Circulating tumor DNA in early breast cancer
Abstract: Introduction: Patients with triple negative breast cancer (TNBC) have the worst prognosis among all breast cancer subtypes. These patients usually receive neoadjuvant chemotherapy (NACT) with the hope of achieving pathological complete response (PCR). The vast majority of patients with pCR have an excellent prognosis similar to other breast cancer subtypes, while those without pCR relapse quickly. Nevertheless, around 10% of patients with pCR relapse and 40% without pCR have a good prognosis. Developing a more accurate biomarker of tumor genetic makeup and response to NACT will help in delivering more efficient treatment. Circulating tumor DNA (ctDNA) in plasma has been introduced as a possible biomarker of cancer that is more representative of tumor genetics and burden. We propose to study the role of ctDNA in monitoring tumor genetic makeup and response to NACT. The ultimate goal is to develop a blood-based biomarker of breast cancer that reveals tumor heterogeneity and measures response in the neoadjuvant setting. Methods: In this pilot study, samples from four patients with triple negative breast cancer (TNBC) receiving NACT were analyzed. Pre-treatment biopsies, post-treatment biopsies, and serial blood samples during treatment have been collected. Pre and post treatment tumors were sequenced to identify somatic variants. Multiple variants were selected for each patient to develop patient-specific droplet digital PCR (ddPCR) ctDNA assays. We analyzed mutational status concordance between tumor biopsy and ctDNA. We also measured the association between changes in ctDNA levels during NACT and tumor response. Results: A total of twenty-six variants have been analyzed in four patients. Eighty-five percent of variants were detected in the plasma. Only sixty percent of variants were shared between tumor biopsy and ctDNA. The remaining variants were detected in one of them, tumor biopsy or ctDNA. In each patient, at least one mutation showed dynamic changes in ctDNA levels analogous to the changes observed in tumor size. Conclusion: Our data show the feasibility of tracking ctDNA during NACT in TNBC patients. Both tumor biopsy and ctDNA are needed to capture tumor heterogeneity. The dynamic changes of ctDNA levels seem to correlate with tumor size changes in response to NACT for selected variants but not for all. Our results will increase the current understanding of the applicability of ctDNA as a biomarker of response to NACT.Résumé: Introduction: Les patients atteints d'un cancer du sein triple négatif (CSTN) ont le pronostic le moins favorable parmi tous les sous-types de cancer du sein. Ces patients reçoivent habituellement une chimiothérapie néoadjuvante (CNA) ayant pour but l'obtention une réponse pathologique complète (RPC). La grande majorité des patients avec une RPC ont un excellent pronostic, tandis que ceux sans une RPC rechutent rapidement. Toutefois, environ 10% des patients avec une RPC rechutent tandis que 40% des patients sans RPC connaissent un bon pronostic. Le développement d'un biomarqueur tumoral précis et prédictif de la réponse à la CNA aiderait à fournir un traitement plus efficace. L'ADN tumoral circulant (ADNtc) a été proposé en tant que biomarqueur potentiel du cancer représentatif de la composition génétique tumorale ainsi que l'ampleur de l'atteinte tumorale. Nous proposons donc d'étudier le rôle de l'ADNtc dans la surveillance de la composition génétique tumoral et de la réponse à la CNA. Le but ultime est de développer un biomarqueur sanguin du cancer du sein qui analyse l'hétérogénéité tumorale et mesure la réponse néoadjuvante. Méthode: Dans cette étude pilote, des échantillons de quatre patients atteints du cancer du sein triple négatif (CSTN) et recevant la CNA ont été analysés. Des biopsies pré- et post-traitement ainsi que des échantillons de sang en série durant le traitement ont été collectés. Les tumeurs pré- et post-traitement ont été séquencées pour identifier les mutations somatiques. Plusieurs mutations somatiques ont été sélectionnées par patient afin de développer des essais de réaction en chaîne par polymérase numérique à gouttelettes (ddPCR) pour l'ADNtc spécifiques au patient. Nous avons analysé la biopsie tumorale et l'ADNtc afin d'évaluer la présence de la mutation et sa concordance entre les deux types d'échantillon. Nous avons également mesuré l'association entre les changements des niveaux d'ADNtc durant la CNA et la réponse tumorale.Résultats:Un total de vingt-six mutations a été analysé chez quatre patients. Quatre-vingt-cinq