Tinnitus psychopharmacology: A comprehensive review of its pathomechanisms and management

Psychopharmacologic agents, although not being the first-line treatment for tinnitus, may modulate cochlear neurotransmission, thereby influencing the subjective tinnitus experience.Method: A comprehensive review of MEDLINE literature (from January 1990–January 2010) was performed searching for: “tinnitus”, major classes of psychopharmacological agents, and psychiatric disorders. The most relevant clinical evidence is reported briefly along with a concise description of the main neurotransmitters purported to be involved in tinnitus, in order to provide the reader with a rational evaluation of tinnitus therapy with psychopharmacological agents. Results: Although strong methodological issues limit the reliability of the current results, a broad number of psychopharmacological agents have already been considered for tinnitus, both as candidate triggers or potential therapies.Conclusions: Selected psychopharmacological drugs may play a role in the clinical management of this disorder. While the rational use of these agents for the treatment of tinnitus should not be overlooked, research should be undertaken on their neuromodulating actions at the cochlea. Statistical mechanics of socio-economic systems with heterogeneous agents
We review the statistical mechanics approach to the study of the emerging collective behavior of systems of heterogeneous interacting agents. The general framework is presented through examples is such contexts as ecosystem dynamics and traffic modeling. We then focus on the analysis of the optimal properties of large random resource-allocation problems and on Minority Games and related models of speculative trading in financial markets, discussing a number of extensions including multi-asset models, Majority Games and models with asymmetric information. Finally, we summarize the main conclusions and outline the major open problems and limitations of the approach
Network Access Capacity Estimation through Passive Traffic Measurement
This work proposes models, techniques and tools aimed at passively estimating the maximum achievable downlink network-layer bandwidth (capacity) of an access link to the Internet from inside a network. The Internet access capacity estimation by mean of passive measurements is an interesting issue from a scientific and from an industrial perspective.
From a scientific perspective the problem, still open, is of interest because of the packet based and best effort based nature of the TCP/IP, that makes the user perceived rate depend not only on the access rate but also on the backbone performance and on the endpoint server performance. Extracting the access rate from TCP/IP observations requires the development of appropriate models, algorithms and techniques.
From an industrial perspective the problem is relevant considering the Internet’s evolution is at a point at where the TCP/IP suite protocols best effort nature needs to be paired with appropriate provisioning policies based on enforceable service level agreements (SLA) between service providers and service users and/or between different types of service providers. The availability of widely accepted techniques to measure the service levels is fundamental to such an evolution.
This thesis proposes a method that extends the well-known packet-pair approach to network capacity estimation by considering longer TCP packet sequences to minimize the impact of measurement noise and to obtain reliable estimation without the need of a large amount of data. In order to obtain such a result, the method augments the traditional packet timing analysis with a model driven data analysis, similar to what is done in pattern recognition to extract known items from large data sets (for example to recognize known objects in images). A two layer process is proposed, in which the first layer performs packet timing analysis, based on statistical techniques, to extract the main traffic features, while the second layer combines the features extracted through appropriate heuristics, to compute the access capacity. The second layer takes advantage of the knowledge of the application environment (i.e. the way TCP/IP networks behave) to analyze the features in a global way, so as to eliminate the false features, i.e. those deriving from noise or artifacts, and identify the true features, i.e. those deriving from the known and expected network behaviour.L’utente di una linea di accesso a Internet (ad es. l’utente di una ADSL) è sensibile alla qualità del servizio, che è determinata da vari aspetti, tra cui la banda disponibile. La stima per mezzo di misure passive della massima banda ottenibile a livello IP (capacity) su un link di accesso ad una rete TCP/IP è un problema interessante sia dal punto di vista scientifico che considerando le sue applicazioni industriali.
Dal punto di vista scientifico, il problema è interessante perché l’estrazione della capacity a partire da osservazioni passive sul TCP/IP richiede lo sviluppo di modelli e algoritmi appropriati.
Dal punto di vista industriale, la stima della banda di accesso è uno strumento fondamentale per la verifica delle condizioni minime di servizio che possono essere stipulate tra un internet service provider ed un utente finale o tra il fornitore dell’accesso (access service provider) e il fornitore della connettività Internet (network service provider).
La tesi propone dei modelli e delle tecniche aventi lo scopo di stimare dall’interno della rete e in maniera passiva la capacity di un link di accesso a Internet. Il metodo proposto estende i tradizionali approcci basati sulle tecniche packet-pair considerando sequenze di pacchetti TCP più lunghe di due elementi allo scopo di minimizzare l’impatto del rumore di misura. A scalable algorithm to explore the Gibbs energy landscape of genome-scale metabolic networks
The integration of various types of genomic data into predictive models of biological networks is one of the main challenges currently faced by computational biology. Constraint-based models in particular play a key role in the attempt to obtain a quantitative understanding of cellular metabolism at genome scale. In essence, their goal is to frame the metabolic capabilities of an organism based on minimal assumptions that describe the steady states of the underlying reaction network via suitable stoichiometric constraints, specifically mass balance and energy balance (i.e. thermodynamic feasibility). The implementation of these requirements to generate viable configurations of reaction fluxes and/or to test given flux profiles for thermodynamic feasibility can however prove to be computationally intensive. We propose here a fast and scalable stoichiometry-based method to explore the Gibbs energy landscape of a biochemical network at steady state. The method is applied to the problem of reconstructing the Gibbs energy landscape underlying metabolic activity in the human red blood cell, and to that of identifying and removing thermodynamically infeasible reaction cycles in the Escherichia coli metabolic network (iAF1260). In the former case, we produce consistent predictions for chemical potentials (or log-concentrations) of intracellular metabolites; in the latter, we identify a restricted set of loops (23 in total) in the periplasmic and cytoplasmic core as the origin of thermodynamic infeasibility in a large sample (106) of flux configurations generated randomly and compatibly with the prior information available on reaction reversibility. © 2012 De Martino et al

