitation, fights, and escape behavior, and can also trigger emotionally traumatic outcomes in children, leading to
psychological and behavioral changes such as: sleep disturbance, including nightmares, nocturnal enuresis, eating disorders, parental separation anxiety, delirium, and postoperative hyperalgesia.
Considering that the level of anxiety and stress can affect the level of pain experienced during medical procedures, several interventions have been proposed to distract and entertain children and, thus, consequently reduce these adverse effects.
Currently available interventions for the management of preoperative anxiety in pediatric patients fall into four categories: the use of preoperative sedatives; allow parents to be present during anesthetic induction; offer patient preparation programs before surgery and pain or anxiety management interventions.
Given this scenario, the use of Virtual Reality (VR) has emerged as a promising non-pharmacological intervention to alleviate preoperative anxiety, reduce pain during and after the procedure, improving the patient's quality of life. Aiming to create an innovative project, an interactive game was developed called “Projeto Treina Dragão” which integrates, using 3D glasses, virtual and real realities simultaneously.
The game tells the story of a small dragon which must be “trained” by the patient and will go through several stages/phases throughout its journey which are equivalent to the sensations that the child will feel during anesthetic induction in real time. Therefore, the objective of this work was to measure the benefits of this unprecedented tool, which integrates the real reality experienced in the surgical center
during anesthetic induction and the virtual reality created by the game.
Objective: Implement the gamification method of monitoring and anesthetic induction using Virtual Reality through the interactive game “Projeto Treina Dragão” in children and adolescents undergoing procedures in the surgical center.
Method: Patients and caregivers were invited to participate in the research and after acceptance, the Free and Informed Consent Form (TCLE) was applied to the guardians of all patients able to participate in the research project. For those patients aged between 12 and 14 years, the Assent Form was also applied.
Children aged 4 to 14 years were included in the study, who underwent a procedure using intravenous sedation or general anesthesia in a surgical center. The patients were divided into two groups, one that had anesthetic induction performed with the aid of VR, and the other that had anesthetic induction without the use of VR. Pain assessment during the procedure was carried out at two different moments, monitoring
and anesthetic induction. Pain scales were applied by the same evaluator according to each age group. For children aged 4 to 7 years, the revised faces scale (FPS-R) and for children aged 8 years or older, the pain visual analogue scale (VAS) was applied.
Anxiety assessment was also carried out at two different pre-operative moments, carried out in the pre-anesthetic room, and post-operative anxiety assessment, carried out in the anesthetic recovery room after the patient's complete recovery. To measure 9 anxiety, the observational scale EAPY-m (Yale Preoperative Anxiety Scale Modified) was applied by the same evaluator at both moments.
Results: In this prospective randomized study, we examined the effects of using Virtual Reality games as a non-pharmacological intervention to reduce preoperative pain; reduction of preoperative anxiety and postoperative anxiety. It is worth noting that there were no reports of any adverse events or side effects related to the study in either group.
The results of this thesis confirmed that the use of VR was associated with a reduction in postoperative anxiety and was associated with a reduction in the perception of pain at the time of anesthetic induction in the subgroup of patients over 8 years of age.
However, when evaluating the use of VR as an intervention to reduce preoperative pain for the age group of 4 to 7 years and preoperative anxiety for all age groups, the results showed that despite the patients in the intervention group (using of VR) presented lower means for both variables, there was no statistical significance.
Conclusion: To date, this is the first study that uses an interactive game using Virtual Reality in conjunction with the reality experienced in the Surgical Center by the patient.