pour cent (85%) des mutations ont été détectées dans le plasma. Seulement soixante pour cent des mutations étaient présentes dans la biopsie de tumeur et l'ADNtc. Les mutations restantes ont été détectées, soient dans la biopsie tumorale ou dans l'ADNtc. Chez chaque patient, au moins une mutation a démontré des changements dynamiques dans le niveau d'ADNtc analogues aux changements observés dans la taille de la tumeur. Conclusion: Nos données démontrent la faisabilité de suivi du ADNtc pendant la CNA chez les patients atteints d'un CSTN. La biopsie tumorale et l'ADNtc sont nécessaires pour capturer l'hétérogénéité tumorale. Les changements dynamiques des niveaux d'ADNtc semblent corréler les changements de taille tumorale en réponse à la CNA pour des mutations sélectionnées, mais pas pour l'ensemble d'entre eux. Nos résultats contribueront à augmenter la compréhension actuelle de l'applicabilité de l'ADNtc comme biomarqueur de la réponse à la CNA
Validation of ABCB1 break points in paclitaxel-resistant triple negative breast cancer
Triple-negative breast cancer (TNBC) is a subtype of breast cancer that shows the most aggressive clinical behavior and the poorest prognosis. It is characterized by the low or lack of expression of the estrogen receptor (ER), progesterone receptor (PR) and the human epidermal growth factor receptor 2 (Her-2). Due to this, TNBC patients do not benefit from hormonal treatments or targeted therapies against Her-2. Taxane-based chemotherapy, mainly paclitaxel (PTX), remains as the standard choice of treatment for TNBC patients. In spite the fact that TNBC patients are initially highly responsive to PTX, the majority of these patients eventually acquire resistance to the treatment and continue to progress in their disease. To mimic acquired resistance in patients, three TNBC cells lines, BT20, MDA-MB-468 and SUM149, have been cultured for 6 months in the presence of PTX. These parental and resistant clones were analyzed with high density array comparative genomic hybridization (array CGH) to assess their genomic amplifications and deletions. A common amplification and break point in ABCB1 between exons 2 and 3 was shared between all three resistant variants. ABCB1 encodes for P-glycoprotein (P-gp), a transmembrane pump that plays a role in multidrug resistance by removing cytotoxic agents from inside the cell. Using PCR techniques and targeted sequencing, we found unrelated genes fused to the ABCB1 mRNA of BT20 and SUM149 resistant (R) cells. In BT20-R, exons 1 and 2 of a pseudogene called TEKT4P2 (Tektin 4 Pseudogene 2) were directly fused to exon 3 of ABCB1. SUM149-R showed several heterogeneous alleles with two separate genes, SCL25A40 (Solute Carrier Family 25, Member 40) and TEFM (Transcription Elongation Factor, Mitochondrial) fused to either exon 3 or to a truncated exon 2 of ABCB1. In MDA-MB-468-R, instead of detecting a fusion event, a deletion of exon 1 and a portion of exon 2 were identified, resulting in a truncated transcript. All fusions were further supported by quantitative real-time PCR (qPCR) results, demonstrating that the fusion partners in BT20-R and SUM149-R were co-expressed with ABCB1. In MDA-MB-468, the deleted exons were validated by a decrease in expression levels compared to the intact exons. Taken together, these findings suggest that the intragenic break point in ABCB1 resulted in the displacement of exons 1 and 2. This event could play a critical role in the translational process of ABCB1 by overcoming a block at the mRNA level.Le cancer du sein triple négatif (TNBC) est le sous-type de cancer du sein le plus agressif et il est associé à un mauvais pronostique. Il est caractérisé par la faible ou l'absence d'expression des récepteurs de l'estrogène (ER) et de la progesterone (PR), ainsi que du récepteur Her-2. Pour cette raison, les patientes TNBC ne peuvent bénéficier de traitements hormonaux ou de thérapies dirigées contre Her-2. La chimiothérapie basée sur les taxanes, notamment le paclitaxel (PTX), reste le choix standard de traitement pour les patientes TNBC. Malgré le fait que les patientes TNBC soient initialement très sensibles au PTX, la majorité de ces patientes finissent par acquérir une résistance au traitement et vont continuer à progresser dans leur maladie. Pour reproduire in vitro la résistance acquise chez les patientes, trois lignées cellulaires TNBC, BT20, MDA-MB-468 et SUM149, ont été cultivées pendant 6 mois en présence de PTX. Les clones parentaux et résistants ont été analysés par hybridation génomique comparative sur puces à ADN (array CGH) pour évaluer leurs