Aus der medizinischen Perspektive ist die Therapie einfach durchzuführen, so dass durch regelmäßige Tabletteneinnahme schwerwiegendere Folgeerkrankungen wie Schlaganfall oder Herzinfarkt verhindert werden können. Die patientenzentrierte Aushandlung einer angemessenen Therapie im Rahmen des Shared Decision-Making wird allgemein als geeignet angesehen, den therapeutischen Erfolg langfristig zu gewährleisten. Bisher ist jedoch nur wenig darüber bekannt, welche Relevanz die Arzt-Patient-Beziehung für die Patient/innen hat und ob diese tatsächlich in der Lage ist, die Umsetzung der therapeutischen Maßnahmen (Adhärenz) zu beeinflussen. In dieser Arbeit wird daher der Frage nachgegangen, welche Bedeutung die Konfrontation mit der Diagnose Bluthochdruck sowie die nachfolgende medikamentöse Therapie für das (Er)Leben der Patient/innen und die Anforderungen an die Arzt-Patient-Beziehung haben

Results: Of 391 students, 56% stated that they felt insufficiently appreciated, 51% had experienced rivalry, and 34% had suffered verbal abuse.     BackgroundCaring for patients with advanced lung cancer is of high relevance in different clinical settings. Lung cancer is among the most common causes of death from malignant neoplasms worldwide; with increasing prevalence and mortality.AimTo get a better understanding of individual patients’ needs, exploring the experiences and meaning of living with advanced lung cancer at the end of life, and to develop strategies for improving patient-centred care in Germany.DesignQualitative explorative interview study with patients, using grounded theory.Setting/ParticipantsA sample of 17 adults living with advanced lung cancer in Lower Saxony/Germany was recruited in two university hospitals. Patients were asked to tell of their experiences of living with advanced lung cancer. The emphasis of this study was the period of palliative tumour therapy.ResultsThe main phenomenon of living with advanced lung cancer is the feeling of having to redefine one’s own existence, such as social roles within and outside the family. The diagnosis trigger powerlessness, which can lead to information passivity, followed by acceptance of aggressive tumour treatment. Patients perceive a high degree of psychological and social stress, without being able to express this. There is a lack of regular appropriate psychosocial care accompanying chemotherapy. Patients ascribe their physical suffering to the side effects of tumour treatment, which may trigger a desire to die. Finally, patients tend to hide their individual needs, even when asked.ConclusionsRegarding the patients’ needs, greater emphasis must be placed on psychosocial care as part of the biopsychosocial model to adequately consider the patients’ concerns. Assessments can be helpful to enhance communication at an early stage across all professions into the multi-professional therapy.</div

    Physicians’ and nurses’ expectations and objections toward a clinical ethics committee

    The study aimed to explore the subjective need of healthcare professionals for ethics consultation, their experience with ethical conflicts, and expectations and objections toward a Clinical Ethics Committee. Staff at a university hospital took part in a survey (January to June 2010) using a questionnaire with open and closed questions. Descriptive data for physicians and nurses (response rate = 13.5%, n = 101) are presented. Physicians and nurses reported similar high frequencies of ethical conflicts but rated the relevance of ethical issues differently. Nurses stated ethical issues as less important to physicians than to themselves. Ethical conflicts were mostly discussed with staff from one's own profession. Respondents predominantly expected the Clinical Ethics Committee to provide competent support. Mostly, nurses feared it might have no influence on clinical practice. Findings suggest that experiences of ethical conflicts might reflect interprofessional communication patterns. Expectations and objections against Clinical Ethics Committees were multifaceted, and should be overcome by providing sufficient information. The Clinical Ethics Committee needs to take different perspectives of professions into account.University of Gottinge
