    Kilic, Raif/0000-0002-8338-4948Objective: The main purpose of this study was to evaluate and compare the in -hospital, 1 -month and 1 -year post -procedure outcomes of patients treated with Evolut-R 34 mm and Evolut-R 23/26/29 mm devices. Additionally, the study aimed to identify factors that could predict the occurrence of >= mild paravalvular leaks (PVL). Methods: Between April 2015 and May 2022, 269 consecutive patients who underwent transcatheter aortic valve implantation (TAVI) with Evolut-R 34 mm (n = 66, 24.5%) and Evolut-R 23/26/29 mm (n = 203, 75.5%) devices in a single center were retrospectively analyzed. Results: Patients in the Evolut-R 34 mm group had a lower female sex ratio (16.7% vs. 66.5%, P = mild PVL in both patient groups in the receiver operating characteristic curve analysis. In logistic regression analysis; In patients with Evolut-R 34 mm valve, pre-TAVI aortic valve peak velocity (odds ratio (OR) = 23.202; P = .019) and calcium volume 800 Hounsfield Units (mm(3)) (OR = 1.017; P = mild PVL. Conclusion: The Evolut-R 34 mm valve has shown comparable in -hospital results with smaller valve sizes. A total of 540 patients, 180 from each of the traditional radial artery, distal radial artery , and transfemoral artery groups, were included. The visual analog scale was applied to the patients as soon as they were taken to bed. RESULTS: When the visual analog scale was compared between the groups, it was found to be significantly different (transfemoral artery: 2.7 & PLUSMN;1.6, traditional radial artery: 3.9 & PLUSMN;1.9, and distal radial artery: 4.9 & PLUSMN;2.1, respectively, p29.8 kg/m2 predicted severe pain with 72.5% sensitivity and 73.2% specificity [(area under the curve: 0.770, 95%CI: 0.724-0.815, p<0.0001)].CONCLUSION: In our study, we found that the femoral approach caused less access site pain and a high body mass index predicts severe pain

