Role of laboratory dental technician in the multidisciplinary treatment of a child with a cleft palate
Uvod: Heilognatopalatoshiza (orofacialni razcep, shiza), znana tudi kot zajčja ustnica in volčje žrelo, predstavlja najpogostejšo kongenitalno nepravilnost na obrazu in ustih ter ima velik vpliv na življenje otroka. Pojavnost shiz v Sloveniji je približno 1 na 600 živorojenih otrok, pri čemer se razcepi delijo na razcepe ustnice, čeljustnega grebena, trdega in mehkega neba. Razcepi so lahko popolni, nepopolni, enostranski in obojestranski. Razvoj obraza se odvija med 4. in 12. tednom nosečnosti, ko se klične plošče združujejov primeru shize ta proces ne poteka pravilno, posledica nepopolnega združevanja izboklin je razcep. Uspešna obravnava otrok s shizo zahteva multidisciplinarno obravnavo, v zdravljenje so vključeni neonatolog, pediater, maksilofacialni kirurg, ortodont, laboratorijski zobni protetik, logoped, otorinolaringolog in po potrebi še drugi strokovnjaki. Zdravljenje v Sloveniji je dobro urejeno in poteka po natančno izdelanem protokolu. Namen: Namen diplomskega dela je predstaviti zdravljenje otroka s shizo v Sloveniji, opisati vlogo laboratorijskega zobnega protetika in tehnično izdelati palatinalno ploščo z nosnim podpornikom. Metode dela: Diplomsko delo smo začeli s pregledom literature o shizah, pri čemer smo uporabili vire v slovenščini in angleščini. Literaturo smo večinoma iskali na platformah PubMed, COBISS, Science Direct in Google Scholar, iskanje pa je bilo omejeno na zadnjih 10 let. Zaradi relevantnosti smo uporabili tudi starejše članke. Postopki izdelave so bili dokumentirani s telefonom iPhone 12 in obdelani v programu Pixlr. Rezultati: Opisali smo postopek izdelave nebne plošče z nosnim podpornikom. V laboratoriju v UKC Ljubljana smo izdelali nebno ploščo za otroka s heilognatopalatoshizo. Opisan in predstavljen je bil postopke izdelave. Razprava in zaključek: V diplomskem delu smo predstavili zdravljenje otroka s heilognatopalatoshizo v Sloveniji in opisali izdelavo palatinalne plošče z nosnim podpornikom. V delu je poudarjena vloga laboratorijskega zobnega protetika pri dojenčkovi predoperativni pripravi. Protokoli za zdravljenje shiz po svetu so različni, v Sloveniji se shize operirajo po ljubljanskem protokolu. Zaradi dobrih rezultatov je postala predoperativna priprava standardni postopek pri zdravljenju heilognatopalatoshiz v Sloveniji.Introduction: Cheilognatopalatoschisis (orofacial cleft), also known as cleft lip and cleft palate, is the most common congenital facial abnormality, significantly impacting a child\u27s life. The incidence of clefts in Slovenia is approximately 1 in 600 live births, with clefts classified into lip clefts, alveolar ridge clefts, and hard and soft palate clefts. Clefts can be complete or incomplete, unilateral or bilateral. Facial development occurs between the 4th and 12th week of pregnancy when processes fusein cases of clefts, this process does not occur properly, leading to incomplete fusion and resulting in a cleft. Successful treatment of children with clefts requires a multidisciplinary approach, involving a neonatologist, pediatrician, maxillofacial surgeon, orthodontist, dental prosthetist, speech therapist, otorhinolaryngologist, and other specialists as needed. Treatment in Slovenia is well-organized and follows a carefully developed protocol. Purpose: The objective of this diploma work is to present the treatment of a child with cleft lip and palate in Slovenia, to describe and technically fabricate a palatal plate with a nasal molding, and to discuss the challenges faced by patients after surgical treatment. Methods: We began the thesis with a literature review on clefts, utilizing sources in both Slovenian and English. Most of the literature was sourced from the platforms PubMed, COBISS, Science Direct and Google Scholar, with a focus on the past 10 years. However, due to the relevance of certain articles, some older sources were also included. The process of making the NAM device was documented using Iphone 12 and edited with Pixlr. Results: in the laboratory at the University Medical Centre Ljubljana a nasoalveolar molding device was made for a child with cheilognatopalatoschisis. The process of making the NAM device was described and presented in detail. Discussion and conclusion: In the thesis, we presented the treatment of a child with cleft lip and palate in Slovenia and described the fabrication of a palatal plate with a nasal support. The role of the laboratory dental prosthetist in the infant\u27s preoperative preparation was emphasized. Treatment protocols for clefts vary around the world, and in Slovenia they are treated according to the Ljubljana protocol. Due to the good results, preoperative preparation has become a standard procedure in the treatment of cleft lip and palate in Slovenia
The evaluation of mandibular prognathism treatment and the fabrication of an intraoperative acrylic occlusal splint