Nell'arco di tre decenni, alla fine del secolo XV, Matteo Civitali (1436-1501) opera una sostanziale riqualificazione degli spazi interni alla Cattedrale di San Martino a Lucca. Con una serrata successione di interventi (altari, cappelle, monumenti, innesti architettonici), l'artista ridisegna l'interna architettura del duomo rinnovandola integralmente. Questa architettura intesa come arredo totale, sintesi di varie arti, diviene la formula vincente con cui Civitali si impone sulla scena artistica quattrocentesca.In the space of three decades, at the end of the 15th century, Matteo Civitali (1436-1501) carried out a substantial redevelopment of the interior spaces of the Cathedral of San Martino in Lucca. With a tight succession of interventions (altars, chapels, monuments, architectural grafts), the artist redesigns the interior architecture of the cathedral, renewing it completely. This architecture understood as total furnishing, synthesis of various arts, becomes the winning formula with which Civitali imposes itself on the artistic scene of the fifteenth century
Adding 5-hydroxytryptamine receptor type 3 antagonists may reduce drug-induced nausea in poor insight obsessive-compulsive patients taking off-label doses of selective serotonin reuptake inhibitors: a 52-week follow-up case report
Abstract Poor-insight obsessive-compulsive disorder (PI-OCD) is a severe form of OCD where the 'typically obsessive' features of intrusive, 'egodystonic' feelings and thoughts are absent. PI-OCD is difficult to treat, often requiring very high doses of serotonergic drugs as well as antipsychotic augmentation. When this occurs, unpleasant side effects as nausea are common, eventually further reducing compliance to medication and increasing the need for pharmacological alternatives. We present the case of a PI-OCD patient who developed severe nausea after response to off-label doses of the selective serotonin reuptake inhibitor (SSRI), fluoxetine. Drug choices are discussed, providing pharmacodynamic rationales and hypotheses along with reports of rating scale scores, administered within a follow-up period of 52 weeks. A slight reduction of fluoxetine dose, augmentation with mirtazapine and a switch from amisulpride to olanzapine led to resolution of nausea while preserving the anti-OCD therapeutic effect. Mirtazapine and olanzapine have already been suggested for OCD treatment, although a lack of evidence exists about their role in the course of PI-OCD. Both mirtazapine and olanzapine also act as 5-hydroxytryptamine receptor type 3 (5-HT3) blockers, making them preferred choices especially in cases of drug-induced nausea.</p

L’indagine si concentra in primo luogo sul trasferimento d’azienda, esaminando la normativa e la giurisprudenza europee per passare poi alla disciplina di diritto interno, alle procedure sindacali e a uno specifico focus sul trasferimento delle aziende in crisi.
Successivamente l’autore si sofferma sull’appalto, prendendone in particolare considerazione gli indici di genuinità, i criteri di distinzione dalla somministrazione illecita di manodopera e la tutela delle maestranze in caso di avvicendamento fra imprese.

Lo studio propone infine una prima lettura storico-artistica dell'opera, attraverso l'individuazione di una nutrita rosa di possibili confronti che ne supportano l'ipotesi di una datazione restringibile ai due decenni a cavallo tra il XV e il XVI secolo
