    Continuous metadata flows for distributed multimedia

    The practical use of temporal multimedia has increased markedly in recent years as enabling technologies for the distribution and streaming of media have become available. As a part of this trend, hypermedia systems and models have adapted accordingly to incorporate such distributed multimedia for presentation. Structured interpretation of information has long been a fundamental feature of both open hypermedia systems and knowledge systems. Metadata, in its many forms, has become the cornerstone for providing this structured knowledge above and beyond basic data and information. This thesis presents the rationale and requirements for continuous metadata, which supports the metadata accompanying distributed multimedia throughout the lifecycle of streamed media, from generation, through distribution, to presentation. Throughout this process it is the temporal and continuous nature of the metadata which is paramount. A conceptual framework for continuous metadata is proposed to encapsulate these principles and ideas. Continuous metadata and the associated framework enable the development, in particular, of real-time, collaborative, semantically enriched distributed multimedia applications. Experience building one such system using continuous metadata is evaluated within the framework. An ontology is developed for the system to enable the collation, distribution, and presentation of structure aiding navigation of multimedia, and it is shown how continuous metadata utilising the ontology can be distributed using multicas

    The regulation of labour and the state in the Sudan : a study of the relationship between the stage of social and economic development and the autonomy of labour relations law

    The thesis is a study of labour regulation and the State in the Sudan in the light of a general theoretical conception of labour law and the State. The first Chapter defines the concepts of analysis that are used throughout the study, isolates the "essential" properties of the Capitalist State and Law from the historically concrete forms which they assume in a particular society and distinguishes between processes which influence development of the form of law and others which influence its sociological development. Drawing on the analysis in Chapter I, Chapter II exposes the inter-relationship between the Sudanese social formation, State and Law and the implication of this inter-relationship for both the form and substance of labour relations law. Chapters III, IV and V are specific verifications of the hypothesis regarding the inter-relationship between the State and labour relations law in the Sudan and that regarding the development of the "substance" and "ideology" of law in general. The thesis considers law as an empirically-founded discipline. But, it distinguishes between various types of empirical facts about law corresponding with respective semi-autonomous social levels at which law asserts its existence. The research method followed describes the empirical facts about law at the particular level and, in order to determine the epistemological significance of these facts, analytically relates them to empirical facts at other levels. Wherever used in the thesis the term "theory" signifies either this methodological procedure of analysing the inter-connection of empirical facts at a certain level and their inter-relation with other facts at other levels, or the substantive generalizations about law which findings at these various levels would allow. I consider my application of this methodology to the study of labour rela tions law, the historical dimension this application introduces in socio-economic analysis of this law, the criticism of certain Marxist and other sociological conceptions of law it enables, and the socio-histor ical relativity of the "substance" and "ideology" of law it reveals as original contributions to the knowledge of labour law. The compilation and evaluation within the framework of the thesis of empirical materials on industrial relations in the Sudan are likewise original contribution to the knowledge of Sudanese "labour law" and labour law in general

    BACKGROUND: Severe acute malnutrition affects 13 million children worldwide and causes 1-2 million deaths every year. Our aim was to assess the clinical and nutritional efficacy of a probiotic and prebiotic functional food for the treatment of severe acute malnutrition in a HIV-prevalent setting. METHODS: We recruited 795 Malawian children (age range 5 to 168 months [median 22, IQR 15 to 32]) from July 12, 2006, to March 7, 2007, into a double-blind, randomised, placebo-controlled efficacy trial. For generalisability, all admissions for severe acute malnutrition treatment were eligible for recruitment. After stabilisation with milk feeds, children were randomly assigned to ready-to-use therapeutic food either with (n=399) or without (n=396) Synbiotic2000 Forte. Average prescribed Synbiotic dose was 10(10) colony-forming units or more of lactic acid bacteria per day for the duration of treatment (median 33 days). Primary outcome was nutritional cure (weight-for-height >80% of National Center for Health Statistics median on two consecutive outpatient visits). Secondary outcomes included death, weight gain, time to cure, and prevalence of clinical symptoms (diarrhoea, fever, and respiratory problems). Analysis was on an intention-to-treat basis. This trial is registered as an International Standard Randomised Controlled Trial, number ISRCTN19364765. FINDINGS: Nutritional cure was similar in both Synbiotic and control groups (53.9% [215 of 399] and 51.3% [203 of 396]; p=0.40). Secondary outcomes were also similar between groups. HIV seropositivity was associated with worse outcomes overall, but did not modify or confound the negative results. Subgroup analyses showed possible trends towards reduced outpatient mortality in the Synbiotic group (p=0.06). INTERPRETATION: In Malawi, Synbiotic2000 Forte did not improve severe acute malnutrition outcomes. The observation of reduced outpatient mortality might be caused by bias, confounding, or chance, but is biologically plausible, has potential for public health impact, and should be explored in future studies.     INTRODUCTION: Surgical procedures in persons with haemophilia A or B with inhibitors (PwHABI) require the use of bypassing agents (BPA) and carry a high risk of complications. Historically, only two BPAs have been available; these are reported to have variable responses. AIM: To prospectively evaluate the efficacy and safety of a new bypassing agent, human recombinant factor VIIa (eptacog beta) in elective surgical procedures in PwHABI in a phase 3 clinical trial, PERSEPT 3. METHODS: Subjects were administered 200 µg/kg (major procedures) or 75 µg/kg eptacog beta (minor procedures) immediately prior to the initial surgical incision; subsequent 75 µg/kg doses were administered to achieve postoperative haemostasis and wound healing. Efficacy was assessed on a 4-point haemostatic scale during the intra- and postoperative periods. Anti-drug antibodies, thrombotic events and changes in clinical/laboratory parameters were monitored throughout the perioperative period. RESULTS: Twelve subjects underwent six major and six minor procedures. The primary efficacy endpoint success proportion was 100% (95% CI: 47.8%-100%) for minor procedures and 66.7% (95% CI: 22.3%-95.7%) for major procedures; 81.8% (95% CI: 48.2%-97.7%) of the procedures were considered successful using eptacog beta. There was one death due to bleeding from a nonsurgical site; this was assessed as unlikely related to eptacog beta. No thrombotic events or anti-eptacog beta antibodies were reported. CONCLUSION: Two eptacog beta dosing regimens in PwHABI undergoing major and minor surgical procedures were well-tolerated, and the majority of procedures were successful based on surgeon/investigator assessments. Eptacog beta offers clinicians a new potential therapeutic option for procedures in PwHABI
