Hypomineralization: aspects related to dental hypersensitivity and anxiety and effectiveness of pre-emptive analgesia through randomized clinical trial
A HMI é uma condição difícil de gerenciar em odontopediatria, pois está associada a vários desafios clínicos, como hipersensibilidade dentária, dificuldade em obter analgesia/anestesia adequada, aumento da ansiedade odontológica. Nenhum estudo até o momento avaliou a ansiedade/medo do tratamento odontológico em associação com a gravidade da HMI e hipersensibilidade dentária no mesmo grupo de pacientes. O primeiro objetivo do estudo foi investigar se a hipersensibilidade dentária e a ansiedade/medo estavam associadas à presença e características da HMI. Este estudo observacional transversal recrutou 1.830 alunos de 6 a 12 anos de quatro escolas selecionadas aleatoriamente na rede municipal de ensino de Ribeirão Preto/SP. Para avaliar os padrões clínicos, utilizou-se o protocolo de Ghanim et al., 2015 e 2017; já para a coleta de informações sobre a hipersensibilidade dentária associada aos dentes acometidos pela HMI, as crianças foram questionadas por um dos examinadores durante o exame clínico sobre o uso da escala facial de Wong-Baker e da escala VAS. A ansiedade e o medo do tratamento odontológico foram avaliados por meio do questionário \"Children\'s Fear Survey Schedule-Dental Subscale\" traduzido e validado para o português. A concordância inter e intra examinadores para a presença de HMI foi calculada pelo teste Kappa ponderado, e as análises estatísticas realizadas foram ANOVA, teste post hoc de Tukey e teste do qui-quadrado, sendo o nível de significância estatística estabelecido em 5%. A média de medo/ansiedade foi verificada em relação ao sexo e idade (<9 e ≥9 anos). O sexo feminino foi o mais prevalente (52,1%) e os pacientes com idade <9 anos também tiveram maior representação em termos de medo/ansiedade (49,35). Já o segundo objetivo do estudo foi avaliar, através de ensaio clínico randomizado controlado por placebo, triplo-cego, a eficácia da analgesia préemptiva e o aumento da anestesia infiltrativa em crianças com hipersensibilidade associada a HMI e fratura pós-eruptiva do esmalte (FPE), utilizando Ibuprofeno (10 mg/kg de peso). O analgésico (identificado por A - Ibuprofeno 100 mg/ml suspensão oral) ou placebo (identificado por B - formulação sem efeito farmacológico desenvolvida pelo Laboratório de P&D Farmacêutico da FCFRP/USP especificamente para este estudo), com as mesmas características e sabor do Ibuprofeno. O clínico que realizou o tratamento restaurador, o participante e seu responsável não sabiam qual suspensão foi administrada ou quem foi o profissional que analisou os dados. Após o tempo estimado de ação de 30 minutos, o tratamento restaurador foi iniciado. Anestesia infiltrativa padrão com um cartucho (1,8 ml) de Mepivacaína a 2% com Epinefrina 1: 100.000 foi administrada para cada dente usando uma seringa aspirativa e uma agulha de calibre 25 mm. O tratamento restaurador foi realizada, sendo removido apenas o esmalte sem suporte, e o tecido afetado pelo defeito. A hipersensibilidade foi medida por meio do teste de jato de ar (10 segundos). Os escores foram determinados pela escala de Wong e Baker, 1988. As análises estatísticas incluíram os testes Qui-quadrado, Fisher e T, e nível de significância foi de 5%. A hipersensibilidade, fator enfatizado neste estudo para pacientes com HMI, foi verificada após 30 minutos da administração do medicamento ou placebo para cada grupo (T1), resultando em uma diferença estatisticamente expressiva entre os grupos, o que de fato enfatiza a eficácia da administração do medicamento (Grupo A) (p = 0,0001). A administração de analgesia pré-emptiva com Ibuprofeno aumenta com sucesso a eficácia anestésica e diminui a hipersensibilidade transoperatória associada ao tratamento restaurador de molares permanentes afetados por HMI com FPE.HMI is a difficult condition to manage in pediatric dentistry as it is associated with several clinical challenges such as dental hypersensitivity, difficulty in obtaining adequate analgesia/anesthesia, increased dental anxiety. No study to date has evaluated anxiety/fear of dental treatment in association with MIH severity and dental hypersensitivity in the same group of patients. The first objective of the study was to investigate whether dental hypersensitivity and anxiety/fear were associated with the presence and characteristics of MIH. This observational cross-sectional study recruited 1,830 students aged 6 to 12 years from four randomly selected schools in the municipal education network of Ribeirão Preto/SP. To assess clinical patterns, the protocol by Ghanim et al., 2015 and 2017 was used; in order to collect information on dental