amplifications et délétions génomiques. Une amplification commune et un point de cassure dans ABCB1 entre les exons 2 et 3 ont été retrouvés dans les trois lignées résistantes. ABCB1 code pour la P-glycoprotéine (P-gp), une pompe transmembranaire qui joue un rôle dans la résistance à de multiples médicaments en expulsant les agents cytotoxiques à l'extérieur de la cellule. En utilisant les techniques de PCR et de séquençage, nous avons trouvé des gènes non relié fusionnés avec l'ARNm d'ABCB1 dans les lignées résistantes (R) de BT20 et SUM149. Dans la lignée BT20-R, les exons 1 et 2 d'un pseudogène appelé TEKT4P2 (Tektin 4 pseudogène 2) ont été fusionnés directement à l'exon 3 d'ABCB1. La lignée SUM149-R démontre quant à elle plusieurs allèles hétérogènes avec deux gènes distincts, SCL25A40 (soluté Transporteur famille 25 membres 40) et TEFM (Transcription Elongation Factor, mitochondrial) fusionnés soit à l'exon 3 ou à l'exon 2 tronqué d'ABCB1. Dans la lignée MDA-MB-468-R, seulement une délétion de l'exon 1 et une partie de l'exon 2 a été identifiée, entraînant la production d'un transcrit tronqué. Toutes les fusions ont été validées par PCR en temps réel (qPCR), ce qui démontre que les partenaires de fusion sont co-exprimés avec ABCB1 dans les lignées BT20-R et SUM 149R. Pour ce qui est de la lignée MDA-MB-468-R, les exons supprimés ont étés validés par une diminution de leur expression par rapport-aux exons intacts. Ces résultats suggèrent donc-que le point de cassure intragénique dans ABCB1 a causé le déplacement des exons 1 et 2. Cet événement pourrait jouer un rôle crucial dans le processus de traduction d'ABCB1 en surmontant un bloc au niveau de l'ARNm
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Molecular profiling ofchemotherapy acquired resistance in triple negatice breast cancer cell lines
Triple-negative breast cancer (TNBCs), characterized by tumors that do not express estrogen receptor (ER), progesterone receptor (PR), or HER-2 genes, shows one of the most aggressive clinical behaviors with distinctive metastatic patterns and very poor prognosis. Since no targeted therapy is available, chemotherapy and, in particular, taxane-based therapy (e.g. paclitaxel) is the treatment of choice for patients with TNBC. TNBCs are initially highly responsive to PTX, however the majority of advanced TNBC patients acquire resistance and develop progressive disease. Here we developed five in vitro models of resistance to better understand and characterize the biological mechanisms underlying resistance to PTX. We used an integrative analysis of array CGH and gene expression data to gain insights into the functional genomic changes in PTX-resistant TNBC cell lines. The results of integrative analysis of array CGH and gene expression identified ABCB1 as the most highly up-regulated gene in all 5 resistant cell lines. This up-regulation was accompanied by gene amplification in three resistant cell lines: BT20R, SUM149R and MDA-468 but not in 2 other lines, MDA-MB-231 and MDA-MB-436. Further analysis of the 7q21.12 chromosome fragment containing the ABCB1 gene in BT20R, SUM149R and MDA-468R revealed similar, but not identical, intra-genic breakpoints in ABCB1. RNAseq analysis identified novel inter-chromosomal translocations of ABCB1 in 2 drug resistance cell lines containing ABCB1 breakpoints. These genomic rearrangements occurred in the 5' un-translated region (UTR) of ABCB1. We hypothesize that the deletion of the 5'UTR of ABCB1 in these resistant cell lines may serve to overcome a translation initiation block, which would then favor the expression of ABCB1. The overexpression of ABCB1 at protein level was confirmed only in the three resistant cell lines with the ABCB1 genomic rearrangement, suggesting that the deletion of 5'UTR of ABCB1 may be a candidate biomarker to predict resistance to paclitaxel in triple negative breast cancers. In order to better understand the biological significance of the observed changes we performed pathway analysis and identified enriched molecular function of deregulated genes common in resistant cell lines. We found that the molecular function "growth factor activity", including several epidermal growth factor receptor (EGFR) ligands was consistently up-regulated in all 5 resistant cell line models. Considering the importance of EGFR ligands and given the up-regulation of EGFR signaling pathway, we examined the sensitivity of all five cell line models to two EGFR inhibitors, lapatinib and neratinib in combination with PTX. Our results revealed that the addition of 100nM of lapatinib or neratinib partially