Uvod: Pri ljudeh z razvojnimi ali prirojenimi nepravilnostmi obraznega skeleta je za izboljšanje estetskih in funkcionalnih nepravilnosti potrebna poleg ortognatsko kirurškega posega še ortodontska obravnava. S tem pristopom lahko pridemo do želenih rezultatov in uspešnega zdravljenja nepravilnosti, ki so bile nedavno nazaj še nerešljive. Namen: Želeli smo ugotoviti, kakšne rezultate pridobimo z meritvami mavčnih modelov zgornje in spodnje čeljustnice, pri pacientih, ki so bili zdravljeni zaradi mandibularnega prognatizma. Namen diplomskega dela je bil predstaviti pristop pri zdravljenju zobnih in čeljustnih nepravilnosti, prikazati, kako pomembno je tesno sodelovanje različnih strokovnjakov za dosego želenega rezultata, in opisati postopek izdelave intraoperativne akrilne okluzijske opornice. Metode dela: Izbrali smo osem pacientov, ki so bili zdravljeni zaradi mandibularnga prognatizma. Na mavčnih modelih smo izvedli meritve sprednje in zadnje transverzalne širine ter meritve incizalnega previsa in incizalne stopnice. Ugotavljali smo, do kakšnih razlik pride med samim potekom zdravljenja in katera vrsta zdravljenja ima pri različnih meritvah največji vpliv. Metode dela, uporabljene za izdelavo diplomskega dela, so bile poleg meritev še pregled in študij strokovne literature, priprava slikovnega gradiva, praktično laboratorijsko delo ter posvet z mentorjem. Rezultati: Meritve smo na modelih izvedli v začetnem stanju - pred samim pričetkom zdravljenja, po ortodontski obravnavi – pred operativnim posegom in na koncu zdravljenja. Pri začetnem ortodontskem zdravljenju in pripravi na operativni poseg pride do sprememb v transverzalni širini čeljustnic, pri operativnem posegu pa pride do sagitalnih premikov, medtem ko ostanejo transverzalne širine enakih vrednosti kot pred operativnim posegom. Nanje lahko v majhni meri vplivamo s končnim ortodontskim zdravljenjem. Predstavili smo tudi postopek in potek izdelave intraoperativne akrilne okluzijske opornice, ki se uporablja med operativnim posegom. Izdelali smo jo v zobnem laboratoriju. Razprava in zaključek: Rezultati so pokazali, kako zelo pomembno je temeljno načrtovanje in sodelovanje vseh strokovnjakov ter pacienta samega. Ortodontska predpriprava je ključnega pomena za nadaljnji operativni poseg in s sodelovanjem obeh strok, tako ortodontske kot kirurške pridemo do zastavljenih rezultatov pri zdravljenju mandibularnega prognatizma.Introduction: People with developmental and congenital anomalies of facial skeleton need orthognathic surgery as well orthodontic treatment in order to improve their aesthetic and functional abnormalities. Including orthodontic treatment in the healing process leads to desired results and enables us to treat anomalies that were thought insoluble only a while ago. Purpose: Our aim was to discover what results can be achieved when measuring dental gypsum models of both upper and lower jaw of patients who were treated for mandibular prognathism. The purpose of this diploma paper was primarily to introduce an approach for the treatment of dental and skeletal irregularities as well as to underline the importance of cooperation between various experts in order to reach the optimal results and describe a procedure of fabrication of intraoperative acrylic occlusal splint. Methods: We have selected eight patients that had previously been treated for mandibular prognathism. Using a dental gypsum model we carried out measurements of front and back transversal width, overjet and overbite. We examined the differences that occurred in the process of healing and determined the type of healing that was most suitable during various stages of measurements. Work methods used for the purpose of diploma paper were not limited to measurements but included also an overview of academic and scientific literature, preparation of visual materials, practical lab work and consultations with mentor. Results: The measurements on dental models were carried out four times: measurements before the treatment (initial stage), following orthodontic treatment, before surgical procedure and at the end of therapy. During the initial orthodontic treatment and while preparation for surgical procedure some changes occur in the transversal width of jaws, however sagittal shifts occurs during the surgical procedure alone, while in this stage the transversal width remains the same as beforehand. The latter can be influenced to a small extent with a final orthodontic treatment. The paper presents the technique of the production of intraoperative acrylic occlusal splint that is used during the surgery. The occlusal splint presented in the diploma paper was produced in a dental laboratory. Discussion and conclusion: The results have underlined and exposed great importance of careful planning and cooperation between experts from various fields and the patient. Orthodontic pre-treatment is of key importance for the subsequent surgical procedure. Only when there is cooperation between orthodontic and surgical experts, the optimal results in treating of mandibular prognathism can occur
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases.
