Key performance indicators and benchmarks in MCI prehospital response using technological tools: a qualitative study assessing the perception of practitioners and tool developers
Purpose The aim of this study is to investigate the opinions and perspectives of The Novel Integrated Toolkit for Enhanced Prehospital Life Support and Triage in Challenging and Large Emergencies (NIGHTINGALE) end-users and tool developers regarding Key Performance Indicators (KPIs) and benchmarks that assess the prehospital response to Mass Casualty Incidents (MCIs) enhanced by the NIT-MR. Methods A qualitative study employing focus group discussions was conducted to collect opinions and perspectives of end-users and tool developers regarding KPIs and benchmarks in MCI response using the NIT-MR. The criteria considered for the selection and distribution of participants within the groups was the nature of their involvement within the NIGHTINGALE project and their familiarity with the tools to be discussed. Results Thirty-one participants from different countries were included. Four themes emerged during data analysis which are: definition/explanation is the personal understanding of participants of the term KPI, process of KPI development and relationship with User Requirements is the decision process for assigning KPIs to user requirements, benchmarking is the mental process of associating a benchmark to a KPI or for developing a benchmark, and technical/medical gap is the gap of understanding between each sides' fields. Conclusion This study emphasized the need for a structured approach to using KPIs and bridging the gap between technological and medical worlds, taking the NIGHTINGALE project, funded by the European Union, which aims to develop a technological toolkit for first responders in mass casualty incidents as an example. These insights are crucial for enhancing disaster response
Key performance indicators in pre-hospital response to disasters and mass casualty incidents: a scoping review
Purpose: The objective of this study was to offer a comprehensive synthesis of the existing Key performance indicators (KPIs) used in the evaluation of the pre-Hospital response to disasters and mass casualty incidents (MCIs). Methods: At the end of December 2022 a scoping review has been performed on PubMed, Scopus, Embase, and Medline to identify articles describing the use of KPIs to assess the performance of first responders during the prehospital phase of an MCI (real or simulated). Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, fourteen articles were included in the analysis. Results: Eleven articles applied indicators in exercises and/or simulations. Two articles proposed new KPIs, and one used KPIs for developing a model for benchmarking pre-Hospital response. All articles analyzed quantitative indicators of time, whereas two studied indicators of structure, of process, and of outcome as well. Conclusion: The findings from this review emphasize the need for employing common terminology and using uniformed data collection tools, if obtaining standardized evaluation method is the goal to be achieved
Navigating challenges, solutions and requirements in the provision of trauma care in conflict settings by humanitarian actors: a scoping literature review
Abstract Background The evolving nature of irregular warfare and the increasingly frequent violations of human rights law and international humanitarian law pose unique challenges for humanitarian actors delivering trauma care in conflict settings. Methods A scoping review was conducted on PubMed, Scopus, and Web of Science and a web search (on Google, Google scholar and Bing) to analyze and review past humanitarian interventions offering trauma care in conflict settings. Relevant records were identified from scientific and grey literature. The thematic areas identified by the framework for a Public Health Emergency Operations Centre were used to facilitate the synthesis and analysis. Results Eleven records examining all phases of conflict identifying gaps throughout the continuum of care in the trauma systems were included. Challenges, solutions and requirements in transportation capacity, data collection methodologies, field coordination mechanisms, and rehabilitative care services were highlighted. Addressing the shortages in skilled healthcare workers, implementing quality improvement measures, and developing standardized training curricula were some of the requirements reported. Conclusions Our findings suggest that a multidimensional approach emphasizing strong coordination, with inclusive partnerships, is fundamental for effective trauma care systems in conflict zones. Key recommendations include robust medical transport, comprehensive healthcare training, preemptive mass casualty planning, standardized educational materials, continuous context reassessment, data protection, and improved transparency. These strategies could enhance trauma care interventions, ensuring they are effective, equitable, accountable, and sustainable
Assessing primary healthcare disaster preparedness: a study in Northern Italy
