Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
lmmunità antipoliomielitica in pazienti pediatrici con trapianto di rene e di fegato
OBIETTIVO: Accertare la necessità dell'adozione di schedule vaccinali specifiche per soggetti con deficit del sistema immunitario confrontando lo stato dell'immunità antipoliomielitica in bambini e adolescenti immunodepressi e in controlli immunocompetenti.
METODI: Sono stati studiati 96 soggetti sottoposti a trapianto di rene (57) e di fegato (39), 27 pazienti in dialisi e 25 con insufficienza epatica cronica e 69 controlli sani di età comparabile. Gli anticorpi sono stati titolati mediante microtitolazione in cellule VERO contro i poliovirus tipo 1, 2 e 3 (stipiti attenuati di Sa(in). I sieri con titoli > 1:8 sono stati considerati positivi.
RISULTATI: Percentuali di copertura e titoli anticorpali per polio 1 e 2 sono risultati significativamente inferiori (p<0.05) nei riceventi di trapianto di rene rispetto ai controlli sani; le medie geometriche dei titoli anti polio 2 sono risultate inferiori anche nei dializzati. Nei pazienti con insufficienza epatica cronica sono stati evidenziati titoli anticorpali verso i poliovirus 1 e 3 maggiori (p<0.05) che nei controlli.
CONCLUSIONI: Esistono differenze nell'effetto che il trattamento immunosoppressivo post-trapianto determina sui livelli di anticorpi circolanti prodotti in seguito alle vaccinazioni. Schedule vaccinali specifiche vanno individuate in rapporto alla malattia che ha portato al trapianto e al tipo di trattamento inununosoppressivo
Conversion from cyclosporine to tacrolimus in pediatric kidney transplant recipients
In pediatric kidney transplant recipients, tacrolimus has been proposed either for primary immunosuppression or as a rescue agent for refractory acute rejection, chronic rejection, and cyclosporine toxicity. This paper describes our experience with tacrolimus conversion from cyclosporine-based therapy in six selected cases: four due to refractory acute rejections unresponsive to conventional therapy, one to chronic graft rejection, and one to cyclosporine-related hypertrichosis. A "simple-switch" conversion was used without any overlap, starting with a dose of 0.2 mg/kg per day. The time to conversion varied from 10 to 730 days after the transplant. In the patients with acute rejection, the median time to reversal after tacrolimus conversion was 12 days. The symptoms of the patient with cyclosporine toxicity completely resolved without any loss of allograft function. The patient with chronic rejection maintained stable renal function for more than 1 year after conversion. A new onset of post-transplant diabetes mellitus and dose-related nephrotoxicity were recorded as adverse events. In conclusion, our experience suggests that tacrolimus can play an important role in the salvage treatment of pediatric kidney transplantations with deteriorating graft function due to acute rejection refractory to standard therapy. Tacrolimus conversion also provides excellent results in the presence of cyclosporine toxicity
Cutaneous and oral lesions in 32 children after renal transplantation
A variety of skin and oral mucosal lesions appearing in patients after renal transplantation have been reported in the literature. Because most of these data pertain to adults, we studied a group of children with kidney transplants to investigate the occurrence of skin and/or oral mucosal lesions, compare them with those observed in adults, and contribute to the knowledge of the pathogenesis of these disorders. Our series consisted of 32 patients aged 5 to 18 years, all of whom had skin and/or oral mucosal lesions that were classified as being either drug induced, probably related to uremia, or due to other causes. A possible relationship between drug-induced lesions and the therapeutic immunosuppressive regimen in use was investigated. In addition, two formerly unreported lesions were seen in these patients: hypertrophy of lingual fungiform papillae and onychoschizia. The former occurred only in children receiving cyclosporine. Ear lobe bilateral necrosis by levamisole induced occlusive vasculitis in a pediatric patient
We describe a bullous-hemorrhagic reaction of the ear lobes in a child with nephrosis after levamisole intake. This drug is used in children as an alternative treatment of corticosteroid-responsive nephrotic syndrome. Histologic evaluation of a lesion revealed necrotizing vasculitis involving mainly the large vessels of the deep plexus. The lesions disappeared 25 days after discontinuation of treatment
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
