Effetti di un programma di attività fisica a lungo termine in centro fitness universitario per pazienti affetti da BPCO
No full textLa sedentarietà è riconosciuta quale principale fattore di rischio indipendente per l’ospedalizzazione e la mortalità in pazienti affetti da Broncopneumopatia Cronica Ostruttiva (BPCO). Infatti, in riabilitazione polmonare, la pratica di attività fisica (AF) è considerata come il miglior intervento a carattere non-farmacologico per incrementare la capacità d’esercizio e lo stato di salute generale dei pazienti con BPCO. Purtroppo, il numero limitato di programmi di assistenza sanitaria disponibili sul territorio rende difficile l’inclusione di iniziative strutturate di AF all’interno del sistema sanitario. Pertanto, dal 2005, la nostra Facoltà ha istituito il Laboratorio “Silver Fitness”, un’iniziativa di Centro Fitness Universitario (UFC) per utenti adulti e/o anziani, affetti da patologie croniche, quale le cardiovascolari, le metaboliche e la broncopneumopatia. L’obiettivo di questo studio è verificare l’efficacia di un programma di allenamento specifico per utenti affetti da BPCO, della durata di 18 mesi, al fine di valutare la possibile disseminazione dell’iniziativa sul campo pratico.
Presso l’UFC, 23 pazienti affetti da BPCO, 4 femmine e 18 maschi (media±dev.st: età 72.1±7.4 anni, BMI 28.2±3.5 kg/m2, FEV1 62.7±12.6% del predetto), sono stati coinvolti in un programma di allenamento (2 volte/settimana, 90 minuti per ciascuna sessione), basato sulle linee guida internazionali ACSM e ATS/ERS. All’inizio, a 12 e a 18 mesi sono state valutate le capacità fisiche salute correlate (Resistenza Aerobica, Forza Muscolare, Mobilità Articolare ed Equilibrio), il BMI e la funzionalità polmonare (FEV1).
Il gruppo di pazienti con BPCO presenta valori iniziali più bassi in resistenza aerobica (p<0.001), e mobilità articolare (p<0.05) rispetto a utenti affetti da altre patologie croniche. Sono stati registrati 4 abbandoni prima del termine dei 12 mesi e cinque nei successivi 6 mesi, a causa della comparsa di altre patologie. Nei 18 mesi, nessun incidente è stato registrato. La media di partecipazione alle attività del UFC è stata del 74.6%±3.24. Le assenze sono largamente riconducibili agli stati patologici (43.0%). Dopo 12 mesi di AF, il gruppo affetto da BPCO presenta un significativo incremento dei parametri al test di resistenza aerobica (5.2%; p<0.05), mentre i miglioramenti registrati sulla forza, la mobilità articolare e l’equilibrio non sono statisticamente significativi.
A 18 mesi, il gruppo incrementa la forza degli arti superiori (23.2%; p<0.05) and l’equilibrio (18.3%; p<0.05), mentre la resistenza aerobica e la mobilità articolare migliorano in modo non statisticamente significativo.
Le differenze prestative iniziali dei pazienti con BPCO rispetto a quelle degli altri gruppi confermano la necessità di prevedere una AF individualizzata, basata sulle linee guida della patologia. Sebbene l’alta prevalenza comorbidità riduca nel tempo la partecipazione, la frequenza all’iniziativa dimostra che una AF supervisionata dovrebbe essere incorporata all’interno delle strategie di trattamento a lungo-termine

For the full list of authors, see article.BACKGROUND: Tools for the prediction of atrial fibrillation (AF) may identify high-risk individuals more likely to benefit from preventive interventions and serve as a benchmark to test novel putative risk factors. METHODS AND RESULTS: Individual-level data from 3 large cohorts in the United States (Atherosclerosis Risk in Communities [ARIC] study, the Cardiovascular Health Study [CHS], and the Framingham Heart Study [FHS]), including 18 556 men and women aged 46 to 94 years (19% African Americans, 81% whites) were pooled to derive predictive models for AF using clinical variables. Validation of the derived models was performed in 7672 participants from the Age, Gene and Environment-Reykjavik study (AGES) and the Rotterdam Study (RS). The analysis included 1186 incident AF cases in the derivation cohorts and 585 in the validation cohorts. A simple 5-year predictive model including the variables age, race, height, weight, systolic and diastolic blood pressure, current smoking, use of antihypertensive medication, diabetes, and history of myocardial infarction and heart failure had good discrimination (C-statistic, 0.765; 95% CI, 0.748 to 0.781). Addition of variables from the electrocardiogram did not improve the overall model discrimination (C-statistic, 0.767; 95% CI, 0.750 to 0.783; categorical net reclassification improvement, -0.0032; 95% CI, -0.0178 to 0.0113). In the validation cohorts, discrimination was acceptable (AGES C-statistic, 0.664; 95% CI, 0.632 to 0.697 and RS C-statistic, 0.705; 95% CI, 0.664 to 0.747) and calibration was adequate. CONCLUSION: A risk model including variables readily available in primary care settings adequately predicted AF in diverse populations from the United States and Europe

It was apparently a sequel to Svend's first book of fables a year earlier, presented as The Fox and the Stork in English. As I mentioned in my comment on the English version, the illustrations in this sequel may be better than those in the original. Among the best are The Affectionate Donkey (7), LS (21), MM (25), The Fox and the Billy-Goat (33), and GGE (39).
