Effect of Coracoid drilling for Acromioclavicular Joint Reconstruction Techniques on Coracoid stability
Für diese biomechanische Studie wurden 18 humane Kadaver-Scapulae präpariert und randomisiert in drei Gruppen mit 2x 4mm, 1x 4mm und 1x 2,4mm-Bohrungen eingeteilt. Nach standardisierter Bohrung wurde Zugkraft an den Conjoined Tendons ausgeübt. Die statistische Auswertung ergab keinen signifikanten Unterschied der Versagenslasten zwischen den Gruppen. Die alleinige Versagensart in den Gruppen mit 1x 4mm und 2x 4mm Bohrungen war die Fraktur des Coracoids (Nachweis der Fraktur durch den Bohrkanal im Micro-CT). In der Gruppe mit 1x 2,4mm-Bohrung versagten fünf Coracoid-Proben durch Ruptur der Conjoined Tendons und ein Coracoid frakturierte.For this biomechanical study, 18 human cadavaric specimen were dissected and randomly assigned to one of three groups with 2x 4mm, 1x 4mm and 1x 2.4mm-tunnels. After standardized coracoid-drilling, load was applied to the conjoined tendons. Statistical analysis showed no significant difference between the groups regarding load to failure. The single mode of failure for the groups with 1x 4mm and 2x 4mm-drill holes was coracoid fracture (micro-CT data showed a fracture running through the drill holes for all fractured coracoids). In the group with 1x 2.4mm-drill holes, five specimen failed by tear of the conjoined tendons, and one coracoid fractured
Long-term Clinical and Radiological Results following Mega-OATS of the femoral Condyle in the Knee Joint
Ziel dieser retrospektiven Fallserie waren die klinischen und kernspintomografischen Langzeitergebnisse nach Mega-OATS Operation am Kniegelenk bei Patienten mit ausgedehnten osteochondralen Defekten des Femurkondylus. Bei ingesamt 39 untersuchten Patienten zeigte sich eine signifikante Verbesserung des Lysholm-Scores. Der IKDC-Score lag 10 Jahre postoperativ bei 73 Punkten und der Tegner-Score blieb bei 4 Punkten konstant. Die kernspintomografischen Untersuchungen zeigten bei allen Patienten ein vitales, kongruent eingewachsenes Transplantat.The aim of this retrospective case series was the clinical and magnetic resonance imaging outcome after Mega-OATS procedure in patients with large osteochondral defects of the femoral condyle. In a total of 39 patients a significant improvement was represented in the Lysholm score. The IKDC score was 73 points at 10 years follow-up and the Tegner Score stayed equal at 4 points in comparison to the preoperative value. The magnetic resonance imaging showed vital and congruent grafts in all cases
A multicenter study to evaluate subscapularis muscle function using 5:30 o’clock portal for antero-inferior shoulder stabilization
Die Studie evaluiert und vergleicht die kurzfristigen postoperativen Ergebnisse nach antero-inferiorer Schulterstabilisierung mittels 5:30-Uhr-Portal (Gruppe I) und 3-Uhr-Portal (Gruppe II). Es wurden jeweils 31 gematchte Patienten mittels Scores, sonographisch sowie einer Kraftmessplatte nachuntersucht. Gruppe I hatte eine signifikant bessere postoperative AR in 0° Abduktion (p<0,05). Gruppe II erreichte einen signifikant höheren SST Score (p<0,05). Signifikante Unterschiede der strukturellen Integrität oder der Kraft des SSC in Abhängigkeit vom Zugangsportal wurden nicht festgestellt. Das 5:30-Uhr-Portal eignet sich somit hervorragend zur antero-inferioren Stabilisierung.This study evaluates and compares the short-term results of antero-inferior shoulder stabilization with either a 5:30 o`clock (group I) or 3:00 o`clock portal (group II). 31 matched patients in each group were evaluated using shoulder scores, ultrasound and a power plate. Group I had a significantly better external rotation in 0° abduction (p<0.05). Group II showed a significantly better SST score (p<0.05). Ultrasound evaluation and strength measurement showed no significant differences in SSC integrity or function. Thus, the 5:30 o`clock portal is a reliable and safe approach to address Bankart lesions
Prospective short-term MRI controlled investigation of the healing properties of meniscus repair
Ziel/Hypothese: Das Ziel dieser Studie war die prospektive Untersuchung der Signalalterationen nach akuter Meniskusnaht anhand spezifischer Magnetresonanztomographie (MRT)-Sequenzen. Die Hypothesen waren, dass 1) es unterschiedlichen Heilungseigenschaften in Abhängigkeit der Risskonfiguration und –größe gibt und 2) die Risslokalisation einen Einfluss auf die Heilungspotenz hat.
