Prevention of preeclampsia with acetylsalicylic acid: a brief history of use and modern prevention methods

By preeclampsia (PE) we mean hypertension, which first occurred after 20 weeks pregnant in combination with proteinuria or other signs of target organ damage. PE is accompanied by high maternal and perinatal mortality, and these indicators are the higher the earlier the disease onset is. Despite the presence of seemingly effective methods for the PE prevention, this pathology incidence has practically not changed in recent decades.
This may be due to the fact that the PE pathogenesis is complex and has not been fully studied. We are still continuing to look for new prognostic markers that would allow us to select high-risk patients. In the high-risk group, acetylsalicylic acid (ASA) intake reduces the PE incidence, but the overall picture does not change significantly. Besides, ASA long-term use may increase the risks of some pregnancy complications. Differential treatment tactics for prevention in different risk groups is likely to be effective. The article summarizes the confirmed data on ASA used for the PE prevention and its complications, as well as promising areas for further research.

Acetylsalicylic acid to prevent preeclampsia and its complications. State-of-the-art of risk groups

This paper reviews current published data on the use of acetylsalicylic acid (ASA) for preventing preeclampsia and its complications and techniques to determine risk groups for im-plementing this preventive treatment. In addition, the authors summarize data on the history of ASA advent and introduction into medical practice (particularly obstetrics) and presumed mech-anisms of its action as first-line therapy to prevent preeclampsia. Recent review papers and guidelines addressing optimal ASA dose, treatment duration, and dosing during pregnancy in Russia and worldwide are discussed. Methods that have been studied as preventive ones for preeclampsia are briefly discussed. It was demonstrated that anamnestic determination of risk groups of early preeclampsia is not always enough. Expanded combined first-trimester screening is a perspective diagnostic tool. Success in determining high-risk groups and optimal prevention of preeclampsia may significantly affect population health by reducing maternal and perinatal mortality and long-term risk of cardiovascular events both in the mother and child.

HIGH-ENERGY PARTICLES FLUX ORIGIN IN THE CLOUDS, DARK LIGHTNING
Problem of high-energy particles flux origin in clouds is discussed. Conditions in which dark lightning preceding the ordinary one and creating additional ionization, fluxes of fast electrons with MeV energy prior to the earthquake detected among lightning initiating ball-lightning, glow, sprites are considered. All above phenomena appear to be of general nature founded on quantum entanglement of hydrogen bonds protons in water clasters inside clouds
The 85th Anniversary of Academician V.V. Alekseev
In the article dedicated to the 85th anniversary of Doctor of Historical Sciences, Professor and Academician of the Russian Academy of Sciences Veniamin V. Alekseev, the author traces the main milestones of the scientific biography of the prominent historian who is a well-known specialist in the field of national history and the theory of historical process and the founder of the Institute of History and Archaeology of the Ural Branch of the Russian Academy of Sciences. The paper considers the main stages of the formation of the scientist, his scientific, organizational and administrative activities in the field of historical science. The author highlights the scientific results of Academician V.V. Alekseev that made him well known in Russia and abroad, his theoretical and methodological studies and such large blocks of scientific research as the history of Russian modernizations, the phenomenon of Russian regionalism, the history of metallurgy of the Urals and industrial heritage. The article also focuses on the features of the scientific method of V.V. Alekseev and his merits in the formation of the documentary base for historical research

The author provides a review of the most interesting and significant works, devoted to the study of game phenomenon in the creative work by V.V. Nabokov. Besides, the meaning of notion "game text" is expanded in the work; there are also made the conclusions about lexical organization of the novel game space (it is a chessboard in this case) and about ways of characterization of this space: 1) direct characterization (through description); 2) indirect characterization (through the comparison with another subject)
