Operation nach Donald Nuss : Anästhesiologisches und intensivmedizinisches Management : eine prospektive Longitudinalstudie
Postoperative Schmerzen sowie postoperative Übelkeit und Erbrechen (PONV) zählen zu den problematischsten Erfahrungen in Zusammenhang mit Operationen. Bei der Trichterbrust-Operation nach Donald Nuss sind die postoperativen Schmerzen meist besonders ausgeprägt und erfordern ein gutes anästhesiologisches und interdisziplinäres Management. In der vorliegenden Arbeit galt es herauszufinden inwieweit die intraoperative Opioidgabe die postoperativen Schmerzen und die postoperative Beatmungsdauer beeinflusst. Eine weitere Fragestellung betraf die patienten-kontrollierten Analgesie (PCA) zur postoperativen Schmerztherapie und ob diese eine gute Alternative zur Periduralanalgesie darstellt. Insgesamt wurden in die prospektive Longitudinalstudie vom 76 Patienten eingeschlossen und im postoperativen Verlauf beobachtet. Anhand der Ergebnisse konnte gezeigt werden, dass die intraoperative Opioidgabe keinen signifikanten Einfluss auf die postoperativen Schmerzen und die Beatmungsdauer hat. Die patienten-kontrollierte Analgesie (PCA) erwies sich als risikoarme und gute Alternative zur Periduralanalgesie
Mittel- bis langfristige Ergebnisse nach Implantation der Daumensattelgelenkprothese Typ Elektra®
Die Daumensattelgelenkprothese zählt zu den modernen Therapieverfahren der Rhizarthrose. Kurze Rekonvaleszenzzeiten und eine frühe Wiederherstellung der Funktionalität stellen die Vorteile gegenüber den bisherigen Therapieverfahren dar. Trotz voranschreitender Weiterentwicklung der Prothesenmodelle fehlt es an publizierten Langzeitergebnissen. Die Elektra® Prothese wird zementfrei implantiert und wurde von der Firma Fixano 1996 auf den Markt gebracht. Die seitdem veröffentlichten Ergebnisse sind sehr heterogen. Zwischen 2003 und 2005 erfolgte bei 50 Patienten in der Orthopädischen Universitätsklinik Magdeburg die Implantation von 56 Elektra® Prothesen. In der vorliegenden Studienarbeit konnten mittel- bis langfristige Resultate von 54 Prothesen bei 48 Patienten, nach einem mittleren Beobachtungszeitraum von 8,2 Jahren, analysiert werden. 36 Frauen und 12 Männer mit einem Durchschnittsalter von 63 Jahren wurden untersucht. Postoperativ konnte eine signifikante Schmerzreduktion von 6 Punkten (7,4 vs. 1,4 VAS) erreicht werden. Beweglichkeit und Kraftentwicklung nahmen zu und näherten sich Normwerten an. Mittlere DASH-Werte (29 für Schmerz und 31 für Funktion) signalisierten gering verbleibende Restbeschwerden. Hohe Werte beim SF 36 bestätigten die positive Auswirkung des Gelenkersatzes auf die Lebensqualität. Im Nachuntersuchungszeitraum bis 2012 kam es bei 30 Prothesen zu klinischen und radiologischen Lockerungszeichen. In 25 Fällen führten Pfannenlockerungen zur Revision. Eine niedrige Überlebensrate von 55,6 % nach 8,2 Jahren steht den guten funktionellen Resultaten gegenüber. Damit bestätigt unsere Analyse weitestgehend die Ergebnisse bisher publizierter Elektra® Studien mit hohen Versagerquoten
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Longterm conditions of mimicked weightlessness influences the cytoskeleton in thyroid cells
Weightlessness influences the human immune and hormone system, reduces bone mass, leads to muscle atrophy and cardiac atrophy. Effects on control mechanisms for proliferation, programmed cell death and differentiation are well documented. The principal aim of this study was to investigate changes of the cytoskeleton in thyroid cells cultured in vector-averaged gravity under clinostat rotation. After 12 hours the formation of multicellular spheroids started. An increase of extracellular matrix proteins and beta 1-integrin was observed. Laser scanning confocal microscopy of ML-1 follicular thyroid carcinoma cells and normal thyroid HTU-5 cells immunostained with anti-cytokeratin to demonstrate these intermediate filaments revealed that cytokeratin filaments extended from centers, were thickened, coalesced and shortened as compared to control cells. Moreover, vimentin was highly disorganized. The vimentin network formed a coiled aggregate closely associated with the nucleus. Western blot analyses of talin, alpha- and beta-tubulin showed a clear increase of these proteins in cells cultured under simulated 0 g. Our data suggest that the effects of microgravity on cultured human thyroid cells are accompanied by noticeable functional cellular changes. Future studies to clarify the pathway that regulate the observed integrin activation and the mechanisms by which they function have to be performed
Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more
