Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Predictors of healthy aging: epigenetic factors and the APOE gene
INTRODUÇÃO: Fatores genéticos estão associados a fenótipos do envelhecimento como a longevidade e algumas doenças relacionadas à idade, mas a identificação dos genes envolvidos no fenótipo complexo do envelhecimento permanece um desafio. Estudos de associação genômica ampla (GWAS), têm apontado o gene da Apolipoproteína E (APOE) como o único associado de maneira consistente à longevidade e alguns fenótipos do envelhecimento. Até o momento, entretanto, a associação do gene APOE com fenótipos alternativos como o envelhecimento saudável (ES), não tem sido demonstrada de maneira consistente. OBJETIVOS: determinar a influência do polimorfismo do APOE na variação de índices prognósticos do envelhecimento (multimorbidade, funcionalidade e acúmulo de déficits) em dez anos. MÉTODOS: estudo longitudinal de coorte retrospectiva de 125 idosos independentes inicialmente para atividades de vida diária (ABVD e AIVD), de um ambulatório assistencial em um hospital escola, em São Paulo. Na avaliação inicial, foram identificados o polimorfismo do gene APOE (alelos E2, E3 e E4), variável preditora primária, e algumas variáveis epigenéticas (clínicas, sociodemográficas e hábitos de vida). No seguimento, avaliações anuais de prontuários foram realizadas para identificar associações com perdas de envelhecimento fisiológico (índice de acúmulo de déficits, DI), ganho de multimorbidade (Cumulative Illness Rating Scale for Geriatrics, CIRS-G e Charlson comorbidity index, CCI) e dependência em ABVD/AIVD, e morte, em dez anos. Curvas de Kaplan-Meier e modelos de regressão de Cox foram utilizados para associações dos fatores genéticos e epigenéticos com piora funcional e morte, e de regressão linear múltipla para associações com os índices CIRS-G, CCI e DI. RESULTADOS: 125 participantes da avaliação inicial, idade média de 74,9 anos, 76,8% mulheres, e 81,6% brancos. A média do CIRS-G foi de 10,24. A distribuição dos alelos: E2 10%, E3 50,8%, e E4 39,2%. A presença de E2 e E4 em relação ao grupo controle E3/E3 não foram preditoras de piora nos índices prognósticos do envelhecimento. Alguns fatores epigenéticos apresentaram associações com desfechos em dez anos. Maior acúmulo de déficits: idade mais elevada p = 80 anos, sedentarismo e tabagismo). A idade >=80 anos, sexo masculino e sedentarismo, foram associados a perda em dez anos de funcionalidade para ABVD e AIVD. Mortalidade em dez anos foi associada a idade >= 80 anos e alta multimorbidade através do CCIBACKGROUND: While genetic factors are linked to longevity and agerelated diseases, the identification of genes responsible for different aging phenotypes remains unclear. Previous studies, including the genome-wide association studies (GWAS), have indicated that Apolipoprotein E (APOE) gene is associated with longevity and other aging phenotypes. However, little is still known on the association of the APOE gene with healthy aging. OBJECTIVES: To determine the influence of the APOE gene polymorphism on the variation of aging prognostic indices (functionality, multimorbidity, and accumulation of deficits) over 10 years. METHODS: A retrospective cohort study comprising 125 older adults who were independent in activities of daily living (ADL and IADL) at baseline. Participants were evaluated at an ambulatory setting from an academical medical center at the University of Sao Paulo Medical School. Baseline assessment included the identification of the APOE gene polymorphism (alleles E2, E3, and E4), which was the primary predictor, and other epigenetic variables such as sociodemographic factors, multimorbidity, and behavior measures. Annual follow-up evaluations over 10 years were conducted to identify dependencies in ADL and IADL, multimorbidity, and death using the hospital medical charts. A cumulative deficit index was computed using the patients\" multimorbidity and functionality every year. Kaplan-Meier curves and Cox proportional hazard models were used to associate the genetic and epigenetic factors with time to dependence in ADL and IADL and death. Multiple linear regression models examined the association of risk factors with the scores of the Cumulative Illness Rating Scale for Geriatrics (CIRS-G), Charlson comorbidity index, and deficit index at 10 years. RESULTS: Participants had a mean age of 74.9 years; 76.8% were female, and 81.6% white. The mean score in CIRS-G was 10.2 points. The distribution of the alleles of APOE was 10% for E2, 50.8% E3, and 39.2% E4. Compared to the allele E3/E3 (control group), the presence of allele E2 did not predict dependence in ADL and IADL. The presence of allele E4 did not predict any outcome. Some epigenetic risk factors were associated with the outcomes over 10 years. For significant increase in deficit index: older age, p < 0,001; sedentarism, p=0,02; tobacco consumption p=0,02 .For dependence in ADL: older age, p=0.002; men p=0,01 ; higher scores in the CIRS-G, p=0.02; sedentarism, p=0.02. For dependence in IADL: older age, p < 0.001; men p=0,01 ; sedentarism, p=0.02 For mortality: older