A Radiomics-Based Machine Learning Approach for Coronary Stenosis Assessment from Coronary Computed Tomography Angiography
A robust radiomic-based machine learning approach to detect cardiac amyloidosis using cardiac computed tomography
Introduction: Cardiac amyloidosis (CA) shares similar clinical and imaging characteristics (e.g., hypertrophic phenotype) with aortic stenosis (AS), but its prognosis is generally worse than severe AS alone. Recent studies suggest that the presence of CA is frequent (1 out of 8 patients) in patients with severe AS. The coexistence of the two diseases complicates the prognosis and therapeutic management of both conditions. Thus, there is an urgent need to standardize and optimize the diagnostic process of CA and AS. The aim of this study is to develop a robust and reliable radiomics-based pipeline to differentiate the two pathologies. Methods: Thirty patients were included in the study, equally divided between CA and AS. For each patient, a cardiac computed tomography (CCT) was analyzed by extracting 107 radiomics features from the LV wall. Feature robustness was evaluated by means of geometrical transformations to the ROIs and intra-class correlation coefficient (ICC) computation. Various correlation thresholds (0.80, 0.85, 0.90, 0.95, 1), feature selection methods [p-value, least absolute shrinkage and selection operator (LASSO), semi-supervised LASSO, principal component analysis (PCA), semi-supervised PCA, sequential forwards selection] and machine learning classifiers (k-nearest neighbors, support vector machine, decision tree, logistic regression and gradient boosting) were assessed using a leave-one-out cross-validation. Data augmentation was performed using the synthetic minority oversampling technique. Finally, explainability analysis was performed by using the SHapley Additive exPlanations (SHAP) method. Results: Ninety-two radiomic features were selected as robust and used in the further steps. Best performances of classification were obtained using a correlation threshold of 0.95, PCA (keeping 95% of the variance, corresponding to 9 PCs) and support vector machine classifier reaching an accuracy, sensitivity and specificity of 0.93. Four PCs were found to be mainly dependent on textural features, two on first-order statistics and three on shape and size features. Conclusion: These preliminary results show that radiomics might be used as non-invasive tool able to differentiate CA from AS using clinical routine available images
A deep learning approach for metal artifact reduction in cardiac computed tomography: a proof-of-concept for radiomic application
LAUREA MAGISTRALEGli artefatti metallici nelle scansioni di tomografia computerizzata cardiaca (CCT) causati da dispositivi impiantati, come i defibrillatori, rappresentano una sfida critica, in quanto degradano la qualità delle immagini e riducono l'affidabilità diagnostica. Le tecniche tradizionali, come il completamento del sinogramma, mitigano questi problemi ma spesso introducono nuovi artefatti o comportano una perdita di dettagli. Le tecniche di deep learning (DL), come residual learning CNNs e CycleGAN, sono emerse come alternative più potenti. Questi metodi richiedono dati accoppiati— immagini CT con artefatti e immagini di riferimento prive di artefatti—per l’addestramento delle reti. La maggior parte degli studi si concentra sugli artefatti metallici causati da protesi d’anca o otturazioni dentali, mentre un solo caso in letteratura affronta il campo cardiaco, evidenziando una significativa lacuna in quest'area. Questa tesi esplora l'uso delle reti neurali per la riduzione degli artefatti metallici (MAR) nella tomografia computerizzata cardiaca (CCT), con particolare attenzione alla parete ventricolare sinistra, che costituisce la regione di interesse per un'ulteriore analisi radiomica. Gli artefatti metallici, causati da impianti come i defibrillatori impiantati, compromettono la qualità dell'immagine e ostacolano il processo diagnostico. Questa ricerca sviluppa e valuta una rete neurale convoluzionale per migliorare la qualità delle immagini cardiache influenzate da artefatti metallici. In questo studio sono stati inclusi 40 pazienti con fibrillazione ventricolare idiopatica e defibrillatore cardiaco impiantato, insieme a 13 famigliari. È stato costruito un dataset di dati simulati con artefatti partendo dalle CCT dei familiari (CCT pulite) e dagli artefatti derivati dalle immagini dei pazienti. Questo set di dati simulato include un totale di 3140 scansioni, divise in 2595 per l'addestramento e 545 per il test. Infine, le caratteristiche radiomiche sono state estratte dalle CCT originali dei famigliari e dalle immagini simulate dopo la riduzione degli artefatti. I risultati dimostrano che il modello proposto supera i metodi tradizionali, ottenendo un indice di similarità strutturale (SSIM) di 0.889±0.04 e un rapporto segnale-rumore di picco (PSNR) di 31.93±2.01 superiori, con un errore quadratico medio (MSE) di 1.05e-04±5.90e-05 inferiore. Inoltre, 374 features radiomiche su 433 sono risultate stabili (dove la stabilità è definita attraverso il coefficiente di correlazione intraclasse, ICC > 0.75) se calcolate sulle immagini simulate dopo la riduzione degli artefatti. Quest’ultimo risultato dimostra Ció dimostra ulteriormente l'efficacia della rete