Relationship between the clinical signs of submucous cleft palate and the specific symptomatology: a preventive approach.
Objetivo: Apontar a prevalência e os achados anatomofuncionais da fissura de palato submucosa (FPSM) com indicação cirúrgica, diante do quadro sintomático, valorizando o diagnóstico e o acompanhamento da evolução do paciente em uma abordagem preventiva. Modelo: Prospectivo, amostra aleatória Local: Hospital de Reabilitação de Anomalias Craniofaciais - Universidade de São Paulo, Bauru. Pacientes: Foram avaliados 50 pacientes com FPSM, sem outros comprometimentos e 7 casos com alterações somente na úvula, na faixa etária de 20 a 46 meses, por ordem de agendamento no HRAC, no período de 1 ano. Principal resultado medido: Análise e descrição quanto à presença dos sinais clínicos específicos da FPSM e à associação com a sintomatologia. Intervenções: Avaliação fonoarticulatória e inspeção clínica. Resultados: Com relação aos sinais clínicos, observaram-se 25 (50,0%)pacientes com a tríade clássica e 25 (50,0%) com 1 ou 2 sinais clínicos. Dos 50 casos de FPSM, 10 (20,0%) mostraram-se sintomáticos e 40 (80,0%), assintomáticos. Houve associação entre a presença da tríade clássica de sinais e a sintomatologia (p= 0,011), associação dos casos sintomáticos e a presença da zona translúcida (p=0,005), alterações na úvula (p= 0,046), bem como do entalhe ósseo (p=0,029). A hipernasalidade esteve presente em 100,0% dos pacientes sintomáticos. A maior parte dos 41 pacientes com fissura pré-forame procurou atendimento nos primeiros meses de vida, devido à fissura labial e apresentou 7,3% de casos sintomáticos. Já dos 9 pacientes com FPSM isolada, 6 procuraram atendimento tardiamente, devido a queixas de fala. O grupo apresentou 77,7% de casos sintomáticos. Conclusão: Uma maior divulgação dos sinais clínicos e sintomas da FPSM entre os profissionais da área de saúde é fundamental para que o diagnóstico e a definição de conduta ocorram a tempo.Objective: Point out the prevalence and anatomic functional findings of submucous cleft palate (SCP) with surgical indication in relation to the symptomatic aspect by giving importance to diagnosis and follow-up of patient\'s evolution in a preventive approach. Pattern: Prospective study, haphazard samples. Setting: Rehabilitation Hospital of Craniofacial Anomalies, University of São Paulo, Bauru, Brazil. Subjects: Fifty patients with submucous cleft palate (SCP) with no other complications and seven patients with alterations only in the uvula, all of them ranging in age from 1 year 8 months to 3 years 10 months. Main Outcome Measures: Assessment and description on the presence of specific clinical signs of SCP and the association with symptomatology. Interventions: Evaluation of articulatory aspects and clinical examination. Results: As for the clinical signs 25 patients (50%) were identified with the classic triad and 25 (50%) with 1 or 2 clinical signs. Out of 50 cases of SCP, 10 (20%) were symptomatic and 40 (80%) asymptomatic. There was an association between the presence of classic triad and symptomatology (p=0,011). It was also observed a correlation of symptomatic cases and presence of a translucid area (p=0,005) as well as a notch (p=0,029). Hipernasality was present in 100% of symptomatic patients. Most of 41 patients with pre foramen cleft searched for treatment in the first months of life due to cleft lip and showed 7,3% of symptomatic cases. In contrast, 6 out of 9 patients with isolated SCP searched for treament very late due to speech complaints and 77,7% of this group were symptomatic. Conclusion: It is essential to improve knowledge of clinical signs and symptoms of SCP among professionals involved in health services so that the diagnosis and definition of procedures can take place in time
Prevalence of the facial pattern and malocclusion in population of two different elementary schools
A avaliação facial no contexto ortodôntico frente ao diagnóstico, a elaboração de planos de tratamento e a avaliação de prognóstico torna-se indispensável. O entendimento do padrão de crescimento facial representa ser possível propor as metas terapêuticas aceitáveis para cada indivíduo. O presente trabalho objetivou identificar a prevalência de determinados padrões faciais e das más oclusões e posteriormente correlacioná-los entre si, em duas escolas distintas, uma escola particular e uma escola pública, na cidade de Bauru - SP. Os sujeitos da amostra foram selecionados por conveniência levando-se em consideração o critério de inclusão para o estudo, cujos indivíduos não poderiam ter sido ou estar sendo submetido a tratamento ortodôntico. Para tanto, foi realizada uma entrevista/anamnese com