    Recent trends in asthma diagnosis, preschool wheeze diagnosis and asthma exacerbations in English children and adolescents: a SABINA Jr study

    Background Asthma-related burden remains poorly characterised in children in the UK. We quantified recent trends in asthma prevalence and burden in a UK population-based cohort (1-17-year-olds). Methods The Clinical Practice Research Datalink Aurum database (2008-2018) was used to assess annual asthma incidence and prevalence in 1-17-year-olds and preschool wheeze in 1-5-year-olds, stratified by sex and age. During the same period, annual asthma exacerbation rates were assessed in those with either a diagnosis of preschool wheeze or asthma. Results Annual asthma incidence rates decreased by 51% from 1403.4 (95% CI 1383.7 to 1423.2) in 2008 to 688.0 (95% CI 676.3 to 699.9) per 10 5 person-years (PYs) in 2018, with the most pronounced decrease observed in 1-5-year olds (decreasing by 65%, from 2556.9 (95% CI 2509.8 to 2604.7) to 892.3 (95% CI 866.9 to 918.3) per 10 5 PYs). The corresponding decreases for the 6-11- and 12-17-year-olds were 36% (1139.9 (95% CI 1110.6 to 1169.7) to 739.9 (95% CI 720.5 to 759.8)) and 20% (572.3 (95% CI 550.4 to 594.9) to 459.5 (95% CI 442.9 to 476.4)) per 10 5 PYs, respectively. The incidence of preschool wheeze decreased over time and was slightly more pronounced in the 1-3 year-olds than in the 4-year-olds. Prevalence of asthma and preschool wheeze also decreased over time, from 18.0% overall in 2008 to 10.2% in 2018 for asthma. Exacerbation rates increased over time from 1.33 (95% CI 1.31 to 1.35) per 10 PYs in 2008 to 1.81 (95% CI 1.78 to 1.83) per 10 PYs in 2018. Conclusion Paediatric asthma incidence decreased in the UK since 2008, particularly in 1-5-year-olds; this was accompanied by a decline in asthma prevalence. Preschool wheeze incidence also decreased in this age group. However, exacerbation rates have been increasing.</p

    The relationship between cardiac troponin in people hospitalised for exacerbation of COPD and major adverse cardiac events (MACE) and COPD readmissions

    Background: no single biomarker currently risk stratifies chronic obstructive pulmonary disease (COPD) patients at the time of an exacerbation, though previous studies have suggested that patients with elevated troponin at exacerbation have worse outcomes. This study evaluated the relationship between peak cardiac troponin and subsequent major adverse cardiac events (MACE) including all-cause mortality and COPD hospital readmission, among patients admitted with COPD exacerbation.Methods: data from five cross-regional hospitals in England were analysed using the National Institute of Health Research Health Informatics Collaborative (NIHR-HIC) acute coronary syndrome database (2008–2017). People hospitalised with a COPD exacerbation were included, and peak troponin levels were standardised relative to the 99th percentile (upper limit of normal). We used Cox Proportional Hazard models adjusting for age, sex, laboratory results and clinical risk factors, and implemented logarithmic transformation (base-10 logarithm). The primary outcome was risk of MACE within 90 days from peak troponin measurement. Secondary outcome was risk of COPD readmission within 90 days from peak troponin measurement.Results: there were 2487 patients included. Of these, 377 (15.2%) patients had a MACE event and 203 (8.2%) were readmitted within 90 days from peak troponin measurement. A total of 1107 (44.5%) patients had an elevated troponin level. Of 1107 patients with elevated troponin at exacerbation, 256 (22.8%) had a MACE event and 101 (9.0%) a COPD readmission within 90 days from peak troponin measurement. Patients with troponin above the upper limit of normal had a higher risk of MACE (adjusted HR 2.20, 95% CI 1.75–2.77) and COPD hospital readmission (adjusted HR 1.37, 95% CI 1.02–1.83) when compared with patients without elevated troponin.Conclusion: an elevated troponin level at the time of COPD exacerbation may be a useful tool for predicting MACE in COPD patients. The relationship between degree of troponin elevation and risk of future events is complex and requires further investigation

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    Trends in asthma incidence in children: a UK population-based cohort study

    Background: Evidence suggests that asthma incidence has steadily declined in the past decade, especially in children. These trends have not been thoroughly explored across age groups in the UK.Aim: To quantify recent trends in asthma incidence in a UK paediatric population using a large population-based cohort.Methods: We identified 498,503 new asthma cases in patients aged 1-17 years contributing data to the Clinical Practice Research Datalink Aurum database from 1/1/07-31/12/19. We calculated annual asthma incidence rates for 3 different age groups (1-5, 6-11, 12-17), stratified by gender.Results: Overall annual incidence rate fell continuously from 1,470 (95% CI;1449–1490) in 2007 to 634 (623–645) per 105 person years (pys) in 2019 (57% decline). Decline was similar between genders. The most pronounced decrease was in the 1-5 age group (Fig), in whom incidence dropped from 2813 (2762–2864) to 759 (735–783) per 105 pys (73% decline). The corresponding data for the 6-11 and 12-17 age group were 1156 (1126 – 1187) to 727 (707 – 747) (37% decline) and 550 (529 – 573) to 430 (415 – 447) (22% decline) per 105 pys, respectively.Conclusion: The rate of new asthma diagnosis has fallen substantially in UK children since 2007. The pronounced decline in the 1-5 age group may be due to a change in diagnostic criteria for wheeze and asthma. Whilst there was some decrease in the 6-11 age group, incidence in 12-17year olds was relatively stable

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Construction and assessment of risk models in medicine

    This thesis investigates the application of classical and contemporary statistical methods in medical research attempting to bridge the gap between statistics and clinical medicine. The importance of using simple and advanced statistical methods in constructing and assessing risk models in medicine will be demonstrated by empirical studies related to vascular complications: namely abdominal aortic aneurysm and diabetic retinopathy. First, data preprocessing and preliminary statistical analysis are examined and their application is investigated using data on abdominal aortic aneurysm. We illustrate that when dealing with missing data, the co-operation between statisticians and clinicians is necessary. Also, we show advantages and disadvantages of exploratory analysis. Second, we describe and compare classification models for AAA selective screening. Tow logistic regression models are proposed. We also show that it is important to assess the performance of classifiers by cross-validation and bootstrapping. We also examine models that include other definitions of abnormality, weighted classification and multiple class models. Third, we consider the application of graphical models. We look at different types of graphical models that can be used for classification and for identifying the underlying data structure. The use of Naïve Bayes classifier (NBC) is shown and subsequently we illustrate the Occam’s window model selection in a statistical package for Mixed Interactions Modelling (MIM). The EM-algorithm and multiple imputation method are used to deal with inconsistent entries in the dataset. Finally, modelling mixture of Normal components is investigated by graphical modelling and compared with an alternative minimisation procedure. Finally, we examine risk factors of diabetic sight threating retinopathy (STR). We show the complexity of data preparation and preliminary analysis as well as the importance of using the clinicians’ opinion on selecting appropriate variables. Blood pressure measurements have been examined as predictors of STR. The fundamental role of imputation and its influence on the conclusions of the study are demonstrated. From this study, we conclude that the application of statistics in medicine is an optimisation procedure where both the statistical and the clinical validity need to be taken into account. Also, the combination of simple and advanced methods should be used as it provides additional information. Data, software and time limitations should be considered before and during statistical analysis and appropriate modifications might be implemented to avoid compromising the quality of the study. Finally, medical research should be regarded for statisticians and clinicians as part of a learning process

    Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods
