Enhancement di parete degli aneurismi intracranici alla RMN ad alta risoluzione: un segno di rottura?
Introduzione: L’aneurisma cerebrale è ancora un’entità clinica potenzialmente devastante. Gli aneurismi sacculari intracranici sono dilatazioni patologiche delle arterie cerebrali che colpiscono il 3-5% della popolazione adulta mondiale e che hanno maggiore incidenza tra la quarta e sesta decade con prevalenza nel sesso femminile. Circa il 20-30% dei pazienti è portatore di aneurismi multipli. E’ stimato che tra 10-15 individui almeno uno sia portatore inconsapevole di un aneurisma cerebrale.
La rottura di un aneurisma sacculare intracranico ha ancora conseguenze devastanti: morte nel 50% dei casi e grave disabilità nel 35% di pazienti giovani.
Il rischio di rottura è tutt’oggi difficilmente prevedibile.
L’infiammazione gioca un ruolo decisivo nella formazione e nella rottura degli aneurismi.
La Risonanza Magnetica della parete dei vasi intracranici costituisce un nuovo approccio diagnostico che permette di visualizzare la “luminosità dell’aneurisma” dovuta ad infiammazione e assottigliamento della parete. La RM della parete dei vasi cerebrali può, infatti, evidenziare un'infiammazione vascolare intracranica visualizzando direttamente anomalie della parete vasale attraverso sequenze di Risonanza Magnetica a 3 Tesla che sopprimono il segnale del sangue ed hanno una eccellente risoluzione spaziale.
La letteratura corrente evidenzia, infatti, come la presa di contrasto della parete vasale alla Risonanza Magnetica (MRI) può suggerire una patologia della parete stessa ed identificare una instabilità nell’aneurisma sacculare intracranico precedente la rottura.
L'identificazione di un segno di rottura precoce può permettere di personalizzare il trattamento offerto, migliorando di conseguenza la sopravvivenza e l’outcome e riducendo il costo sociale e l’impatto familiare.
Ipotesi: L'ipotesi principale dello studio è che l'enhancement della parete vasale alla risonanza magnetica ad alta risoluzione (3T-MRI) sia un marcatore potente ed affidabile della infiammazione della parete dell'aneurisma e della sua conseguente rottura,
Obiettivo:
Lo scopo principale di questo studio è di valutare la capacità diagnostica della presa di contrasto della parete vasale come biomarker d'infiammazione della parete dell’aneurisma, confermando che la presa di contrasto è correlata ad una alterazione istologica della parete ed al conseguente aumentato rischio di rottura.
Altro obiettivo è quello di studiare una correlazione tra presa di contrasto della parete vasale e biomarkers infiammatori nel sangue periferico prelevando un campione ematico nei pazienti con presa di contrasto della parete aneurismatica al fine di valutare un incremento degli indici di flogosi.
Metodi: Una serie consecutiva di 50 pazienti sintomatici o asintomatici con aneurisma rotto o non rotto sono stati inclusi nello studio. I fattori di rischio per rottura sono stati registrati. Lo studio MRI della parete degli aneurismi è stato eseguito su Risonanza 3 Tesla, con sequenze ad alta risoluzione T1 e T2-pesate prima e dopo iniezione di mezzo di contrasto.
Due neuroradiologi esperti in patologia vascolare hanno studiato le immagini indipendentemente.
Nei pazienti sottoposti ad intervento chirurgico, è stata effettuato uno studio istologico ed immunoistochimico.
Su tutti i pazienti è stato inoltre eseguito prelievo ematico per analizzare molteplici biomarkers di infiammazione periferica (IL-1, MMP-9, TNF-α et al).
L’analisi di regressione logistica univariata e multivariata è stata effettuata per investigare l’associazione tra presa di contrasto della parete vasale degli aneurismi con fattori di rischio noti, caratteristiche dell’aneurisma (dimensione, sede, conformazione, molteplicità) ed altri fattori clinici e laboratoristici.
Rilevanza scientifica: Le attuali conoscenze sulle caratteristiche delle pareti aneurismatiche sugli studi MRI delle pareti vasali sono in continua evoluzione.
I dati presenti in letteratura sono basati su piccole serie o dati retrospettivi.
