The clinical and microbiological characteristics of infections in burn patients from the Formosa Fun Coast Dust Explosion

    No full text
Background/Purpose: Bloodstream infection is a leading cause of mortality among burn patients. This study aimed to evaluate the risk factors, causative pathogens, and the relationship between bloodstream infections and other infections among burn patients from the Formosa Fun Coast Dust Explosion. Methods: This retrospective study evaluated the demographic and clinical characteristics, infection types, causative pathogen(s), and isolates' antibiotic susceptibilities from patients who were hospitalized between June 27 and September 31, 2015. Results: Fifty-eight patients were admitted during the study period (36 males, mean age: 22.6 years). The mean burned total body surface area (TBSA) was 40% for all patients. Eighteen (31%) patients with mean TBSA of 80% had 66 episodes of bloodstream infections caused by 92 isolates. Twelve (18.2%) episodes of bloodstream infections were polymicrobial. Acinetobacter baumannii (19, 20.7%), Ralstonia pickettii (17, 18.5%), and Chryseobacterium meningosepticum (13, 14.1%) were the most common pathogens causing bloodstream infections. A high concordance rate of wound cultures with blood cultures was seen in Staphylococcus aureus (3, 75%) and C. meningosepticum (8, 61.5%) infections. However, no Ralstonia isolate was found in burn wounds of patients with Ralstonia bacteremia. A high concordance rate of central venous catheter cultures with blood cultures was noted in Ralstonia mannitolilytica (5, 62.5%) and Chryseobacterium indologenes (3, 60%) infections. Approximately 21.1% of A. baumannii strains were resistant to carbapenem. All S. aureus isolates were susceptible to methicillin. Conclusions: Waterborne bacteria should be considered in patients of burns with possible water contact. Empirical broad-spectrum antibiotics should be considered for patients who were hospitalized for severe sepsis, or septic shock with a large burn. Antibiotic treatment should be administered based on the specific pathogens and their detection points. Background/Purpose: Infection is the most common cause of death following burn injury. The study was conducted to compare the diagnostic value of serum procalcitonin (PCT) with the other current benchmarks as early predictors of septic shock and bloodstream infection in burn patients. Methods: We included 24 patients admitted to the Burn Unit of a medical center from June 2015 to December 2015 from the Formosa Fun Coast dust explosion. We categorized all patients at initial admission into either sepsis or septic shock groups. Laboratory tests including the worst PCT and C-reactive protein (CRP) levels, platelet (PLT), and white blood cell (WBC) count were performed at <48 h after admission. Patients were also classified in two groups with subsequent bacteremia and non-bacteremia groups during hospitalization. Results: Significantly higher PCT levels were observed among participants with septic shock compared to those with sepsis (47.19 vs. 1.18 ng/mL, respectively; p < 0.001). Patients with bacteremia had significantly elevated PCT levels compared to patients without bacteremia (29.54 versus 1.81 ng/mL, respectively, p < 0.05). No significant differences were found in CRP levels, PLT, and WBC count between the two groups. PCT levels showed reasonable discriminative power (cut-off: 5.12 ng/mL; p = 0.01) in predicting of bloodstream infection in burn patients and the area under receiver operating curves was 0.92. Conclusions: PCT levels can be helpful in determining the septic shock and bloodstream infection in burn patients but CRP levels, PLT, and WBC count were of little diagnostic value. Ultrasonically dispersed dyed water mists as a substitute for colored powders

    No full text
AbstractColored powders such as colored corn starch are used in events such as The Color Run to blanket the air and surroundings with decorative color. Their use, however, presents a risk of dust explosion. The fact that such powders are used at events drawing large crowds makes any fire or explosion likely to result in a mass casualty incident. For instance, the Formosa Fun Coast explosion on June 27, 2015, resulted in multiple deaths and injuries. Development of alternatives to flammable/explosive colored powders would eliminate this hazard. We demonstrate as a proof-of-concept that an ultrasonically driven water mist can produce effects similar to those created by colored powders. Food coloring was dissolved in water, and the colored water was subsequently dispersed from various ultrasonic devices and photographed. Colored clouds were formed from the colored water. Colored clouds were visible under various conditions, and color combinations were possible. Possible risks of colored corn starch and water mists are discussed. An ultrasonic misting system is capable of safely replacing colored powders with regard to appearance

Emotion finds a way to users from designers: assessing product images to convey designer's emotion

Along with a growing interest in emotional design and pleasurable products, it is necessary to understand how designers are able to maintain emotional impacts of their design solutions throughout the design process, and how these solutions can find a way to evoke the intended emotional feeling of the users. The present study first examined emotional responses to early design sketches, and how these responses could be used to check if the emotional impacts of their early sketches were observed in a combinative way of cognitive-linguistic and physiological approaches. Based on the findings of the empirical study, we further discussed how the emotional differences of product images would be generated by product forms. We figured out that good form factors of early design sketches might elicit positive feelings and high arousal states, thereby incorporating semantically meaningful features in the product images

Propulsion Dynamics Analysis of Gun/Waterjet Rocket

Abstract The gas-pressurized waterjet rocket is a new product characterized with originality, interest, exercise, science and environmental friendliness in addition to education and fun. Its recent popularity is due to minimal cost, easiness of acquiring the propellant ( air and water ) and changing design. News media show that there are various models of gas-pressurized waterjet rocket. Traditional theory is not good enough to analyze these cases, because it lacks proper concepts and equations of total kinetic power. In that respect, the dynamics laboratory of National Chung-Hsing University currently leads the advanced research trend. While take advantages of Lagrangian Reynolds transport equation, momentum equation, total kinetic power, available propulsion power, and thrust power, we can make a more complete, clear, natural and systematic analysis of waterjet rocket. In this research, we enhance flight performance by improving its propulsive efficiency. In that sense, we develop the gun/waterjet rocket by the idea of farmer transplanting rice seeds. It features a long rod extruding from launcher deep into the rocket mainbody to increase initial rocket velocity on launching. Further, we analyze the flight dynamic of gun/waterjet rocket by solving the momentum and total kinetic power equation simultaneously to predict the flight histogram, computationally, and convolutionally. In this approach, we thoroughly solve the complicated internal flow field with the aid of conservation laws and isentropic process. The analysis results are reasonably agreeable to the experimental flight data. Meanwhile it proves the correctness of our theory and numerical simulation. The gun/waterjet rocket is a good model with minimal cost for demonstrating propulsion theory, it may have the potential for propelling the smokeless antitank missile and associated technology in the future.