os alunos das 2 escolas de Ensino Fundamental, que cursavam de 1ª a 4ª série. Após selecionados, tais sujeitos foram divididos em dois grupos, de acordo com a escola que cursavam, e fotografados nas vistas extrabucais e intrabucais, sendo três e cinco fotos, respectivamente, e estas fotografias avaliadas por três examinadores. Os sujeitos foram determinados em tipos de padrão de crescimento facial segundo a classificação proposta por Capelozza (2004), através do exame das fotografias extrabucais de frente e de perfil. Também se verificou a presença das más oclusões no plano sagital pela classificação de Angle e as demais alterações no sentido transversal e vertical por meio das fotografias intrabucais. A avaliação dos examinadores foi submetida à análise estatística descritiva e comparativa (teste quiquadrado), além de exames intra e inter-examinadores (teste Kappa). Os resultados apontaram maior prevalência de padrão facial I e má oclusão de classe I para ambas as escolas, porém na escola particular apresentou-se seguida da classe II e na escola pública, da classe III de Angle. O padrão de crescimento facial não esteve relacionado diretamente a diferença entre as escolas, mas indiretamente devido à distribuição étnica dentro da amostra de cada uma. Já as más oclusões dentárias tendem a serem maiores na escola pública, com suposta dependência da perda dentária precoce, aceita como quebra da seqüência de irrupção ou perda dentária por outros motivos.The Facial Evaluation in the orthodontics context facing the diagnosis, the elaboration of treatment plans and prognostics evaluation is essential. The understanding of the pattern of facial growth represents to be possible to propose acceptable therapeutic goals for each and every individual. The present work aimed to identify the prevalence of certain facial patterns of malocclusion and later on to relate them among themselves, in two distinct schools, a private and a public one, in the city of Bauru - SP. The subjects of the sample have been selected by convenience considering the inclusion criteria for the study, which individuals could not have been or be subjected to orthodontics treatment. For that, it has been conducted an interview with the students of the 2 elementary schools, who were from the 1st to 4th grades. After being selected, such subjects have been divided in two groups, according to the school where they studied, and pictures of the extrabucal and intrabucal ways have been taken being three and five pictures, respectively, and three examiners have analyzed these pictures. The subjects then have been determined in types of the facial growth according to the classification, which was proposed by Capelozza (2004), through the examination of the front view and side view of the extrabucal photographs. It also has been verified the presence of malocclusion in the saggital plan through the Angle´s classification and the other alterations in the transversal and vertical direction through the intrabucal pictures. The evaluations of the examiners have been submitted to statistic descriptive and comparative analysis (qui-squared test), besides the intra and inter - examiners tests (Kappa test). The results have pointed to a greater prevalence of the facial pattern I and class I malocclusion for both schools, however in the private school it has been noticed followed by the class II and in the public school by the Angle\'s class III. The pattern of facial growth has not been directly related to the differences between the schools, but indirectly due to the ethnic distribution inside the sample of each one. As for the dental malocclusions, they tend to be greater in the public school with the supposed dependency on the early dental loss, it is acceptable as the interruption of the sequence the either the irruption or the loss of teeth by any other reasons
Influence of anteroposterior and transverse spatial relationship of maxillary segments before primary surgeries on the occlusal index in individuals with complete unilateral cleft lip and palate (CUCLP)