Material und Methoden: In diese Studie wurden Patienten mit traumatischen Meniskusläsionen und folgender arthroskopischer Meniskusnaht innerhalb von 6 Wochen eingeschlossen. Ausschlusskriterien waren ein Alter 45, degenerative Veränderungen > ICRS III und Patienten mit Multiligamentverletzungen. Die radiologischen Nachuntersuchungen fanden nach 2, 4, 6 und 12 Wochen sowie nach 6 Monaten mittels 3T-MRT statt. Die Auswertung basierte auf der ISAKOS Meniskusklassifikation. Postoperativ wurde die Meniskusheilung entsprechend der Henning Klassifikation in A) kontinuierlich, B) partiell kontinuierlich (>50%) und C) diskontinuierlich klassifiziert. Das klinische Ergebnis wurde anhand einer Untersuchung und validierte Outcome-Scores (IKDC, KOOS, Lysholm) nach 6 und 12 Wochen sowie nach 6 Monaten untersucht. Die Daten wurden mit SPSS Statistiksoftware Version 21 (IBM, New York, USA) analysiert und die Signifikanz bei p < 0.05 gesetzt.
Resultate: Es konnten 30 Patienten (28m, 2w) mit insgesamt 35 Meniskusrissen (19 medial, 16 lateral) eingeschlossen werden. Die Risslänge war durchschnittlichen 19.2 ± 11.0 mm. 26 Risse waren in Zone 1 und 9 Risse in Zone 2 lokalisiert. In 16 Patienten (53%) wurde zusätzlich eine Ersatzbandplastik des vorderen Kreuzbands durchgeführt. Die klinischen Scores verbesserten sich zum finalen Untersuchungszeitpunkt wie folgt: IKDC Score (präOP: 39.4 ± 18.5, postOP: 78.8 ± 15.3) KOOS (präOP: 45.7 ± 22.1, postOP: 82.7 ± 12.5) und Lysholm Score (präOP: 42.8 ± 23.7, postOP: 84.4 ± 13.8) (p < 0.01). In der MRT Untersuchung zeigten sich 6 Wochen postoperativ 54.5% der Menisken als kontinuierlich, 33.3% als partiell kontinuierlich und 12.1% als diskontinuierlich. Nach 12 Wochen waren 60.6% kontinuierlich, 33.3% partiell kontinuierlich und 6.1% zeigten intraläsionale Flüssigkeit und wurden als diskontinuierlich gewertet. Zum finalen Untersuchungszeitpunkt wurden 64.7% der Menisken als kontinuierlich, 26.5% als partiell kontinuierlich und 8.8% als diskontinuierlich klassifiziert. Zwei Korbhenkelriss und ein Longitudinalriss waren nach 6 Monaten radiologisch nicht geheilt.
Schlussfolgerung: Das Ergebnis dieser Studie zeigt eine stetige Signalveränderung in der MRT Bildgebung innerhalb der Meniskusrisszone während der ersten Heilungsphase. Diese Strukturveränderung weist auf einen kontinuierlichen Heilungsprozess in den ersten sechs Monaten hin. Die Anzahl der in Querschnitt über 50% kontinuierlich geheilten Menisken liegt nach 6 Wochen bei 87.9%, nach 3 Monaten bei 93.9% bzw. nach 6 Monaten 91.2%.Purpose/ Hypotheses: The purpose of the study was to prospectively investigate signal alterations in short-term follow-up after acute meniscus repair on specific magnetic resonance imaging (MRI) scan sequences. It was hypothesized that 1) there are different meniscus healing properties depending on lesion configuration and size, and 2) the tear zone has an influence on the healing properties of the meniscus.