age, p=0,002; higher score in the Charlson comorbidity index, p=0.001. While metabolic activities (beta=-4,37; p=0.003) and living alone (beta=-2,28; p=0.005) were associated with a lower score in the CIRS-G at 10 years of follow-up, alcohol consumption (beta=1,78; p=0.04) was associated with higher scores in this index. CONCLUSIONS: The APOE gene polymorphism did not influence the prognosis of aging. Older age, sedentarism and tobacco consumption were associated with a significant increase in deficit index. Older age, men and sedentarism were predictors of functional loss. Metabolic activities and living alone were associated with a lower score in the CIRS-G at 10 years of follow-up. Self care of elderly people admitted at a geriatric ward of a school hospital: comparison between subjective and objective evaluations
Estudo observacional quantitativo longitudinal prospectivo em idosos e seus cuidadores, de ambos os sexos, internados na enfermaria de geriatria de um hospital-escola, com coleta de dados no período de agosto de 2013 a março de 2014. Objetivo: Avaliar a condição funcional subjetiva e objetiva de idosos internados em uma enfermaria geriátrica; entre os relatos de desempenho no autocuidado e suas necessidades de ajuda. Método: avaliações subjetivas e objetiva, usando o instrumento Performance test of Activities of Daily Living (PADL), com 16 tarefas associado a três itens da Escala de Atividades Instrumentais de Vida Diária (AIVD). Resultados: o sexo feminino predominou dentre os idosos (32 - 58,2%) e os cuidadores (46 - 83,6%); a idade dos clientes variou entre 64 e 99 (x=80) anos, e dos cuidadores entre 37 e 84 anos (x=58,7a); a escolaridade básica foi mais frequente entre os idosos (36 - 65,4%) e a superior (18 - 32,7%) para os cuidadores; quanto ao parentesco dos cuidadores, 30 (54,4%) eram filhas e 12 (21,5%) esposas. Para comparar as funcionalidades subjetivas e objetiva no desempenho do autocuidado, calculou-se o Índice de Concordância (Ind.Con%), que variou de 62 a 93%, com Kappa oscilando entre 0,20 e 0,59. Tanto os idosos como os cuidadores referiram desempenhar as tarefas sem ou com ajuda, porém, na avaliação objetiva, detectou-se maior necessidade de ajuda ou a incapacidade da realização da tarefa. Saliente-se que os idosos se autoavaliaram um pouco melhor que os seus cuidadores. Discussão: Nas avaliações subjetivas idosos e subjetivas cuidadores, em comparação com a objetiva, as respostas dos idosos para o autocuidado foram confirmadas na avaliação objetiva e com os relatos reportados subjetivamente pelos cuidadores. Verificou que o Nível de funcionalidade nos idosos, na maioria foi o nível 3 (fazer sem ajuda) para as atividades de vida diária PADL e atividades instrumentais AIVD. Os relatos subjetivos idosos em comparação com a avaliação objetiva podem apresentar que alguns idosos fazem as atividades sem ajuda, outros com ajuda e aqueles que não fazem as tarefas sozinhos, e, assim, necessitaram de algum tipo de ajuda. As informações subjetivas reportadas pelos cuidadores foram discordantes em comparações com a avaliação objetiva em alguns itens das escalas. Os idosos tendem a se avaliar melhor que seus cuidadores. Os cuidadores reportam que os idosos conseguem realizar a tarefa, mas, na avaliação objetiva, alguns não fazem. Conclusões: Há importantes diferenças entre as capacidades funcionais relatadas pelo próprio idoso e seus cuidadores quando comparadas com a observação direta destas tarefas pelo enfermeiro, o que indica a necessidade de basear as ações e os cuidados muito mais na avaliação direta do que exclusivamente nos relatos dos clientes e de seus cuidadoresProspective, longitudinal quantitative study in elderly people and their proxies, of both genders, admitted at a geriatric ward of a school hospital, whose data collection was August 2013 to March 2014. Objective: to evaluate subjective and objective functional condition of elderly people admitted at a geriatric ward; among the reports of self care performance and their needs for help. Method: subjective and objective evaluations using the instrument Performance test of Activities of Daily Living (PADL), with 16 tasks associated to 3 items of Instrumental Activities of Daily Living (IADL). Results: Female gender was predominant among the elderly (32 - 58.2%) and the proxies (46 - 83.6%); clients´ age varied between 64 and 99 (x=80) years old and their proxies between 37 and 84 years old (x=58.7y); basic education was more frequent among the elderly (36 - 65.4%) and higher (18 - 32.7%) for their proxies. Regarding the family connection of proxies, 30 (54.4%) were daughters and 12 (21.5%) wives. In order to compare the subjective and objective functionalities to perform self care, it was calculated the Concordance Index (CI), which varied from 62 to 93%, whose Kappa was between 0.20 and 0.59. Both elderly people and their proxies referred performing tasks with or without help. However, on the objective evaluation it was detected greater need for help or incapability to perform a task. It must be highlighted that elderly people evaluated themselves a little better than their proxies. Discussion: