    Mapping of the current transition of care practice for patients with pituitary disease at Endo-ERN reference centers

    Rare endocrine conditions present specific diagnostic and management challenges for healthcare providers, one of which is the understudied transition of care. Despite the need for guidance regarding transition, consensus on structured and protocolled approaches is lacking. Therefore, we aimed to map the current clinical practice and identify unmet needs regarding transition of care for patients with pituitary disease in the reference centers (RCs) of the European Reference Network on Rare Endocrine Conditions (Endo-ERN). A survey-based, cross-sectional study using the EU Survey tool was performed and completed by 46 physicians (n) from 30 RCs (N). Transition is a common practice among RCs (n = 44/46), usually accomplished by a multidisciplinary team meeting (N = 20/30). Criteria for start and end of transition were defined in half of the RCs, with 16.7% of centers providing dissimilar answers. Transition readiness was assessed by >75% of the RCs, mostly by unvalidated means (e.g. subjective opinions, informal consultations). Pituitary-specific transition assessment tool was applied in one RC only. Transition protocols were present in only 9% of RCs, while in many RCs, transition decisions were taken in combined adult-pediatric meetings or based on clinicians’ personal judgment. A minority of physicians evaluated the effectiveness of transition-related interventions (n = 11/46) or medical outcomes (n = 8/46). Patient-reported outcome measures were infrequently used (n = 4/46). Identified unmet needs included the development of guidelines (n = 5/46) and EU-wide approach (n = 2/46). This study exemplifies the unmet needs for a structural definition of the transition period and transition management for patients with rare hypothalamic and pituitary conditions from healthcare providers’ perspective

    Blood pressure measurement methodology and technology in the SWEET diabetes centers: An international SWEET database survey

    Introduction The accuracy of blood pressure (BP) measurement is a prerequisite for the reliable diagnosis and management of hypertension. Objectives This survey evaluated the use of office and out-of-office BP measurements and the antihypertensive pharmacological treatment in expert pediatric diabetes centers. Methods A questionnaire was distributed in 78 reference pediatric diabetes centers of the SWEET international consortium. The methodology, devices, indications, and interpretation of office BP measurements (OBPM), 24-hour ambulatory BP monitoring (ABPM) and home BP monitoring (HBPM), and the preference for antihypertensive drug treatment was assessed. A grading score was developed to evaluate centers for overall BP measurement performance. Results Fifty-two centers responded. The average score for OBPM methodology was 72.5%, for technology 77.5% and the overall center score was 74.75%.The majority of the centers used appropriate methodology and technology, however, there was heterogeneity among them. Manual auscultatory or automated devices specifically validated for children were used by 26/52 centers. ABPM was recommended by 35/52 centers (27/35 had health insurance coverage) and HBPM by 18/52 centers. The BP measurement methodology and devices used for ABPM and HBPM were frequently inadequate. Angiotensin converting enzyme inhibitors were the most frequently prescribed drugs for treating hypertension. Conclusions The majority of SWEET pediatric diabetes centers use adequate methodology and devices for BP measurement. ABPM is recommended by two thirds of the centers, whereas HBPM is less widely used. Further improvement in the quality of office and out-of-office BP measurements and harmonization among centers is necessary according to current guidelines

    Circulating proinflammatory peptides related to abdominal adi posity and cardiometabolic risk factors in healthy prepubertal children

    Abstract OBJECTIVES: To analyze the circulating levels of proinflammatory peptides in healthy prepubertal children in relation to abdominal obesity, measured by waist circumference (WC), and to investigate their interactions with cardiometabolic risk factors. DESIGN AND METHODS: A cross-sectional study of 137 healthy prepubertal children with a mean age of 8.0±0.1 years divided into three groups according to their WC as a measure of abdominal obesity: 'normal-WC' children (25th-75th percentile, n=48), 'children at risk' (75th-90th percentile, n=39), and 'abdominally obese' (≥90th percentile, n=50) children. Auxological measurements and blood pressure (BP) were taken. Fasting levels of high-sensitivity C-reactive protein (hsCRP), interleukin 6 (IL6), tumor necrosis factor-α (TNF-α), glucose, insulin, and lipid profile were measured. Insulin resistance (IR) was assessed by homeostasis model assessment of IR (HOMA-IR). RESULTS: Abdominally obese children had significantly higher BP, insulin, HOMA-IR, total cholesterol and triglycerides (TG) compared with their normal-WC counterparts (P<0.05). HsCRP concentrations increased proportionally with the degree of abdominal obesity (r=0.443, P<0.0001), whereas IL6 and TNF-α were not significantly associated with any of the adiposity variables. After controlling for adiposity, hsCRP was significantly correlated with systolic BP (r=0.257, P=0.004), TNF-α levels were related to high-density lipoprotein cholesterol (HDL-C; r=-0.216, P=0.016) and TG (r=0.196, P=0.029), whereas the relationship between IL6 and HDL-C reduced its magnitude to an insignificant level (r=-0.173, P=0.055). CONCLUSIONS: Healthy prepubertal children with abdominal obesity have associated inflammatory and cardiometabolic alterations, interacting with each other