neurale, poiché la maggior parte delle features radiomiche estratte dalle immagini dopo la riduzione degli artefatti sono compatibili con le features estratte dalle immagini CT prive di artefatti.Metal artifacts in cardiac computed tomography (CCT) scans caused by implanted devices like defibrillators remain a critical challenge, as they degrade image quality and reduce diagnostic reliability. Traditional techniques, such as sinogram completion, mitigate these issues but often introduce new artifacts or result in a loss of detail. Deep learning (DL) techniques have emerged as more powerful alternatives. These methods require paired data—CT images with artifacts and corresponding artifact-free references—for network training. Most of these studies focus on metal artifacts caused by hip prostheses or dental fillings, while only one case in the literature addressing the cardiac field, highlighting a significant gap in this area. The aim of this study is to develop and evaluate a convolutional neural network to enhance the quality of CCT images affected by metal artifacts. A population of 40 patients with idiopathic ventricular fibrillation and implanted cardiac defibrillator are included, together with 13 family members were included in the research. A dataset of simulated data with artifacts was built starting from CCT of the family members (clean CCT) and artifacts derived from the patient scans. This simulated dataset includes a total of 3140 scans, divided into 2595 for training and 545 for test. Finally, radiomic features were extracted from the original CCT of the family members as well as from the simulated images after artifact reduction. The results demonstrate that the proposed model outperforms traditional methods, achieving superior Structural Similarity Index (SSIM), with a value of 0.889±0.04 and Peak Signal-to-Noise Ratio (PSNR), with value of 31.93±2.01, with a lower Mean Squared Error (MSE), of 1.05e-04±5.90e-05. Radiomic stability analysis reported 374 over 433 radiomic features as stable (where stability is defined by means of the intraclass correlation coefficient) when computed on the simulated images after artifact reduction. This latter results further proves the effectiveness of the neural network, as most of the radiomic features extracted from the images after artifact reduction are compatible with the features extracted from the artifact-free CT images
Automated left ventricle segmentation in cardiac CT Images using deep learning techniques
LAUREA MAGISTRALELa segmentazione del ventricolo sinistro (LV) nelle immagini di tomografia computerizzata (TC) cardiaca è un'importante attività per la diagnosi e il trattamento delle malattie cardiovascolari. La segmentazione manuale richiede molto tempo ed è soggetta a variabilità inter- e intra-osservatore. Pertanto, lo sviluppo di metodi di segmentazione accurati e automatizzati è cruciale. In questa tesi, abbiamo studiato l'uso di modelli di deep learning per la segmentazione del LV nelle immagini TC cardiache.
Abbiamo valutato tre diversi modelli di deep learning: U-Net, U-Net 2.5D e U-Net con LSTM. I modelli sono stati addestrati e valutati utilizzando un dataset di 85 pazienti, i cui LV sono stati manualmente segmentati da un radiologo esperto. Le prestazioni di segmentazione dei modelli sono state valutate utilizzando metriche quantitative come il coefficiente di Dice e il coefficiente di Intersezione sull'Unione, nonché l'ispezione visiva dei risultati di segmentazione. I nostri risultati mostrano che il modello 2.5D U-Net raggiunge le migliori prestazioni di segmentazione. La valutazione qualitativa dei risultati di segmentazione utilizzando l'ispezione visiva delle immagini e delle maschere corrispondenti rivela che il modello 2.5D U-Net produce le segmentazioni più accurate in generale, con meno errori e artefatti.
Complessivamente, i nostri risultati dimostrano il potenziale dei modelli di deep learning per la segmentazione del ventricolo sinistro nelle immagini TC cardiache. In particolare, il modello 2.5D U-Net mostra prestazioni superiori, con elevata precisione e pochi errori o artefatti.The segmentation of the left ventricle (LV) in cardiac computed tomography (CT) images is an important task for the diagnosis and treatment of cardiovascular diseases. Manual segmentation is time-consuming and subject to inter- and intra-observer variability. Therefore, the development of accurate and automated segmentation methods is crucial. In this thesis, we investigate the use of deep learning models for the segmentation of the LV in cardiac CT images.
We evaluated three different deep learning models: U-Net, 2.5D U-Net, and U-Net with LSTM. The models are trained and evaluated using a dataset of 85 patients, which are manually segmented by an expert radiologist. The segmentation performance of the models is evaluated using quantitative metrics such as the Dice coefficient and Intersection over Union coefficient, as well as visual inspection of the segmentation results.Our results show that the 2.5D U-Net model achieves the best segmentation performance.The qualitative evaluation of the segmentation results using visual inspection of the images and corresponding masks reveals that the 2.5D U-Net model produces the most accurate segmentations overall, with the fewest errors and artifacts.