re-sensitized resistant cell lines to PTX to various degrees, with one exception: BT20R did not show sensitivity to the addition of 100nM lapatinib to increasing concentration of PTX. These results suggest that anti-EGFR therapies can re-sensitize PTX-resistant TNBC cells to PTX and that up-regulation of EGFR ligands in PTX resistant cell lines may be a biomarker of sensitivity to EGFR inhibitors.Le cancer du sein triple-négatif (TNBC), caractérisé par des tumeurs qui n'expriment ni le récepteur aux estrogènes (RO), ni le récepteur à la progestérone (PR) ou ni les gènes HER-2, constitue une évolution des plus agressive avec des profils métastatiques particuliers et un très sombre pronostic. Puisqu'aucune thérapie ciblée n'est à ce jour disponible, la chimiothérapie, et en particulier la thérapie à base de Taxanes (Placlitaxel), représente le traitement de référence pour les patientes atteint de TNBC. Les TNBC répondent initialement fortement au paclitaxel (PTX), cependant la majorité des patientes aux stades avancés développent des résistances et la maladie progresse. In vitro, nous avons développé cinq modèles de résistance dans le but de mieux comprendre et caractériser les mécanismes biologiques sous-tendant la résistance. Nous avons utilisé une analyse intégrée de array-CGH et des données d'expression de gènes pour comprendre les changements génomiques fonctionnels dans des lignées cellulaires TNBC résistantes au PTX. Les résultats de ces analyses ont identifié ABCB1 comme étant le gène plus fortement sur-exprimé parmi les cinq lignées cellulaires résistantes. Cette sur-expression était accompagnée d'une amplification du gène dans trois lignées résistantes : BT20R, SUM149R et MDA-468, mais pas dans les deux autres lignées, MDA-MB-231 et MDA-MB-436. Des analyses complémentaires d'un fragment du chromosome 7q21.12 contenant le gène ABCB1 dans BT20R, SUM149R et MDA-468R révèlent des cassures intra-géniques similaires mais non identiques. Une analyse RNAseq a identifié de nouvelles translocations intra-chromosomiques de ABCB1 dans deux lignées cellulaires résistantes contenant les cassures de ABCB1. Ces réarrangements génomiques se localisaient dans la région «5' non-traduite region» (UTR) de ABCB1. Notre hypothèse est que la délétion de la région 5'UTR de ABCB1 dans ces lignées cellulaires résistantes pourrait servir à surmonter un blocage initial de la traduction et pourrait ensuite favoriser l'expression de la protéïne Abcb1. La surexpression de ABCB1 à un niveau protéique a été confirmée seulement dans les trois lignées résistantes contenant les réarrangements génomiques ABCB1, suggérant que la délétion de la région 5'UTR dans ABCB1 serait un bio-marqueur potentiel pour prédire la résistance au Paclitaxel dans les TNBC. Dans le but de mieux comprendre la signification biologique des changements observés, nous avons pratiqué des analyses de voies moléculaires et avons identifié une fonction moléculaire enrichie chez les gènes dérégulés dans les lignées résistantes. Nous avons trouvé que la fonction moléculaire d'« activité de facteur de croissance », incluant plusieurs ligands du récepteur « Epidermal Growth Factor Receptor » (EGFR), étaient systématiquement sur-exprimés dans les cinq modèles de lignées cellulaires résistantes. Considérant l'importance des ligands au EGFR et étant donné leur sur-expression, nous avons examiné la sensibilité des cinq modèles cellulaires à deux inhibiteurs de EGFR, Lapatinib et Neratinib en association avec le PTX. Nos résultats ont révélé que l'ajout de 100nM de Lapatinib ou Neratinib re-sensibilisait partiellement les lignées résistantes au PTX à des degrés variables, avec une exception : BT20R n'a pas montré de sensibilisation à l'addition de 100nM de Lapatinib à des concentrations croissantes de PTX. Ces résultats suggèrent que des thérapies anti-EGFR pourraient re-sensibiliser les cellules de TNBC résistantes au PTX ; et cette sur-expression des ligands du EGFR dans les cellules résistantes au PTX pourrait constituer un bio-marqueur de sensibilité aux inhibiteurs du EGFR
A study of SPEN functions in breast cancer