Introdução: Sabe-se que as cirurgias primárias de lábio e palato em indivíduos com fissura transforame incisivo unilateral (FTIU) provoca inibição do crescente deslocamento da base maxilar, bem como da região dentoalveolar. No entanto, a maxila dividida em dois segmentos diferentes, com as condições anatomicas variadas, assim como as alterações transversais e ântero-posterior, pode ser um dos fatores etiológicos para proporcionar deficiência maxilar e dentoalveolar. Proposição: Este estudo teve como objetivo avaliar, por modelos de gesso obtidos antes de cirurgias primárias de indivíduos com FTIU, a influência da relação espacial anteroposterior e transversal dos segmentos maxilares e da amplitude da fissura no crescimento maxilar e relação oclusal avaliado pelo índice de cinco anos de idade. Métodos: Um total de 357 modelos de gesso do arco maxilar, da fase prévia as cirurgias primárias de lábio e palato, foram digitalizados e avaliados quanto à amplitude da fissura e relação espacial anteroposterior e transversal dos segmentos maxilares. Os grupos para classificação foram: Grupo 1: segmentos maxilares em contato na região anterior; Grupo 2: segmentos maxilares com espaçamento entre os segmentos maxilares e diferentes graus de discrepância ântero-posterior (a = leve, b = moderada e c = grave), e Grupo 3: segmentos maxilares em colapso, com o segmento menor apresentando flexão para a região mediana. A amplitude da fissura foi classificada em estreita (E), regular (R), ampla (A) e muito ampla (AA), pelo método visual subjetivo na região anterior, média e posterior. Fotografias intra-orais aos seis anos de idade foram utilizadas para a classificação do relacionamento intra e inter arcos dentro de escores de 1 a 5, seguindo as características oclusais do índice dos cinco anos e depois relacionada com a relação espacial anteroposterior e transversal dos segmentos maxilares e com a amplitude da fissura. Resultados e discussão: Um único examinador realizou as avaliações e o teste kappa demonstrou que a concordância intra-examinador foi quase perfeita. Para a avaliação estatística entre o índice de oclusal, a relação espacial e a amplitude da fissura foi realizada o coeficiente de correlação de Spearman. Foi definida a relação espacial anteroposterior dos segmentos maxilares, demonstrando uma prevalência de 56,82% de Grupo 2b e, menos prevalente obteve-se o Grupo 3. Na análise dos índices oclusais deste trabalho observou-se a maior prevalência do índice 3, com 36,33%, cujo prognóstico para o tratamento ortodôntico é regular. Não houve significância estatística na correlação entre o índice oclusal e discrepância anteroposterior entre os segmentos maxilares. No entanto, isso não foi observado para a amplitude da fissura que estatisticamente influenciou os resultados das relações interarcos, com tendências de índices oclusais maiores em maiores amplitudes de fissura, demonstrando prognosticos de crescimento maxilar mais pobre. Conclusões: Os diferentes graus de discrepância ânteroposterior entre os segmentos maior e menor não influenciou a relação interarcos. A amplitude da fissura entre os segmentos prévio às cirurgias primárias influenciou estatisticamente na relação interarcos, com crescimento maxilar mais pobre em indivíduos que apresentaram maiores amplitudes de fissura.Background and Purpose: It is known that primary lip and palate repair in complete unilateral cleft lip and palate (CUCLP) causes inhibition of increasing displacement of the maxillary base, as well as of the dentoalveolar region. However, the maxilla divided in two different segments with varied anatomical conditions, as well as the transverse and anteroposterior changes, may be among the etiologic factors to yield deficient maxillary and dentoalveolar growth. This study aimed to evaluate, by dental casts obtained before primary surgeries of patients with CUCLP, the influence of the anteroposterior and transverse spatial relationship of maxillary segments and the cleft width on the maxillary growth and occlusal relationship scored by the Five years old Index. Methods: A total of 357 dental casts of the maxillary arch before lip and palate closure were scanned and evaluated as to the cleft width and anteroposterior and transverse spatial relationship of maxillary segments. Group 1 had maxillary segments contacting in the anterior region; Group 2 presented maxillary segments with spacing between them and different degrees of anteroposterior discrepancy (a=mild; b=moderate and c=severe), and Group 3 exhibited collapse between the two segments, being that the lesser segment presented flexure to the median region. The cleft width was scored as narrow (N), regular (R), wide (W) and very wide (WW) by the visual method in anterior, medium and posterior regions. The intraoral photographs at six years of age were used for the rating dental relationship from 1 to 5 scores, following characteristics of Five years old Index.and then relating to the spatial relationship of the anteroposterior and lateral segments jaws. Results: A single investigator performed the assessments and the kappa test showed that the intraexaminer agreement was almost perfect. The statistical evaluation between the occlusal index and the spatial relationship and cleft width was performed using the Spearman correlation coefficient. It