    The evaluation of compliance to different treatment modalities in COPD patients: pharmacotherapy, different oxygen administration systems

    Long-term oxygen therapy (LTOT) is the only therapeutic modality proven to alter the late course of chronic obstructive pulmonary disease (COPD). In particular, patients with chronic hypoxaemia (resting PaO₂ level<55mmHg in room air) under steady disease state as well as those hypoxaemic patients (resting PaO₂:56-59mmHg) in the presence of cor pulmonale and/or polycythemia require LTOT administration. It is widely known that LTOT improves survival rates in these patients if oxygen is received for at least 15h/day, as it exerts beneficial effects on several pulmonary haemodynamic variables (pulmonary artery pressure, right and left ventricular function, haematocrit, mixed venous PaO₂). Two different oxygen administration systems have been developed for LTOT: concentrators and liquid oxygen, which have actually eliminated high-pressure compressed gas cylinders, present in the early years of LTOT implementation. Several studies have been conducted in different European countries, concerning patients under LTOT and its influence on compliance rate, exercise capacity and quality of life as primary outcome measures for survival. Greece is one of the countries where the issue of LTOT is poorly investigated. Our study is divided into two phases. The aim of the first phase was to investigate the compliance of patients under LTOT in the Prefecture of Ioannina (North-Western Greece) and to identify factors that might affect it. The study included 249 patients under LTOT during one year period. They were visited at home by the investigating staff (a chest specialist and a primary care nurse). Compliance to therapy was defined by the daily duration of the O₂ concentrator use (hours). Patients underwent pulse oximetry, basic spirometry, and trial inhalation of bronchodilators. The results were as follows: Only 67 patients (26.9%) complied with therapy, while the daily concentrator use lasted for 9.7 ± 6.09 hours. SaO₂, FEV₁ and FVC were negatively correlated to patients’ compliance. No significant differences concerning LTOT compliance were noted between men and women, urban and rural population, and between smokers, ex-smokers and non-smokers. A chest physician recommended LTOT for the majority of patients (86.3%), while COPD was the main reason for LTOT prescription (74.7%). Nevertheless, compliance to LTOT was not significantly higher, when prescribed by a respiratory physician. No specific medical instructions and information was recorded for the majority of the examined patients, under LTOT. In conclusion, LTOT overuse and the concomitant poor compliance to therapy is a strong message indicating further investigation to all links included (medical and nursing staff, concentrator dealers and, of course, patients). The relative results and conclusions would reinforce any attempt to settle a national LTOT policy. An organized home-care program for COPD patients is the cornerstone of a positive patients’ attitude regarding LTOT thus improving its efficacy. The aim of the second phase was to investigate the effect of two oxygen sources (concentrator and liquid oxygen) on quality of life, exercise tolerance and compliance to LTOT in patients with COPD. The survey included 104 patients with COPD (31 on concentrator treatment and 73 on liquid). Patients were subjected to pulse oximetry, basic spirometry, measurement of health-related quality of life by the St. George’s Hospital Respiratory Questionnaire instrument (SGRQ) and assessment