    REABILITAÇÃO RESPIRATÓRIA NAS EXACERBAÇÕES DA DOENÇA PULMONAR OBSTRUTIVA CRÓNICA: UMA INVESTIGAÇÃO CENTRADA NA PESSOA

    Symptomen, functionele status en exacerbaties zijn belangrijke problemen van mensen met chronisch obstructief longlijden (COPD) en dragen in hoge mate bij aan de ziektelast; toch worden deze problemen nog steeds niet volledig begrepen. Het verder begrijpen van de symptomen, de functionele status en het beloop van exacerbaties van COPD (ECOPD), evenals het identificeren van strategieën om de last ervan te minimaliseren, zijn daarom erkende onderzoeksprioriteiten. Deze thesis had als doel i) de symptomen en functionele status bij mensen met COPD te begrijpen, ii) de impact van ECOPD vanuit klinisch en patiëntperspectief vast te stellen en iii) te onderzoeken hoe respiratoire revalidatie (RR) deze problemen kan tegengaan. Een narratieve review (Hoofdstuk 2), een cross-sectionele studie (Hoofdstuk 3) en een secundaire analyse van deze studie (Hoofdstuk 4), een scoping review (Hoofdstuk 5), een kwalitatieve studie (Hoofdstuk 6), een systematische review (Hoofdstuk 7) en een gerandomiseerde gecontroleerde studie (Hoofdstuk 8) werden uitgevoerd. De hoofdstukken 2, 3 en 4 gingen over doel i), de hoofdstukken 5 en 6 gingen over doel ii), en de hoofdstukken 2, 6, 7 en 8 gingen over doel iii). Hoofdstuk 2 toonde de hoge prevalentie van respiratoire en niet-respiratoire symtpomen, hun onderliggende mechanismen en impact op het dagelijks leven van mensen met COPD, en de rol van interdisciplinaire RR bij het effectief aanpakken van de complexiteit van deze symptomen. Hoofdstuk 3 toonde aan dat mensen met COPD, ongeacht de ernst van de ziekte, een verminderde functionele status hebben en dat deze beperking niet nauwkeurig kan worden ingeschat op basis van andere patiënt gerapporteerde of klinische uitkomsten. Hoofdstuk 4 toonde aan hoe het gebruik van verschillende criteria een invloed heeft op de classificatie van functionele beperkingen en identificeerde de meest consistente methoden om functionele beperkingen bij mensen met COPD te bepalen. Hoofdstuk 5 toonde aan dat ECOPD resulteert in een acute verslechtering van de symptomen en de impact van de ziekte evenals een afname van de functionele status, die op de lange termijn aanhoudt en bijdraagt aan een afname van de kwaliteit van leven. Hoofdstuk 6 toonde aan dat ECOPD die poliklinisch worden behandeld een enorme negatieve impact hebben op het leven, wat tot uiting komt in symptomen, fysiologische veranderingen, beperkingen in de activiteiten van het dagelijks leven, sociale beperkingen, psychologische en emotionele uitdagingen en gezinsproblemen. Hoofdstuk 7 toonde aan dat RR veilig is bij mensen met ECOPD en dat programma's voor deze populatie moeten bestaan uit oefhentherapie, ademhalingstechnieken, voorlichting en psychosociale ondersteuning. Ten slotte blijkt uit Hoofdstuk 8 dat een drie weken durend RR in de thuissetting voor mensen met een ECOPD die poliklinisch werd behandeld, veilig is, effectiever is dan alleen farmacologische behandeling, en resulteert in positieve kortetermijnseffecten en ervaren impact op symptomen, fysieke activiteit, activiteiten van het dagelijks leven, functionele capaciteit en gezondheidsstatus. Deze thesis biedt belangrijke informatie over de uitgebreide evaluatie en behandeling (via RR) van mensen met COPD, met name tijdens ECOPD, en ondersteunt de implementatie van RR-programma's voor ECOPD (die poliklinisch worden behandeld). Deze nieuwe kennis kan worden gebruikt als leidraad voor toekomstige interventies, richtlijnen en politieke beslissingen om het bewustzijn en de toegang tot RR voor alle mensen met ECOPD te vergroten.Embarg

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     Variations on the Author

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    Lead as an environmental toxicant in models of synucleinopathies

This is an open access article under the CC BY license (http://creativecommons.org/licenses/by/4.0/).Lead, a toxic heavy metal, is prevalent in various industrial applications, contributing to environmental contamination and significant health concerns. Lead affects various body systems, especially the brain, causing long-lasting cognitive and behavioral changes. While most studies have focused on continuous lead exposure, intermittent exposure, such as that caused by migration or relocations, has received less attention. Importantly, lead exposure intensifies the severity of Parkinson's disease (PD) and dementia with Lewy bodies, diseases involving the accumulation of alpha-synuclein (aSyn) in the brain and in the gut. Although the precise mechanisms underlying these observations remain unclear, oxidative stress and mitochondrial dysfunction likely play a role. Here, we investigated how two different profiles of lead exposure - continuous and intermittent - affect models of synucleinopathies. We found that lead exposure enhances the formation of aSyn inclusions, resulting in an increase in both their number and size in cell models. In addition, we found that animals injected with aSyn pre-formed fibrils display serine 129-phosphorylated aSyn inclusions and a reduction in astrocytes in the substantia nigra. These animals also display neuronal damage and alterations in locomotor activity, exploration behavior, anxiety, memory impairments and hypertension. Our results suggest a mechanistic link between environmental lead exposure and the onset and progression of diseases associated with aSyn pathology. Understanding the molecular and cellular interactions between lead and aSyn is crucial for shaping public health policies and may provide novel insight into strategies for mitigating the impact of environmental toxins on neurodegenerative processes involved in Parkinson's disease and related synucleinopathies.L.S. was supported by the Foundation for Science and Technology (FCT), SFRH/BD/143286/2019.