elderly subjective and proxy subjective evaluations in comparison to objective ones, elderly responses for self care were confirmed in objective evaluation and the reports provided subjectively by their proxies. It was verified that the functionality level of elderly people in majority was level 3 (do it without help) for PADL daily life activities and IADL instrumental activities. Elderly subjective reports in comparison to objective evaluation, it can be seen that some elderly people perform their activities without help, others with help and the ones who don´t perform tasks by themselves required some kind of help. Subjective information reported by proxies was discordant in comparison to objective evaluation in some items of the instruments. Elderly people are more prone to evaluate themselves better than their proxies. Proxies reported that elderly people can perform their tasks, but in the objective evaluation it was not observed. Conclusions: there are important differences between the functional capacities reported by elderly themselves and their proxies when compared to the direct observation of these tasks by the nurse, which points to the need of actions and care more focused on the direct evaluation rather than reports by clients and their proxie
Association between cystatin C, frailty and clinical outcomes in older adults with cancer
Introdução: A idade é um fator de risco independente para câncer, portanto, os idosos são afetados desproporcionalmente pelas neoplasias. Da mesma forma, a Síndrome de Fragilidade se torna mais frequente ao longo dos anos, refletindo a vulnerabilidade dos idosos a insultos orgânicos. Os idosos frágeis com câncer têm um risco mais alto de resultados desfavoráveis; no entanto, poucos biomarcadores demonstram identificar adequadamente esses pacientes de alto risco. A cistatina C (CisC) é uma proteína de baixo peso molecular que se correlaciona negativamente com o envelhecimento saudável, de forma independente da função renal. Associou-se, em estudos prévios, à fragilidade e a aumento de mortalidade. Objetivos: Investigar a associação entre os níveis de CisC e a fragilidade em idosos com câncer; e investigar a associação entre níveis de CisC ou fragilidade e declínio funcional em até 12 meses em idosos com câncer. Métodos: Estudo de coorte prospectivo, incluindo idosos com 60 anos ou mais, em São Paulo, Brasil. Os participantes foram recrutados no ambulatório de um hospital universitário oncológico, no período de abril de 2015 a setembro de 2017. Foram avaliados quanto a variáveis sociodemográficas, clínicas, funcionais e laboratoriais. Os desfechos primários foram o diagnóstico de fragilidade de acordo com os critérios adaptados de Fragilidade de Fried e o estado funcional de acordo com as atividades da vida diária. A principal variável independente foi a dosagem de CisC na linha de base. Após a avaliação inicial, foram coletados dados de estado funcional, hospitalizações e óbito aos 3, 6 e 12 meses de seguimento. Foi realizada uma análise descritiva da população de acordo com os níveis de CisC. Resultados: Foram incluídos no estudo 605 idosos. A idade média foi de 69 anos (±7), 65% eram do sexo masculino, e as comorbidades mais comuns foram hipertensão (64%) e diabetes mellitus (29%). No seguimento de 12 meses, houve 14,7% de perdas de seguimento e recusa (dropouts), e 7,7% de óbitos. O câncer de próstata (31%) e o câncer de mama (15%) foram as neoplasias mais comumente encontradas. As taxas de filtração glomerular medidas foram superiores a 60 ml/min em 70% dos pacientes e a mediana de CisC foi de 1,14 mg/dL (intervalo interquartil = 1,0; 1,36). Considerando a avaliação inicial, 82,7% dos pacientes eram totalmente independentes para as atividades básicas de vida diária e 72% totalmente funcionais para as atividades instrumentais de vida diária. Respectivamente, 51% e 8% foram considerados pré-frágeis e frágeis. Os pacientes com valores séricos de CisC acima do percentil 50 apresentavam idade mais avançada, piores índices de comorbidade de Charlson, maior prevalência de hipertensão, doença arterial coronariana e níveis de hemoglobina mais baixos. Em uma análise multivariada, níveis mais altos de CisC foram associados à fragilidade (p 50) had more advanced age, worse rates of Charlson\'s comorbidity, higher prevalence of hypertension, coronary artery disease and lower hemoglobin levels. In a multivariate analysis, higher levels of CysC were associated with frailty (p < 0.001), but not with negative outcomes of mortality, hospitalizations, and loss of functionality. In the multivariate analysis by logistic regression, the loss of basic activities of daily living correlated only with sex and there was a tendency for octogenarians to have a higher risk of functional loss. Conclusion: Advanced age, comorbidities, male gender and being pre/frail increased the risk of functional deterioration. Elevated serum cystatin C was unequivocally correlated with frailty in elderly patients with cancer, however, it was not isolatedly determinant for functional loss and negative outcomes. The 10-minute targeted geriatric assessment (10-TaGA) predictive validity for adverse outcomes in outpatient older adults: a prospective cohort study
