Environment Bureau of Epidemiology and Public Health Informatics"--Page 6.
Includes bibliographical references."At approximately 2:00 p.m. on April 18, 2011, the Kansas Department of Health and Environment Bureau of Epidemiology and Public Health Informatics (KDHE) was notified of two laboratory-confirmed cases of salmonellosis among a group who recently traveled to Negril, Jamaica to attend a destination wedding held at the Beaches Negril resort. Some guests arrived as early as March 27, 2011, and all departed on April 3, 2011. A spokesperson for the wedding group estimated four others became ill following the trip. KDHE began an investigation immediately, working to obtain names and contact information for the group, which included 31 individuals from 17 different households -- residents of Kansas, Missouri, Arizona, Arkansas, South Dakota, Virginia, New York, New Jersey, and Germany. Because of the multijurisdictional nature of this outbreak, KDHE distributed an alert on April 26 via the Epidemic Information Exchange (Epi-X), a communication network maintained by the Centers for Disease Control and Prevention (CDC) that allows state, local, and federal partners to exchange disease outbreak information. KDHE requested to be notified of additional cases of salmonellosis among travelers to Beaches Negril. A similar notification was distributed via CDC's foodborne outbreak listserv on April 27.

FCHD reviewed the case on the day of notification, but contact with the parent of the case did not occur until the afternoon of Monday, June 13. Further investigation and surveillance identified additional cases; all cases were associated to Dodge City, Kansas. There were approximately 54 residents and had 80 staff members at the LTCF at the time of the report. Within the next 24 hours, two staff members and three residents exhibiting gastrointestinal symptoms tested positive for influenza A. The LTCF was asked to evaluate residents and staff for respiratory symptoms in addition to gastrointestinal symptoms. SCHD and KDHE-BEPHI began an outbreak investigation to determine the causes of illness and to recommend prevention and control measures.

Scott Harrington, DMD were potentially exposed to blood-borne pathogens and that testing for HIV, hepatitis C (HCV), and hepatitis B (HBV) was recommended. This announcement resulted from the discovery of numerous, longstanding improper infection control practices after an epidemiologic investigation of an acute case of hepatitis C was associated to a dental surgical procedure The Kansas Department of Health and Environment (KDHE) was notified on April 1, 2013 at 9:30 AM by the Montgomery County Health Department of an individual reporting to be a patient of an implicated clinic. KDHE immediately worked to coordinate with the OSDH to obtain a laboratory specimen collection protocol, intake summary, and a listing of potential Kansas residents who were notified through OSDH patient notification letters. Patient records available from 2007 were used to notify current and previous patients of the situation and to offer free HIV, HCV, and HBV testing through the OSDH Public Health Laboratory (PHL) for persons who identified themselves as direct patients of Dr. Harrington's offices in Tulsa and Owasso, Oklahoma. OSDH notified other state health departments via the Epidemic Information Exchange (Epi-X) on Sunday, March 31, 2013, providing a summary of actions taken and OSDH contact information.

By 10:38 a.m., a questionnaire was emailed to the complainant and the affected group of employees."