Breast cancer is the most common type of cancer and the second-leading cause of cancer deaths in women worldwide. Breast tumors are classified into three distinct therapeutic groups based on the expression of the estrogen receptor (ER), progesterone receptor (PR) and human epidermal growth factor receptor 2 (HER2). The establishment of the ER, PR and HER2 as prognostic and predictive biomarkers in breast cancer has greatly improved treatment decisions and overall patient survival. However, thousands of women relapse and eventually die from their disease every year due to intrinsic or acquired resistance to therapy. While most studies have focused on oncogene-driven drug resistance, we proposed that tumour suppressor genes (TSG) might have a greater influence on the tumorigenic process and possibly a superior clinical relevance than oncogenes in predicting therapeutic responses. Using an unbiased genomic approach, we identified a list of candidate tumour suppressor genes mutated in breast cancer cells, including SPEN. The SPEN gene encodes a large protein that has previously been shown to repress transcription by nuclear receptors, including the ER. Given the latter, we proposed to examine the 1) hormone-dependent and 2) -independent biological functions of SPEN in breast cancer cells. We report the finding of SPEN, a cofactor of the ER transcriptional complex, as a novel tumour suppressor gene mutated in breast cancer that regulates cell survival and tumour growth by repressing the expression of ER-dependent genes, including the PR as well as apoptosis-related genes. In addition, our work provides functional and clinical evidence for SPEN as a candidate predictive biomarker of Tamoxifen response in ER-positive breast cancers, possibly due to its recruitment of histone-deacetylase enzymes to DNA. Having shown that SPEN attenuates hormonal signalling in breast cancer, we then investigated the hormone-independent transcriptional program regulated by SPEN in breast cancer cells and uncovered a significant enrichment for genes associated with the cilium, including the ciliogenic transcription factor, RFX3. This work led to the identification of SPEN as a novel regulator of primary cilia formation in hormone-independent breast cancers. We further demonstrated that SPEN regulates migration in ciliated breast cells exclusively, suggesting that it may coordinate cellular movement through primary-cilia dependent mechanisms. Importantly, we found that SPEN expression is strongly predictive of early metastasis in patients with ER-negative breast cancers. The findings reported in this study therefore establish SPEN as a corepressor of the ER, novel tumour suppressor gene and candidate predictive biomarker of Tamoxifen response in patients with hormone receptor positive breast cancers. Furthermore, they show that SPEN regulates primary cilia formation, cell migration and is associated with metastasis in patients with ER-negative breast cancers. Altogether, the data contained within this thesis reveal that SPEN has dual roles in breast cancer and that its expression levels are prognostic in both hormone-dependent and -independent breast cancers.Le cancer du sein est le type de cancer le plus fréquent et la deuxième cause en importance de décès des suites du cancer chez la femme à travers le monde. Les tumeurs du sein sont classées en trois groupes thérapeutiques en function de l'expression du recepteur à l'estrogène (ER), à la progesterone (PR) et au facteur de croisssance épidermique humain 2 (HER2). Bien que l'instauration du ER, PR et HER2 comme marqueurs prognostics et predictifs dans le cancer du sein aie grandement contribué a l'amélioration des décisions thérapeutiques ainsi qu'à la survie des patientes, des milliers de femmes meurent chaque année des suites de leur maladie en raison d'une forme intrinsèque ou acquise de résistance aux médicaments. Alors que la plupart des groupes de recherche ont etudié la résistance thérapeutique induite par les oncogenes, nous proposons que les gènes suppresseurs de tumeurs pourraient avoir une plus grande influence dans le processus tumorigénique et possiblement une valeur clinique supérieure aux oncogènes pour prédire les réponses thérapeutiques. À l'aide d'une approche génomique non-biaisée, nous avons identifié une liste de gènes potentiellement suppresseurs de tumeur et mutés dans le cancer du sein, dont SPEN. Le gène SPEN code pour une protéine qui est impliquée dans la repression de la transcription médiée par des récepteurs nucléaires, y compris le ER. Compte tenu de ces données, nous avons proposé d'étudier les fonctions biologiques de SPEN controlées de façon 1) dépendante et 2) indépendante des hormones dans le cancer du sein. Nous rapportons la découverte de SPEN, un cofacteur du complexe transcriptionnel du ER, comme nouveau gène suppresseur de tumeur fréquemment muté dans le cancer du sein. Nos résultats démontrent que SPEN régule la survie des cellules et la croissance tumorale en réprimant l'expression des gènes dépendants du ER, notamment le PR, ainsi que des gènes associés à