was defined spatial relation, showing a prevalence of 56.82% 2b Group and the less prevalent obtained in Group 3. In the occlusal analysis of this work observed a higher prevalence of index 3, with 36.33%, whose prognosis for orthodontic treatment is regular. There was no statistically significant in the correlation between the occlusal and anteroposterior discrepancy between the maxillary segments. However, this was not observed for the cleft width before primary surgery, which statistically influenced the outcomes of the occlusal index. Conclusions: The different degrees of anteroposterior discrepancy of greater and lesser segments did not influence the interarch relationship. The cleft width between the maxillary segments influenced the interarch relationship, with poor maxillary growth in greater cleft widths
Evaluation of the facial esthetics of patients with complete bilateral cleft lip and palate submitted to the surgical protocol of HRAC-USP
Objetivo: avaliar por meio de fotografias de perfil e um questionário específico, a estética facial de indivíduos com fissura transforame incisivo bilateral (FTB), operados segundo o protocolo do HRAC-USP. Modelo: Estudo transversal, com análise das fotografias, atribuindo escores aos pacientes e comparando-os estatisticamente. Local de Execução: HRAC-USP. Participantes: Amostra: 30 pacientes, 24 do gênero masculino e 6 do gênero feminino, com idades entre 5 anos e 8 meses a 10 anos e 4 meses, leucodermas, com FTB . Examinadores: 5 Ortodontistas do HRAC-USP (OHRAC), 5 ortodontistas não relacionados à área de fissura (ONF), 5 Cirurgiões Plásticos do HRAC-USP (CPHRAC), 5 Cirurgiões Plásticos não relacionados à área de fissura (CPNF), 5 leigos (L) com formação superior. Variáveis: classificação da agradabilidade facial, em escala de 1 a 9, estruturas reconhecidas como responsáveis pela pior classificação. Resultados: O grupo OHRAC atribuiu mediana de 7 (aparência esteticamente agradável), o grupo CPHRAC atribuiu 5 (esteticamente aceitável). O grupo L atribuiu 4 (esteticamente aceitável). Os grupos ONF e CPNF, designaram 3 (esteticamente desagradável). Existiu diferença significante entre todas as categorias, exceto para os ONF e CPNF. Foram associadas à classificação esteticamente desagradável o nariz, o terço médio da face e o lábio superior. Conclusões: A estética facial da amostra foi classificada em agradável (grupo OHRAC), aceitável (grupos CPHRAC e L) e desagradável (grupos ONF e CPNF). Houve diferença significante entre os grupos, exceto para os ONF e CPNF. As estruturas associadas aos piores escores foram o nariz, o terço médio da face e o lábio superior.Objective: to evaluate, by means of photographs of profile and a specific questionnaire, the facial esthetics of individuals with complete bilateral cleft lip and palate (CBCLP) operated according to the protocol of HRAC-USP. Design: cross-sectional study with analysis of the photographs, by assignment of scores to the patients and statistical comparison. Setting: HRAC-USP. Participants: sample: 30 white patients with CBCLP, being 24 males and 6 females, aged 5 years 8 months to 10 years 4 months. Examines: 5 orthodontists of HRAC-USP (OHRAC), 5 orthodontists not treating patients with clefts (ONC), 5 plastic surgeons of HRAC-USP (PSHRAC), 5 plastic surgeons not treating patients with clefts (PSNC), and 5 lay graduated examiners (L). Variables: classification of facial pleasantness in a scale from 1 to 9, structures recognized as responsible for the worst classification. Results: the OHRAC group assigned a median of 7 (esthetically pleasant appearance) and the PSHRAC assigned 5 (esthetically acceptable). Group L assigned 4 (esthetically acceptable). Groups ONC and PSNC assigned 3 (esthetically unpleasant). There was a significant difference between all categories, except for ONC and PSNC. The structures associated to the esthetically unpleasant classification were the nose, middle third of the face and upper lip. Conclusions: the facial esthetics of the sample was classified as pleasant (OHRAC group), acceptable (PSHRAC and L groups) and unpleasant (ONC and PSNC groups). here was a significant difference between groups, except for the ONC and PSNC. The structures associated to the worst scores were the nose, middle third of the face and upper lip
Long axis and tipping evaluation of mandible and maxilla anterior teeth by cone beam tomography in patients with unilateral cleft lip and palate
