A novel MAG variant causes hereditary spastic paraplegia in a consanguineous Pakistani family

No full text
Background and objectives: Hereditary spastic paraplegia (HSP) is characterized by unsteady gait, motor incoordination, speech impairment, abnormal eye movement, progressive spasticity and lower limb weakness. Spastic paraplegia 75 (SPG75) results from a mutation in the gene that encodes myelin associated glycoprotein (MAG). Only a limited number of MAG variants associated with SPG75 in families of European, Middle Eastern, North African, Turkish and Palestinian ancestry have been documented so far. This study aims to provide further insight into the clinical and molecular manifestations of HSP.Methods: Using whole-exome sequencing, we investigated a consanguineous Pakistani family where three individuals presented with clinical signs of HSP. Sanger sequencing was used to carry out segregation analysis on available family members, and a minigene splicing assay was utilized to evaluate the effect of the splicing variant. Results: We identified a novel homozygous pathogenic splice donor variant in MAG (c.46 + 1G \u3e T) associated with SPG75. RNA analysis revealed exon skipping that resulted in the loss of a start codon for ENST00000361922.8 isoform. Affected individuals exhibited variable combinations of nystagmus, developmental delay, cognitive impairments, spasticity, dysarthria, delayed gait and ataxia. The proband displayed a quadrupedal stride, and his siblings experienced frequent falls and ataxic gait as one of the prominent features that have not been previously reported in SPG75.Conclusions: Thus, the present study presents an uncommon manifestation of SPG75, the first from the Pakistani population, and broadens the spectrum of MAG variants

Bio-mechanical behaviour of articular cartilage: characterisation of surface damage, friction and replacement

No full text
The progression of cartilage damage has the potential to develop into the joint disease of osteoarthritis. Cartilage repair methodologies involve the implementation of biomaterials, for which the bio-mechanical characteristics should closely align with articular cartilage. To date, the frictional torque of articular cartilage has not been assessed, nor has the coefficient of friction of replacement biomaterials inserted within a defect. Additionally, cartilage damage inducing factors have not been considered for a likely dual effect. Consequently, this thesis presents three novel developments to fulfil the lack of knowledge outlined above. The first novel outcome is the assessment of the dual impact of loading frequency and underlying substrate density, as a replication of bone mineral density, on resulting cartilage damage. The second novel development is a technique to determine the frictional torque of articular cartilage, corresponding to an inserted cartilage replacement biomaterial. The third novel development is a technique to assess the coefficient of friction of a cartilage replacement biomaterial, in line with osteochondral graft surgical procedures. Various approaches have been implemented to include three materials testing machines, Hertzian contact stress analysis and chemical hydrogel manufacture. For this thesis, bovine articular cartilage was modelled for human articular cartilage. The results demonstrate significant cartilage off-bone damage with the multi-factorial effect of a loading frequency of 10 Hz, in combination with an increase in substrate density (p < 0.05). The torque range and characteristics have been identified for which a cartilage replacement biomaterial should adopt, followed by the prospect to assess the coefficient of friction of a biomaterial amongst a defect. The work presented in this thesis has implications on the contribution to osteoarthritis and the manufacture of cartilage replacement biomaterial

The integration of modern technology in healthcare education, mainly through digital media platforms, is pivotal for enhancing healthcare professionals' proficiency in birth preparedness and complication readiness (BP/CR). This study aimed to assess the effectiveness of digital media interventions on BP/CR knowledge among healthcare providers, reflecting on broader implications for global healthcare delivery and patient education. In this quasi-experimental study, 70 healthcare professionals were recruited using a nonprobability convenience sampling method. The training protocol was developed after an in-depth literature review, while the evaluation questionnaire incorporated provider-level index indicators aligned with Johns Hopkins University's BP/CR guidelines to ensure a comprehensive assessment framework. The intervention utilized widely accessible digital platforms, i.e., WhatsApp and email, aimed to deliver educational content, with its effectiveness gauged through pre- and postintervention assessments analyzed using SPSS 25.00. The study findings demonstrated significant improvements in BP/CR knowledge postintervention across various care domains. The results showed significant increases in BP/CR knowledge postintervention: focused antenatal care knowledge improved from 34.00% to 69.00% (p < 0.001), and knowledge of third-stage labor management increased from 21.00% to 56.00% (p < 0.001). Furthermore, significant improvements were also observed in neonatal care and emergency obstetric and neonatal care management knowledge (p < 0.001). These results affirm the role of digital media in improving healthcare education, suggesting its integration into healthcare policies to reduce maternal mortality and support professionals in resource-limited settings