Lo studio MRI della parete vascolare può potenzialmente permettere di identificare immagini suggestive di patologia della parete vascolare (inclusi infiammazione, neoangiogenesi, emorragia intramurale e rottura) e, a volte, di identificare biomarkers neuroradiologici del comportamento dell’aneurisma.
Lo scopo dello studio è di valutare la capacità della MRI della parete vascolare di prevedere il comportamento biologico confrontandola con fattori di rischio noti per rottura.
Risultati: L'analisi della relazione tra la captazione di parete nei tre gruppi di pazienti studiati ha mostrato come la captazione di contrasto fosse significativamente presente nel 100% degli aneurismi rotti; nei pazienti sintomatici la captazione era presente nel 50% dei casi mentre tra gli asintomatici nel 33% (p=0.003). Un’elevazione dello score PHASES ha mostrato una correlazione statistica con la captazione e con i sottotipi di paziente. E’ stata individuata una linea di taglio di circa 4 al di sopra della quale la maggior parte degli aneurismi comincia a captare (p<0.001).
L'analisi di regressione logistica ha dimostrato che sono predittori indipendenti di rottura: la captazione (p=0.018), la presenza di aneurismi multipli (p=0.016). Predittori indipendenti di captazione dell'aneurisma: le dimensioni dell'aneurisma (p=0.001), la presenza di variabili anatomiche del circolo di Willis (p=0.005) e la morfologia irregolare dell'aneurisma (p=0.016). L’analisi di sensibilità e specificità della captazione come predittore di rottura ha dimostrato (p=0.002): sensibilità: 100%, specificità: 64%, VP positivo: 28%, VP negativo: 100%. I markers CD3 e CD34 (linfociti T e vasa vasorum) sono tendenzialmente più alti nei pazienti captanti (p=0.061 e p=0.083). Esisteva una correlazione diretta tra il numero di marcatori immunoistochimici d’infiammazione alterati e la presenza di captazione di parete con un cut-off di circa 7. Pazienti con un indice al di sotto del 7 non mostravano captazione mentre tutti i pazienti con un indice superiore mostravano captazione alla RM (p=0.043). Un punteggio superiore a 7, offre un 100% di sensibilità, specificità, VPP e VPN in relazione alla presenza di captazione aneurismatica.
Conclusioni: I nostri risultati preliminari di uno studio su una coorte di pazienti più ampia, confermano che la presa di contrasto della parete ed i markers infiammatori aumentati siano in grado di rilevare, negli aneurismi non rotti, una tendenza alla rottura. Questi dati sono stati, inoltre, confermati dagli studi neuroradiologici ed anatomopatologici su aneurismi rotti con evidenza, dapprima, della captazione di contrasto da parte della parete, successivamente di alterazioni di struttura allo studio istologico del campione operatorio.
Il presente studio conferma l’importanza di eseguire accertamenti accurati durante la valutazione di aneurismi non rotti al fine di poter stratificare l’individuale rischio di rottura, basato sulle caratteristiche stesse dell’aneurisma e non su fattori ambientali o di rischio calcolati sulla popolazione generale.Background:
Saccular intracranial aneurysm (SIA) rupture has still devastating consequences: death and serious disability in up to 35%-50% of often young patients. Instability state is still difficult to estimate(1). Inflammation plays a major role in aneurysm formation and rupture and literature suggests that aneurysm wall enhancement (WE) on magnetic resonance imaging (MRI) may be a surrogate of vessel wall inflammation(2).
Aim:
The main aim of the study is to assess the capability of wall enhancement to identify intracranial aneurysm in real danger for rupturing.
Methods:
Cohort prospective observational study. Consecutive patients with ruptured and un-ruptured aneurysms underwent contrast 3T-MRI and histology to confirm wall inflammation. Statistical analysis was performed to investigate the association of WE with risk factors and histological markers of inflammation(3). Patients were divided into three groups in relation to clinical features:
• Asymptomatic aneurysms,
• Symptomatic aneurysms,
• Ruptured aneurysms.
