LA VENTOSA OSTETRICA: IL RACCONTO DELLE DONNE E LA VISIONE DEL RUOLO DELL’ OSTETRICA.

La maggior parte delle donne partoriscono in modo naturale, ma alcune hanno bisogno di interventi medici durante la seconda fase del travaglio come l’uso della ventosa ostetrica.
L’esperienza parto e la soddisfazione ad esso connessa rimangono un argomento poco studiato, poco compreso ed influenzato da numerosi e diversi fattori.
E’ riconosciuto in tutta la letteratura che, sebbene l’evento nascita sia un evento spesso sereno e dovrebbe avere un impatto positivo sulla vita della donna, può avere anche un
effetto deprimente sul suo benessere fisico e mentale.
Circa il 25%- 33% delle donne riportano sintomi di trauma legati alla nascita che possono essere connessi ad interventi ostetrici. La ventosa ostetrica, facendo parte degli interventi medici inaspettati durante il parto, è uno dei fattori di rischio.
L’ ostetrica durante un parto con ventosa, intervento di competenza medica, ha il ruolo fondamentale di erogare un’assistenza che salvaguardi le emozioni della donna e il trauma che potrebbe insorgere.
Deve essere un riferimento che spiega alla coppia cosa sta succedendo, fornendo tutti i dettagli delle procedure e le sensazioni che la donna potrebbe avvertire. Sarà importante essere concise, sincere e chiare utilizzando un linguaggio comprensibile. L’empatia in questi casi sarà fondamentale per riconoscere paure e preoccupazioni. La donna ha bisogno dell’ostetrica che l’ha seguita durante il travaglio e di cui ha fiducia.
Obiettivo dello studio
L’obbiettivo di questo studio qualitativo è di ascoltare ed indagare il vissuto delle donne che hanno avuto un parto vaginale operativo, analizzando le emozioni e le sensazioni che hanno provato in quel momento. Attraverso i racconti delle donne, si è cercato di comprendere il ruolo svolto dall’ostetrica in questa loro esperienza.
Materiali e metodi
Nella ricerca sono state coinvolte sedici puerpere che sono state intervistate tra le 24 alle 72 ore dal parto. Il campione è stato di convenienza. Il metodo usato è l’intervista semistrutturata, formata da nove domande, registrata e trascritta integralmente su supporto informatico.
Risultati
Nove donne su sedici descrivono il proprio parto negativamente. Dieci donne su sedici associano al parto con ventosa ostetrica emozioni e sensazioni negative.
Il ruolo dell’ostetrica è visto dalla maggior parte delle donne positivamente. L’ostetrica viene descritta come un punto di riferimento conferendole caratteristiche come la pazienza, l’educazione, la bravura, la disponibilità, l’essere presente, la spiegazione continua, l’uso di posizioni alternative e la chiarezza nella comunicazione. Una minoranza di donne, però, afferma di non aver percepito l’ostetrica durante la ventosa ma solo la figura del medico. Altre ancora descrivono un rapporto complicato con l’ostetrica.
Le donne risultano in linea generale informate sulla possibilità di un parto con ventosa dai corsi di accompagnamento alla nascita. Tuttavia una parte afferma di avere informazioni legate al “sentito dire” o di non averne affatto. Nonostante le spiegazioni il parto con ventosa rimane un evento inaspettato.
Risulta che i motivi della ventosa ostetrica vengano spiegati prima dopo o durante la procedura e risultano ben compresi dalle donne. Solo qualche intervistata afferma di non aver avuto informazioni da parte degli operatori presenti, condizione che ha suscitato disagio e paura.
Infine la visione delle donne su una gravidanza e parto futuro rimane controversa, le donne danno opinioni differenti. Tuttavia in generale possiamo riconoscere due pensieri: da un lato alcune donne desiderano ancora un parto vaginale ma fisiologico, dall’altro altre dichiarano di voler ricorrere ad un taglio cesareo elettivo non volendo più rivivere l’esperienza del parto vaginale.
L’analisi delle interviste ha fatto emergere otto temi e ventisei categorie.
Conclusione e proposta operativa
La ricerca ha fatto emergere che le donne hanno una visione negativa del parto distocico e le emozioni connesse alla ventosa ostetrica sono profondamente negative.
Tutti i neonati hanno avuto un buon outcome e anche le donne riferiscono di stare bene riferendo solo disturbi che caratterizzano il periodo puerperale.
Il ruolo dell’ostetrica è visto dalla maggior parte delle donne positivamente. L’ostetrica viene descritta come un punto di riferimento.
La proposta operativa prende spunto dalla realtà inglese e si tratta della realizzazione di un foglio informativo sul parto vaginale operativo chiaro semplice e sintetico da consegnare alle primipare.
Pur essendo una modalità di parto che rappresenta una minoranza, si ritiene importante parlare in modo chiaro della possibilità di un parto con ventosa in tutti i corsi di accompagnamento alla nascita, consegnando anche il foglio informativo.
Soprattutto però alle gestanti che non frequentano i corsi preparto dovrebbe essere consegnato dall’operatore che segue la gravidanza.
Viste le caratteristiche del foglio informativo potrebbe essere anche consegnato alle puerpere da parto vaginale operativo che non hanno avuto informazioni in gravidanza oppure che desiderano conoscere meglio i fattori di rischio in puerperio e chiarire le possibilità di un parto futuro
Going Beyond Counting First Authors in Author Co-citation Analysis

counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
Full text link“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
Full text linkWe provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