    THE FINANCIAL PERFORMANCE OF EUROPEAN COMPANIES: A COMPARATIVE APPROACH

    The present study empirically examines the financial performance of European companies and tries to identify the profitability differences that exist between: enterprises with same profile, economic sectors and between several countries from the European Union. Based on data of the year 2009, the newest available on the BACH-ESD database, three financial performance main indicators are being calculated, namely: Return on Assets, Return on Equity and Net Margin. Results are discussed and compared with the ones obtained in former similar studies and some conclusions are drawn. The most performing sectors are found to be mining, quarrying, professional, scientific and technical activities, utilities and IT&C. The sectors that perform the worst are transport and storage, agriculture, accommodation and food service. As shown in other studies the most profitable and competitive companies in the sample belong to the Nederland, Belgium and Austria and the least profitable ones are situated in Italy and Portugal. As a complementary useful result of the study, the average values obtained for the three variables, in the various sectors considered, can be utilized for future comparisons, for assessing the financial performance of companies that belong to same sectors as those surveyed.     This article explores the controversial issue of Bilingual Education. Violeta Mendoza Wagner argues against the notion that Bilingual Education has failed in California schools. Borrowing from personal experience and based on a review of relevant literature the author concludes that Bilingual Education is a means to provide immigrants with a gateway to becoming full participants in this society. Dismantling Bilingual Education will not only weaken the social fabric of the state of California, but could also have a negative impact on its economic future at a time when multiculturalism and diversity are of utmost importance in an increasingly globalized world

    Дисертационният труд разработва рискови фактори (кардиометаболитни и кардиоренални) при доносени здрави деца в перипубертетна възраст, родени малки за гестационната си възраст (МГВ). Проследени са постнаталният им растеж и наваксване до 10 г. възраст, както и ролята му за повишаване на риска. Извършено е измерване на мастната тъкан и обиколка на талията (ОТ), артериално налягане и пулс, бъбречни размери, както и редица метаболитни маркери (адипонектин, SHBG, инсулинова резистентност - HOMA-IR, TNF-alfa, IL-6, полови стероиди и DHEA-S). Всички те са значимо влошени при иначе здрави МГВ деца, като най-лоши са показателите на тези, които са натрупали най-много мазнини Извършено е сравнение данни от по-късно проучване (на затлъстяването) със сходна методика и е установено значимо увеличение на общото и най-вече абдоминалното затлъстяване. Изработени са изгладени персентилни криви за ИТМ и ОТ при деца и юноши на възраст 3 до 18 г., от които е видно пълното изглаждане на характерното за предните поколения снижение около 3-6 г. възраст. В сравнителен анализ са проследени 3 изследвани групи на 9 г. възраст относно честота на затлъстяването и е установено утрояване в последните 5 г. В проучване на деца от 3 до 5 г. възраст, които все още са с нисък относителен дял на затлъстяване, е установено сигнификантно увеличение с възрастта при момичетата. Отделно са изследвани някои обществено-здравни аспекти на затлъстяването - влияние на намалената физическа активност, отношение на личните лекари към затлъстяването, излагане на българските деца на телевизионни реклами. Основните изводи са в посока необходимост от незабавна интервенция за ограничаване на затлъстяването.The dissertation works out risk factors (cardio-metabolic and cardio-renal) in peripubertal full-term healthy children born small for gestational age (SGA). Their postnatal growth and catch-up until 10 years of age were followed, as well as their role for increased risk factors. Measurements of fat tissue and waist circumference (WC) were performed, blood pressure and pulse, kidney size as well as a number of metabolic markers (adiponectin, SHBG, HOMA-IR, TNF-alfa, IL-6, DHEA-S and sex steroids). All of them were significantly impaired in the otherwise healthy SGA children, with the worst values in children who accumulated most fat. We compared the data with data from more recent study of obesity with similar methodology and significant increase of total and particularly of the abdominal obesity was found. Smoothed BMI and WC percentile curves for children and adolescents aged 3 to 18 years showed full disappearance of the characteristics for the previous decades adiposity rebound at the age 3 to 6 years. In a comparative analysis 3 groups of children aged 9 years from 3 separate studies in the last 15 years showed tripled prevalence in the last 5 years. In a study of 3 to 5 year olds with relatively low prevalence of obesity, significant trend of increase with age was found in girls. Additionally, some public health aspects of obesity were studied - the influence of decreased physical activity, the attitude of family doctors towards obesity and the TV advertisement exposure of Bulgarian children. The main conclusions lead to the need of mandatory intervention for the restriction of obesity