l'apoptose. De plus, notre étude apporte une preuve fonctionnelle et clinique que SPEN pourrait être employé comme biomarqueur prédisant la réponse des patientes ayant un cancer positif pour le ER au Tamoxifen et ce, possiblement dû à sa capacité de recruiter des histone déacetylases au niveau de l'ADN. Ayant démontré que SPEN atténue la signalisation hormonale dans le cancer du sein, nous avons ensuite entrepris de définir le programme transcriptionnel régulé par SPEN de façon indépendante des hormones. Cette étude nous a permis de découvrir que SPEN est co-exprimé avec un grand nombre de gènes associés au cil primaire, y compris le facteur de transcription, RFX3. Ces travaux ont conduits à l'identification de SPEN comme un nouveau régulateur de la formation des cils primaires dans les cancers du sein hormono-indépendants. Nous exposons aussi que SPEN contrôle la migration des cellules mammaires, mais uniquement de celles qui sont ciliées, ce qui suggère que SPEN coordonne le movement cellulaire par l'intermédiaire du cil primaire. Fait important, nous avons constaté que l'expression de SPEN est fortement predictive de la détection précoce de metastases chez les patients atteintes d'un cancer du sein négatif pour le ER. Les résultats presentés dans cette étude établissent donc SPEN comme corépresseur du ER, nouveau gène suppresseur de tumeur et biomarqueur potentiellement prédictif d'une réponse clinique au Tamoxifen chez les patientes atteintes d'un cancer du sein positif pour les récepteurs hormonaux. Aussi, les résultats rapportés dans cette étude démontrent que SPEN régule la formation du cil primaire, la migration des cellules et que sont expression est associée au développement de métastases chez les patientes ayant un cancer négatif pour le ER. En général, les données contenues dans cette thèse révèlent que SPEN est une protéine ayant un double rôle dans le cancer du sein et mettent en évidence que son expression pourrait avoir un potential pronostic et prédictif dans les cancers du sein hormono-dépendants et –indépendants
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Functional validation of morphological and gene expression changes related to metabolic adaptation in chemoresistant triple negative breast cancer cells
Triple Negative Breast Cancer (TNBC) is the most aggressive subtype of breast cancer (ER-, PR-, or HER2-) and chemotherapy is the only treatment option, but 40% of patients in early TNBC stages do not respond to therapy and will develop chemoresistance. It is imperative to identify biomarkers that will help identify TNBC subpopulations' response to chemotherapy. An in vitro model of chemoresistance was generated in our lab by administering a single high dose of DOX to a TNBC cell line, MDA-MB-436. These DOX-resistant cells had an enrichment of lipid droplets (LDs) and fragmented mitochondria as revealed by electron microscopy. This correlated with increased copy number and/or overexpression of genes and proteins involved in mitochondrial fission (MFF), LD biogenesis (PLIN4), and lipid metabolism (PPAR). Moreover, inhibition of OXPHOS by oligomycin in DOXO-R cells had a greater impact on ATP production compared to parental cells, suggesting the mitochondria are functional and crucial for energy production in DOXO-R cells. Beta-oxidation of free fatty acids occurs in mitochondria, thus we hypothesized these morphological anomalies and gene expression changes indicate an adapted metabolism in DOXO-R cells that could confer survival and/or chemoresistance. This project aimed to determine the roles of MFF, PLIN4, and PPAR, in LD dynamics and metabolism and how they contribute to these phenotypes. We confirmed increased mRNA and protein expression of MFF, PLIN4, and PPAR. The hypothesis of altered mitochondrial dynamics in DOXO-R cells was supported by an increase of mitochondrial proteins (VDAC, PGC1), a decrease in regulation of fusion by OPA-1, and an increase in regulation of fission by p-MFF. PLIN4 was revealed as a marker of lipid droplets and is crucial to DOXO-R cell survival, as transient silencing via siRNA significantly reduced cell viability. PPAR regulates PLIN4 expression, and is likely not directly implicated in DOXO-R cell survival, but led us to investigate autophagy as a novel vulnerability of DOXO-R cells. Our work better defines the functional importance of morphological and metabolic changes as novel actors conferring survival benefits and chemoresistance in TNBC.Le cancer du sein triple négatif (TNBC) est le sous-type le plus agressif du cancer du sein (ER, PR ou HER2) et la chimiothérapie est