Assessment and clinical implications of functional vitamin B6 deficiency
Vitaminen zijn essentieel voor een gezond en lang leven. Tegenwoordig worden vitaminesupplementen dan ook veelvuldig gebruikt om het algemeen welbevingen te verbeteren en aandoeningen te verlichten of te voorkomen. Echter, bij veel groepen mensen, zoals patiënten met fibromyalgie en het chronisch vermoeidheidssyndroom, ontbreekt de wetenschappelijke basis hiervoor met als gevolg dat men mogelijk onnodig risico loopt op vitaminetoxiciteit, zoals onlangs met vitamine B6 is gebleken bij Sven Kramer. In dit proefschrift laten wij zien dat het tot nu toe uitgevoerde onderzoek naar een mogelijk verband tussen vitaminen en de ziekteactiviteit van fibromyalgie en het chronisch vermoeidheidssyndroom laag van kwaliteit is en dat er op basis van dit onderzoek geen duidelijk verband tussen vitaminen en ziekteactiviteit aanwijsbaar is. Hiermee hebben wij laten zien dat er vooralsnog geen goede wetenschappelijke basis bestaat voor het gebruik van vitaminesupplementen ter verlichting van de symptomen van fibromyalgie en het chronisch vermoeidheidssyndroom. Verder hebben wij aangetoond dat een vitamine B6 tekort relatief vaak voorkomt bij niertransplantatiepatiënten en dat dit tekort bepaalde intracellulaire processen op moleculair niveau lijkt te verstoren. Bovendien wijzen onze data erop dat vitamine B6 deficiënte niertransplantatiepatiënten en vrouwen uit de algemene bevolking een hogere kans hebben op voortijdige sterfte ten gevolge van cardiovasculaire aandoeningen, vergeleken met personen met een adequate vitamine B6 status. Wij concluderen dan ook dat niertransplantatiepatiënten en vrouwen mogelijk baat zouden kunnen hebben bij vitamine B6 supplementen. Voordat dit in de richtlijnen opgenomen kan worden, dienen interventiestudies met vitamine B6 supplementen echter eerst sluitend bewijs te leveren.Vitamins are essential for a healthy and long life. Furthermore, we show that deficiency of vitamin B6, a vitamin that is present in virtually all standard multivitamin pills, occurs frequently in stable outpatient kidney transplant recipients, despite adequate vitamin B6 intake through diet. Importantly, this deficient state confers increased risk of long-term mortality due to cardiovascular causes, not only in kidney transplant recipients but also in women from the general population. We therefore conclude that kidney transplant recipients and women may possibly benefit from vitamin B6 supplementation. However, before concrete recommendations can be made, intervention studies involving vitamin B6 supplementation should provide conclusive evidence

The fate of sulfate in chronic heart failure
New leads to advance our understanding of heart failure (HF) pathophysiology are urgently needed. Previous studies have linked urinary sulfate excretion to a favourable cardiovascular risk profile. Sulfate is not only the end-product of hydrogen sulfide metabolism, but is also directly involved in various (patho)physiological processes, provoking scientific interest in its renal handling. This study investigates sulfate clearance in chronic HF (CHF) patients and healthy individuals and considers its relationship with disease outcome. Parameters related to renal sulfate handling were determined in and compared between 96 previously characterized CHF patients and sex-matched healthy individuals. Among patients, sulfate clearance was analysed for associations with clinical and outcome parameters. In CHF patients, plasma sulfate concentrations are significantly higher, whereas 24-h urinary excretion, fractional excretion and clearance of sulfate are significantly lower, compared to healthy individuals. Among patients, sulfate clearance is independently associated with diuretics use, creatinine clearance and 24-h urinary sodium excretion. Sulfate clearance is associated with favourable disease outcome (HR per SD increase 0.38 (95% CI 0.23-0.63), P<0.001). Although significance was lost after adjustment for creatinine clearance, the decrease of sulfate clearance in patients is independent of this parameter, indicating that sulfate clearance is not merely a reflection of renal function. This exploratory study reveals aberrant sulfate clearance as a potential contributor to CHF pathophysiology, with reduced levels in patients and a positive association with favourable disease outcome. Further research is needed to unravel the nature of its involvement and to determine its potential as a biomarker and target for therapy
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