Com a evolução da imaginologia, a tomografia computadorizada vem sendo amplamente utilizada na Odontologia. Este exame nos fornece detalhada identificação das estruturas anatômicas, e possibilita inúmeras mensurações em diversos planos, sendo uma boa indicação para avaliar indivíduos com fissura transforame incisivo, onde há a ruptura da continuidade óssea na região do rebordo alveolar e palato duro, causando várias alterações dentárias e faciais, tornando a reabilitação deste paciente um processo muito complexo. O presente estudo tem por objetivo avaliar as angulações e inclinações dentárias induzidas pelo tratamento ortodôntico corretivo, em pacientes com fissura transforame incisivo unilateral, e analisar morfologicamente o osso alveolar e contorno radicular após finalização do tratamento ortodôntico. Foram selecionados 10 pacientes, na fase de contenção após finalização do tratamento ortodôntico corretivo, e foram realizados cortes tomográficos dos seis dentes anteriores superiores e inferiores. Os dados coletados foram submetidos aos testes estatísticos. Concluímos que houve diferença significativa nas medidas do arco superior entre os lados com e sem fissuras; também foi possível a nítida visualização da relação entre as raízes dentárias com o osso alveolar. Com relação ao método utilizado, contatamos que este se mostrou confiável e reproduzível. Entretanto, novas pesquisas serão necessárias para sua consolidação.Recent developments on the field of imaginology allowed computed tomography to become largely used in dentistry. Details of anatomic structures can be visualized with high quality images and measurements can be taken on several plans helping all professionals involved on the treatment of patients with alveolar bone defect, malocclusion and facial deformities caused by cleft lip and palate. Objectives: to evaluate tooth tipping and long axis angulation of mandible and maxilla anterior teeth produced by corrective orthodontic treatment in patients with unilateral cleft lip and palate and to analyze the morphology of alveolar bone and teeth roots after orthodontic treatment in the same group of patients. Methods: Ten patients that had received orthodontic treatment at the Craniofacial Anomalies Rehabilitation Hospital (HRAC-USP) were selected and submitted to computed tomography exam. All images taken during the exam were transferred to Dolphin Imaging Software and specific areas of anterior teeth were selected to be cutted. Measurements were taken and submitted to statistical analyses. Results: There were statistical differences between measurements of anterior teeth on both sides of the upper arch and the alveolar bone and roots interface could be well visualized. Conclusion: The results showed that this method is reliable and reproducible; however, new researches are necessary
Facial and dental parameters of reference used to indicate orthodonticsurgical treatment to patients with Class II dentofacial deformities
O objetivo do presente estudo foi determinar os parâmetros morfológicos, faciais e dentários, que devem ser utilizados como referência pelo profissional, ortodontista e cirurgião, para indicação do tratamento ortodôntico-cirúrgico em pacientes com deformidades dentofaciais do Padrão II. Foram selecionados 40 pacientes brasileiros, com crescimento finalizado, 26 do gênero feminino e 14 do masculino, com ângulo de convexidade facial maior que 12º e exposição gengival no sorriso igual ou menor a 3 mm. Foram excluídos os indivíduos xantodermas e melanodermas ou aqueles que já se submeteram a qualquer cirurgia facial. Foram obtidas fotografias extrabucais de frente, de perfil e do sorriso, fotografias intrabucais frontal e laterais direita e esquerda, modelos de gesso e telerradiografias do perfil facial de todos os pacientes. Toda essa documentação foi digitalizada e os arquivos enviados para 30 professores de cursos de Pós-graduação em Ortodontia. Os avaliadores foram solicitados a determinar, em uma lista de fatores relacionados ao diagnóstico ortodôntico, os que eles consideravam de extrema, moderada ou pequena/nenhuma importância na indicação da cirurgia ortognática. Posteriormente, deveriam avaliar os arquivos com as documentações ortodônticas para classificar a estética facial (agradável; aceitável; desagradável) e a melhor opção de tratamento (tratamento ortodôntico compensatório; tratamento ortodôntico-cirúrgico; nenhum tratamento) para cada paciente da amostra. Foram obtidas as medidas das variáveis da análise facial numérica do perfil nas fotografias do perfil facial, da sobressaliência e da sobremordida nos modelos de gesso, das inclinações dos incisivos superiores e inferiores, além do tipo facial, nas telerradiografias do perfil. Testou-se, então, a associação entre cada uma das variáveis acima, além da idade e do gênero, e a indicação do tratamento ortodôntico-cirúrgico e, a partir das variáveis que apresentaram associação, estabeleceu-se