Longitudinal plasticity across the neural axis in acute stroke

With the advent of novel brain stimulation techniques aimed at improving functional outcome, understanding poststroke plasticity becomes critical for the appropriate selection of patients and optimal timing to introduce neuromodulatory interventions. Objective. To better define the temporal evolution of central and peripheral neuroplastic changes in the first 3 months after stroke and their clinical implications. Methods. Transcranial magnetic stimulation, peripheral nerve excitability, and clinical assessments were undertaken longitudinally in 31 acute stroke patients, comprising a total of 384 clinical studies. Results. During the hyperacute phase (<7 days), short-interval intracortical inhibition (SICI) was significantly reduced in lesioned (4.3% ± 1.3%) and contralesional hemispheres (3.6% ± 1.9%) compared with controls (11.4% ± 1.3%, P = .001). There were also significant alterations in accommodative properties of motor axons in the affected limb. At follow-up, SICI remained suppressed in both hemispheres in the context of significant clinical improvement. Conclusion. Simultaneous assessment of central and peripheral motor pathways has identified bilateral plastic changes that develop throughout the neural axis in acute stroke patients. It is proposed that these changes represent an adaptive response and that the persistent bihemispheric reduction in SICI may act to promote stroke recovery through cortical reorganization
Purple Pigments: The Pathophysiology of Acute Porphyric Neuropathy
No full textThe porphyrias are inherited metabolic disorders arising from disturbance in the haem biosynthesis pathway. The neuropathy associated with acute intermittent porphyria (AIP ) occurs due to mutation involving the enzyme porphobilinogen deaminase (PBGD) and is characterised by motor-predominant features. Definitive diagnosis often encompasses a combination of biochemical, enzyme analysis and genetic testing, with clinical neurophysiological findings of a predominantly motor axonal neuropathy. Symptomatic and supportive treatment are the mainstays during an acute attack. If administered early, intravenous haemin may prevent progression of neuropathy. While the pathophysiology of AIP neuropathy remains unclear , axonal dysfunction appears intrinsically linked to the effects of neural energy deficits acquired through haem deficiency coupled to the neurotoxic effects of porphyrin precursors. The present review will provide an overview of AIP neuropathy, including discussion of recent advances in understanding developed through neurophysiological approaches that have further delineated the pathophysiology of axonal degeneration
Botulinum toxin modulates cortical maladaptation in post-stroke spasticity
No full textIntroduction: Maladaptive plasticity involving the unaffected hemisphere (UH) in stroke patients may contribute to post-stroke deficits, including spasticity. We investigated the central and peripheral effects of botulinum toxin in post-stroke spasticity to determine whether there is modulation of cortical processes in the UH. Methods: Transcranial magnetic stimulation and peripheral nerve excitability studies were undertaken in 5 stroke patients with upper limb spasticity before (T1) and 6 weeks after (T2) botulinum injection. Results: Transcranial magnetic stimulation demonstrated inexcitable motor cortices of the affected hemisphere at T1 and T2, and short-interval intracortical inhibition (SICI) in the UH was significantly reduced at T1. At T2, SICI in the UH increased significantly compared with T1, normalizing to controls, and was found to be associated with clinical improvements in spasticity. Peripheral excitability parameters were unchanged after injection. Conclusion: Cortical excitability changes were demonstrated in UH, suggesting that the clinical benefits of botulinum toxin relate to modulation of abnormal central reorganization (maladaptive plasticity) in post-stroke spasticity. Gay communities have developed forms of service organization and responses to the epidemic based on expertise, thus receiving recognition and support from the general public as well as government agencies, while Haitian populations developed a more multi-service integrated approach, less recognized and therefore less funded. The author also describes how volunteer work, intrinsically linked to the State’s disengagement from public health, is structured in terms of gender, the labor division in the AIDS industry assigning the majority of women to positions of altruistic care-takers
Multifunctional microsphere formulation of fluorescent magnetic properties for drug delivery system
Get PDF© 2017 Author(s). The microsphere formulations of Chit/TPP/Sm/Fe3O4/Rn were prepared by an ionic gelation technique, where Chit=chitosan, TPP=tripolyphosphate, Sm=samarium and Rn=ranitidine. Optimum of microsphere formulation exhibit magnetic and fluorescent properties with adsorption efficiency of ∼92% was obtained for Chit/TPP/Sm/Fe3O4/Rn with ratio 400:500:50:1:20. Fluorescence intensity of microsphere formulations increased with the cumulative amount release of ranitidine, so that the changing of fluorescence intensity at wavelength of 590 nm referring to the Sm3+ ion could be used as indicator in DDS. With the demonstration of sustained release from microsphere formulation, it allows to investigate the applications to other drugs