INTRODUÇÃO: A implementação na prática ambulatorial da Avaliação Geriátrica Ampla, componente central na atenção à saúde do idoso, mostra-se bastante desafiadora, especialmente quando observamos um sistema de saúde constituído por serviços com alto fluxo de atendimentos. A Avaliação Geriátrica Compacta de 10 minutos (AGC-10) é um instrumento multidimensional breve, recém desenvolvido e de fácil aplicação, com o objetivo de rastrear síndromes geriátricas e prever desfechos adversos. No entanto, a AGC-10 foi somente avaliada em pacientes idosos com condições agudas em hospital dia, fazendose necessário avaliar seu poder de predição em outras modalidades de atendimento. OBJETIVOS: Avaliar se a AGC-10 é capaz de predizer sobrevida, declínio funcional e desfechos adversos em 12 meses em idosos ambulatoriais. MÉTODOS: Estudo de coorte prospectivo, no qual foram incluídos pacientes com 60 anos ou mais que passaram em consulta médica de abril a dezembro de 2017 em um ambulatório acadêmico de geriatria em São Paulo, Brasil. A avaliação inicial foi constituída pela AGC-10, dados sociodemográficos e de multimorbidade. Fragilidade, avaliada pelo Frailty Index, foi utilizada para explorar a validade da AGC-10. Após a inclusão, os participantes receberam ligações telefônicas trimestrais por um período de 12 meses para a detecção dos desfechos morte, perda funcional, hospitalização, ida ao pronto socorro (PS) e queda. Regressão logística, modelos de riscos proporcionais de Cox e de riscos competitivos foram utilizados na associação da AGC-10 com os desfechos, após ajuste para fatores sociodemográficos (gênero, idade, cor/etnia, renda familiar per capita) e índice de comorbidades de Charlson. RESULTADOS: 1.336 participantes foram incluídos, com média de idade de 82,2 (±7,6) anos, 70,2% pertencente ao gênero feminino, escolaridade mediana de 4 (1-5) anos, oriundos de todas as regiões da cidade. A amostra apresentou média de 0,4 (±0,2) no índice AGC-10 e distribuiu-se da seguinte forma nas categorias de risco: 12% baixo (0-0,24), 55% médio (0,25-0,49) e 33% alto risco (0,5-1,0). O índice AGC-10 e o Frailty Index apresentaram alta correlação (coeficiente de Pearson: r=0,72; p < 0,001). No seguimento de 12 meses, 9,9% dos participantes morreram, 20,3% perderam funcionalidade para atividade básica de vida diária, 24,1% internaram, 55,6% foram ao PS e 38,7% apresentaram queda. Pacientes classificados como médio e alto risco na AGC- 10, quando comparados aos de baixo risco, tiveram, respectivamente, maior incidência de morte (HR=10,3 IC95%=1,4-74,6 e HR=22,3 IC95%=3,1-161,4), perda funcional (sub-HR=5,3 IC95%=2,2-13,0 e sub-HR=10,2 IC95%=4,1-24,9), hospitalização (HR=1,9 IC95%=1,2-3,2 e HR=2,8 IC95%=1,7-4,6), ida ao PS (OR=2,3 IC95%=1,6-3,3 e OR=4,2 IC95%=2,8-6,3) e queda (OR=1,8 IC95%=1,2-2,7 e OR=3,4 IC95%=2,2-5,2) em um ano. CONCLUSÕES: A AGC- 10 foi preditora de morte, perda funcional, internação hospitalar, ida ao PS e queda em um ano em uma coorte idosos ambulatoriais. A AGC-10 mostrou ainda apresentar alta correlação com o Frailty Index. Os dados do presente estudo favorecem o uso do índice AGC-10 e sua classificação em categorias de risco no ambiente ambulatorial. O reconhecimento de forma simples e breve de síndromes geriátricas e vulnerabilidades pode aprimorar o tratamento de pacientes ambulatoriais e possibilitar o gerenciamento de programas de atenção voltados ao cuidado da pessoa idosaBACKGROUND: The integration of the Comprehensive Geriatric Assessment, a central component of healthcare for older adults, into outpatient clinical practice is quite challenging, especially in busy healthcare settings of care. The 10-minute targeted geriatric assessment (10-TaGA) is a quick and easy-toadminister multidimensional assessment, newly developed to screen geriatric syndromes and predict adverse outcomes in older adults. However, the 10- TaGA was only evaluated in acutely ill older patients in a day hospital setting, and new studies are needed to evaluate its predictive validity in other settings of care. OBJECTIVES: To examine whether 10-TaGA predicts mortality, disability and adverse outcomes in 12 months in outpatient older adults. METHODS: Prospective cohort study, which included patients aged 60 years and older who had a medical appointment at an academic geriatric outpatient clinic, from April to December 2017, in São Paulo, Brazil. The baseline clinical assessment included: the 10-TaGA, sociodemographic data and multimorbidity evaluation. Frailty, assessed by the Frailty Index, was used to explore 10-TaGAs validity. After recruitment, participants were interviewed by telephone quarterly for a period of 12 months to collect data for the following outcomes: death, functional status, hospital admissions, emergency room (ER) visits and falls. Logistic regression, Cox proportional hazards and competing risks regression models were used for the association of 10-TaGA with the outcomes, adjusted for sociodemographic factors (gender, age, color/ethnicity, family income per capita) and Charlson comorbidity index. RESULTS: 