uma equação matemática que permite prever a probabilidade de indicação da cirurgia para pacientes Padrão II. Os fatores considerados de extrema importância na indicação da cirurgia ortognática, por pelo menos 50% dos avaliadores, em ordem decrescente de importância, foram os seguintes: estética facial, assimetria facial, convexidade do perfil facial, proporção entre os terços faciais médio e inferior, comprimento da linha queixo-pescoço, idade, exposição gengival no sorriso, projeção anterior do mento, exposição dos incisivos em repouso e sobressaliência. As variáveis que apresentaram associação com a indicação do tratamento ortodôntico-cirúrgico foram a estética facial, o ângulo de convexidade facial total, a sobressaliência e a inclinação do incisivo superior (1.PP). O modelo matemático proposto para prever a porcentagem de indicação de cirurgia ortognática em pacientes com deformidades dentofaciais do Padrão II inclui a estética facial, a sobressaliência e o ângulo de convexidade facial total. A equação visa prover o clínico de uma ferramenta auxiliar na decisão do tratamento dos pacientes Padrão II, principalmente nos casos limítrofes.The aim of the present study was to determine the morphological parameters, facial and dental, that may be used as references by the professional, either orthodontist or surgeon, to indicate compensatory orthodontic treatment or orthodontic-surgical treatment to patients with Class II dentofacial deformities. Forty brazilian leucoderma patients have been selected, 26 female and 14 male, all of them with growth finished, convexity facial angle higher than 12° and gingival exposure at smile equal or less than 3mm. It has been excluded the ones who have gone through facial surgery. Extrabucal photographs at the frontal, profile and smile views and intrabucal at the frontal, right lateral and left lateral views, plaster casts and lateral cephalometric radiograph have been taken of all patients. This documentation was digitalized and the archives sent to 30 post-graduation course orthodontic teachers of different parts of the country. The appraisers were asked to determine, in a list of factors related to orthodontic diagnosis, the ones they considered of extreme, moderate, little or no importance at the indication of orthognathic surgery. Subsequently, they evaluated the archives with the orthodontic documentation to classify the facial esthetic (pleasant, acceptable, and unpleasant) and the best treatment option (orthodontic compensatory treatment; orthodontic-surgical treatment; no treatment) to each patient of the sample. The measurements of the variables of the numerical facial analysis of the profile have been obtained at the photographs of the facial profile, the overjet and overbite at the plaster casts, and the inclination of the upper and lower incisors and the facial type at the lateral cephalometric radiograph. Then, the association between each of the variables mentioned above, age and gender, and the indication of the orthodontic-surgical treatment were tested. From the variables that showed association, a mathematic equation that allowed to foresee the indication of surgery to patients with Class II dentofacial deformities was established. The factors considered of extreme importance to indicate orthognathic surgery for at least fifty percent of the appraisers, in decreasing order of importance were: facial esthetics, facial assimetry, convexity of the facial profile, proportion between medium and lower facial thirds, length of the chin-neck line, age, gingival exposure at smile, chin anterior projection, incisor exposure at rest and overjet. The variables that showed association with the indication of orthodontic-surgical treatment were: facial esthetics, total facial convexity angle, overjet and upper incisor inclination (1.PP). The mathematic model proposed to foresee the percentage of indication of orthognathic surgery to patients with Class II dentofacial deformities includes facial esthetics, total facial convexity angle and overjet. The equation aims to provide the clinicians with an auxiliary tool at the treatment decision of the patients with Class II dentofacial deformities, especially at borderline cases