of dyspnea severity by the modified Borg category scale. Exercise tolerance was evaluated by the SaO₂ value after 3 minutes walking. SaO₂ was measured before and after treatment with each device. Compliance with therapy was defined by the daily duration of oxygen use. The results were the following: The two LTOT groups did not differ significantly concerning PFTs (p=0.49 for FEV₁, 0.98 for FVC, 0.15 for FEV₁/FVC% and 0.38 for FEF₅₀), SaO₂ before (p=0.28), after (p=0.80) O₂ treatment and after the 3 min walking trial (p=0.47) and SGRQ score (p=0.45). The liquid oxygen group was less dyspneic before treatment (p=0.02) but this difference disappeared after treatment (p=0.95). A highly significant difference (p<0.001) was noted concerning the daily oxygen use. Complete compliance to LTOT (≥ 15 h/day) was observed in 12.9% and 42.5% respectively (p<0.005). Noise disturbance was noted only in the concentrator group (80%). In conclusion, stationary liquid oxygen seems to be a satisfactory alternative mode for LTOT treatment in COPD patients, with higher patients’ compliance to therapy in comparison with concentrators.Η χρόνια «κατ’ οίκον» οξυγονοθεραπεία θεωρείται το μόνο θεραπευτικό μέσο, που αποδεδειγμένα μεταβάλλει το τελευταίο στάδιο εξέλιξης της χρόνιας αποφρακτικής πνευμονοπάθειας. Συγκεκριμένα, οι ασθενείς με χρόνια υποξαιμία σε σταθερή κατάσταση (PaO₂ στον ατμοσφαιρικό αέρα σε ηρεμία <55mmHg) καθώς επίσης και οι υποξαιμικοί ασθενείς (PaO₂ στον ατμοσφαιρικό αέρα σε ηρεμία 56- 59mmHg) παρουσία πνευμονικής καρδίας ή/και πολυερυθραιμίας χρειάζονται χρόνια «κατ’ οίκον» οξυγονοθεραπεία. Είναι ευρέως γνωστό, ότι η χρόνια κατ’ οίκον οξυγονοθεραπεία βελτιώνει την επιβίωση σ’ αυτούς τους ασθενείς, εφόσον το οξυγόνο λαμβάνεται για τουλάχιστον 15 ώρες την ημέρα, και ασκεί θετική επίδραση σε διάφορες αιμοδυναμικές μεταβλητές (πίεση πνευμονικής αρτηρίας, λειτουργία της δεξιάς και αριστερής κοιλίας, αιματοκρίτης και μικτή φλεβική PaO₂). Δύο διαφορετικά συστήματα παροχής οξυγόνου κυκλοφορούν σήμερα: ο συμπυκνωτής και το υγρό οξυγόνο, εκτοπίζοντας τη φιάλη αέριου συμπιεσμένου οξυγόνου, που χρησιμοποιήθηκε αρκετά τα πρώτα χρόνια εφαρμογής της χρόνιας οξυγονοθεραπείας. Αρκετές μελέτες έχουν διεξαχθεί σε διάφορες Ευρωπαϊκές χώρες, σχετικά με τη χρόνια «κατ' οίκον» οξυγονοθεραπεία και τα διάφορα συστήματα παροχής αυτής. Στη χώρα μας το σημαντικό ζήτημα της εφαρμογής της χρόνιας «κατ’ οίκον» οξυγονοθεραπείας και κατ’ επέκταση των κατευθυντήριων οδηγιών συνταγογράφησης και συμμόρφωσης σ’ αυτή τη θεραπεία δεν έχει διεξοδικά και ενδελεχώς διερευνηθεί. Η παρούσα μελέτη χωρίζεται σε δύο φάσεις. Σκοπός της πρώτης φάσης της μελέτης ήταν η διερεύνηση της συμμόρφωσης των ασθενών, που λαμβάνουν χρόνια «κατ’ οίκον» οξυγονοθεραπεία και ο προσδιορισμός τυχόν παραγόντων, που μπορεί να την επηρεάζουν. Στη μελέτη έλαβαν μέρος 249 ασθενείς που υποβάλλονταν σε χρόνια κατ’ οίκον οξυγονοθεραπεία. Η μελέτη διήρκησε ένα έτος. Εξειδικευμένο ιατρικό και νοσηλευτικό προσωπικό επισκεπτόταν τους ασθενείς στην οικία τους και τους υπέβαλε σε μέτρηση κορεσμού αρτηριακού αίματος, σπιρομέτρηση και έλεγχο της ικανότητας λήψης βρογχοδιασταλτικής αγωγής. Η συμμόρφωση προς την οξυγονοθεραπεία ορίστηκε ως η καθημερινή χρήση του συστήματος παροχής οξυγόνου (στην προκειμένη περίπτωση συμπυκνωτής). Μόνο εξήντα επτά (67) ασθενείς (26.9%) συμμορφώθηκαν προς την οξυγονοθεραπεία, ενώ η ημερήσια χρήση του οξυγόνου μέσω συμπυκνωτή βρέθηκε 9.7±6.09 ώρες. Το επίπεδο του κορεσμού του αρτηριακού αίματος, ο μέγιστος εκπνεόμενος όγκος στο πρώτο δευτερόλεπτο και η δυναμική ζωτική χωρητικότητα συσχετίστηκαν αρνητικά με τη συμμόρφωση