Primary pulmonary blastoma of monophasic variety- diagnosis and management
Pulmonary blastoma is a rare primary lung neoplasm, in that monophasic variety is far too rare. There are no specific clinical features seen for pulmonary blastoma; computed tomography and histopathology are diagnostic. Surgical excision is the treatment of choice; however, adjuvant chemotherapy and radiotherapy may be required in large and aggressive tumors
Developing competency in post-graduate students of anaesthesiology for taking informed consent for elective caesarean section
Background and Aims: Post-graduate medical students (residents) generally lack effective communication skills required to obtain informed consent. The aim of this study was to assess role play and group discussion as teaching/learning tools for improving residents' knowledge on informed consent and competency in communicating while taking informed consent. Methods: This prospective, observational study was conducted on 30 anaesthesia residents. They were first observed while obtaining informed consent and their basic knowledge regarding communication skills was checked with a pre-test questionnaire. Then, lecture and group discussion were carried out to increase the knowledge base, and their knowledge gain was checked by the same questionnaire as a post-test. Communication skills were demonstrated by role play and residents were assessed by direct unobtrusive observation using a checklist. Feedback regarding effectiveness of programme was taken from students. Statistical analyses were done using Microsoft Office Excel and SPSS software. Results: Percentage gain was 122.37% for knowledge domain. For communication skills, mean ± standard deviation for checklist was 8.93 ± 1.43 before role play and it improved to 17.96 ± 1.29 after role play. Regarding effectiveness of role play as a teaching/learning tool, 76.66% of residents said they strongly agreed and 23.33% of residents said they agreed. Likert scale for evaluation of programme was graded 4 or 5 by all residents. Conclusion: The knowledge and communication skills required for obtaining informed consent was improved significantly after role playing
Longitudinal Analysis of Renal Function using ZIP GEE on OLT Transplant Patients Undergoing NAC Prophylaxis
Organ dysfunction is associated with oxidative stress following Orthotropic Liver Transplant (OLT) surgery. N-Acetyl Cysteine (NAC) is an acetylated form of the amino acid cysteine. NAC is known to replenish glutathione in the bloodstream which helps relieve cell damage caused by oxidative stress. NAC was used in a placebo controlled study to discover its effects on organ dysfunction caused by oxidative stress following OLT surgery. A standard NAC treatment, as used to treat acetaminophen toxicity, was used as a treatment during surgery. Measures of hepatic and renal dysfunction were recorded at unequally spaced time-points for a follow-up time of one year. The Generalized Estimating Equation (GEE) approach was used to model continuous hepatic responses. Discrete renal dysfunction responses are shown to follow a unique distribution. This unique distribution was accommodated by the GEE procedure proposed by Liang and Zeger to produce consistent and efficient estimates of the treatment effect of NAC. The estimates produced contradictory results for a hypothesized protective effect of NAC against hepatic dysfunction. The public health relevance of this work is that NAC treatment, if shown to be efficacious with respect to renal function, can benefit over six thousand OLT patients each year

This thesis is performed as the finalization of the master degree of the master Building Technology – track Façade Design – at the faculty of Architecture. The topic is about the NEXT Active façade and its detailed application. This final report is the result of the master thesis. The graduation project started in Nov 2011 and included a broad literature study on the background of decentralized systems. The concept of decentralizedclimate system is comparatively new but it is widely becoming popular especially in Western Europe. This report unfolds the concept of NEXT Active facade with respect to the application of the system in aa given condition. In the era of globalization and the world economy, corporations and governments are focusing more on the concept of sustainability and far-going security to the stakeholders and investors. Insider trading is a deep-rooted evil opportunity in the stock market. The transfer of unpublished price-sensitive information to an outsider or the use of it for unfair gains by the insider is a governance issue per se. The current Indian regulation i.e. SEBI (Insider Trading) Regulation 2015 has traveled a long journey to curb the root cause but still must move on the international parameters. It has a major objective to prevent the unauthorized use of price-sensitive information. The review paper will critically analyze the historical development in the Indian Capital market with reference to the incidents of insider trading and preventive steps by the Indian market regulations and regulator activism. It will also review the regulatory advancement of the insider trading norms after the Harshad