hypersensitivity associated with teeth affected by HMI, the children were asked by one of the examiners during the clinical examination about the use of the Wong-Baker facial scale and the VAS scale. Anxiety and fear of dental treatment were assessed using the \"Children\'s Fear Survey Schedule-Dental Subscale\" questionnaire translated and validated into Portuguese. Inter- and intra-examiner agreement for the presence of HMI was calculated using the weighted Kappa test, and the statistical analyzes performed were ANOVA, Tukey\'s post hoc test and the chi-square test, with the level of statistical significance set at 5%. Mean fear/anxiety was verified in relation to sex and age (<9 and ≥9 years). Females were the most prevalent (52.1%) and patients aged <9 years were also more represented in terms of fear/anxiety (49.35). The second objective of the study was to evaluate, through a randomized, placebo-controlled, triple-blind clinical trial, the efficacy of preemptive analgesia and increased infiltrative anesthesia in children with hypersensitivity associated with HMI and post-eruptive enamel fracture ( FPE), using Ibuprofen (10 mg/kg body weight). The analgesic (identified by A - Ibuprofen 100 mg/ml oral suspension) or placebo (identified by B - formulation without pharmacological effect developed by the Pharmaceutical R&D Laboratory of FCFRP/USP specifically for this study), with the same characteristics and taste as Ibuprofen . The clinician who performed the restorative treatment, the participant and his/her guardian did not know which suspension was administered or who the professional who analyzed the data was. After the estimated time of action of 30 minutes, the restorative treatment was started. Standard infiltrative anesthesia with a cartridge (1.8 ml) of 2% Mepivacaine with 1:100,000 Epinephrine was administered to each tooth using an aspirating syringe and a 25 mm needle. Restorative treatment was performed, removing only the unsupported enamel and the tissue affected by the defect. Hypersensitivity was measured using the air blast test (10 seconds). The scores were determined by the scale of Wong and Baker, 1988. Statistical analyzes included the chi-square, Fisher and T tests, and the significance level was 5%. Hypersensitivity, a factor emphasized in this study for patients with HMI, was verified 30 minutes after the administration of the drug or placebo for each group (T1), resulting in a statistically significant difference between the groups, which in fact emphasizes the effectiveness of the administration of the drug. drug (Group A) (p = 0.0001). Orthopedic treatment of sleep disorders: clinical case report
O presente trabalho aborda os distúrbios respiratórios do sono em crianças e adolescentes, focando na eficácia do uso de um aparelho disjuntor tipo Hyrax e de um micro sensor eletrônico para monitorar o tempo de uso do aparelho ortopédico Bionator de Balters. A pesquisa foi realizada com um paciente diagnosticado com má oclusão Classe II e Síndrome de Apneia Obstrutiva do Sono (SAOS) de grau moderado. Os resultados mostraram que a implementação do sensor de monitoramento foi eficaz, refletindo uma melhora significativa na cooperação do paciente ao longo do tratamento. Além disso, observou-se uma redução considerável no índice de apneia/hipopneia após a Expansão Rápida da Maxila (ERM) e o uso contínuo do aparelho por 12 meses. Esses resultados ressaltam a importância de intervenções precoces e adequadas, não apenas para melhorar os sintomas, mas também para promover uma melhor qualidade de vida para as crianças afetadas. O estudo sublinha a relevância do diagnóstico precoce e do tratamento contínuo, sugerindo que a combinação de ERM e aparelhos ortopédicos pode trazer benefícios significativos, contribuindo para o desenvolvimento saudável e o bem-estar emocional dos pacientes.This study addresses sleep-disordered breathing in children and adolescents, focusing on the effectiveness of using a Hyrax-type breaker device and an electronic microsensor to monitor the wearing time of the Balters Bionator orthopedic appliance. The research was carried out with a patient diagnosed with Class II malocclusion and moderate Obstructive Sleep Apnea Syndrome (OSAS). The results showed that the
implementation of the monitoring sensor was effective, reflecting a significant improvement in the patient's cooperation throughout the treatment. In addition, a considerable reduction in the apnea/hypopnea index was observed after Rapid Jaw Expansion (RME) and continuous use of the device for 12 months. These results highlight the importance of early and appropriate interventions, not only to improve symptoms, but also to promote a better quality of life for affected children. The study underlines the relevance of early diagnosis and continuous treatment, suggesting that the combination of RME and orthopedic appliances can bring