The results obtained emphasize the benefits of using VR and suggest the ability to alleviate postoperative anxiety and modulate pain in pediatric patients undergoing procedures in the Operating Room. We believe that it is necessary to carry out new complementary studies
Analysis of the effects of mobility level in intensive care patients on improving functional outcomes and length of stay
INTRODUÇÃO: A mortalidade em Unidades de Terapia Intensiva ((UTIs) pode ser associada a diversos fatores como aumento do tempo de ventilação mecânica e internação, insucesso no processo de extubação, alto risco de infecções e fraqueza muscular adquirida. Esses fatores são relevantes nesse contexto, pois levam à perda da funcionalidade. OBJETIVOS: Avaliar a importância da mobilização precoce e utilização de métricas, e analisar desfechos clínicos e funcionais relacionados as intervenções fisioterapêuticas mais comuns utilizadas nas UTIs. MÉTODOS: Essa revisão sistemática foi registrada em 30 de setembro de 2022 na International Prospective Register of Systematic Reviews, sob número CRD42022361431, conduzida de acordo com as diretrizes Cochrane para revisões sistemáticas e relatada de acordo com o Preferred Reporting Items Systematic Reviews and Meta-Analyses (PRISMA) declaração de 2020. A avaliação do risco de viés nos estudos incluídos foi realizada pela ferramenta RoB 1.0 em todos os seus domínios em duplicata dos revisores, além da análise estatística demonstrada pelos gráficos de forrest-plot. RESULTADOS: Foram incluídos e contabilizados 50 pacientes entre os diferentes estudos desta revisão. Na análise do risco de viés, todos foram avaliados como baixo risco de viés em todas as categorias. Em relação aos gráficos da floresta com representações de meta-análise, observamos que não houve diferença estatisticamente significativa em relação ao tempo de ventilação mecânica e nas demais intervenções convencionais ou com eletroestimulação no desfecho de fraqueza muscular adquirida. CONCLUSÃO: Podemos inferir que os estudos apresentam boa qualidade metodológica. Em relação as diferentes intervenções fisioterapêuticas utilizadas para melhora da capacidade funcional, não observamos significância nas análises. A fim de obter maior definição e qualidade metodológica se faz necessário padronizar as avaliações por meio de métricas funcionais em pacientes internados nas Unidades de Terapia Intensiva
Impact of the pharmaceutical industry on clinical trials comparing inhalational anesthesia versus intravenous anesthesia: systematic quantitative research
Introdução: Pesquisas científicas sempre foram essenciais ao desenvolvimento da medicina no decorrer da história da humanidade, considerando seu caráter investigador e inovador para diversos tratamentos das doenças existentes. Juntamente ao crescimento da produção científica mundial estão as indústrias farmacêuticas, que atualmente, configuram-se grandes protagonistas financeiras, principalmente de ensaios clínicos na área da saúde. Contudo, é necessário analisar cuidadosamente a condução desses estudos, pois muitas vezes o interesse econômico e capitalista se sobrepõe à ética, podendo apresentar resultados tendenciosos e favoráveis a determinado medicamento fabricado pela indústria patrocinadora. Nesse sentido, ao longo da história mundial, alguns estudos patrocinados foram questionados quanto aos métodos utilizados pelos pesquisadores, e principalmente quanto aos resultados obtidos, detectando-se uma quantidade impressionante de estudos fraudulentos publicados em revistas médicas de alto impacto. Objetivo: Avaliar se há impacto da indústria farmacêutica nos resultados de ensaios clínicos randomizados que compararam a eficácia e segurança da utilização de anestesia inalatória versus anestesia intravenosa em cirurgia de revascularização do miocárdio com circulação extracorpórea e intracorpórea. Métodos: Pesquisa sistemática quantitativa de ensaios clínicos randomizados (ECRs) que avaliaram a eficácia e segurança da anestesia intravenosa (e.g. propofol, fentanil) quando comparada a anestesia inalatória (e.g. sevoflurano, isoflurano, desflurano) em pacientes submetidos à cirurgia de revascularização do miocárdio com ou sem circulação extracorpórea. Os estudos foram obtidos das seguintes bases de dados eletrônicas: PubMed, EMBASE, LILACS e CENTRAL. A data da última busca foi em 02 de Março de 2016. Dois revisores independentemente selecionaram os estudos, extraíram os dados dos mesmos e, avaliaram o risco de viés dos estudos