    ÖZET Raif Necdet, 1881 yılında İzmir'de doğmuştur, izmir'in köklü ailelerinden Kestellioğullan'na mensuptur. Babası gümrük müdürlüğünde çalışan Eyüp Sabri Beydir. Eğitimine İzmir'de başlayan Raif Necdet, 1897'de Harbiye Mektebi'ne girmiş ve 1900 yılında mezun olmuştur. Askerî kimlikli birisi olmasına rağmen, edebiyata gönül veren yazar, 1921 yılına kadar Edirne ve İstanbul'da edebiyat ve kitabet muallimi olarak görev yapmıştır. I. Balkan Harbi'nin başladığı 1912 yılında yüzbaşı olarak Edirne savunmasına çağırılan yazar, altı ay cephede mücadele ettikten sonra, altı ay da Sofya'da esir kalmıştır. İstanbul'a döndükten sonra edebi faaliyetlerine devam eden yazar, çeşitli türlerde on yedi eser vermiştir. 1937 yılında, 56 yaşında vefat etmiştir. Ölüm haberi 18 Ağustos 1937 tarihli gazetelerde yer almıştır. Raif Necdet, tenkid roman, hikaye, mensure, hatıra, mersiye, piyes, tercüme, mektup, vecize, antoloji türlerinde eserler yazmıştır. Bizim incelememizde, romanları, hikayeleri, piyesi ve hatıraları yer almaktadır. Çalışmamız giriş ve sonuç bölümlerinin dışında, dört bölüme ayrılmıştır. Giriş bölümünde, "II. Meşrutiyet Dönemi Edebiyatı" başlığı ile Fecr-i Âti ve Milli Edebiyat cereyanları hakkında bilgiler verilmiştir. I. bölüm, yazarın hayatım; doğumu, ailesi, öğretmenliği, askerliği, esirliği, edebî faaliyetleri ve ölümünü kapsamaktadır. II. bölümün başlığı, "Raif Necdet'in Romancıhğı"dır. Yazarın kaleme aldığı Ufül ve Semavî İhtiras romanları, olay örgüsü, tema, mekan, zaman, şahıs kadrosu ve üslûp açılarından incelenmiş ve romanlarda bulunan ortak hususiyetler tespit edilmiştir. Raif Necdet'in yazdığı on üç hikaye, IH. bölümde "Raif Necdet'in Hikayeleri" başlığı altında ele alınmış ve roman incelemesinde kullanılan teknik ile değerlendirilmiştir. IV. bölümde "Raif Necdet'in Piyesi ve Hatıraları" başlığında, M. Rauf la birlikte yazdığı piyese ve üç adet hatıra yazısına yer verilmiştir. Sonuç bölümünde yazarın edebî ve felsefî görüşleri belirtilerek, bu görüşlerin eserlerine nasıl yansıdığım gösterilmiştir. His wel-known family who worked called for customs in Izmir. His fat er is Mr. Eyüp Sabri who worked for customs directorate. Raif necdet who began his education in İzmir went to War Acedemy in 1887 and gradueted in 1900. Although he has a military identity, the author who was interested in litarature worked as a literature and secrataryship teacher in Edirne and Istanbul until 1921. In 1912 in w ic I. Balkan War started, t e aut or w o was called as a captain for the protection of Edirne became a slave in Sofya for six months after he had struggled in the frontline for six months. After he had returned to İstanbul, t e aut or wo continued his contributions gave seventen various kinds of works. He died at the age of fifty-six in 1937. The news of his death took place in the newspaper dated 18th of April, 1937. Riaf Necdet wrote critical novel, story, prose, memory, elegy, play, translation, letter, epigramand antology. His novels, stories, plays and memories took place in our revision. Our work is divided into four parts except the introduction and the resolution parts. İn t e introduction part, same information about Fecr-i Ati and Milli Edebiyat movements titled as "II. Constitutional Literary Monarcy Period." The first part includes his life; his birth, family, teaching, military life, literary contributions and death. The title of the second part is "Raif Necdet As A Novelist." His novels, Uful and Semavi I tiras" are evaluated for t eir plot, main idea, place, time, cast and style and assocated charac teristics are determined. Thirteen storyes, that Raif Necdet wrote, are evaluated in the title of "The Stories Of Raif Necdet" in third part and they are also evaluated with the tecniques used for criticizing novel. İn t e fourt part a play and t ree memories written toget er wit M. Rauf are taken place titled as "Raif Necdet' s Play and Memoryes." İn t e resolution part, t e aut or' s literary and p ilosop ical ideas are determined and how there ideas show parallelism with his works are displayed. The information about Raif Necdet' s works which are not included by our work and critics about him in different sources aren't taken place in this part.     The Role of Statins in Modulating Subclinical Inflammatory Markers in Coronary Slow Flow Phenomenon

    Kilic, Raif/0000-0002-8338-4948; Oguz, Mustafa/0000-0002-5165-1212;Coronary slow flow phenomenon (CSFP) is characterized by slow coronary blood flow in the absence of significant stenosis, and its pathophysiology is associated with endothelial dysfunction, microvascular abnormalities, and inflammation. This study aimed to investigate the effects of statin therapy on subclinical inflammatory markers in CSFP patients. This retrospective cohort study included patients diagnosed with CSFP by using coronary angiography. The patients were divided into statin and control groups based on the initiation of statin therapy. Inflammatory markers, including neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), systemic immune-inflammation index (SII), and systemic inflammation response index (SIRI) were assessed at baseline and 3 months later. At baseline, NLR, PLR, SII, and SIRI levels were comparable between the 2 groups. After 3 months, these markers were significantly lower in the statin group. In the statin group, the NLR, PLR, SII, and SIRI levels significantly decreased from baseline, whereas no significant changes were observed in the control group. The reduction in inflammatory marker levels was more pronounced in patients receiving high-dose statins. Statin therapy is associated with reduced subclinical inflammation in CSFP, supporting the inflammatory basis and therapeutic value of statins

