Bmc Public Health, 16, 11. doi:10.1186/s12889-016-3155-1Background: Nutrition transition in developing countries were induced by rapid changes in food patterns and nutrient intake when populations adopt modern lifestyles during economic and social development, urbanization and acculturation. Consequently, these countries suffer from the double burden of malnutrition, consisting of unresolved undernutrition and the rise of overweight/obesity. The prevalence of the double burden of malnutrition tends to be highest for moderate levels (third quintile) of socioeconomic status. Evidence suggests that modifiable factors such as intra-household food distribution and dietary diversity are associated with the double burden of malnutrition, given household food security. This article describes the study protocol of a behaviorally based nutrition education intervention for overweight/obese mothers with stunted children (NEO-MOM) in reducing the double burden of malnutrition. Methods: NEO-MOM is a randomized controlled trial with a three-month behavioral intervention for households involving pairs of 72 stunted children aged 2-5 years old and overweight/obese mothers (SCOWT) in urban Indonesia. The SCOWT pairs were randomly assigned to either an intervention group or to a comparison group that received usual care plus printed educational materials. The intervention consisted of six classroom sessions on nutrition education and home visits performed by trained community health workers using a motivational interviewing approach. The primary outcomes of this study are the prevalence of double burden of malnutrition as measured in SCOWT, child's height-for-age z-score (HAZ) and maternal body mass index (BMI). Discussion: Because previous studies are mainly observational in nature, this study advances understanding of the double burden of malnutrition through a fully powered randomized controlled trial. The intervention assists participants in self-administered goal setting to improve diet and child feeding behaviors by improving self-efficacy. Maternal self-efficacy may be enhanced through vicarious and active mastery of experiences gained during six sessions of nutrition education and verbal persuasion during home visits

    Comparison of maternal nutrition literacy, dietary diversity, and food security among households with and without double burden of malnutrition in Surabaya, Indonesia

    Introduction: Double burden of malnutrition in the form of stunted child and overweight/obese mother has been increasing in countries experiencing nutrition transition. This study aimed to compare maternal nutrition, literacy, dietary diversity, and food security of households categorised by nutritional status of mother-child pairs. Methods: This cross-sectional study included a total of 685 children under 5 years of age from 14 community health centres (posyandu) in a poor urban area of Surabaya, Indonesia. The Lemeshow formula was used to compute sample size. The children and their mothers were categorised into motherchild pairs based on maternal body mass index (BMI) and height-for-age z-score for the child: non-overweight mother-non-stunted child (NM), non-overweight mother and stunted child (SC), overweight/obese mother and non-stunted child (OWT), stunted child and overweight/obese mother (SCOWT). Nutrition literacy was determined using questionnaires, dietary diversity was estimated using household dietary diversity scores, and food security was determined using the Household Food Insecurity Access Scale (HFIAS). Results: The prevalence of households with SCOWT double burden of malnutrition was 21.2%, OWT 37.7%, SC 15.3% and 25.8% were normal (NM). There were significant differences in maternal nutrition literacy, dietary diversity and household food security status. Households with NM pairs were shown to be most food secure followed by OWT, SC and lastly, the SCOWT pairs. Conclusion: Households with double burden of malnutrition have relatively lower nutrition literacy, dietary diversity, and food security. This study indicates the importance of developing nutritional strategies to enable low-income households to make healthy food choices

    Background: good nutrition for mothers since pre-conception and conception is an important preparation for the health of the fetus and the child in the future. Objective: The purpose in writing this literature review is to explain argumentatively how nutrition from the time of conception can affect the child's biological health. Discussion: The results of the literature review show evidence that nutrition from the time of conception can affect the child's biological health. Several articles show a mechanism that explains the possible mechanisms of nutritional influence as conception on the child's biological health as in the future as Barker Hyphothesis or fetal programming. However, the data obtained to date is still not fully convincing. It is necessary to develop a long or longitudinal and experimental research that can guarantee the truth of the theory. Conclusion: the argument that nutrition from the time of conception is believed to affect the biological health of children in the future is increasingly stronger in the current literature although evidence of experimental research results still need to be accumulated to ensure the truth. ABSTRAK   Latar Belakang: gizi yang baik bagi ibu sejak masa pra konsepsi dan konsepsi merupakan persiapan yang penting untuk kesehatan janin dan anak dimasa dating. Tujuan: tujuan dalam penulisan literature review ini adalah untuk menjelaskan secara argumentative bagaimana gizi sejak masa konsepsi dapat mempengaruhi kesehatan biologis anak. Pembahasan: hasil literature review menunjukkan bukti bahwa gizi sejak masa konsepsi dapat mempengaruhi kesehatan biologis anak. Beberapa artikel menunjukkan mekanisme yang menjelaskan kemungkinan mekanisme pengaruh gizi saat konsepsi terhadap kesehatan biologis anak dimasa datang seperti halnya Barker Hyphothesis atau fetal programming. Namun begitu, data yang diperoleh hingga saat ini masih belum meyakinkan sepenuhnya. Untuk itu perlu dikembangkan penelitian yang panjang atau longitudinal dan eksperimental yang dapat menggaransi kebenaran teori tersebut. Kesimpulan: argumentasi bahwa gizi sejak masa konsepsi diyakini berpengaruh terhadap kesehatan biologis anak dimasa depan semakin menguat dalam literature terkini walaupun bukti hasil penelitian eksperimental masih perlu banyak diakumulasikan untuk memastikan kebenarannya