Aim: The aim of this paper is to outline the steps taken to develop an operational checklist to assess primary healthcare (PHC) all-hazards disaster preparedness. It then describes a study testing the applicability of the checklist.Background: A PHC approach is an essential foundation for health emergency and disaster risk management (H-EDRM) because it can prevent and mitigate risks prior to disasters and support an effective response and recovery, thereby contributing to communities' and countries' resilience across the continuum of the disaster cycle. This approach is in line with the H-EDRM framework, published by the World Health Organization (WHO) in 2019, which emphasizes a whole-of-health system approach in disaster management and highlights the importance of integrating PHC into countries' H-EDRM. Nevertheless, literature focusing on how to practically integrate PHC into disaster management, both at the facility and at the policy level, is in its infancy. As of yet, there is no standardized, validated way to assess the specific characteristics that render PHC prepared for disasters nor a method to evaluate its role in H-EDRM.Methods: The checklist was developed through an iterative process that leveraged academic literature and expert consultations at different stages of the elaboration process. It was then used to assess primary care facilities in a province in Italy.Findings: The checklist offers a practical instrument for assessing and enhancing PHC disaster preparedness and for improving planning, coordination, and funding allocation. The study identified three critical areas for improvement in the province's PHC disaster preparedness. First, primary care teams should be more interdisciplinary. Second, primary care services should be more thoroughly integrated into the broader health system. Third, there is a notable lack of awareness of H-EDRM principles among PHC professionals. In the future, the checklist can be elaborated into a weighted tool to be more broadly applicable
Factors causing emergency medical care overload during heatwaves: A Delphi study
Heatwaves pose an important risk for population health and are associated with an increased demand for emergency care. To find factors causing such overload, an online Delphi study included 15 experts in emergency medicine, disaster medicine, or public health. One open-ended question was delivered in the first round. After content analysis, the obtained statements were sent to the experts in two rounds to be rated on a 7-point linear scale. Consensus was defined as a standard deviation ≤ 1.0. Thirty-one statements were obtained after content analysis. The experts agreed on 18 statements, mostly focusing on the input section of patient processing and identifying stakeholders, the population, and primary care as targets of potential interventions. Additional dedicated resources and bed capacity were deemed important as per throughput and output sections, respectively. These findings could be used in the future to implement and test solutions to increase emergency healthcare resilience during heatwaves and reduce disaster risk due to climatic change

ncesi, Felsefe, Ahmet Hamdi Akseki, Rûh ÖZET AHMET HAMDİ AKSEKİ'NİN RÛH VE BEKÂ-YI RÛH ADLI ESERİ Ahmet Hamdi Akseki'nin “Rûh ve Bekâ-yı Rûh” adlı yazma eseriyle ilgili olan çalışmamız iki bölümden oluşmaktadır. Birinci bölümde yazar ve eser hakkında incelemelerimiz yer aldı. Burada “Rûh ve Bekâ-yı Rûh” adlı eserin müellifi olan Ahmet Hamdi Akseki'nin hayatı, ilmî şahsiyeti ve eserlerine ayrıca tezimizin asıl mevzuunu teşkil eden “Rûh ve Bekâ-yı Rûh” adlı eserin şekil ve muhteva açısından tanıtımı ve eserde yer alan başlıca görüşlere yer ayırdık. Tezimizin ikinci bölümünde ise “Rûh ve Bekâ-yı Rûh” adlı eseri dipnotlarda değindiğimiz çeşitli ilâve ve değişikliklerle Latin harflerine aktardık. Bu çalışmayı yaparken gerekli gördüğümüz noktalarda müellifin eserden üç yıl önce kaleme aldığı “Felsefe-i Rûh” ismini verdiği eserin önceki versiyonu olan yazma eserden de istifade ettik. In the first chapter, we considered about the author and his book. The author’s life, intellectual personality, books and form and content of “Rûh ve Bekâ-yı Rûh” were discussed in the first chapter. In the second chapter, we transliterated “Rûh ve Bekâ-yı Rûh” into Latin alphabet with footnotes which contain additions and changes about the book. In the important points we applied to “Felsefe-i Rûh” which is the former version of “Rûh ve Bekâ-yı Rûh.

Bu çalışmamızın konusu, Cumhuriyet dönemi yazarlarından bireyin iç dünyasını ele alan, modernizmin öncüsü, Ahmet Hamdi Tanpınar'ın hikâye ve romanlarında beş duyunun eserlerdeki yeridir. Beş duyumuz vasıtasıyla dış dünyamızın bilgilerini duyumsadığımız uzun zamandan beri bilinmektedir. Duyu organlarımız vasıtasıyla dışımızdaki dünyayı görürüz, kokusunu alırız, sesini işitiriz, onun değişik tatlarını alırız veya bunların hiçbirisi yetmiyorsa bu sefer de dokunarak ne olduğunu anlamaya çalışırız. Biz duyu organlarımız vasıtasıyla dış dünyamızın, sinir sistemimiz vasıtasıyla kendi iç dünyamızın bilgilerine vakıf olmaya çalışırız. Yazarın hikâye ve romanlarında beş duyunun ele alınışı, duyuların duygulara ve esere etkisi, duyuların metaforik çağrışımları, yazarın bunları eserlerindeki kullanım biçimleridir Ahmet Hamdi Tanpınar, Türk edebiyatının kendisi ve eserleri üzerinde en çok durulan şair / yazarlarından biridir. Bu çalışmada, diğer çalışmalardan farklı olarak beş duyunun ve duyuların esere ve yazara etkisi, katkısı üzerinde durulacak olması özgün tarafıdır. Bu tezimizin konusu, Cumhuriyet Dönemi Türk Edebiyatının en önemli yazarlarından, bireyin iç dünyasını esas alan, modernizmin öncüsü Ahmet Hamdi Tanpınar'ın roman ve hikâyelerinde ayna motifinin eserlerindeki yansımasıdır. Dünya kültür tarihinde mitolojiden, felsefeye, psikolojiye, tasavvufa kadar ve daha da sayılabilecek birçok alanda aynanın simge, sembol, nesne olarak yer aldığını görülür. Bu insanın doğuşundan itibaren içinde yer etmiş olan görme ve görünme ihtiyacının bir neticesidir. Ahmet Hamdi Tanpınar, Türk edebiyatının kendisi ve eserleri üzerinde en çok araştırılan, yazılan şair/yazarlardan biridir. Bu tezde, diğer çalışmalardan farklı olarak aynanın tarihe, edebiyata, yazara ve yazarın eserlerine olan etkisi ve yansıması üzerinde durulacak olması tezin özgün tarafıdır. In this thesis, unlike other studies, the effect and reflection of mirror on history, literature, author and author's Works will be emphasized is the original side of the thesis.