1,336 participants were admitted, with a mean age of 82.2 (±7.6) years, 70.2% female, median schooling duration of 4 (1-5) years, coming from all the different regions of the city. Participants had a mean 10-TaGA score of 0.4 (±0.2) and, according to risk categories, 12% participants were classified as having low-risk (0-0.24), 55% as medium-risk (0.25-0.49) and 33% as high-risk (0.5-1.0). The 10-TaGA score was highly correlated with the Frailty Index (Pearson\'s coefficient: r=0.72; p < 0.001). At follow-up, 9.9% of the participants had died and 20.3% increased dependency in activities of daily living. Hospitalizations, ER visits and falls were reported in 24.1%, 55.6% and 38.7%, respectively. Patients classified as having medium and high-risk, according to 10-TaGA, when compared to those at low-risk, had, respectively, a higher incidence of death (HR=10.3 95%CI=1.4-74.6 and HR=22.3 95%CI=3.1-161.4), disability (sub-HR=5.3 95%CI=2.2-13.0 and sub- HR=10.2 95%CI=4.1-24.9), hospitalization (HR=1.9 95%CI=1.2-3.2 and HR=2.8 95%CI=1.7-4.6), ER visit (OR=2.3 95%CI=1,6-3.3 and OR=4.2 95%CI= 2.8-6.3), and fall (OR=1.8 95%CI=1.2-2.7 and OR=3.4 95%CI=2,2-5,2) during the 1-year follow-up period. CONCLUSIONS: The 10-TaGA predicted death, disability, hospitalizations, visits to the ER and falls among outpatient older adults within one year. Moreover, the 10-TaGA showed a high correlation with the Frailty Index. The results from this study supports the use of the 10-TaGA score and its classification into risk categories for outpatient older adults. The early identification of outpatients with geriatric syndromes or at high risk for adverse outcomes by an easy-to-administer tool can enable tailored treatments and help the management of healthcare programs for older adult
Evaluation of a network building multicentric teleconference about knowledge in geriatrics and gerontology: the TeleGero
INTRODUÇÃO: A TeleGero é uma rede multicêntrica, multidisciplinar, de interação bidirecional, que, através de uma construção experiencial, consegue ressignificar o olhar sobre diversos temas e contribui para a evolução dos centros participantes. As reuniões têm duração de duas horas e ocorrem na última quinta-feira do mês, em onze meses do ano. OBJETIVO: Ensejar a reflexão sobre as características que tornaram a TeleGero uma proposta educativa transformadora, sustentável academicamente ao longo de dez anos. DESENHO: Estudo primário, observacional, longitudinal. CASUÍSTICA: Seus doze centros distribuem-se em cinco estados do Brasil (São Paulo, Rio de Janeiro, Rio Grande do Sul, Pernambuco, Amazonas) e um em Portugal (Porto). MÉTODO: Foram realizadas entrevista semiestruturada com informantes-chave de cada um dos centros participantes, para entender como o programa influenciou no comportamento do corpo profissional dos respectivos centros. Aquelas foram gravadas e transcritas em estilo literal e analisadas pelo método de condensação de significados. Todas as teleconferências gravadas, foram editadas e separadas em dois grandes grupos: apresentação e debate. Seu conteúdo foi analisado e estruturado em um banco de dados. RESULTADOS: Os responsáveis pelas apresentações caracterizaram-se por serem do sexo feminino (56%); médicos (69%), e por escolherem seminários (50%) como forma de apresentação. Foram identificados 172 descritores em ciências da saúde diferentes, sendo \"tratamento\" e \"estratégia de saúde da família\" os mais encontrados. A TeleGero foi identificada como uma proposta singular pela pluralidade dos centros participantes, que permitiu a aproximação de realidades distantes, mas também pela qualidade das apresentações e dos debates. A motivação para sua criação foi, segundo os informantes, a necessidade de aproximação entre centros formadores em Geriatria e em Gerontologia como também uma motivação relevante para todos os entrevistados. A TeleGero enfrentou várias dificuldades, especialmente, problemas técnicos, mas estruturou soluções e permitiu-se novas perspectivas de ação conjunta. CONCLUSÃO: A TeleGero formou uma rede multicêntrica, multiprofissional e colaborativa, cujas estratégias de construção podem contribuir para a formação de atividades educativas em telesaúde.INTRODUCTION: TeleGero is a multicentric, multidisciplinary network, of bidirectional interaction, which, through an experiential construction, manages to reframe the look on different themes and contributes to the evolution of the participating centers. The meetings have two hours and occur on the last Thursday of the month, eleven months per year. OBJECTIVE: Signify a reflection about the characteristics that made TeleGero a transformative educational proposal, academically sustainable over ten years. DESIGN: Observational, longitudinal study. CASUISTIC: Twelve centers distributed in five states of Brazil (São Paulo, Rio de Janeiro, Rio Grande do Sul, Pernambuco, Amazonas) and one in Portugal (Porto). METHOD: Semi-structured interviews were conducted with a key integrant of each of the participating centers, to understand how the program influenced the behavior of the professional staff of the respective centers. Those were recorded and transcribed in literal style and analyzed by the method of condensation of meanings. All recorded conference calls were edited and separated into two major groups: presentation and debate. Its content was analyzed and structured in a database. RESULTS: The responsibles for the presentations were predominantly female (56%); doctors (69%), and chosen seminars (50%) as a form of presentation. 