la seule option de traitement, mais 40% des patients TNBC au stade TNBC précoces ne répondent pas au traitement et développent la chimiorésistance. Il est impératif d'identifier des biomarqueurs qui aideront à identifier la réponse des sous-populations TNBC à la chimiothérapie. Un modèle in vitro de chimiorésistance a été généré dans notre laboratoire en administrant une seule dose élevée de DOX à une lignée cellulaire TNBC, MDA-MB-436. Ces cellules résistantes à la DOX présentaient un enrichissement en gouttelettes lipidiques (LD) et en mitochondries fragmentées, révélées par microscopie électronique. Ceci est corrélé à l'augmentation du nombre de copies et/ou à la sur-expression des gènes et des protéines impliqués dans la fission mitochondriale (MFF), la biogenèse de la LD (PLIN4) et le métabolisme des lipides (PPAR). De plus, l'inhibition d'OXPHOS par l'oligomycine dans les cellules DOXO-R a eu un impact plus important sur la production d'ATP que sur les cellules parentales, ce qui suggère que les mitochondries sont fonctionnelles et cruciales pour la production d'énergie dans les cellules DOXO-R. Comme la bêta-oxydation des acides gras libres se produit dans les mitochondries, nous avons conçu l'hypothèse que ces anomalies morphologiques et les modifications de l'expression génique indiquent un métabolisme adapté dans les cellules DOXO-R qui pourrait entraîner la survie et/ou la chimiorésistance. Ce projet visait à déterminer les rôles du MFF, du PLIN4 et du PPAR dans la dynamique et le métabolisme des LD, ainsi que leur contribution à ces phénotypes. Nous avons confirmé une expression accrue de l'ARNm et des protéines de MFF, PLIN4 et PPAR. L'hypothèse d'une altération de la dynamique mitochondriale dans les cellules DOXO-R était soutenue par une augmentation des protéines mitochondriales (VDAC, PGC1), une diminution de la régulation de la fusion par OPA-1 et une augmentation de la régulation de la fission par le p-MFF. Le PLIN4 a été révélé comme un marqueur des gouttelettes lipidiques et il est crucial pour la survie des cellules DOXO-R, car le silençage transitoire via les ARNsi a considérablement réduit la viabilité cellulaire. PPAR régule l'expression de PLIN4 et n'est probablement pas directement impliquée dans la survie des cellules DOXO-R, mais nous a conduit à étudier l'autophagie comme nouvelle vulnérabilité des cellules DOXO-R. Nos travaux définissent mieux l'importance fonctionnelle des modifications morphologiques et métaboliques en tant que nouveaux acteurs conférant des avantages de survie et une chimiorésistance au TNBC
The diagnostic and therapeutic role of the stromal cell-derived factor (SDF)-1/CXCR4 axis in breast cancer metastasis
Breast cancer kills through the process of metastasis. In order to improve the prognosis of patients with breast cancer, a better understanding of the underlying factors driving the metastatic process in patients is required. One theory that helps explain the metastatic process suggests that chemokines, such as stromal cell-derived factor (SDF)-1, are overexpressed in specific distant metastatic organs, such as lung, liver, and bone, and serve to home in cancer cells that express their receptors, like CXCR4. The hypothesis of this thesis is that the SDF-1/CXCR4 axis plays an important role in the process of metastasis in breast cancer, and that this ligand/receptor axis can be exploited in the diagnostics and therapy of breast cancer. The first objective of this thesis was to determine if circulating levels of SDF-1 could predict breast cancer metastasis. We found low levels of plasma SDF-1 to be a strong independent prognostic marker, suggesting that the concentration gradient of low plasma SDF-1 and high SDF-1 expressed in the metastatic organ may be critical in driving cancer cells from the circulation to the target organ. We further determined that the levels of plasma SDF-1 were tumor-independent, identifying the first host-derived blood marker predictive of distant metastasis. The second objective was to determine if tumor expression of CXCR4 could modulate the prognostic effect of plasma SDF-1 levels. We found that patients with tumors that highly expressed the activated form of the receptor, phosphorylated-CXCR4, and low plasma SDF-1 levels had a much poorer prognosis than those patients with either risk factor alone. These results highlighted the importance of the dysfunctional relationship between the tumor and the host in the metastatic process. The third objective assessed the therapeutic potential of targeting CXCR4 with a peptide antagonist in a transgenic mouse model.
