    Pazienti Lungosopravviventi al cancro in età pediatrica: dall'Inflamm-Aging alla Ferroptosi

    Il miglioramento dei protocolli terapeutici oncologici ha significativamente aumentato la sopravvivenza nei pazienti pediatrici affetti da tumori, ma ha anche esposto i long-term cancer survivors (CCS) a un aumentato rischio di comorbidità precoci legate all’invecchiamento, tra cui osteoporosi, disfunzioni metaboliche e infiammazione cronica sistemica (inflamm-aging). L’obiettivo di questo lavoro è stato esplorare alcuni meccanismi cellulari e molecolari alla base di tali condizioni, con particolare attenzione al metabolismo del ferro e all’attività immunitaria nei CCS. Nel primo studio sperimentale, è stata valutata l’attività degli osteoclasti derivati da CCS, evidenziando un incremento significativo dell’attività riassorbitiva e dell'espressione della fosfatasi acida resistente al tartrato (TRAP), in associazione ad alterazioni del metabolismo del ferro e sovraregolazione dei recettori CB2 e TRPV1. Il trattamento con un chelante del ferro (deferasirox) e la modulazione farmacologica dei recettori CB2/TRPV1 hanno ridotto l’iperattività osteoclastica, suggerendo nuove possibilità terapeutiche nel controllo della perdita ossea secondaria ai trattamenti oncologici. Nel secondo studio, sono stati analizzati i macrofagi derivati da CCS, osservando uno shift pro-infiammatorio (M1-like), alterazioni nel metabolismo del ferro e aumentata secrezione di citochine infiammatorie. L’esposizione a nutraceutici quali resveratrolo, curcumina e licopene ha indotto un parziale ripristino del fenotipo M2, con riduzione dell’infiammazione e normalizzazione dei parametri ferro-correlati, suggerendo il ferro come potenziale bersaglio immuno-metabolico. Infine, sono state esplorate le basi molecolari della ferroptosi, una forma non convenzionale di morte cellulare ferro-dipendente, in relazione alla condizione infiammatoria persistente nei CCS. I risultati preliminari supportano l’ipotesi che la ferroptosi contribuisca sia al danno tissutale sia alla modulazione immunitaria, aprendo nuove prospettive terapeutiche per il controllo dell’infiammazione cronica e delle sequele tardive nei long-term survivors pediatrici

    Emotional facial palsy: an unusual and rarely explored neurological sign

    The anatomy of the cortico-bulbar tract that drives voluntary movements of mimic muscles is well described. Some cases of facial palsy with inverse automatic-voluntary dissociation (emotional facial palsy; EFP) are reported in the literature. These cases suggested a completely independent path of the fibers whose lesion results in EFP. We aimed to review the clinical reports of EFP available in the literature to characterize the anatomical aspect of the fibers whose lesion results in the isolated impairment of spontaneous smiling. Cortico-pontine fibers that control spontaneous smiling arise from the medial surface of the prefrontal cortex and descend through the anterior limb of the internal capsule, thalamus, and brain steam, independently from those that control voluntary movement. The mesial temporal lobe, particularly the amygdala, plays a crucial role in the network driving emotionally evoked facial expressions. We would highlight the relevance of an unusual and rarely explored neurological sign that could be added to clinical examination in ruling out focal brain pathology, such as stroke, tumors, or multiple sclerosis

    Online Bruxism‐related information: Can people understand what they read? A Cross‐Sectional Study

    Background: Bruxism patients often access the web to search for information. Unfortunately, the low text readability of online material and the limited medical literacy in the general population may prevent patients' understanding of health information. Objective(s): We aimed to assess the home page's readability of the top 10 patients-oriented bruxism-related websites and the educational level required to approach them. Methods: Using the word bruxism in the “no country redirect” extension of Google Chrome browser (www.google.com/ncr), we identified the first 10 patient-oriented English language websites. The readability of the material was determined using six commonly recommended readability tests (1): Gunning Fog Index (GFI), Coleman Liau Index (CLI), Automated Readability Index (ARI) Simple Measure of Gobbledygook (SMOG), Flesch Kincald Grade Level (FKGL), Flesh Reasing Ease (FRE). Results: The USA National Institutes of Health requirements, which call for websites to be readable at a 6th to 7th-grade level, were not followed by any of the most popular websites. Conclusion: The average consumer frequently finds the health information on the Internet to be too complex to understand, which can lead to misinterpretation, a delay in diagnosis, and poorer health consequences