significant benefits, contributing to the healthy development and emotional well-being of patients
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Physicochemical properties and cytotoxicity of root canal filling materials of permanent teeth
O presente estudo teve como objetivo avaliar as propriedades físico-químicas e citotoxicidade quatro cimentos endodônticos, com diferentes bases químicas, utilizados na rotina clínica do Cirurgião-dentista. Foram utilizados os cimentos AH Plus®, BioRoot RCS®, Endomethasone N® e Sealapex®. Foram realizados os testes físico-químicos de tempo de endurecimento, radiopacidade e escoamento, segundo a especificação nº 57 da ANSI/ADA (2012). Para o teste de citotoxicidade, foram coletadas amostras sanguíneas de seis doadores voluntários adultos, com idade entre 18 e 35 anos, de ambos os gêneros. Foi realizada a cultura de células primárias de linfócitos do sangue periférico humano em contato com o eluato dos cimentos obturadores, e a viabilidade celular foi analisada por meio do ensaio de MTT. Cada teste foi realizado em triplicata, tanto para propriedades físico-químicas como para o ensaio do MTT, sendo os resultados analisados empregando o programa GraphPad Prism 5®, por meio do teste Oneway ANOVA e pós-teste de Tukey, com nível de significância de 5%. De acordo com os resultados obtidos, tanto o AH Plus® quanto o BioRoot RCS® apresentaram tempo de endurecimento de 1450 minutos e 255 minutos, respectivamente; Endomethasone N® não tem tempo de endurecimento citado pelo fabricante, e Sealapex® não tomou presa no período do estudo (168 horas). Para o teste de radiopacidade, todos os materiais avaliados apresentaram radiopacidade acima de 3 mm alumínio, como recomendado pela especificação nº 57 da ANSI/ADA (2012). Para o teste de escoamento, segundo a ISO 6876:2012, um cimento endodôntico não deve apresentar um diâmetro inferior a 17 mm. Observamos a maior média para AH Plus® (39,68 mm), seguido em ordem decrescente por Sealapex® (35,73 mm), Endomethasone N® (34,22 mm) e BioRoot RCS® (23,81 mm). Quanto à viabilidade celular, Endomethasone N® foi citotóxico sobre os linfócitos em todas as concentrações (p0,05). Podemos concluir que todos os materiais avaliados atenderam aos parâmetros estabelecidos pela ANSI/ADA para os testes físico-químicos realizados. Além disso, o Endomethasone N® apresentou citotoxicidade sobre os linfócitos humanos, e os demais materiais analisados apresentaram viabilidade celular em níveis aceitáveisThe present study had as objective to evaluate the physicochemical properties and cytotoxicity of four root canal sealers with different chemical bases, used in the clinical routine of the Dentistry. The AH Plus®, BioRoot RCS®, Endomethasone N® and Sealapex® cements were used. The physicochemical tests of setting time, radiopacity and flowability were performed according to ANSI/ADA specification (nº 57). For the cytotoxicity test, blood samples were collected from six adult voluntary donors, aged 18-35 years, of both genders. Primary human lymphocyte cell cultures were placed in contact with the eluate from the plug cements, and cell viability was analyzed by the MTT assay. Each test was performed in triplicate, both for physicochemical properties and for the MTT assay. The results were analyzed using the GraphPad Prism 5® program, using the Oneway ANOVA test and Tukey\'s post-test, with a significance level of 5%. According to the results obtained, both AH Plus® and BioRoot RCS® presented setting time of 1450 minutes and 255 minutes, respectively; Endomethasone N® does not have setting time quoted by the manufacturer, and Sealapex® did not set in the study period (168 hours). For the radiopacity test, all evaluated materials had radiopacity above 3 mm aluminum, as recommended by the ANSI/ADA specification (nº 57). For the flow test, according to ISO 6876:2012, an endodontic cement must not have a diameter of less than 17mm. We observed the highest mean for AH Plus® (39.68 mm), followed in descending order by Sealapex® (35.73 mm), Endomethasone N® (34.22 mm) and BioRoot RCS® (23.81 mm). Regarding cell viability, Endomethasone N® was cytotoxic on lymphocytes at all concentrations (p 0.05). We can conclude that all materials evaluated met the parameters established by ANSI/ADA for the physicochemical tests performed. In addition, Endomethasone N® showed cytotoxicity on human lymphocytes, and the other materials analyzed presented cell viability at acceptable level
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more

We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