172 descriptors were identified in different health sciences, with \"treatment\" and \"family health strategy\" being the most common. TeleGero was identified as a singular proposal due to the plurality of participating centers, which allowed distant realities to come together, but also due to the quality of the presentations and debates. The motivation for the creation was, according to the informants, the need for approximation between training centers in Geriatrics and Gerontology as well as a relevant motivation for all interviewees. TeleGero faced several difficulties, especially technical problems, but found structured solutions and allowed new perspectives for joint action. CONCLUSION: TeleGero formed a multicenter, multiprofessional and collaborative network, whose construction strategies can contribute to the formation of educational activities in telehealth
Clinical use of posturography to assess community-dwelling elderly fallers
Introdução: Recorrência de quedas está presente em 50% dos casos de idosos caidores, fato que sinaliza piores desfechos na saúde dessa população. Daí a relevância dos fatores associados aos eventos recorrentes: se bem entendidos, eles podem conferir maior objetividade às avaliações de riscos e, consequentemente, produzir uma melhor correspondência entre as intervenções propostas e as reais necessidades dessa população. Objetivo: Investigar se a posturografia - uma das propostas de avaliação de risco de quedas -- adiciona à avaliação clínica a capacidade de discriminar a recorrência de quedas em idosos. Método: Estudo transversal, realizado no período de agosto de 2011 a novembro de 2012, com 124 idosos do Programa de Prevenção de Quedas em um hospital terciário na cidade de São Paulo. Foram avaliados dados sociodemográficos, clínicos e testes de performance como Timed Up and Go Test (TUGT), Velocidade de Marcha, Escala de Equilíbrio de Berg (Berg Balance Scale, BBS), teste do sentar e levantar, medo de cair (Falls Efficacy Scale International, FES-I) e sintomas depressivos (Escala de Depressão Geriátrica, EDG). A posturografia estática foi realizada por meio de plataforma de força em três diferentes situações: olhos abertos (OA), olhos fechados (OF) e OA com dupla tarefa. Resultados: Foram avaliados 124 sujeitos com idade entre 60 e 88 anos, sendo 35 caidores únicos e 89 caidores recorrentes. Houve diferença entre os grupos quanto ao relato de medo de cair (P= 0,01), sintomas depressivos (EDG) (P= 0,007), escala de eficácia em quedas - Internacional (FES-I) (P=0,01), consequência grave, como traumatismo cranioencefálico e fratura (P=0,002) e necessidade de ajuda para se levantar (P=0,007), sendo que essas duas últimas características são mais prevalentes no caidor único. A variável de amplitude média de deslocamento do centro de pressão (COP) no plano médio lateral (XSD) na condição de OF apresentou diferença entre grupos (P=0,011), assim como a velocidade média calculada pelo deslocamento total do COP em todas as direções (VAvg) na condição OA (P=0,014). Após regressão logística de forma hierárquica e adicionada à avaliação clínica, nenhuma variável da posturografia mostrou-se capaz de incrementar de forma significante o poder de diferenciação entre quedas recorrentes e únicas. No modelo final ajustado, sintomas depressivos (EDG) (P < 0,05), TUGT com distrator (P < 0,05) e BBS (P < 0,01) apresentaram poder preditivo significativo de forma independente. Conclusão: No que tange a discriminação da recorrência de quedas em idosos da comunidade, a posturografia realizada em diferentes situações (olhos abertos, olhos fechados e dupla tarefa) não agregou valor à avaliação clínica composta por dados sociodemográficos, variáveis clínicas e testes de performanceIntroduction: The recurrence of falls has been present in 50% of cases of elderly fallers, which predicts bad health outcomes for this population. Thus, it is important to understand the factors associated with these recurrent events so that the risk assessments are objective and allow the proposed interventions to meet the real needs of this population. Objective: To investigate whether the posturography test adds to the clinical evaluation in the ability to discriminate the recurrence of falls in the elderly. Method: This cross sectional study was conducted from August 2011 to November 2012, with 124 elderly from a Falls Prevention Program at a tertiary level hospital in the city of São Paulo. Sociodemographic, clinical and functional testing such as the Timed Up and Go Test (TUGT), Speed Gait, Berg Balance Scale (BBS), five times sit-to-stand test, fear of falling (Falls Efficacy Scale - International, FES-I) and depressive symptoms (Geriatric Depressive Scale, GDS) were evaluated. The static