    The Role of Telemedicine in Oral and Head Cancer Management

    Background: Telemedicine has recently emerged as a powerful tool for medical care from a distance. This new technology has the potential to revolutionise how healthcare is delivered and has been used for various medical specialities, including orthodontics. The purpose of this is to evaluate the ‘use and aid of telemedicine in the diagnosis of oral cancer. Methods: PubMed, Web of Science and Lilacs were systematically searched until 31/01/2023. In addition, a manual search was performed using the bibliography of selected articles and a Google Scholar search. It was completed that the articles were read to assess their eligibility. Results: 240 papers were selected. Specifically, 146 articles were found in PubMed, four from Lilacs, and 90 from Web of Science. Randomised clinical trials (RCTs) and clinical trials were selected as inclusion criteria. Therefore, three articles that met the inclusion criteria were selected; All of the selected studies had and showed the possible influence and ease with which mouth neoplasms are diagnosed. Conclusions: Telemedicine has greatly facilitated the diagnosis of oral cavity malignancies and is especially useful in suburban areas where the diagnosis is always very late. Telemedicine has the potential to revolutionize the diagnosis, treatment, and monitoring of oral cancer. It could provide remote access to healthcare services, allowing individuals in remote areas to access the care they need. The potential of telemedicine in oral cancer diagnosis and management is significant, and further research is required to understand its potential fully

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    EFFECTIVENESS OF DIGITAL OCCLUSAL SPLINTS IN MANAGING TEMPOROMANDIBULAR JOINT DISORDERS: A SYSTEMATIC REVIEW AND META ANALYSIS

    Background: This systematic review aims to assess the effectiveness of 3D occlusal splints (OS) for managing pain and improving maximum mouth opening in temporomandibular joint disorders (TMDs) via randomized controlled trials (RCTs). Materials and Methods: Searches were conducted in PUBMED, SCOPUS, and the COCHRANE library. Eligible studies were evaluated for risk of bias (RoB) by two reviewers based on predetermined criteria: generation of random sequences, implementation of single-operator protocol, inclusion of a control group, blinding of testing machine operator, uniformity in sample preparation, evaluation of failure modes, adherence to manufacturer's instructions for materials, and explanation of sample size calculation. If the author provided information about the examined parameter, the study was categorized as having a "YES." Conversely, if the information was absent, the parameter was marked as "NO." The RoB for each study was then determined according to the total number of "YES" responses: 1 to 3 indicated a high RoB, 4-6 signified a medium risk, and 7 to 8 indicated low RoB. Out of 14 articles, 5 RCTs underwent qualitative analysis. Results: Results indicate that OS therapy effectively manages TMD-related pain and reduces TMJ clicking. Additionally, it improves mouth opening in patients with initial limitations. Better symptom management correlates with improved quality of life in TMD sufferers. Conclusions: Further comparative clinical trials are necessary due to remaining uncertainties regarding how specific OS design and fabrication methods may impact TMD outcomes

    Prevalence of temporomandibular disorders in clear aligner patients using orthodontic intermaxillary elastics assessed with diagnostic criteria for temporomandibular disorders (DC/TMD) axis II evaluation: A cross-sectional study

    Background: The objective of this investigation is to assess the relationship between the utilisation of orthodontic intermaxillary elastics and temporomandibular disorder (TMD) symptoms in clear aligner patients and to examine the correlation between the elastic usage time with the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD)-Axis II Evaluation Forms.Methods: This study was carried out on a total of 40 clear aligner patients using intermaxillary elastics in the experimental group and 30 clear aligner patients who did not use any intermaxillary elastics in the control group. The data were evaluated using the Mann-Whitney U, chi-square, Fisher's exact chi-square, and Fisher Freeman Halton exact chi-square tests.Results: The characteristic pain intensity, mastication, mobility, communication, global and PHQ-9 scores of the experimental group were significantly higher than those of the control group (p < .05). The characteristic pain intensity score, interference score and chronic pain grade score of patients using Class III elastics were statistically significantly higher than those of patients using Class II elastics (p < .05). Patients who used elastics for less than 6 months had statistically significantly higher PHQ-9 scores than those who used elastics for more than 6 months (p < .05).Conclusions: Orthodontic treatment may affect occlusion, bite force and jaw movement, which may cause or worsen TMD symptoms, and the DC/TMD questionnaires can determine if orthodontic patients acquire TMD by assessing their psychosocial state and pain-related problems