incluídos. Além disso, os estudos foram divididos em dois grupos: estudos patrocinados por indústrias farmacêuticas e, estudos não patrocinados pela indústria. Resultados: 12 estudos com um total de 1.845 pacientes foram incluídos, sendo seis estudos (n= 1.209 pacientes) patrocinados por indústrias farmacêuticas e seis estudos (n= 640 pacientes) não patrocinados por indústrias. Em relação a avaliação do risco de viés, a maioria dos estudos foi dividido uniformemente entre baixo risco de viés e risco incerto de viés, independentemente se patrocinados ou não por indústrias farmacêuticas. Metanálise com quatro ECRs sugere uma redução do risco de mortalidade com o uso da anestesia inalatória quando comparada ao anestésico intravenoso no grupo dos estudos patrocinados por indústrias farmacêuticas [Risco relativo (RR) 0.45, 95% intervalo de confiança (IC) 0.23 à 0.89; I2 = 2%; p=0.02; quatro estudos, n = 927 pacientes]. Entretanto, na metanálise considerando os estudos não patrocinados pela indústria, não houve diferença estatisticamente significante entre o uso da anestesia inalatória quando comparada à anestesia intravenosa em relação ao risco de mortalidade (0.21 RR, 95% IC 0.01 à 4.24; I2 = não aplicado; p=0.31; três estudos, n = 493 pacientes). Resultados apresentaram-se semelhantes em relação ao uso de inotrópicos, ou seja, embora ambas metanálises considerando os estudos patrocinados por indústrias farmacêuticas (RR 0.71, 95% IC 0.48 à 1.06; I2 = 27%; p =0.09; três estudos, n = 382 pacientes) e os estudos não patrocinados pela indústria (RR 0.77, IC 95% 0.47 à 1.26; I2 = 74%; p=0.30; seis estudos, n = 615 pacientes) não apresentaram diferenças estatisticamente significantes entre o uso da anestesia inalatória quando comparada ao da intravenosa, houve uma maior redução de risco no grupo inalatório sobre o uso de inotrópicos nos estudos patrocinados por indústrias farmacêuticas (29%) quando comparado aos estudos não patrocinados pela indústria (23%). Conclusões: Essa pesquisa demonstrou uma tendenciosidade na estimativa da magnitude dos efeitos em ensaios clínicos patrocinados por indústrias farmacêuticas quando comparados aos estudos não patrocinados por indústrias farmacêuticas sobre a eficácia e segurança da anestesia inalatória versus a intravenosa em relação à mortalidade e ao uso de inotrópicos em pacientes submetidos à cirurgia de revascularização do miocárdio com ou sem circulação extracorpórea. Entretanto, nós não podemos descartar a possibilidade de viés devido ao número discrepante de pacientes nos estudos incluídos entre os dois grupos avaliados (i.e. estudos patrocinados por indústrias farmacêuticas e estudos não patrocinados pela indústria), além dos achados dessa revisão serem limitados à apenas uma questão clínica na área da Anestesiologia; outras áreas da saúde devem ser exploradas.Introduction: Scientific research have always been essential to the development of medicine in the course of human history, considering its researcher and innovative character for various treatments of existing disease. Along with the growth of world scientific production are pharmaceutical companies that currently make up to major financial players, particularly clinical trials in health. However, it is necessary to carefully examine the conduct of these studies because often the capitalist economic and interest overrides the ethical and may have biased results and favorable to certain medication manufactured by sponsoring industry. In this sense, throughout world history, some sponsored studies were questioned about the methods used by the researchers, and especially about the results, detecting an impressive amount of fraudulent studies published in medical journals of high impact. Objective: To evaluate whether there is an impact of the pharmaceutical industry on the results of randomized clinical trials comparing the efficacy and safety of the use of inhalational anesthesia versus intravenous anesthesia in cardiopulmonary bypass and intracorporeal circulation. Methods: Quantitative systematic search of randomized controlled trials (RCTs) evaluating the efficacy and safety of intravenous anesthesia (eg propofol, fentanyl) when compared to inhalation anesthesia (eg sevoflurane, isoflurane, desflurane) in patients undergoing coronary artery bypass grafting or without cardiopulmonary bypass. The studies were obtained from the following electronic databases: PubMed, EMBASE, LILACS and CENTRAL. The date of last search was March 02, 2016. Two reviewers