    Exploration et modélisation de l'écoulement inspiratoire au passage des fosses nasales

    In clinical routine the exploration of the obstructive disorders in the nose is done primarily using techniques (tomodensitometry, endoscopic clinical examination) giving only one static image of the nose. Here we present two noninvasive investigation methods allowing to have a functional image of the nasal fossae. The posterior rhinomanometry gives an index of the resistance of each nasal fossae. The acoustic rhinometry gives the longitudinal cross-sectional area along the airways. A model with individualized compliant elements was associated with these methods in order to better understanding the fluid-structure coupling in the nasal fossae during inspiration. The proposed model, used in steady state conditions, allowed to simulate the pressure flow relationship in vivo observed in normal conditions as well as in pathological conditions. Our results suggest that the wall compliance is an essential parameter to understand the inspiratory flow limitation phenomenon well known by physiologists. This model, by predicting the functional pressure flow relationship, could be a useful tool for the clinician to evaluate the potential effects of treatments.En routine l'exploration des troubles obstructifs du nez se fait essentiellement à l'aide de techniques ne donnant qu'une image très statique du nez (tomodensitométrie, examen clinique endoscopique, ...). Ici, nous présentons deux méthodes d'investigation non-invasives permettant d'avoir une image fonctionnelle de la fosse nasale. La rhinomanométrie postérieure donne un index de la résistance globale de chaque fosse nasale. La rhinométrie acoustique apporte une visualisation du calibre des voies aériennes sous forme de l'évolution longitudinale de l'aire des sections droites de ce conduit. Un modèle à éléments individualisés compliants a été associé à ces 2 méthodes dans le but de mieux comprendre le couplage fluide structure se développant dans la fosse nasale à l'inspiration. Ce modèle, utilisé en conditions statiques, permet de simuler la relation pression débit observée in vivo chez le sujet sain comme chez le sujet pathologique. Nos résultats suggèrent que la compliance des fosses nasales est un paramètre essentiel pour comprendre les phénomènes de limitation de débit observés par les physiologistes à l'inspiration. Ce model par sa faculté de prédire la relation fonctionnelle entre pression et débit pourrait devenir un outil pour le clinicien dans le but d'évaluer les effets potentiels du traitement envisagé.Louis Bruno, Fodil Redouane, Isabey Daniel.     Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     Variations on the Author

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Upper airways are characterized by a succession of anatomical and functional singularities (naso/oro/hypopharynx) evidenced by a number of studies in patients with Sleep Apnea Syndrom. Moreover, these studies show that the inspiratory flow is strongly limited during periods of severe obstructive hypopnea and most often resemble to a flow plateau at values considerably smaller than those of normal inspirations. Mechanical models proposed in the literature to describe the coupling between the inspiratory flow and the pharynx did not take into account these mechanical and geometrical singularities as well as the possible interdependence between the different pharyngeal segments. The aim of this work is firstly to consider the pharyngeal singularities using a model based on several individualized compliant elements. The downstream element is pressure controlled whereas one (or several) free element(s) are located upstream. Derivation of the conservation equations versus the controlled pressure leads to a system which allows to describe variations of both upstream cross-sectional area and flow. These equations reveal the fundamental role of the speed indexes associated to the distensibility of each segment. The condition corresponding to the singular point has been shown to play a fundamental role in the generation of the flow plateau which actually corresponds to a succession of close maximum and minimum flows with multiple passages through the singular point, depending on the number and the properties of free elements.Les voies aériennes supérieures se caractérisent par une succession de singularités anatomiques et fonctionnelles (naso/oro/hypopharynx) dont le rôle spécifique est de plus en plus mis en évidence chez les patients apnéiques. Par ailleurs, le débit inspiré est fortement altéré dans les hypopnées obstructives sévères et se présente le plus souvent sous forme de plateau à des valeurs de débit très diminuées par rapport à celles de la normale. Les modèles mécaniques proposés dans la littérature pour décrire le couplage entre l’air inspiré et le pharynx ne prennent pas en compte les singularités géométriques et mécaniques et leurs interdépendances possibles entre les différents segments pharyngés. Nous proposons de prendre en compte les singularités pharyngées dans un modèle à plusieurs éléments compliant individualisés, dont un élément situé en aval est contrôlé en pression et un (ou plusieurs) élément(s) libre(s) situé(s) en amont. Les équations de conservation, différentiées par rapport à la pression de contrôle, aboutissent à un système d’équations qui permet de mettre en évidence le rôle fondamental des indexes de vitesse associés à la distensibilité sur les variations de la section d’amont et le débit. Ainsi, le modèle montre que la succession de passages par la condition du point singulier s’avère jouer un rôle fondamental dans l’apparition du plateau de débit qui se présente sous la forme une succession rapprochée d’extrema autour d’une valeur (faible) de débit.Fodil Redouane, Louis Bruno, Isabey Daniel.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    Modélisation du transport et du dépôt de particules dans la région acinaire du poumon

