ObjectiveThe aim was to explore the perceptions of rheumatology health-care professionals (HCPs) of interacting with patients of South Asian origin attending early inflammatory arthritis clinics.MethodsWe used face-to-face semi-structured interviews, designed in partnership with a clinician partner, to interview 10 HCPs involved in the running of early inflammatory arthritis clinics across seven centres in the UK. Data were recorded, transcribed by an independent company and analysed using inductive thematic analysis.ResultsThree emerging themes were identified that characterized consulting experiences of HCPs: varied approaches were used in early inflammatory arthritis clinic; the challenges for rheumatology HCPs in managing and delivering information to patients of South Asian origin in early inflammatory arthritis clinics; and moving towards good practice, the views on managing future patients of South Asian origin in early inflammatory arthritis clinics. Overall, HCPs found that they required additional skills to support the engagement and management for patients of South Asian origin living with inflammatory arthritis. The HCPs felt that they were less effective in addressing self-management issues for this patient group, and they found it difficult to determine adherence to medication. In such consultations, HCPs perceived that their own limitation of inadequate training contributed towards poor consultations.ConclusionFor the first time, our data demonstrate that the management of patients of South Asian origin in early inflammatory arthritis clinics is under-served. To address this, HCPs have identified training needs to improve knowledge and skills in engaging with and supporting patients of South Asian origin. These findings provide a good direction for future research

New approaches to asymmetric nucleophilic catalysis and the development of redox-neutral reaction cascades

A new concept for nucleophilic catalysis has been developed and this strategy was successfully applied to the kinetic resolution of benzylic amines and propargylic amines. The first catalytic enantioselective desymmetrization of meso-1,2-diarylaminoethanes has been introduced. A combination of nucleophilic catalyst DMAP and hydrogen bonding catalyst afforded the monoacylated products in good yields and good to excellent enantioselectivity. A catalytic enantioselective Steglich rearrangement reaction has also been developed using our dual catalytic approach for nucleophilic catalysis. A novel catalytic enantioselective transformation between azlactones and isoquinolines has been developed. To the best of our knowledge, this type of functionalization of isoquinolines with azlactones is unprecedented. A redox neutral reaction cascade that involves the functionalization of the relatively unactivated C–H bond α to the nitrogen of cyclic amines has been developed. This transformation represents a fast route to synthetically useful aminal building blocks. A mechanistically unique 2,3-annulation of indole based on the tert-amino effect 1,5-hydride shift/ring closure sequence has been shown. This redox-neutral C–H functionalization leads to a new class of ring fused indolobenzazepines and features short reaction times. Apart from indole, the substrate scope includes pyrroles and 1,2-diarylhydrazines.Ph. D.Includes bibliographical referencesby Chandra Kanta D

    Kanta-Hämeen asiakastallien turvallisuus

Tämän opinnäytetyön aiheena on Kanta-Hämeen asiakastallien turvallisuus. The subject for this thesis is safety of riding stables with customers in Kanta-Häme area. It was commissioned by the fire department of Kanta-Häme. One of the goals was to research the quantity of riders and horses in Finland and to expand in this area and also to find out the quantity of the accidents in the equine sector either to the entrepreneur or customers and also the number of accidents in which horses were involved. The primary goal was to research stable security taking both customer and the rescue team perspectives into consideration. The thesis includes a lot of information gathered concerning the safety regulation which the horse businesses have to take care of. The author`s own opinions are based on both professional skill and experiences with horses and the rescue sector. A lot of written information about horses, rescue and agriculture was used in this study. Facts about accidents were based on a publication by healthcare sector and rescue sectors own database Pronto. All the rules and safety regulations are based on Kuluttajavirasto and Turvatekniikan keskus. Safety study for riding stables with customers was made by a questionnaire and the response percentage was 41 %. According to the questionnaire it is safe to have a horse sport hobby in Kanta-Häme area. From the fire department`s standpoint the stable buildings in the area are quite old and contain many risk factors

Pure threshold strategies for a two-node tandem network under partial information

In a two node tandem network, customers decide to join or balk by maximizing a given profit function whose costs are proportional to the sojourn time they spend at each queue. Assuming that their choicesare taken without knowing the complete state of the system, we show that a pure threshold equilibrium policy exists. In particular we analyze the case when the partial information consists in informing the arrival customers of the total number of users in the network. The research of the first author is partially supported by the Spanish Ministry of Education and Science Grants MTM2013-42104-P (FEDER funds), MTM2010-16519 and RYC-2009-0467

Overview Description Table of Contents Author InformationCoverBusiness and Politics in IndiaEdited by Christophe Jaffrelot, Edited by Atul Kohli, and Edited by Kanta MuraliModern South AsiaDescriptionOver the last few decades, politics in India has moved steadily in a pro-business direction. This shift has important implications for both government and citizens. In Business and Politics in India, leading scholars of Indian politics have gathered to offer an analytical synthesis of this vast topic. Collectively, they cover the many strategies that businesses have used to exert their newfound power in recent times and organize the book around a few central concerns. They first analyze the nature of business power and how it shapes political change in India. Second, they look at the consequences of business' growing power on some important issue areas-labor, land, urban governance, and the media. Finally, they take account of regional variation and analyze state-business relations. This definitive account offers significant insights into how and why corporations have increased their power in contemporary Indian politics