independently selected studies, extracted data from them and assessed the risk of bias of the included studies. Moreover, studies were divided into two groups: studies sponsored by pharmaceutical industries and industry-sponsored trials do not. Results: 12 studies with a total of 1,845 participants were included, six studies (n = 1,209 patients) sponsored by pharmaceutical companies and six studies (n = 640 patients) not sponsored by industries. Regarding the assessment of risk of bias, most of the studies were divided evenly between low risk of bias and uncertain risk of bias, whether sponsored or not by pharmaceutical companies. Meta-analysis of four RCTs suggests a reduction in mortality risk with the use of inhalational anesthesia when compared to intravenous anesthetic in the group of the studies sponsored by pharmaceutical industries [relative risk (RR) 00:45, 95% confidence interval (CI) 0.89 to 0:23; I2 = 2%; P = 0.02; four studies, n = 927 patients]. However, the metaanalysis considering the studies not sponsored by industry, there was no statistically significant difference between the use of inhalational anesthesia when compared to intravenous anesthesia on the risk of mortality (0:21 RR, 95% CI 0.01 to 4.24; I2 = not applied; p = 0:31; three studies, n = 493 patients). Results were similar regarding the use of inotropic agents, that is, although both meta-analyzes considering the studies sponsored by pharmaceutical companies (RR 0.71, 95% CI 0.48 to 1.06; I2 = 27%, p = 0.09; three studies, n = 382 patients) and studies not sponsored by industry (RR 0.77, 95% CI 0:47 to 1:26; I2 = 74%, p = 0:30; six studies, n = 615 patients) showed no statistically significant differences between the use of inhalational anesthesia when compared to intravenous, there was a greater risk reduction in inhalational group on the use of inotropic agents in studies sponsored by pharmaceutical companies (29%) when compared to studies not sponsored by industry (23%). Conclusions: This study demonstrated a bias in the estimate of the magnitude of the effects in clinical trials sponsored by pharmaceutical companies when compared to studies not sponsored by pharmaceutical companies on the efficacy and safety of inhaled anesthesia versus intravenous regarding mortality and the use of inotropic agents in patients undergoing coronary artery bypass grafting with or without cardiopulmonary bypass. Ultrasound guided nerve blockade in children : a meta-analysis of randonized clinical trials
Background: it has been suggested that ultrasound-guided regional anesthesia could improve the blockade efficacy and decrease complication rates in the pediatric population, but this hypothesis has not previously been adequately tested in a quantitative manner. Aim: the purpose of this systematic review of randomized clinical trials (RCT) is to assess whether the use of ultrasonography in the pediatric population has advantages over any other method of nerve location, such as the anatomical landmark-based technique, paraesthesia or use of an electrical nerve stimulator. Methods: the systematic search, data extraction, critical appraisal and pooled analysis were performed according to the Cochrane Handbook and PRISMA guidelines. The search strategy included the CENTRAL of the Cochrane Library, Medline, EMBASE, and Lilacs. The date of the last search was November 5, 2014. The relative risk (RR), mean difference (MD) and their corresponding 95% confidence intervals (95% CIs) were calculated using the Revman statistical software for dichotomous and continuous outcomes, respectively. Results: eleven randomized, clinical, controlled trials (published between 2004 and 2013) with a total 634 patients met the inclusion criteria. The methodological quality of the included studies was considered regular. There was a slight benefit of ultrasound to the success rate (RR 1.19, 95% CI 1.07 to 1.33), which was variable according to the anatomical region studied and a more important reduction of the analgesia requirement in the postoperative period with the use of ultrasound for peripheral nerve blocks (RR 0.2, 95% CI 0.11-0.34). There was only one complication reported (blood vessel puncture in one control group). Métodos: a pesquisa sistemática, extração dos dados, avaliação crítica e análise conjunta dos dados seguiram as recomendações para a realização de revisões sistemáticas propostas pela Colaboração Cochrane e pela declaração PRISMA. Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