των ασθενών στην οξυγονοθεραπεία. Δε σημειώθηκαν στατιστικά σημαντικές διαφορές, όσον αφορά τη συμμόρφωση, μεταξύ ανδρών-γυναικών, αστικού-αγροτικού πληθυσμού και ενεργών καπνιστών, πρώην καπνιστών και μη καπνιστών. Πνευμονολόγοι συνταγογράφησαν οξυγόνο στην πλειοψηφία των ασθενών (86.3%), ενώ η ΧΑΠ ήταν η κυριότερη πάθηση για την οποία συνεστήθη οξυγονοθεραπεία (74.7%). Ωστόσο, η συμμόρφωση προς την οξυγονοθεραπεία δεν ήταν σημαντικά μεγαλύτερη στους ασθενείς, που το οξυγόνο συνταγογραφήθηκε από πνευμονολόγο. Επίσης, 136 ασθενείς (52.6%) δεν είχαν λάβει από το θεράποντα ιατρό τους ακριβείς και γραπτές οδηγίες σχετικά με τη χρήση της χρόνιας «κατ’ οίκον» οξυγονοθεραπείας, ενώ 157 ασθενείς (63%) δεν είχαν επίγνωση του ρόλου της χρόνιας οξυγονοθεραπείας στη συνολική θεραπευτική τους αντιμετώπιση. Η χρησιμοποίηση σε υπερβολικό βαθμό της χρόνιας «κατ’ οίκον» οξυγονοθεραπείας και η συνεπακόλουθη ελλιπής συμμόρφωση στη θεραπευτική αυτή μορφή καταδεικνύει κατηγορηματικά, ότι απαιτείται περαιτέρω διερεύνηση συμπεριλαμβανομένων του ιατρικού και νοσηλευτικού προσωπικού, των εταιρειών παροχής και προμήθειας συμπυκνωτών και οπωσδήποτε των ασθενών. Τα αποτελέσματα, που θα προκύψουν και τα συμπεράσματα, που θα εξαχθούν, θα ενισχύσουν οποιαδήποτε προσπάθεια για την εφαρμογή μιας εθνικής πολιτικής, αναφορικά με το ζήτημα της χρόνιας «κατ’ οίκον» οξυγονοθεραπείας στη χώρα μας. Ένα οργανωμένο πρόγραμμα ιατρικής φροντίδας «κατ’ οίκον» για τους ασθενείς με ΧΑΠ θα είναι ο ακρογωνιαίος λίθος της θετικής στάσης των ασθενών απέναντι στη χρόνια «κατ’ οίκον» οξυγονοθεραπεία βελτιώνοντας έτσι την αποτελεσματικότητά της. Σκοπός της δεύτερης φάσης της μελέτης ήταν η διερεύνηση της επίδρασης δύο συστημάτων παροχής οξυγόνου (συμπυκνωτής και υγρό οξυγόνο) στην ποιότητα ζωής, την ανοχή στην άσκηση και τη συμμόρφωση σε ασθενείς με ΧΑΠ. Η μελέτη περιέλαβε 104 ασθενείς με ΧΑΠ (31 χρησιμοποιούσαν συμπυκνωτή και 73 υγρό οξυγόνο). Οι ασθενείς υποβλήθηκαν σε παλμική οξυμετρία, σπιρομέτρηση, αξιολόγηση της ποιότητας ζωής βάσει ειδικού ερωτηματολογίου (ερωτηματολόγιο του Νοσοκομείου St. George) και αξιολόγηση του βαθμού της δύσπνοιας από την τροποποιημένη κλίμακα Borg. Η ικανότητα ανοχής στην άσκηση εκτιμήθηκε από το επίπεδο του κορεσμού του αρτηριακού αίματος μετά από δοκιμασία άσκησης 3 λεπτών. Η συμμόρφωση στην οξυγονοθεραπεία ορίστηκε από την ημερήσια χρήση της κάθε συσκευής. Οι δύο ομάδες των ασθενών δε διέφεραν σημαντικά όσον αφορά τις παραμέτρους της αναπνευστικής λειτουργίας (p=0.49 για το μέγιστο εκπνεόμενο όγκο στο πρώτο δευτερόλεπτο, p=0.98 για τη δυναμική ζωτική χωρητικότητα, p=0.15 για το δείκτη Tiffeneau και p=0.38 για τη μέγιστη μεσοεκπνευστική ροή), το επίπεδο κορεσμού πριν (p=0.28) και μετά τη χορήγηση οξυγόνου (p=0.80) και μετά τη δοκιμασία βάδισης 3 λεπτών (p=0.47) και το ερωτηματολόγιο St. George (p=0.47). Η ομάδα του υγρού οξυγόνου παρουσίασε λιγότερο δύσπνοια πριν τη θεραπεία (ρ=0.02), αλλά αυτή η διαφορά εξαφανίστηκε μετά τη θεραπεία (p=0.95). Μία στατιστικά σημαντική διαφορά (p<0.001) σημειώθηκε σχετικά με την ημερήσια χρήση του οξυγόνου υπέρ της ομάδας ασθενών με το υγρό οξυγόνο. Πλήρης συμμόρφωση στην οξυγονοθεραπεία παρατηρήθηκε στο 12.9% της ομάδας του συμπυκνωτή και στο 42.5% της ομάδας του υγρού οξυγόνου (p<0.005). Ο θόρυβος από τη συσκευή παρατηρήθηκε μόνο στην ομάδα ασθενών, που χρησιμοποιούσε συμπυκνωτή (80%). Συμπερασματικά, με τη δεύτερη φάση διαπιστώθηκε ότι το σταθερό υγρό οξυγόνο αποτελεί ικανοποιητικό εναλλακτικό τρόπο οξυγονοθεραπείας στους ασθενείς με ΧΑΠ με υψηλότερη συμμόρφωση συγκριτικά με τους συμπυκνωτές

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