Mehta scandal in the year 1992 to the happening of ICICI-Videocon loan scam and thereafter. The objective of the review paper is a structural analysis of the historical development in the regulatory framework of insider trading in India. Also, to demonstrate the legal transformation of Indian Stock Exchanges in response to insider trading issues. A transferência de informações não publicadas sensíveis ao preço para alguém de fora ou a sua utilização para ganhos injustos por parte de quem está dentro é uma questão de governação em si. A atual regulamentação indiana, ou seja, o Regulamento SEBI (Insider Trading) de 2015, percorreu um longo caminho para conter a causa raiz, mas ainda deve avançar nos parâmetros internacionais. Tem como objetivo principal impedir a utilização não autorizada de informações sensíveis em matéria de preços. O artigo de revisão analisará criticamente o desenvolvimento histórico no mercado de capitais indiano com referência aos incidentes de abuso de informação privilegiada e às medidas preventivas tomadas pelas regulamentações do mercado indiano e pelo ativismo regulador. Ele também revisará o avanço regulatório das normas de uso de informações privilegiadas após o escândalo Harshad Mehta no ano de 1992 até o acontecimento do golpe de empréstimo ICICI-Videocon e depois disso. O objetivo do artigo de revisão é uma análise estrutural do desenvolvimento histórico no quadro regulamentar do abuso de informação privilegiada na Índia. Navigating challenges for supply chain transparency in the digital enterprises
Purpose:
In the age of digital businesses, the rise of innovative digital transformation has made supply chain transparency (SCT) an essential research topic. While digital technologies provide possibilities for enhancing transparency in supply chains, there is limited research on how digital businesses overcome different SCT issues or provide real-time visibility and traceability. This research investigates the different challenges of transparency in supply chains for digital businesses in the age of digital transformation.
Design/methodology/approach:
Challenges are identified through a comprehensive literature review and finalized through the fuzzy-Delphi method (FDM) after industry experts’ validation. Data is gathered, ranked and prioritized through the analytic hierarchy process (AHP) entropy method.
Findings:
The study reveals that challenges related to data privacy and security, supplier resistance, and lack of data standardization grouped under Cluster 1 (Critical Digital Friction Points) pose the most significant barriers to supply chain transparency in digital enterprises. These are followed by Cluster 2 (Operational Capability Gaps) and Cluster 3 (External and Strategic Constraints), highlighting a clear prioritization framework for addressing SCT issues in a structured, phased manner.
Implications: By leveraging advanced digital technologies to gather, share, and analyze supply chain data, digital enterprises can overcome these challenges, enhance operational effectiveness, and gain stakeholders' trust. These findings can be utilized by policymakers to develop guidelines that enhance transparency while maintaining data security. Practitioners can develop targeted strategies to address supplier resistance and infrastructure deficiencies.
Originality:
This study makes three distinct contributions. Firstly, it fills a gap in current literature by initiating a clear debate on supply chain transparency beyond generic or sustainability-focused models, illuminating its unique dimensions in digital enterprises. Secondly, it demonstrates the practical importance of SCT by identifying and prioritizing the key challenges that are faced by digitally transforming companies. Thirdly, it advances theory by contrasting traditional perspectives with new insights drawn from our findings, offering both actionable guidance and novel conceptual frameworks for transparency in digitally enabled supply chains
Surgical consideration in posterior C1-2 instrumentation in case of vertebral artery anomaly
Anatomical variations in the course of the vertebral artery have been previously described in the literature. Generally, these predictable patterns of variations commonly observed in lower cervical vertebral artery anatomy and less commonly described for upper cervical vertebral artery anatomy. Due to presence of these variations, treatment options for upper cervical spine pathology may be influenced and sometimes prevent commonly performed stabilization procedures. Herein author presented a case of vertebral artery anatomic variation at the craniovertebral junction and management option for such variations
Iatrogenic lumbar Pseudomeningocele: A case report and review of literature
Lumbar pseudomeningocele is an uncommon complication of lumbar spine surgeries. This occurred due to extradural encapsulated cerebrospinal fluid collection after accidental durotomy or un-repairable dural tear. On review of literature only 58 cases have been reported so far. Magnetic resonance imaging is the investigation of choice for diagnosis of pseudomeningocele. Herein author has described a case of iatrogenic pseudomeningocele after lumbar spine surgery and its management
