    Care associated infection (ICA): clinical risk assessment Annamaria Vullo Dipartimento di Scienze anatomiche, istologiche, medico-legali e dell'apparato locomotore, Sapienza - Università di Roma The aim of this study is to evaluate the risk of care associated infection (ICA) during the ordinary activity. The study include a retrospective analysis about epidemiological and statistical data collected by simple operative unit (UOS) of Igiene and hospital techniques at Sant’Andrea Hospital in Rome . The time of observation included five years 2011-2015. The cases were 2.495 patients (53% male and 47% female) and were collected in clinical Departments. The analysis was elaborated on 255 records: 21 are related to professional responsibility and 21 to ICA. By our preliminary results we speculate the importance of the quality of the hospital assistance; in particular we observed a higher risk of infection in the patients with deteriorated clinical condition. Moreover the correct application of the protocols for the use of disinfectants and antibiotics, the respect of criteria for the use of drugs and invasive procedures plays an important role as regards the transmission of pathogens and causes of ICA. In conclusion, the adoption of a risk management policy based on the synergy between risk assessment adhering to the real business context and the adoption of appropriate organizational models (Procedures-Protocols) as well as future regulatory references being approved by the Senate acts for the prevention and control of the ICA. It will represent the best strategy by the Hospital Companies will be able to guarantee the quality of the assistance and where it is necessary to demonstrate in court the correctness and completeness of the clinical and organizational models adopted

    Nuove Prospettive diagnostiche dell'infezione da Pnumocystis carini

    Nel corso di recenti ricerche sono emersi alcuni dati di rilevante interesse: 1- Evidenziazione di P. carinii in soggetti HIV positivi asintomatici ed HIV negativi esenti da infezioni respiratorie in atto (1). Sono in corso a tal proposito ricerche sistematiche volte a stabilire il significato del riscontro di P. carinii in secrezioni broncoalveolari di pazienti con o senza infezione da HIV, dell'eventuale presenza di stipiti differenti del microrganismo e del loro ruolo nella malattia; 2- Isolamento di P. carinii da cellule mononucleate di pazienti di AIDS con PCP, coltivate su linea A549 di derivazione umana (2). Tale sistema, che ha consentito di porre diagnosi di PCP in una vasta percentuale di casi, potrebbe rappresentare, se i risultati verranno confermati da più ampie casistiche, un approccio alternativo o complementare alla diagnosi dell'infezione da P. carinii. 3- Evidenziazione di alcune componenti ad attività proteasica (MW 100-50 kDa) mediante copolimerizzazione del substrato in gel di poliacrilamide (3). L'impiego di questa metodica ha permesso di visualizzare in Pneumocystis 6 bande ad attività proteasica. In particolare, é stato osservato che la banda 90 kDa viene inibita dall'EDTA, inibitore selettivo delle metalloproteasi. La purificazione di tale banda mediante cromatografia su HPT, suggerisce che questa proteasi é fortemente basica in quanto eluita da cationi bivalenti.     Structural analysis of riveted structures using a new FE modelling technique

    A theoretical approach, in order to define the structural behaviour of riveted joints, is presented. The closed form solutions lead to the definition of a Rivet Element useful to FE models of multi-riveted structures. The objective is an accurate evaluation of the local stiffness of riveted joints in FE analysis, which is fundamental to perform a reliable simulation of multi-joint structures and, consequently, a good estimate of loads acting on connections; this makes it possible to introduce new general criteria allowing, for example, to predict fatigue behaviour. On the other hand, a low number of degrees of freedom is needed when several connections are present in a complex structure. The goal is to reach a reliable model of the rivet region which can be used as the basis to develop a Rivet Element in FE analysis. The proposed Rivet Element combines the precision in the simulation with a very limited number degrees of freedom in the finite element model of a complex structure having several rivets. In the present paper the structural behavior of two simple riveted specimens is investigated experimentally and numerically using a new Rivet Element. A comparison with a joint model performed with very refined non-linear 3D models of rivet and with experimental data is performed and a good agreement is shown.</jats:p

    Performance di frenatura di un treno merci con trazione distribuita

    Il presente lavoro focalizza l’attenzione sulle performance di frenatura e sulla dinamica longitudinale di un convoglio ferroviario formato dall’unione di due sotto-treni identici, a configurazione di un treno con trazione e frenatura distribuite, avente lunghezza di 1500 m e massa di circa 3500 t e dotato di comunicazione radio tra le due locomotive. Attualmente non esistono treni del genere in Europa, ma essi contribuiranno, con la loro prossima entrata in circolazione, ad aumentare l’efficienza del trasporto ferroviario merci, specialmente per lunghe distanze. Il lavoro stimolato dal grande interesse europeo, propone una modalità operativa attraverso la quale il treno possa circolare, anche in condizioni degradate, cioè senza la comunicazione radio tra le locomotive. Infine, si dimostra come, attraverso un’opportuna distribuzione, delle masse sia possibile ottimizzare fortemente la dinamica longitudinale del treno.This paper is focused on the braking performance and on the longitudinal dynamics of a train-set built by coupling two equal sub-trains in way of assembling a train with distributed traction and braking, having overall length and mass of 1500 m and 3500 t, respectively, and provided by a radio connection between the locomotives. Nowadays such trains do not exist in Europe, but they will contribute with their circulation to the increase of railway freight efficiency, especially on long distance routes. This paper, claimed by an high European interest on the subject, proposes an operative rule by means of which the train can run also in degraded mode, i.e. without the radio communication among the locomotives. Lastly, it is shown that by means of an appropriate mass distribution it is possible to strongly optimize the longitudinal dynamics of the train

NOV.&lt;/i&gt; &lt;p&gt; &lt;i&gt;Type species: Pietraroiasuchus ormezzanoi&lt;/i&gt;&lt;/p&gt; &lt;p&gt; &lt;i&gt;Diagnosis of the genus:&lt;/i&gt; Small crocodylomorph about 1 m long, with flat skull (rostrum and mandible) and round rostral contour. Rostral profile convex in front of orbits; orbits with anterior rectilinear contour; premaxillary constituting about one-third of rostral length; maxillary alveoli medially placed and laterally bordered by a ridge (shared with &lt;i&gt;Pachycheilosuchus&lt;/i&gt;); three final maxillary teeth medially arranged in a platform (autapomorphy); maxillary bone with minimal sculpture and pitting aligned along lateral margins; maxillae send a short caudal process between the lachrymal and prefrontal, thus avoiding lachrymonasal contact; maxillary tooth row ends well anterior to posterior tip of maxilla; posteromedial edge of maxillae forming the margin of a depression or fossa in front of the maxillojugal suture; prefrontal and lachrymal about same length; preorbital fenestra slit-like; choana in pterygoid, anteriorly placed, and bordered by palatines; compound quadrate&ndash; quadratojugal condyles (autapomorphy); furrowed quadratojugal and quadrate suture (autapomorphy); pterygoidean flange not reaching laterally the medial quadrate condyle (shared with &lt;i&gt;Iharkutosuchus&lt;/i&gt;); lateroventral protuberance in exoccipitals form a vertical ridge (shared with &lt;i&gt;Hylaeochampsa&lt;/i&gt; and &lt;i&gt;Iharkutosuchus&lt;/i&gt;); unforked and unique posterior dentary process (shared with &lt;i&gt;Pachycheilosuchus&lt;/i&gt;); medial extension of dentary; surangular anterodorsally expanded; posterodorsal smooth dentary projection lacking alveolar setting; homodont dentition; absence of external mandibular fenestra.