clientes, sin embargo, un problema que viene arrastrando desde hace muchos años es
la relacionada con la gestión de sus reclamos, el cual en los últimos meses se han
incrementado en gran medida y a su vez ha ocasionado la insatisfacción de gran cantidad
de usuarios.El presente estudio tubo como finalidad Determinar la relación existente
entre la gestión de reclamaciones y la satisfacción del cliente en una empresa tercerizada
de telecomunicaciones, Moyobamba – 2023 para lo cual se optó por elegir un estudio de
tipo básico y diseño no experimental – correlacional, tomando para ello una población
muestral de 152 clientes a los cuales se les aplico 2 encuestas para evaluar cada una de
las variables identificando así a través del estadístico de correlación Rho de Spearman
una relación significativa entre la gestión de reclamaciones y cada una de las
dimensiones de satisfacción del cliente (tangibilidad 0.758, fiabilidad 0,692, seguridad
0,745, capacidad de respuesta 0,683 y empatía 0,625) finalmente el estudio concluye
que la gestión de reclamaciones está positivamente asociada con la satisfacción del
cliente en la organización (Rho = 0,841)

No full textHoy, que está sobre la mesa y con una condición de urgencia preguntarse sobre la enseñabilidad de nuestras músicas populares y tradicionales, así como la influencia secular y aún dominante de las lógicas de estudio centroeuropeas, este trabajo de la artista y pedagoga Cindy Gómez Gutiérrez nos da una mano de cara anuestra actitud respecto al relacionamiento que nuestros instrumentos debieran tener con la academia. Escrita en un tono académico riguroso, pero también ameno y a veces muy divertido, con unas referencias certeras, oportunas y válidas, y poniendo sobre la mesa teorías en torno a la cultura, las sociedades, la enseñabilidad en el arte y el acervo patrimonial, la autora nos invita a recuperar una actitud acaso perdida de relacionamiento con nuestro tiple. Esta tiene que vercon reivindicar —desde el ejercicio académico y apoyado desde las lógicas curriculares— las prácticas y las variantes interpretativas regionales de este instrumento, que cada vez que suena cuenta historias y leyendas sobre las que quisiéramos seguir construyendo nuestra identidad y el sentido que quisiéramos darle a nuestra condición singular de colombianos y latinoamericanos.Today, when it is a matter of urgency to ask ourselves about the teachability of our popular and traditional music, as well as the secular and still dominant influence of Central European study logics, this work, made by artist and pedagogue Cindy Gómez Gutiérrez, gives us a hand in our attitude towards the relationship that our instruments should have with the academy. Written in a rigorous academic tone, but also entertaining and sometimes very funny writing, with accurate, timely and valid references, and putting on the table theories about culture, societies, teachability in art and heritage, the author invites us to recover a perhaps lost attitude of relationship with our tiple. This has to do with vindicating the practices (from the academic exercise and supported from the curricular logics) and regional interpretative variants of this instrument, which every time it sounds tells stories and legends on which we would like to continue building our identity and the sense we would like to give to our singular condition as Colombians and Latin Americans
