Using a weaning immunosuppression protocol in liver transplantation recipients with hepatocellular carcinoma: A compromise between the risk of recurrence and the risk of rejection?
Hepatocellular carcinoma (HCC) recurrence rate after liver transplantation (LT) is still up to 1520%, despite a careful selection of candidates and optimization of the management within the waiting list. To reduce tumour recurrence, the currently adopted post-transplant strategies are based on the administration of a tailored immunosuppression (IS) regimen. Drug-induced depression of the immune system is essential in preventing graft rejection, however has a well-established association with oncogenesis. The immune system has a key role as a defending mechanism against cancer development, preventing vascular invasion and metastasis. Thus, IS drugs represent one of few modifiable non-oncological risk factors for tumour recurrence. In HCC recipients, a tailored IS therapy, with the aim to minimize drugs' doses, is essential to gain the optimal balance between the risk of rejection and the risk of tumour recurrence. So far, a complete withdrawal of IS drugs after LT is reported to be safely achievable in 25% of patients (defined as "operational tolerant"), without the risk of patient and graft loss. The recent identification of non-invasive "bio-markers of tolerance", which permit to identify patients who could successfully withdraw IS therapies, opens new perspectives in the management of HCC after LT. IS withdrawal could potentially reduce the risk of tumour recurrence, which represents the major drawback in HCC recipients. Herein, we review the current literature on IS weaning in patients who underwent LT for HCC as primary indication and we report the largest experiences on IS withdrawal in HCC recipients
Pandora's box: unpredictable evolution of a 20-year history of a bariatric patient-report of small bowel migrated gastric band after redo banded gastric bypass
Introduction: morbid obesity is a chronic disease, with resistance to multiple therapies. Bariatric surgery is the most efficient nowadays treatment, but with a certain price when we speak of surgeon’s efforts, patient’s compliance and tolerance, available technology and long term evolution. Materials: we present the case of 20 years evolution of a female obese patient, with multiple, sequential bariatric minimally-invasive interventions: intragastric balloon, gastric banding, gastric bypass (GBP), banding positioning on GBP, all complicated. Last attempt to treat weight regain was laparoscopic positioning of an adjustable banding over a failed gastric bypass in 2015, complicated 12 months after by acute intestinal occlusion due to band migration, leading to open emergency band removal, wound dehiscence and finally incisional hernia. Results: after multiple interventions, the patient has a BMI of 38 kg/m2, large incisional hernia, depression, over a non-satisfactory gastric bypass. Conclusion: bariatric surgery has some limitations in case of patient’s non-compliance and bad-luck, even with persistent, experienced tailored bariatric treatment. Non-responsive obese patients should be considered as possible subcategory of long-term outcomes
Life is short: The impact of power states on base station lifetime
We study the impact of power state transitions on the lifetime of base stations (BSs) in mobile networks. In particular, we propose a model to estimate the lifetime decrease/increase as a consequence of the application of power state changes. The model takes into account both hardware (HW) parameters, which depend on the materials used to build the device, and power state parameters, that instead depend on how and when power state transitions take place. More in depth, we consider the impact of different power states when a BS is active, and one sleep mode state when a BS is powered off. When a BS reduces the power consumption, its lifetime tends to increase. However, when a BS changes the power state, its lifetime tends to be decreased. Thus, there is a tradeoff between these two effects. Our results, obtained over universal mobile telecommunication system (UMTS) and long term evolution (LTE) case studies, indicate the need of a careful management of the power state transitions in order to not deteriorate the BS lifetime, and consequently to not increase the associated reparation/replacement costs

Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