    Cette thèse a pour cadre la modélisation du dépôt de particules dans le poumon humain afin d'optimiser l'administration de médicaments par voie inhalée. La région alvéolaire du poumon jouant un rôle physiologique et fonctionnel crucial, l'objectif de ce travail est de mettre en place un modèle de dépôt au sein de la région acinaire qui soit intégrable à un modèle intégrant le poumon complet. Les deux premiers chapitres rappellent les caractéristiques anatomiques et fonctionnelles du poumon et en particulier de la région alvéolaire ainsi que les principes physiques mis en jeu lors de l'écoulement de l'air et du transport de particules dans l'arbre pulmonaire. Puis un modèle numérique d'écoulement dans une géométrie alvéolaire simplifiée est présenté. Le transport d'un bolus d'aérosol y est étudié par une approche eulérienne, au cours de plusieurs cycles respiratoires ; l'impact des irréversibilités de l'écoulement sur la dispersion du bolus est ensuite quantifié. Le dernier chapitre présente l'intégration des résultats précédents au sein d'un modèle analytique de dépôt de particules dans le poumon. Les résultats générés par ce modèle sont ensuite comparés aux données expérimentales issues de la littérature ou obtenues lors d'une étude clinique en cours, spécifiquement orientée sur la mesure du dépôt de particules dans les voies aériennes. Les résultats du modèle montrent une augmentation du dépôt de particules dans la région acinaire, présentant un bon accord avec les données expérimentales. Ce modèle pourrait aider à la conception de thérapies ciblant spécifiquement la région alvéolaire du poumonThe context of this thesis is the modelling of particle deposition in the human lung in order to optimise the administration of inhaled drugs. As the alveolar region plays a crucial role both physiologically and functionally, especially for systemic delivery, the objective of this work is to set-up a particle deposition model specific to the acinar region which could be integrated in whole lung deposition model. The first two chapters concentrate on the anatomical and functional aspects of the lung and on the physical principles involved in the flow and particle transport mechanisms in the lung. Then a computational fluid dynamics model was setup in a simplified alveolar geometry. Aerosol bolus transport was studied through an Eulerian approach, for one or several breathing cycles. The impact of flow irreversibilities on bolus dispersion was quantified. The last chapter deals with the integration of the previous results in an analytical model of particle deposition in the whole lung. The results generated by this model are then compared to experimental data from the literature or obtained from an ongoing clinical trial. The results of the new theoretical model show an increase of particle deposition in the acinar region which improves correlation of theory with experimental data. This model could favourably help designing therapies targeting the alveolar region of the lun

    Physical modelling and numerical resolution of aerosol medicine particles transport and deposition in human extrathoracic airways