    Prevalence of Sarcopenia in Heart Failure With Mildly Reduced Ejection Fraction and Its Impact on Clinical Outcomes

In this study, the prevalence of sarcopenia and its effect on clinical outcomes in heart failure with mildly reduced ejection fraction (HFmrEF) patients were examined. Methods: A total of 722 patients from three centres who applied to the outpatient clinic with the diagnosis of HFmrEF between 01 January 2020 and 01 June 2021 were included in the study retrospectively. Sarcopenia was diagnosed with a screening test using age, grip srength and calf circumference. At least two-year follow-up results were reviewed from the date the patients were included in the study. Results: Of the 722 HFmrEF patients, 169 (23.4%) were sarcopenic. During the follow-up of sarcopenic patients, a higher rate of hospitalisation and two-year mortality was detected compared to the non-sarcopenic group (49.7% vs 33.3%, p < .001 and 23.7% vs 13.2%, p = .001, respectively). Additionally, atrial fibrillation (AF), chronic obstructive pulmonary disease (COPD), chronic renal failure (CRF) and smoking were detected at higher rates in sarcopenic patients. In subgroup analysis, AF was found to be significantly higher in overweight/obese sarcopenia patients compared to other groups. According to Receiver operating characteristic (ROC) analysis, the sarcopenia score cut-off of 73.61 predicted mortality with 65% sensitivity and 63% specificity, and the cut-off level of 71.10 predicted hospitalisation with 68% sensitivity and 69% specificity. Conclusion: In HFmrEF patients, sarcopenia is associated with adverse events and is an important prognostic marker

    This study is about the book titled Kosovo Lodges- Tombs and Tombstones with Inscriptions written by Raif Virmica. Kosovo lodges, tombs and tombstones with inscriptions, which reflect the architectural tradition and taste of the Ottoman period in the simplest style are included in this book. According to the author, since there have not been for any published works about these structures in Kosovo so far, these structures are subject to extensive research for the first time with this study and introduced to the science public. The purpose of this book, which we will try to introduce, is to identify and present the year of foundation, architectural features and functions of these structures in Kosovo, which have been neglected by both the science community and local people and the state, and sometimes destroyed deliberately , leaving no trace of them. The spread of Islam in the Balkans and Kosovo and the superior success of the Ottoman civilization determined the establishment, development and identity of most cities. In the book, it is stated that it is not known how many lodges have been built since the Ottomans settled in the region and up today, but it is stated that ten sects have been active in Kosovo and one hundred and three lodges have been built since the Ottomans left the region politically. It has also been emphasized that the sects identified in Kosovo are the Halveti Kadiri, Sadi, Melami, Rifai, Bektashi, Nakşibendi, Şazeli and Mevlevi sects and that the lodges belong to these sects. Since the issue of tombs in Kosovo has not been researched before, there is no exact information about how many tombs there are, but as a result of the research, it is stated that there are 350-400 tombs. A lot of detailed information related to 45 lodge tombs, 80 private tombs and the people buried there, the purposes of the visits, and interesting never - known narrations about the tombs and lodges has been revealed in this piece of work. The information given in this book was compiled by the author himself by visiting the lodges and shrines; and the lodges and shrines that still exist in Kosovo have been documented. This piece of work is a very important reference book for studies on the Balkans, specifically Kosovo, as it contains valuable and factual information. The book consists of 623 pages and includes the author’s biography, a short preface, three main chapters, bibliography and photographs

    Association Between ABO Blood Group, Peripheral Artery Disease Lesion Severity, and Coronary Artery Disease Coexistence