The goal of the research, "Lexical Analysis of English Borrowings in Balochi Language," was to close the theoretical divide between English borrowing and linguistic contact in Pakistan's regional languages. The purposes and varieties of English borrowings in the Balochi language. The nature of the investigation was qualitative. In order to examine English borrowings in the text, sixteen short stories by a well-known Balochi author Saba Dashtyari were purposefully chosen as a sample. Students at the BS level are taught the short tales chosen for analysis as part of the Balochi course curriculum. Thematic analysis was used to examine the texts. After the chosen publications were analyzed, over 65 instances of borrowed English terminology were found. It was discovered that the bulk of the imported terms (20%) had Balochi counterparts. Merely 80% of the terms were found to have no counterpart in the Balochi language. The study's conclusions also made clear that borrowed terms were accepted in a variety of real-world contexts, such as politics, sports, education, science and technology, and medical. This suggests that English has had a significant effect on Balochi language and literature. The study's conclusions have consequences for educators and legislators who create the Balochi courses that are taught at the college and university levels. The research came to the conclusion that by describing how borrowing might impact local languages, authors, students, and speakers of those languages can all learn about various facets of borrowing

A survey-based study of challenges of multilingualism in Pakistan

No full text
This paper aims to investigate how people face different challenges in a multilingual and multicultural society, especially in Pakistan. The English enjoys official language status, while Urdu is the national language in Pakistan. Minority languages, on the other hand, are a sign of stigma and destitution, and most speakers of minority languages face discrimination. In this paper, a binary approach of qualitative and quantitative is used to explore multilingualism in Pakistan. In order to understand how Pakistan's language policy favors some languages over others and what impact that has appeared on politics, society, education, and the economy. A closed-ended questionnaire was given to 30 participants as part of the quantitative data gathering process utilizing the purposive sampling technique. In the qualitative phase, researchers used content analysis as a strategy to gather information from secondary sources in search of strategies and solutions to the challenges. It also briefly examines the effects of globalization on Pakistani languages. The findings of the study revealed that there is a need to use such strategies that help overcome these challenges and help in the development of English as an international language. Future researchers can undertake a longitudinal study to investigate the language development and experiences of multilingual persons in Pakistan and they can perform a comparative examination of multilingualism in various regions of Pakistan

No full text
This exploratory study has investigated the relevance of the translation universals hypothesis of normalization to the English Urdu language pair genre specific parallel, comparable corpus (USTC). The corpus compiled for this study consisted of three genres i.e legal statutes (LSC), newspaper articles (NSC) and academic prose (ASC). The study has investigated if genre specific specialized translated texts normalize in an Indo Aryan language Urdu. The analytical framework to investigate the translation specific linguistic traits is based on Zanettin(2013) WordSmith 7 has been used to find the distribution of list heads for the purpose of analysis . The analysis has revealed that the translated Urdu component (TUT) in USTC has shown a denormalizing tendency in two sub corpus. Moreover, comparable genre specific non translated language (CUT) and general Urdu language corpus components REF(GEN) also favor the same tendency in all the supcorpora. An adherence to generic conventions, the context of the production and consumption of specialized translated texts, and the content-oriented informative role of the texts have provided sufficient justification for contrary to the hypothesis findings. The study would add to the existing knowledge of translators, post-editors, trainers, contrastive genre analysts, terminology banks, and pedagogy developers in the domain of translation studies and machine translation