O objetivo do presente estudo foi avaliar subjetivamente a estética do perfil facial de 30 pacientes adultos leucodermos, com fissura transforame incisivo unilateral, não sindrômicos, de ambos os gêneros, com idades entre 17 anos e 3 meses e 30 anos e 9 meses, e média de 23 anos e 3 meses, reabilitados no Hospital de Reabilitação de Anomalias Craniofaciais (HRACUSP). Foram obtidas 60 fotografias faciais da amostra selecionada, 30 do perfil direito e 30 do perfil esquerdo. Essas fotografias foram avaliadas subjetivamente por 25 examinadores divididos em 5 categorias de acordo com sua área de atuação profissional: 5 ortodontistas e 5 cirurgiões plásticos atuantes na reabilitação de fissuras no HRAC, 5 ortodontistas e 5 cirurgiões plásticos inexperientes na área de fissuras e 5 leigos com formação superior. Por meio de notas que variavam de 1 a 9, os perfis faciais foram classificados em 3 categorias, esteticamente desagradável (notas de 1 a 3), esteticamente aceitável (notas de 4 a 6) e esteticamente agradável (notas de 7 a 9). Os resultados mostraram que os perfis faciais dos pacientes foram classificados em esteticamente aceitáveis, com variações entre as categorias de examinadores. Os profissionais do HRAC, por conhecerem as limitações que o processo de reabilitação impõe aos indivíduos com fissuras, foram os mais condescendentes no julgamento estético.The aim of this study was to evaluate esthetically the facial profile of 30 adult Caucasian unilateral cleft lip and palate (CLP)patients, without syndromes, of both genders, with ages ranging from 17 years 3 months to 30 years 9 months (mean age = 23 years 3 months), rehabilitated at the Hospital for Rehabilitation of Craniofacial Anomalies (HRACUSP). Sixty profile photographs of the selected sample were taken, 30 of the right profile and 30 of the profile. These photographs were subjectively evaluated by 25 examiners divided into 5 categories according to their professional activity: 5 orthodontists and 5 plastic surgeons dealing with CLP, 5 orthodontists and 5 plastic surgeons without experience in the CLP field and 5 laymen by means of scores that ranged from 1 to 9, the facial profiles were classified into 3 categories: esthetically unpleasant (scores 1 to 3) esthetically acceptable (scores 4 to 6) and esthetically pleasant (scores 7 to 9). The findings showed that the patients facial profiles were classified as esthetically acceptable, with variations among the categories of examiners. The examiners of the HRAC gave the best scores in the esthetic judgement due to their Knowledge of limitations related to the rehabilitation process
Computed tomography: angulation and inclination of upper and lower anterior teeth in patients with bilateral cleft lip and palate
Objetivos: Avaliação quantitativa e qualitativa de um método para mensuração das inclinações e angulações dentárias por meio de tomografia computadorizada e avaliação morfológica cãs estruturas dentárias nas imagens tomográficas em pacientes com fissura transforame incisivo bilateral. Metodologia: Foram selecionados dez pacientes com fissura transforame bilateral em fase de contenção do tratamento ortodôntico para realização do exame tomográfico. Foram realizados cortes parassagitais no longo eixo dos dentes anteriores superiores e inferiores e a obtenção de uma imagem panorâmica a partir da TC para mensuração da inclinação e angulação dentária, respectivamente. As mensurações foram repetidas após um mês para cálculo do erro metodológico. As medidas feitas inicialmente foram denominadas M1 e as medidas repetidas após 1 mês M2. Resultados: Os dados coletados foram submetidos à análise estatística para cálculo dos valores de média de M1 e M2, média total (M1+M2/2) e desvio padrão da amostra e também aos testes de normalidade (Shapiro-Wilk), erro metodológico com a fórmula de Dahlberg e erro sistemático com teste t. Os dentes superiores apresentaram os valores mais discrepantes mostrando haver grande influência da fissura no posicionamento mésio-distal e vestíbulolingual ao final do tratamento ortodôntico. O arco inferior apresentou maior simetria e valores mais próximos daqueles encontrados na literatura mostrando haver pouca influência da fissura. Conclusão: O exame tomográfico forneceu imagens de excelente qualidade para avaliação da relação dente/osso e a metodologia testada mostrou-se reproduzível com valores bastante próximos entre as duas medições.Objectives: (1) To evaluate tooth tipping and long axis angulation using computed tomography (CT), after orthodontic treatment in patients with bilateral cleft lip and palate. (2) Morphological evaluation of bone and dental structures and their relationship offered by CT. Methods: Ten patients with bilateral cleft lip and palate were select. Long axis´ tomography cuts of all upper and lower anterior teeth and a panoramic view were obtained in order to measure tooth tipping and angulation. All measurements were repeated after one month (M1 and M2) and submitted to statistical analysis. Results: The results showed no statistical differences between M1 and M2, however measurements for all upper teeth were higher compared to literature showing that for these patients, variation is rule not exception. Lower teeth measurements were much more uniform and predictable. Conclusions: This method produced very accurate measurements and computed tomography provides high quality images to evaluate the relationship between roots and alveolar bone like no other source available