postGDurography was performed by means of a force platform in three different situations: eyes open (EO), eyes closed (EC) and EO dual task. Results: 124 subjects aged 60 to 88 years were evaluated, consisting of 35 single fallers and 89 recurrent fallers. Differences were found among the groups in terms of the reported fear of falling (p= 0.010), depressive symptoms (GDS) (p=0.007), Falls Efficacy Scale - International, (FES-I) (p=0.010), serious consequences such as traumatic brain injury and fracture (p=0.002) and the need for help to stand up (p= 0.007); the two last characteristics were more prevalent in single fallers. The variable of mean amplitude of center of pressure displacement (COP) in the median lateral plane (XSD) in the EC condition was different among groups (p =0.011), as was the average speed calculated through the total displacement of the COP in all directions (VAvg) in the EO condition (p= 0.014). After hierarchical logistic regression added to clinical evaluation, no posturography variable was capable of significantly increase the power of differentiation between recurrent and single falls. In the final adjusted model, depressive symptoms (EDG) (P < 0.05), the TUGT with a distractor (P < 0.05) and the BBS (P < 0.01) presented significant independent predictive power. Conclusion: Posturography performed in different situations (eyes open, eyes closed and dual task) did not add value to clinical evaluation - consisting of sociodemographic data, clinical variables and functional tests - in discriminating the recurrence of falls in elderly individuals living in the communit
Oral health as a predictor of frailty in older adult outpatients
No processo de envelhecimento, a saúde oral tem sido entendida como componente importante da saúde sistêmica, propiciando uma efetiva correlação entre os marcadores de vulnerabilidade oral, como edentulismo, higiene precária, protetização deficiente, com as frequentes multimorbidades e consequente polifarmácia que caracterizam a fragilidade no avançar da vida. Avaliar os diferentes componentes da saúde oral, correlacionando-os com os parâmetros de saúde sistêmica, em idosos com diferentes classificações na Síndrome da Fragilidade: Frágeis, Pré Frágeis e Não Frágeis. Este é um estudo observacional transversal de 88 idosos, com 60 anos ou mais - sendo 72 mulheres (81,9%), dos quais 44 matriculados em um Ambulatório de Fragilidade (GAF) e 44, pareados por idade e sexo, em um Ambulatório de Promoção de Saúde GAMIA (GAG). Todos foram avaliados pelo mesmo protocolo clínicoodontológico, identificando o número de dentes, o tipo de protetizações e a presença de lesões na cavidade oral. Foram também avaliados pelo GOHAI, instrumento que correlaciona subjetivamente a qualidade de vida com a saúde oral. As informações sobre as doenças sistêmicas e medicações prescritas foram obtidas no prontuário médico na última consulta prévia à avaliação presencial. A seguir, os pacientes foram classificados, segundo o Teste de Fried Autorreferido, nas três possibilidades da Síndrome da Fragilidade: Frágeis (GF), Pré Frágeis (GPF) e Não Frágeis (GNF). Os pacientes do GAG mostraram maior média do número de dentes (p=0,03) e melhor pontuação no GOHAI (p=0,008), além de medianas mais altas de Próteses Sobre Implantes (p=0,007) e Próteses Fixas Convencionais (p=0,047). Paralelamente, o número de doenças sistêmicas teve média mais elevada entre os pacientes GAF (p<0,001), sendo a Hipertensão Arterial Sistêmica a mais prevalente em ambos os grupos (80,6%) e, junto da Depressão, as que mais superaram em prevalência no GAF. O número de medicações utilizadas também teve média mais elevada no GAF (p<0,001). Os idosos matriculados no Ambulatório de Fragilidade (GAF),sejam Frágeis ou Pré Frágeis, têm menor número de dentes, protetizações menos eficientes, higiene oral mais precária, maior número de doenças sistêmicas e de medicações utilizadas do que os matriculados no Ambulatório de Promoção de Saúde GAMIA (GAG), sejam eles Pré Frágeis ou Não Frágeis, demonstrando que a saúde oral deve ser entendida como um dos fatores preditores da Síndrome da Fragilidade em idososIn the aging process, oral health has been understood as an important component of systemic health, providing an effective correlation between markers of oral vulnerability, such as edentulism, bad hygiene, and poor prosthetics, with the frequent multimorbidities and consequent polypharmacy that characterize frailty in later life. To evaluate the different components of oral health in older adults with different classifications in the Frailty Syndrome: Frail, Pre-Frail and Non-Frail, correlating them with systemic health parameters. This is a cross-sectional observational study of 88 older adults aged 60 years or older - of which 72 were women (81.9%) -, 44 enrolled in a Frailty Outpatient Clinic (GAF) and 44, matched for age and sex, in a Health Promotion Outpatient Clinic GAMIA- (GAG). All were evaluated by the same clinicaldental protocol, identifying the number of teeth, the type of dental prosthetics and the presence of lesions in the oral cavity. They were also evaluated by the GOHAI, an instrument