The role of physiotherapy in reducing deformity and pain caused by Hallux valgus. A systematic review

Darba aktualitāte: Hallux valgus deformācija ir viena no visbiežāk sastopamajām hroniskajām pēdu slimībām, kas rodas, kad Hallux (kājas īkšķis) novirzās laterāli uz pārējiem kājas pirkstiem, kas savukārt izraisa funkcionālos traucējumus, piemēram, sāpes pēdā, līdzsvara traucējumus, gaitas stereotipa izmaiņas kā arī palielinātu kritienu risku gados vecākiem pacientiem. Hallux valgus izplatība populācijā ir 23 % pieaugušo, vecumā no 18 līdz 65 gadiem un 35,7 % cilvēkiem vecumā virs 65 gadiem.
Darba mērķis: Apkopot un izanalizēt pieejamo zinātnisko literatūru par fizioterapijas metožu efektivitāti Hallux valgus radīto sāpju un deformācijas mazināšanā.
Pielietotās metodes: Pētījumu atlasei tika izmantotas Rīgas Stradiņa Universitātes bibliotēkā pieejamās tiešsaistes datubāzes: Cohrane Library, EBSCO, PubMed un ScienceDirect. Pētījumu meklēšanai tika izmantoti atslēgas vārdi: Hallux valgus, foot, Big/Great toe, bunion, physiotherapy, non-surgical, treatment, pain.
Rezultāti: Pētījumu atlases rezultātā tika identificēti 5 pētījumi, kuri tika izmantoti detalizētai analīzei. No identificētajiem pētījumiem divi (5/10 punkti pēc PEDro) bija vidējas kvalitātes un trīs (6-8/10 punkti pēc PEDro) bija labas kvalitātes pētījumi. Pētījumos kopumā piedalījās 188 dalībnieki, no kuriem 173 bija sievietes un 15 vīrieši, kā arī sievietes bija ņēmušas dalību četros pētījumos bet vīrieši tikai vienā.
Pētījumos tika izmantotas dažādas fizioterapijas metodes, lai noskaidroto šo metožu efektivitāti, Hallux valgus deformācijas leņķa un sāpju samazināšanā. Visas pielietotās fizioterapijas metodes uzrādīja statistiski nozīmīgus rezultātus pēc terapijas mērījumos, bet atkārtotos mērījumos, pēc noteikta laika, bija vērojama iegūto rezultātu regresija.
Secinājumi: Fizioterapijas metožu pielietošana samazina Hallux valgus deformācijas leņķi un ar to saistītās sāpes, īslaicīgā periodā. Neskatoties uz to, ka vairākām fizioterapijas metodēm ir pozitīva ietekme uz Hallux valgus deformācijas samazināšanu, vairumā gadījumu, Hallux valgus leņķis arī pēc terapijas pārsniedz >150. Hallux valgus leņķim, un ar to saistītām sāpēm, pēc fizioterapijas metožu pielietošanas ir pozitīva ietekme, savukārt ilglaicīgā periodā ir tendence atkal pasliktināties, tāpēc būtu nepieciešami pētījumi, kuri būtu ar ilgāku terapijas un novērošanas laiku, lai varētu novērtēt fizioterapijas metožu efektivitāti ilglaicīgā periodā.Title: The role of physiotherapy in Hallux valgus deformity and pain relief: a systematic review of the literature.
Background: Hallux valgus deformity is one of the most common chronic foot disorders, occurring when the hallux (big toe) deviates laterally towards the other toes, leading to functional impairments such as foot pain, balance problems, gait stereotype changes and increased risk of falls in elderly patients. The prevalence of hallux valgus in the population is 23% in adults aged 18-65 years and 35.7% in people over 65 years.
Methods: Online databases available in the library of Riga Stradiņš University were used to select the studies. Hallux valgus, foot, Big/Great toe, bunion, physiotherapy, non-surgical, treatment, pain.
Results: The selection of studies led to the identification of 5 studies, which were used for detailed analysis. Of the identified studies, two (5/10 PEDro score) were of moderate quality and three (6-8/10 PEDro score) were of good quality. A total of 188 participants took part in the studies, of which 173 were women and 15 were men, and women had taken part in four studies and men in only one.
The studies used different physiotherapy methods to investigate their effectiveness in reducing the angle and pain of Hallux valgus deformity. All physiotherapy methods used showed statistically significant results in the post-treatment measurements, but regression of the results was observed in the repeated measurements after a certain time.
Conclusions: Physiotherapy techniques reduce the deformation angle of the Hallux valgus and the associated pain in the short term. Although several physiotherapy techniques have a positive effect on reducing the Hallux valgus deformity, in most cases, the Hallux valgus angle continues to exceed >150 after therapy. The Hallux valgus angle and associated pain have a positive effect after physiotherapy, but tend to worsen again in the long term, so studies with longer treatment and follow-up times are needed to assess the long-term effectiveness of physiotherapy techniques
Going Beyond Counting First Authors in Author Co-citation Analysis

counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

Get PDF“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis

We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