Stability of open bite treatment with occlusal adjustment
O objetivo desse trabalho foi avaliar cefalométrica e clinicamente a estabilidade em longo prazo do tratamento da mordida aberta anterior por meio do ajuste oclusal, bem como a sensibilidade dentinária causada por esse procedimento em longo prazo. A amostra consistiu de 17 pacientes dentre mais de 4000 pacientes tratados nos cursos de Pós-Graduação (Latu e Strictu sensu) da Disciplina de Ortodontia da Faculdade de Odontologia de Bauru - USP, com mordida aberta anterior e que haviam sido previamente tratados ortodonticamente e apresentaram recidiva do trespasse vertical negativo (em média de -1,06mm) e foram retratados com a técnica do ajuste oclusal. As alterações cefalométricas foram avaliadas em telerradiografias em norma lateral, obtidas de cada um dos pacientes antes e logo após a realização do procedimento de ajuste oclusal, e em longo prazo (média de 3,4 anos após o ajuste). Concomitantemente, foi realizada uma avaliação da sensibilidade dentinária em longo prazo (3,4 anos pós-ajuste), que foi comparada com a sensibilidade encontrada nos pacientes antes, 1,35 e 4,61 meses após a realização do ajuste oclusal. Os resultados demonstraram que algumas das alterações conseguidas com a realização do ajuste oclusal, como a diminuição da altura facial ântero-inferior (AFAI) e da altura posterior molar (APM) e o aumento do trespasse vertical apresentaram uma recidiva significante em longo prazo. Apesar disso, a maioria das alterações obtidas com o tratamento permaneceu estável em longo prazo. Os pacientes com menos de 21 anos apresentaram o mesmo comportamento em longo prazo que a amostra total. Porém, os pacientes com 21 anos ou mais apresentaram estabilidade do trespasse vertical, apesar de apresentarem também uma recidiva da AFAI e da APM em longo prazo. Houve correlação da recidiva da mordida aberta anterior com a recidiva da AFAI e com a retrusão do lábio inferior em longo prazo. Houve diferença significante da sensibilidade à mastigação, calor, frio e percussão entre as fases antes e 1,35 meses após o ajuste, porém 4,61 meses depois, a sensibilidade já havia retornado aos níveis normais prévios ao desgaste, e isto se manteve em longo prazo. Apesar da recidiva estatisticamente significante da mordida aberta anterior observada, houve estabilidade \"clinicamente significante\" em 66,7% dos casos .The objective of this study was to evaluate the cephalometric and clinical longterm stability of anterior open bite treatment with occlusal adjustment, as well as the dentinal sensitivity caused by this procedure in the long-term. The sample consisted of 17 patients among more than 4,000 treated in Graduating Program (Latu and Strictu sensu) of the Orthodontics Discipline at Bauru Dental School - USP, with open bite and who were previously treated and that presented a relapse of the negative vertical overbite (mean of -1.06mm) and were retreated with occlusal adjustment technique. The cephalometric changes were evaluated in lateral headfilms obtained from each patient before and after the occlusal adjustment procedure, and in the long-term (mean of 3,4 years after occlusal adjustment). Concurrently, dentinal sensitivity was also evaluated before, after 1.35 and 4.61 months of the occlusal adjustment and in the long-term (3,4 years after adjustment). Results demonstrated that some of the alterations obtained with the occlusal adjustment, such as the decrease of lower anterior face height (LAFH) and posterior molar height (PMH) and the increase of the overbite, presented a significant relapse in the long-term. However, most of the changes obtained with treatment remained stable in the long-term. Patients younger than 21 years-old showed the same long-term behaviour than the whole sample. Therefore, patients aging 21 years or older presented stability of the overbite, despite they also presented a relapse of LAFH and PMH in the long-term. There was correlation of open bite relapse with the LAFH relapse and with the retrusion of the lower lip in the long-term. There was significant difference of dentinal sensitivity to mastication, heat, cold and percussion among the stages before and 1,35 months after occlusal adjustment, but 4,61 months after, sensitivity returned to normal levels exhibited previous to adjustment, and this remained stable in the long-term.