that subjectively correlates quality of life with oral health. Information about systemic diseases and prescribed medications was obtained from the medical chart in the last visit prior to the face-to-face evaluation. Then, the patients were classified, according to the Self-Referred Fried Test, in the three possibilities of the frailty syndrome: Fragile (GF), Pre-Fragile (GPF) and Non-Fragile (GNF) Patients in the GAG showed higher median number of teeth (p=0.03) and better GOHAI score (p=0.008), as well as higher medians for Prostheses on Implants (p=0.007) and Conventional Fixed Prostheses (p=0.047). In parallel, the number of systemic diseases had a higher median among the GAF patients (p<0.001), with Systemic Arterial Hypertension being the most prevalent in both groups (80.6%) and, together with Depression, the most prevalent in the GAF. The number of medications used also had a higher mean in the GAF (p<0.001). The older adults enrolled in the Frailty Outpatient Clinic (GAF), whether Frail or Pre-Frail, have fewer teeth, less efficient prosthetics, poorer oral hygiene, a higher number of systemic diseases and of medications used than those enrolled in the Health Promotion Outpatient Clinic - GAMIA - (GAG), whether Pre-Frail or Non-Frail, showing that oral health should be understood as one of the predictors of the Frailty Syndrome in older adult
Histomorphometric analysis of the diaphragm muscle at different ages: a clinicopathological correlation study of smoking load
O tabagismo é um importante fator de risco para várias doenças cardiovasculares e pulmonares, e também têm sido associado à perda de massa e força muscular esquelética levando à sarcopenia. Assim, o presente estudo analisou os efeitos histopatológicos do processo de envelhecimento do diafragma, em indivíduos de ambos os sexos, em diferentes faixas etárias, a fim de avaliar se a carga tabágica é responsável por acréscimo das correlações clinicopatológicas. Amostras do músculo diafragma post-mortem foram coletadas e processadas para análise histológica através das colorações histológicas de Picrosirius red e hematoxilina e eosina. Utilizou-se a estereologia e a morfometria para mensurar as alterações nas fibras musculares, bem como a densidade de volume das fibras colágenas, adipócitos e vasos sanguíneos (incluindo capilares). A análise estatística, com nível de significância P < 0,05, foi realizada através do programa SPSS. Os dados foram apresentados como média ± erro padrão da média para vaiáveis contínuas e frequência absoluta e relativa para as categóricas. Utilizamos o Teste T de Student (bicaudal e não pareado) para comparar as variáveis continuas entre os indivíduos fumantes com os não-fumantes (controle). As variáveis categóricas foram analisadas através do Teste Qui-quadrado ou Exato de Fisher quando necessário. Para testar se as distribuições de tamanhos de fibras normais eram diferentes entre os grupos, foi utilizado o teste de Kolmogorov-Smirnov. A fim de entender como as variáveis demográficas e clínicas podem atuar nos achados histopatológicos, realizamos uma análise de regressão multivariada. Observamos que com o avanço da idade, o diafragma acumula características histopatológicas que podem levar a complicações na mecânica respiratória. Ademais, o tabagismo demonstrou atuar negativamente na histologia diafragmática, sendo que a carga tabágica desempenha papel de suma importância no acréscimo de características histopatológicas neste músculoSmoking is a major risk factor for several cardiovascular and pulmonary diseases, and has also been associated with loss of skeletal muscle mass and strength leading to sarcopenia. Thus, the present study analyzed the histopathological effects of the aging process of the diaphragm, in individuals of both sexes, in different age groups, in order to assess whether the smoking load is responsible for the increase in clinicopathological correlations. Post-mortem diaphragm muscle samples were collected and processed for histological analysis through Picrosirius red and hematoxylin and eosin staining techniques. Stereology and morphometry were used to measure changes in muscle fibers, as well as the volume density of collagen fibers, adipocytes and blood vessels (including capillaries). Statistical analysis, with a significance level of P < 0.05, was performed using the SPSS software. Data were presented as mean ± standard error of mean for continuous variables and absolute and relative frequency for categorical variables. We used Student\'s T test (two-tailed and unpaired) to compare continuous variables between smokers and non-smokers (control). Categorical variables were analyzed using the chi-square test or Fisher\'s exact test when necessary. To test whether normal fiber size distributions were different between groups, the Kolmogorov-Smirnov test was used. In order to understand how demographic and clinical variables may affect histopathological findings, we performed a multivariate regression analysis. We observed that with advancing age, the diaphragm accumulates histopathological characteristics that can lead to complications in respiratory mechanics.