Results:
A preliminary group of 50 patients was included. WE was present in 100% of the ruptured aneurysms, in 50% of unruptured symptomatic and in 33% of asymptomatics(p=0.003). Cut-offline of 4 in PHASES score was found, above which most of the aneurysms begin to enhance(p<0.001). Logistic regression analysis demonstrated as independent predictors of rupture: WE (p=0.018) and presence of multiple aneurysms(p=0.016). Independent predictors of wall enhancement were: size(p=0.001), presence of variables of circle of Willis (p=0.005) and irregular morphology(p=0.016). Analysis of WE as a predictor of rupture has shown (p=0.002):100% sensitivity, 64% specificity, 28%VPP, 100%VPN. At a cut-off of 7, there is a direct correlation between the number of altered immunohistochemical inflammation markers and presence of WE (p=0.043). A score higher than 7 entails a 100% sensitivity, specificity, VPP and VPN in relation to presence of WE.
Conclusions:
Wall enhancement identifies instability-state in SIA. WE is a new and powerful rupture sign that could be crucial in deciding whether and how to treat SIA, even in small aneurysms. Final purpose is to offer a practice tool to personalize the therapeutic approach, distinguishing different aneurysm “activation” stages, in order to reduce intracranial bleeding and improving long-term outcome. Taking into account not only the dimensions and the morphology but, above all, the pathophysiology of the aneurysm itself, will allow to tailor therapeutic options. Further implication of the results will be the development of new personalized devices aiming at lowering the inflammation of the wall
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Static Visual Agnosia Following Awake Resection of a Left Frontal Low-Grade Glioma: A Case Report of Ventral Stream Network Disruption ("Astatopsia")
Background and Clinical Significance: Visual agnosia and speech production deficits are well-described sequelae of neurosurgical interventions, but their selective dissociation remains rare. This report presents an unusual combination of postoperative deficits following awake resection of a left frontal low-grade glioma. Case Presentation: We present the case of a right-handed female with left hemisphere language dominance who had a left frontal low-grade glioma. Preoperatively, she exhibited anomia and dysexecutive syndrome, including difficulty completing everyday goal-directed tasks such as sending emails and paying for parking. Following awake tumor resection, she developed two rare, dissociated deficits: (1) speech restricted to infinitive verb forms and (2) selective visual agnosia for static images, with preserved recognition of dynamic stimuli. Conclusions: This uncommon clinical constellation highlights the vulnerability of left frontal language and ventral visual processing networks during surgery and supports the dual-stream model of vision and language production. We describe a selective form of static visual agnosia affecting static images with relative preservation of dynamic and object recognition, for which we use the descriptive label “astatopsia”. This peculiar clinical condition is rarely documented in this particular combination and has not, to the best of our knowledge, previously been denominated in such a manner in the literature
Long-term seizure and behavioral outcomes after corpus callosotomy
Outcomes of corpus callosotomy (CC) have been mainly focused on seizures. The present study aimed to evaluate the long-term effects of CC on adaptive behaviors and caregivers' satisfaction in addition to seizures and to identify clinical predictors of postsurgical outcomes. Medical records of 26 patients (mean age at study time: 40 years, mean follow-up: 14 years) with childhood-onset epilepsy who underwent anterior or 2-stage complete CC were reviewed. A structured questionnaire was submitted to caregivers asking about relative changes in different seizure types, behavioral functions, and satisfaction with the postoperative outcomes. Formal neuropsychological assessment was carried out in a subgroup of patients. Selected clinical variables including age at surgery, extent of callosal section, length of follow-up, epilepsy syndrome, and presurgical cognitive level were submitted to multiple regression analysis. At the last follow-up visit, a reduction greater than 50% was observed mainly for drop attacks (65% of patients), followed by generalized tonic-clonic seizures (53%), and complex partial seizures (50%). No presurgical variables were significantly associated with seizure outcome. After surgery, more than half of patients showed attention enhancement, which was related to drop seizure improvement. Early age at surgery was associated with better behavioral regulation; complete CC slightly worsened language abilities. Satisfaction with surgery outcomes was expressed by 73% of caregivers and was dependent on drop seizure reduction and improvements in activities of daily living. A long-term positive psychosocial outcome is likely after CC also in severely disabled patients, especially if surgery is performed early
Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more

We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