Overall, our results demonstrate the potential of deep learning models for the segmentation of the left ventricle in cardiac CT images. The 2.5D U-Net model, in particular, shows superior performance, with high accuracy and few errors or artifacts
Automated left ventricle segmentation from CINE cardiac magnetic resonance images
LAUREA MAGISTRALEQuesto studio si propone di sviluppare un framework automatizzato per segmentare il ventricolo sinistro dalle immagini CINE di risonanza magnetica cardiaca, destinato a supportare la diagnosi e l'analisi delle malattie cardiovascolari. Sfruttando approcci di deep learning, sono stati implementati e confrontati due modelli, U-Net e Swin-U-Net, per l'estrazione accurata delle strutture cardiache, in particolare della regione del miocardio. Le prestazioni di queste reti sono state valutate utilizzando il coefficiente Dice e l'Intersection-over-Union (IoU), dimostrando capacità di segmentazione robuste, soprattutto con l'architettura U-Net.
Per perfezionare ulteriormente le maschere di segmentazione, è stata sviluppata una pipeline di post-elaborazione dedicata, che impiega tecniche di levigatura dei contorni e filtraggio a livello di slice. Questo garantisce che vengano mantenute solo le slice di maggiore rilevanza diagnostica durante l'intero ciclo cardiaco, migliorando la continuità dei confini, eliminando artefatti e ottenendo, in ultima analisi, segmentazioni più accurate e clinicamente significative.
Nel complesso, questo approccio multidisciplinare mira a fornire uno strumento affidabile per il supporto diagnostico nel campo cardiovascolare, contribuendo al progresso dei metodi computer-assistiti in medicina.This study aims to develop an automated framework for segmenting the left ventricle from CINE cardiac magnetic resonance images, intended to support the diagnosis and analysis of cardiovascular diseases. By leveraging deep learning approaches, two models U-Net and Swin-U-Net have been implemented and compared for the accurate extraction of cardiac structures, particularly the myocardium region. The performance of these networks has been evaluated using the Dice coefficient and Intersection-over-Union (IoU), demonstrating robust segmentation capabilities, especially with the U-Net architecture.
To further refine the segmentation masks, a dedicated post-processing pipeline was developed, employing contour smoothing and slice-level filtering techniques. This ensures that only the most diagnostically relevant slices are retained throughout the cardiac cycle, thereby enhancing boundary continuity, eliminating artifacts, and ultimately yielding more accurate and clinically meaningful segmentations.
Overall, this multidisciplinary approach seeks to provide a reliable tool for diagnostic support in the cardiovascular field, contributing to the advancement of computer-assisted methods in medicine
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Radiomic analysis of epicardial adipose tissue in cardiac MRI to improve cardiac risk assessment
LAUREA MAGISTRALEL’obiettivo di questo studio è quello di migliorare la valutazione del rischio di ospedalizzazione e morte per motivi cardiaci attraverso la selezione delle migliori features radiomiche estratte dalle immagini di risonanza magnetica di grasso epicardico. Per raggiungere questo obbiettivo sono state estratte delle features radiomiche da immagini di risonanza magnetica effettuate su 560 pazienti, tra le features ottenute sono state selezionete le più rilevanti attraverso l’utilizzo di metodi di feature selection. Per ogni paziente è stata poi calcolata una signature radiomica utilizzando un Cox model multivariato, e successivamente con il log-rank test è stata valutata la capacità della signature ottenuta di dividere i pazienti in due gruppi, uno ad alto e uno a basso rischio. I risultati dell’analisi di sopravvivenza mostrano che le signatures create, sia per l’ospedalizzazione che per la morte causate da eventi cardiaci, potrebbero essere dei buoni predittori di rischio avendo ottenuto in entrambi i casi p-value significativi (<0.05) nel log-rank test. Questo studio permette di individuare un processo per selezionare un insieme di features estratte dalle immagini di risonanza magnetica del gr per creare una signature radiomica che permetta di dividere la popolazione in due gruppi in base al rischio di ospedalizzazione e morte per motivi cardiaci.This study aims to improve the assessment of the risk of cardiac hospitalization and death by selecting the best set of radiomic features extracted from magnetic resonance images of epicardial fat. Radiomic features were extracted from MRI images of 560 patients, and the best set of features was determined by feature selection methods. A signature was created for each patient using Cox multivariate model, then the log-rank test was used to evaluate the ability of the signatures to divide high- and low-risk patients. The results of the survival analysis show that the signatures created for both hospitalization and death can be associated with the risk of the cardiac event (p-value<0.05 in the log-rank test). This study allows us to find a workflow to select a set of radiomic features and create a radiomic signature able to divide the population into two groups according to the risk of hospitalization and cardiac death
Methodological advancements in cardiovascular radiomics for enhanced disease phenotyping and risk profiling
DOTTORATOLe malattie cardiovascolari rappresentano la principale causa di mortalità globale, causando 8,9 milioni di decessi annualmente. Nonostante i notevoli progressi nelle tecnologie di imaging, come la tomografia computerizzata cardiaca e la risonanza magnetica cardiaca, l'interpretazione di queste modalità si basa ancora sulla valutazione visiva soggettiva da parte dei clinici. In questo contesto, la radiomica ha acquisito grande interesse nel campo cardiovascolare, offrendo una soluzione non invasiva mirata all'estrazione di caratteristiche quantitative dalle immagini mediche, trasformando le informazioni visive in dati numerici che possono essere elaborati e interpretati attraverso metodi di intelligenza artificiale.
