    Scanning electron microscopic features of Kaposi's sarcoma.

    In order to clarify the three-dimensional structure of Kaposi's sarcoma (KS) five cases of KS of the skin were observed with a scanning electron microscope (SEM). KS lesions were investigated both in vascular and spindle cell areas and were compared with similar areas observed by light microscopy. KS appears as a spongelike neoplasm consisting of irregular vascular channels lacking endothelial cells and well-developed basement membranes. Vascular spaces and vascular slits are packed with numerous red blood cells (RBCs). SEM features of KS share some similarities with those of hemangiopericytoma, a vascular tumor composed of mature endothelial cells surrounded by pericytes. These findings are consistent with the hypothesis according to which the proliferating cells of KS are viewed as immature endothelial cells with partial differentiation into pericytes

    Riassunto. Vengono riportati 48 casi di balanopostite xerotica obliterans (lichen scleroatrofico del pene)osservati consecutivamente in 8 anni presso la nostra clinica dermatologica . La malattia, interessa giovani adulti principalmente tra i 25 e i 35 anni e determina in due terzi dei casi un fimosi e nei restanti casi lesioni sclero-atrofiche.Sul piano istopatologico le lesioni sono riconducibili atre stadi del lichen scleroatrofico.La terapia locale comprende un trattamento medico -topico e uno chirurgico; nei casi iniziali sono utili le emulsioni a base di corticosteroidi a bassa potenza, il progesterone percutaneo in gel all'1% e le sostanze emollienti.Il trattamento chirurgico, effettuato in 20 pazienti, consiste nella circoncisione allargata, nella decorticazione con lisi delle aderenze balaniche e/o nella copertura con innesti a tutto spessore e nella meatoplastica. on questa terapia abbiamo ottenuto risultati funzionali ed estetici soddisfacenti e duraturi