    Background: This study aims to investigate the relationship between ABO blood groups and the severity of peripheral artery disease (PAD) lesions, the coexistence of coronary artery disease (CAD) with PAD, and to identify which blood groups may be more predisposed to these conditions. Methods: This study, which has a single-center and retrospective design, includes 305 patients diagnosed with peripheral artery disease (PAD) between 2015 and 2021. The patients were selected from those with at least 50% stenosis detected by computed tomography (CT) angiography. The severity of PAD lesions was evaluated according to the TransAtlantic Inter-Society Consensus (TASC-II) classification, and the presence of CAD was determined by simultaneous conventional coronary angiography. Demographic data, comorbidities, and laboratory results were collected, and statistical analyses were performed using chi-square tests, logistic regression, and t-tests. All of these patients were divided into 2 groups based on their ABO blood types: O and non-O blood groups. Results: This study demonstrates a significant relationship between blood group classification and both the severity of PAD and the prevalence of CAD. Patients with non-O blood groups exhibited a higher likelihood of having severe PAD lesions (TASC-C and TASC-D) compared to those with O blood groups, who more frequently had milder lesions (TASC-A and TASC-B) (P < 0.001). The prevalence of CAD was also significantly higher among patients with non-O blood groups than those with the O blood group (54.4% vs. 36.4%; P = 0.003). In the logistic regression analysis, non-O blood groups, alongside age, hypertension (HT), and chronic kidney disease (CKD), emerged as independent predictors of severe PAD (P < 0.001 for non-O blood groups). Similarly, older age and non-O blood group status were identified as significant independent predictors of CAD (P = 0.004). These findings suggest that blood group classification, particularly non-O groups, may influence the severity and coexistence of PAD and CAD. Conclusion: Our study reveals a significant association between ABO blood groups and both the severity of PAD and the association of CAD, and shows that non-O blood groups are linked to more severe forms of these conditions. These findings highlight the potential role of ABO blood groups in cardiovascular risk stratification, suggesting a need for further research to confirm these associations and understand their clinical implications. © 2025 Elsevier Inc

    Dynamic system simulation of small satellite projects

    A prerequisite to accomplish a system simulation is to have a system model holding all necessary project information in a centralized repository that can be accessed and edited by all parties involved. At the Institute of Astronautics of the Technische Universitaet Muenchen a modular approach for modeling and dynamic simulation of satellite systems has been developed called Dynamic System Simulation (DySyS). DySyS is based on the platform independent description language SysML to model a small satellite project with respect to the system composition and dynamic behavior. A library of specific building blocks and possible relations between these blocks have been developed. From this library a system model of the satellite of interest can be created. A mapping into a C++ simulation allows the creation of an executable system model with which simulations are performed to observe the dynamic behavior of the satellite. In this paper DySyS is used to model and simulate the dynamic behavior of small satellites, because small satellite projects can act as a precursor to demonstrate the feasibility of a system model since they are less complex compared to a large scale satellite project.Space EngineeringAerospace Engineerin

    The effect of atherogenic plasma index on collateral development in patients with chronic coronary total occlusion

    Aim: To demonstrate the correlation between coronary collateral circulation (CCC) and atherogenic plasma index (AIP), one of the factors associated with the formation of collateral vessels. Methods: Hospital records of patients with chronic total occlusion (100% stenosis) in at least one coronary artery were evaluated. Triglycerides, HDL level, triglyceride/HDL ratio and atherogenic plasma index before coronary angiography were assessed for the 451 patients who met the study criteria. Results: Comparison of the two groups in terms of laboratory findings showed that triglyceride/HDL ratio (5.04±3.13 vs 3.56±2.12, p&lt;0.001) and AIP (0.63±0.25 vs 0.48±0.25, p&lt;0.001) were higher with statistical significance in the weak collateral group. The ROC analysis revealed an association between weak collateral formation and atherogenic plasma index with 64.7% sensitivity and 66.2% specificity using a cut-off value of 0.58 for AIP. Accordingly, low AIP was found to be an independent predictor of good collateral artery formation. Conclusion: This study suggests that a high atherogenic plasma index may be an independent factor associated with poor collateral formation