Nonostante il potenziale dimostrato nella classificazione delle malattie, nella caratterizzazione delle placche e nella predizione degli esiti, la radiomica affronta ancora sfide significative per l'integrazione clinica, tra cui scarsa riproducibilità, mancanza di standardizzazione e limitata robustezza. Considerando queste lacune, questa tesi si propone di sviluppare e validare soluzioni metodologiche avanzate per la radiomica cardiaca, investigando al contempo applicazioni cardiovascolari clinicamente rilevanti.
La ricerca si concentra su tre sviluppi metodologici chiave: (i) progettare una pipeline standardizzata per lo sviluppo di modelli radiomici cardiaci con metodi robusti di estrazione e selezione delle caratteristiche; (ii) implementare strategie innovative di data augmentation per superare le limitazioni legate alle dimensioni dei dataset; e (iii) definire un framework comprensivo per l'integrazione multimodale che combini la radiomica con parametri clinici, caratteristiche derivate dal deep learning e misurazioni fisiologiche come i marcatori basati sull'elettrocardiografia.
Le soluzioni metodologiche proposte vengono testate su due problematiche cardiovascolari clinicamente rilevanti: la fenotipizzazione delle malattie cardiache e la stratificazione del rischio dei pazienti nella malattia coronarica. La prima applicazione mira a migliorare l'accuratezza diagnostica e fornire intuizioni sui pattern di malattia differenziando tra condizioni cardiache complesse, incluse l'amiloidosi cardiaca, la stenosi aortica, la cardiomiopatia ipertrofica, la sarcoidosi cardiaca e la cardiomiopatia aritmogena del ventricolo destro, mentre la seconda si concentra sulla stratificazione del rischio dei pazienti nella malattia coronarica, una delle principali cause di mortalità e disabilità a livello mondiale.
Con questi sviluppi metodologici, il lavoro presentato contribuisce a colmare il divario tra i progressi della ricerca e le applicazioni cliniche, potenzialmente migliorando la precisione diagnostica, il valore prognostico e le capacità di pianificazione del trattamento dell'imaging cardiovascolare.Cardiovascular diseases are the leading cause of global mortality, accounting for 8.9 million
deaths annually. Despite remarkable advancements in imaging technologies, such as cardiac
computed tomography and cardiac magnetic resonance, interpretation of these modalities still
relies on subjective visual assessment by clinicians. In this framework, radiomics has gained
high interest in the cardiovascular field offering a non-invasive solution aimed at extracting
quantitative features from medical images, transforming visual information into numerical data
that can be processed and interpreted through artificial intelligence methods.
Despite demonstrated potential in disease classification, plaque characterization, and outcome
prediction, radiomics still faces significant challenges to clinical integration, including poor
reproducibility, lack of standardization, and limited robustness. Given these gaps, this thesis aims
to develop and validate advanced methodological solutions for cardiac radiomics while
investigating clinically relevant cardiovascular applications.
The research focuses on three key methodological developments: (i) designing a standardized
pipeline for cardiac radiomic model development with robust feature extraction and selection
methods; (ii) implementing novel data augmentation strategies to overcome dataset size
limitations; and (iii) defining a comprehensive framework for multimodal integration that
combines radiomics with clinical parameters, deep learning-derived features, and physiological
measurements such as electrocardiographic-based markers.
The proposed methodological solutions are tested on two clinically relevant cardiovascular
issues: cardiac disease phenotyping and patient risk stratification in coronary artery disease. The
first application aims to enhance diagnostic accuracy and provide insights into disease patterns
by differentiating among complex cardiac conditions including cardiac amyloidosis, aortic
stenosis, hypertrophic cardiomyopathy, cardiac sarcoidosis, and arrhythmogenic right
ventricular cardiomyopathy, while the second focuses on patient risk stratification in coronary
artery disease, one of the leading causes of mortality and disability worldwide.
With these methodological developments, the presented work contributes to fill the gap between
research advances and clinical applications, potentially enhancing the diagnostic precision,

Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