Material and Methods: We enrolled patients (age 18-45 years) with traumatic meniscus lesion, subsequent arthroscopic meniscus repair within 6 weeks and available preoperative MRI. Exclusion criteria were age 45 years, osteoarthritis > ICRS III and multiligamentous knee injuries. Radiological follow-up examinations were performed 2, 4, 6, 12 weeks and 6 months after operation using a 3T-MRI. Results evaluation was based on ISAKOS meniscus classification system. Meniscus healing were classified according to Henning’s criteria in A) continuous, B) partially continuous (> 50%) and C) separated. Furthermore, clinical examination and outcome scores (IKDC, KOOS, Lysholm Score) were surveyed preoperatively and 6 and 12 weeks and 6 months after surgery. Data were analyzed using SPSS statistics software version 21 (IBM, New York, USA). Statistical significance was set at a p value of < 0.05.
Results: At final follow-up (FU), 30 patients (28m, 2w) with a total of 35 meniscus tears (19 medial, 16 lateral) met the inclusion criteria. The average tear length was 19.2 ± 11.0 mm. 26 tears were located in the rim zone 1 and 9 tears were more medially in zone 2. In 16 (53%) patients an additional anterior cruciate ligament (ACL) reconstruction was performed. Clinical scores improved significantly after surgery: IKDC Score (preOP: 39.4 ± 18.5, final FU: 78.8 ± 15.3) KOOS (preOP: 45.7 ± 22.1, final FU: 82.7 ± 12.5) and Lysholm Score (preOP: 42.8 ± 23.7, final FU: 84.4 ± 13.8) (p < 0.01). Regarding MRI examination at 6 weeks follow-up, 54.5% were rated as continuous, 33.3% as partially continuous and 12.1% as separated. At 12 weeks follow-up 60.6% were considered continuous, 33.3% partially continuous and 6.1% showed intrameniscal joint fluid in more than 50% of meniscus thickness and were consequently rated as not separated. At final FU 64.7% of the menisci were rated continuous, 26.5% as partially continuous and 8.8% as separated. Two bucket handle tears and one longitudinal tear were not healed after 6 months.
Conclusion: At final FU all investigated menisci showed signal alteration on MRI, in most instances considered as scar tissue without intrameniscal joint fluid. According to Henning’s criteria 91.2% of the menisci were rated as continuous or partially continuous and clinical follow-up showed good short-term results after meniscus repair
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Refixation of the supraspinatus tendon in a rat model - influence of continuous growth factor application on tendon structure

A prospective study of clinical outcomes comparing stationary vs. outpatient rehabilitation following rotator cuff repair
Die Studie verglich das klinische Outcome nach Rotatorenmanschetten-Rekonstruktion von 23 Patienten nach stationärer Rehabilitation mit 15 Patienten nach ambulanter Rehabilitation. Es wurde ein Follow up von sechs Monaten zugrunde gelegt. Zu vier Untersuchungszeitpunkten wurden verschiedene klinische Scores eingesetzt. Die Ergebnisse zeigten in den wesentlichen Punkten keine statistisch signifikanten Unterschiede. Prinzipiell relevant sind die Durchführung einer Rehabilitation und die individuelle Motivation des Patienten.The study reviewed the clinical outcome of stationary compared to outpatient rehabilitation following rotator cuff repair. A group of 23 patients completed a stationary rehabilitation program and were followed for six months after surgery. They were compared to 15 patients completing an outpatient rehabilitation program using different clinical scores. The results did not show any statistical significance in major perspectives. However, the performance of rehabilitation and the individual motivation of the patient seemed to be relevant for the clinical outcome
