    Assessing Equity in the Provision of Primary Healthcare Centers in Buenos Aires Province (Argentina): A Stochastic Frontier Analysis

    Background: Unequal access to healthcare between individuals or populations does not imply the existence of inequities as it must be controlled for differences in need. This is usually done either via direct or indirect standardization methods. Objective: We study the degree of equity in the availability of public primary healthcare centers between the 134 municipalities of the Buenos Aires province (Argentina) with a novel application of an existing statistical tool. Methods: Stochastic frontier analysis was applied to estimate the number of primary care facilities that should be available in each municipality according to demographic variables (population under age 4 years and above age 65 years), as they are the population groups with higher healthcare needs. Results: Under all specifications tried, the endowment of primary healthcare centers at the municipal level tends to increase (but less than proportionally) with the size of the population at risk. In addition, wealthier jurisdictions tend to be more equitable but also more heterogeneous. Conclusions: Stochastic frontier analysis allows us to discriminate between those districts that, according to their needs, are underserved, from those who have a surplus of health facilities.     Argentine pension replacement rate

    El objetivo del artículo es comparar la tasa de reemplazo teórica del sistema previsional argentino con la tasa de reemplazo efectiva. Mientras la primera se deduce de las normativas previsionales vigentes en cada momento, la segunda se estima en términos promedio en el aglomerado del Gran Buenos Aires entre 1974 y 2018. Se encuentra que el sistema previsional argentino no ha sido capaz de cumplir en términos promedio con las tasas de reemplazo teóricas prometidas debido a que el retraso en la actualización de los haberes previsionales ha sido históricamente uno de los mecanismos de ajuste más utilizados para retornar al equilibrio financiero del sistema. Se concluye que, mientras el mercado laboral continúe reflejando un mal desempeño en términos de su capacidad para generar empleo formal y no se modifique su diseño mayormente contributivo, el sistema previsional continuará aplicando este y otros mecanismos de ajuste en perjuicio de sus beneficiarios.The objective of the article is to compare the theoretical replacement rate of the Argentine pension system with the effective replacement rate. While the first is inferred from the current pension laws, the second is estimated in average terms in the Greater Buenos Aires agglomerate between 1974 and 2018. It is found that the Argentine pension system has not been able to meet in average terms with the theoretical replacement rates promised because the delay in updating pension benefits has historically been one of the most used adjustment mechanisms to return to the financial equilibrium of the system. It is concluded that, while the labor market continues to reflect poor performance in terms of its capacity to generate formal employment and the largely contributory design of pension system is not modified, it will continue to apply this and other adjustment mechanisms to the detriment of its beneficiaries.Fil: Geri, Milva.     Assessing the spatial efficiency in the location of primary health care facilities: A local application in Argentina

    If scarce resources are not used efficiently, welfare could be increased without any cost. Efficiency metrics have a long tradition in economics. However, measures of spatial efficiency (SE) are far less common in the literature. In health care, SE is crucial to guarantee population’s access to health care providers. We develop a metric of SE of Primary Health Care Centres (PHCCs) location based on the comparison of the minimum distances users must travel to reach a PHCC, considering their current location, against the distances they should travel if all PHCCs were optimally located. We apply this metric to assess the spatial efficiency of the PHCCs of Bahía Blanca City (Argentina). To determine the optimal location of the centres we used a capacitated P-Median model. The annual demand for medical consultations of each demand node was estimated by adjusting the number of inhabitants of that node by a socioeconomic index.Fil: Arnaudo, María Florencia.     Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    Variations on the Author

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Conceptual delimitation of the primary health care

    La relevancia de los bienes y servicios provistos en el marco de la estrategia de atención primaria de salud se sustenta en la evidencia teórica y empírica recopilada en las últimas décadas. Sin embargo, aún persisten ciertas imprecisiones en torno al concepto que dificultan su estudio. Objetivo: Delimitar el concepto y diferenciarlo de algunos términos relacionados: atención primaria y primer nivel de atención. Fuente de datos: Se realizó una revisión temática de investigaciones teórico-conceptuales y empíricas correspondientes al período 1970- 2015. Se consultaron los estudios originales y de revisión publicados en español, inglés y portugués en las bases Medline, EconLit y en buscadores genéricos de internet (Google Scholar). Al mismo tiempo, y para evitar el sesgo de publicación, se revisaron las referencias bibliográficas de las investigaciones encontradas en la primera etapa de búsqueda. Los descriptores empleados fueron: primary health care, primary care, atención primaria y atención primaria de salud. Síntesis de los datos: Existen variaciones conceptuales del término atención primaria de salud que pueden responder a características propias de los sistemas de salud donde fueron formulados. Diversos autores han intentado establecer las diferencias entre los mismos.The relevance of the goods and services provided as part of the strategy of Primary Health Care (PHC) is based on the theoretical and empirical evidence found in the last decades. However, there are certain inaccuracies around this concept that hinder its study. Objective.We try to define the concept of primary health care and differentiate it from some related terms: primary care (PC) and first level care (FLC). Methods: we perform a thematic review of the literature related to the concepts under study. Results. The literature review reveals that there are variations of the term PHC that may depend on the specific characteristics of the health systems where they were formulated. Several authors have attempted to establish the differences between them. Conclusions. Although the concepts of PHC, PC and FLC are frequently confused and used as synonyms, the first concept refers to a health policy while the second must be associated with a model of care; finally, the last term corresponds to a level of care in the health system.Fil: Elorza, Maria Eugenia.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods
