    Imaging techniques are a keystone for diagnosing and monitoring spinal disorders and are widely used in both clinical practice and research studies. Planar X-rays are employed to assess spinal morphology, to assess spinal motion in cases of suspect instability and after implantation of motion-preserving devices, and to identify bone-related disorders such as fractures. Because of the possibility of acquiring scans in various postures, including standing, planar X-rays are widely used to measure relevant anatomical parameters in spinal deformities such as scoliosis and sagittal disorders. Magnetic resonance imaging, which does not employ ionizing radiation and is the gold standard for the characterization of intervertebral disc degeneration, is commonly used for soft tissue imaging. Other frequently used techniques include computed tomography, ultrasonography, and surface topography. After a brief introduction of the various techniques, this chapter describes their uses in spinal imaging, with a special focus on the aspects that are particularly important in biomechanical in vivo studies, such as the analysis of spinal motion

    Bleeding and Hemostasis in Acute Renal Failure

    Hemostasis and coagulation disorders are common and potentially life-threatening complications of acute renal failure, which are observed more frequently in the critical care setting. On one hand, patients with acute renal failure are more prone to bleeding (with manifestations ranging from mild skin/mucosal to overt gastrointestinal hemorrhage). On the other hand, studies in this same population showed an increased incidence of arterial and venous thrombosis. The reasons of this apparent paradox can be understood by analyzing the pathogenesis of these two conditions, which is multifactorial and extensive. Increased bleeding susceptibility has been linked to thrombocytopenia, platelet dysfunction, anemia, and use of anticoagulants, which can be due to the patient conditions or to the medical interventions that such conditions imply. A similar approach can be used to explain thrombosis in this setting: inflammation, endothelial dysfunction, and coagulation activation can result from uremia, other critical illnesses, and the use of hemodialysis. In this chapter, we review the key concepts of normal hemostasis, coagulation, and fibrinolysis, and we discuss the recent advances on the pathogenesis of bleeding and thrombosis in acute renal failure

    Design concepts in lumbar total disc arthroplasty

    The implantation of lumbar disc prostheses based on different design concepts is widely accepted. This paper reviews currently available literature studies on the biomechanics of TDA in the lumbar spine, and is targeted at the evaluation of possible relationships between the aims of TDA and the geometrical, mechanical and material properties of the various available disc prostheses. Both theoretical and experimental studies were analyzed, by a PUBMED search (performed in February 2007, revised in January 2008), focusing on single level TDA. Both semi-constrained and unconstrained lumbar discs seem to be able to restore nearly physiological IAR locations and ROM values. However, both increased and decreased ROM was stated in some papers, unrelated to the clinical outcome. Segmental lordosis alterations after TDA were reported in most cases, for both constrained and unconstrained disc prostheses. An increase in the load through the facet joints was documented, for both semi-constrained and unconstrained artificial discs, but with some contrasting results. Semi-constrained devices may be able to share a greater part of the load, thus protecting the surrounding biological structure from overloading and possible early degeneration, but may be more susceptible to wear. The next level of development will be the biomechanical integration of compression across the motion segment. All these findings need to be supported by long-term clinical outcome studies