    D'abord développées pour traiter les maladies respiratoires (asthme, BPCO...), les thérapies inhalées sont depuis peu l'objet d'un intérêt particulier pour délivrer des molécules dans le sang via la barrière alvéolo-capillaire. Quelle que soit la cible de ces thérapies, leur efficacité dépend de la quantité de médicament qui arrive sur la zone à traiter dans l'arbre respiratoire. La région extrathoracique (appelée "région ET" et comprenant la bouche, l'oropharynx, le larynx et le début de la trachée) se comporte alors comme un "filtre" où se dépose une certaine masse de médicament. Afin d'optimiser une telle administration, il est nécessaire de connaître au mieux l'influence des facteurs caractéristiques de l'inhalation (entre autres, le débit d'inhalation, les diamètres des particules inhalées, les propriétés physiques du gaz porteur). Pour cela, un modèle réaliste de région ET a été construit à partir des données issues de l'imagerie médicale. Après la définition d'un maillage et le choix d'un modèle de turbulence et de paramètres adéquats, les équations de Navier-Stokes ont été résolues numériquement pour (i) décrire l'écoulement dans ce modèle et (ii) pour déterminer les trajectoires des particules inhalées. En parallèle, deux études expérimentales ont été menées sur l'exacte reproduction physique du modèle : l'une pour estimer les pertes de pression provoquées par des écoulements d'air et d'oxhel (mélange hélium (78 %) - oxygène (22 %)), l'autre pour mesurer les vitesses par IRM d'un écoulement d'hélium-3 hyperpolarisé. Les résultats obtenus en terme d'écoulement et de quantité de particules déposées dans la région ET sont en accord avec les rares données expérimentales existantes. Ils montrent l'influence prépondérante de la géométrie tant sur l'écoulement (marqué par la présence, vérifiée expérimentalement, du jet laryngeal) que sur le dépôt. Les zones préférentielles de dépôt se situent là où des variations géométriques brusques surviennent (rétrécissement de section et courbures près de l'épiglotte et en amont de la glotte), et là où le caractère turbulent de l'écoulement est prononcé (en fin de larynx notamment). Ces constatations ont permis de caractériser avec précision le dépôt à l'aide de deux nombres adimensionnés : le nombre de Stokes, caractéristique de l'impaction inertielle et le nombre de Reynolds, caractéristique du régime de l'écoulement. L'étude a permis une meilleure appréhension des influences des divers paramètres à considérer pour modéliser une inhalation d'aérosol médicament et, de surcroît, une prévision des quantités de médicament déposée et transmise à l'arbre respiratoire, soit un outil précieux pour l'optimisation des thérapies inhalées. La méthodologie, ainsi développée, est applicable aux morphologies issues d'une étude clinique en cours et permettra d'affiner la caractérisationInhaled therapies were originally developed to treat respiratory diseases (e.g., asthma and COPD). They are now the subject of research to deliver medicine to the blood through the alveolar-capillary barrier. The efficacy of such therapies lies in the quantity of medicine that reaches the targeted area in the respiratoy tree. The extrathoracic region (called "ET" and including the mouth, the oropharynx, the larynx and the beginning of the trachea) has the role of a "filter" limiting the amount of medicine that is deposited in the targeted area. In order to optimize the administration of aerosol medicine, it is necessary to know the influence of the various parameters of an inhalation (among others, the inhalation flow rate, the diameter of inhaled particles, the physical properties of the cariier gas). Therefore, a realistic model of the ET region has been constructed from data obtained from medical imaging. The Navier-Stokes equations were numerically solved (i) to describe the flow in the model and (ii) to determine the trajectories of inhaled particles. The numerical analysis required the development of an appropriate mesh and the choice of turbulence model and parameters. In parallel, two experimental studies were performed on an exact physical reproduction of the model : one to estimate the pressure losses in air and oxhel (a helium (78 %) - oxygen (22 %) mixture) flows and another to measure the velocity field using a MRI of a hyperpolarized helium-3 flow. The flow and deposition results that were obtained in the model of the ET region are in agreement with the few existing experimental data. They show the dominant effect of the geometry not only on the flow (marked by the experimentally verified presence of the laryngeal jet) but also on the particle deposition. The major areas of deposition are located where sudden geometrical changes are observed (like the constrictions and the bends near the epiglottis and upstream the glottis) and where the turbulent character of the flow is pronounced (particulary at the end of the larynx). These observations allowed for the accurate characterization of the deposition with non-dimensional numbers : the Stokes number, characteristic number of the inertial impaction, and the Reynolds number, characteristic number of the flow regime. The study allows for the improved comprehension of the influences of the various parameters that must be considered to model the inhalation of aerosol medicine. Furthermore, predictions in terms of the deposited and transmitted mass of medicine were possible.
