The WHO BMI System Misclassifies Weight Status in Adults from the General Population in North Italy: A DXA-Based Assessment Study (18-98 Years)
Background/Objectives: The body mass index (BMI) is considered a key method for the classification of individuals' weight status, according to cut-off points proposed by the World Health Organization (WHO); however, the use of this classification is still the subject of debate and criticism. We aimed to evaluate the accuracy of the WHO BMI classification in reflecting true adiposity in the Italian general population. Methods: This cross-sectional study included 1351 adults of mixed gender aged between 18 and 98 years, comprising 19 (1.4%) underweight individuals, 787 (58.3%) normal weight, 354 (26.2%) overweight, and 191 (14.1%) with obesity according to the WHO BMI. After that they were re-categorized according to adiposity based on body fat percentage (BF%) measured by dual-energy X-ray absorptiometry (DXA). The agreement between the two classification systems was tested using the kappa statistic (κ), with the system based on BF% being considered the gold standard. Results: According to the BMI classification, 78.1% of the individuals who were in the normal weight range were correctly classified. However, 53.4% of the overweight group and 68.4% of the underweight group were misclassified according to the BMI, as the majority of those misclassified fell within the normal weight range according to their BF%. Finally, regarding the obesity group, 34% who were classified as having obesity according to the BMI were misclassified, since they were revealed to be only affected by overweight according to adiposity status. Conclusions: Despite the fact that the BMI seems to be reliable in determining body weight status in the normal weight range, over a third of the general population was misclassified, as the current BMI classification appears to inflate the prevalence of underweight, overweight, and obesity among the general population. Accordingly, this may warrant consideration of revising the National Guidelines in Italy related to weight status classification. Healthcare practitioners should be advised not to rely solely on the BMI, and should integrate its use with adiposity measures (i.e., BF%) or alternative surrogate indicators (i.e., waist-based) in routine evaluations, especially in those with a BMI below or above 18.5 kg/m2 or 25 kg/m2
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. New BMI Cut-Off Points for Obesity in Middle-Aged and Older Adults in Clinical Nutrition Settings in Italy: A Cross-Sectional Study
Obesity is a major health problem defined as an excess accumulation of body fat (BF). The World Health Organization (WHO) usually relies on a body mass index (BMI) ≥ 30 kg/m2 as an indicator of obesity. Due to changes in body composition that occur across the lifespan, with an increase in BF and a decrease in lean mass, we aimed to test the validity of this BMI cut-off point for adiposity in middle-aged and older adults. This cross-sectional study, composed of 4800 adults of mixed gender aged between 40 and 80 years, included (according to the WHO BMI classification) 1087 normal-weight, 1826 overweight, and 1887 obese individuals who were referred to the Department of Biomedicine and Prevention, University of Rome "Tor Vergata", Italy. The sample was then categorized by adiposity status based on the total BF% as measured by dual-energy X-ray absorptiometry (DXA), and the best sensitivity and specificity were attained for predicting obesity according to the receiver operating characteristic curve (ROC) analysis. In a real-world clinical setting, a new BMI cut-off point (BMI = 27.27 kg/m2) has been identified for predicting obesity in middle-aged and older adults. Obesity guidelines in Italy therefore need to be revised accordingly
The Risk of Sarcopenia among Adults with Normal-Weight Obesity in a Nutritional Management Setting
Normal-weight obesity (NWO) is a phenotype characterized by excessive body fat (BF) despite normal body weight. We aimed to assess the association between NWO and the risk of sarcopenia. Two groups of patients with a normal body mass index [BMI (20–24.9 kg/m2)] were selected from a large cohort of participants. Body composition was measured using dual-energy X-ray absorptiometry (DXA), and 748 participants were categorized as NWO or normal-weight without obesity (NWNO) and were classed according to whether or not they were at risk of sarcopenia. The “NWO group” included 374 participants (cases), compared to 374 participants (controls) in the “NWNO group”, all of a similar BMI, age and gender. The participants in the “NWO group” displayed a higher prevalence of the risk of sarcopenia than the control group across both genders (0.6% vs. 14.1% in males; 1.4% vs. 36.5% in females). Regression analysis showed that being in the NWO category increased the risk of sarcopenia 22-fold in males (RR = 22.27; 95%CI: 3.35–147.98) and 25-fold in females (RR = 25.22; 95%CI: 8.12–78.36), compared to those in the NWNO category. In a “real-world” nutritional setting, the assessment of body composition to identify NWO syndrome is vital since it is also associated with a higher risk of sarcopenia
Revising BMI Cut-Off Points for Overweight and Obesity in Male Athletes: An Analysis Based on Multivariable Model-Building
Background: Body composition in athletes is characterized by pronounced muscle mass and low body fat (BF). Over and excessive adiposity are thus expected in athletes at higher body mass index (BMI) levels than those suggested by the World Health Organization (WHO). Therefore, we aimed to test the validity of WHO BMI cut-off points for overweight and obesity, respectively (i.e., >= 25 kg/m2 and 30 kg/m2) in young male athletes from different sport disciplines in Italy. Methods: This study includes 622 male young adult athletes of mean age 25.7 +/- 4.7 years who were initially categorized according to the WHO BMI classification, and then re-categorized by adiposity status based on total BF% as measured by dual-energy X-ray absorptiometry (DXA). A predictive equation has been developed utilizing multivariable model-building to predict the best BMI cut-offs for identifying overweight and obesity in this population. The agreement between the different classification systems was assessed with the kappa statistic (kappa). Results: According to the WHO BMI classification, 451 (72.5%) individuals were of normal weight, 148 (23.8%) were with overweight and 23 (3.7%) were with obesity, but based on the total BF%, 598 (96.1%) were of normal weight, and only 19 (3.1%) were with overweight and 5 (0.8%) were with obesity, revealing a weak agreement between the two classification systems (WHO BMI vs. BF%; kappa = 0.169). On the other hand, new BMI cut-off points were identified (BMI >= 28.2 kg/m2 for overweight and 33.7 kg/m2 for obesity) and showed good agreement with the BF% classification system (kappa = 0.522). Conclusions: The currently used WHO BMI cut-offs are not suitable for determining weight status in young male athletes, and since the newly proposed ones demonstrated a good performance, these should be implemented in new guidelines
Do Lifestyle Interventions before Gastric Bypass Prevent Weight Regain after Surgery? A Five-Year Longitudinal Study
It is unclear whether weight loss (WL) achieved by means of lifestyle interventions (LSIs) before bariatric surgery (BS) can improve long-term WL outcomes after surgery. We aimed to assess the impact of a structured LSI on WL% after gastric bypass (GBP). Two groups of patients were selected from a large cohort of participants with obesity who underwent GBP surgery at Santa Maria Nuova Hospital (Reggio Emilia, Italy). The groups were categorized as those who have or have not received LSI prior to GBP. The LSI group included 91 participants (cases) compared to 123 participants (controls) in the non-LSI group. WL% was measured at follow-up times of 1, 3, 6, 12, 24, 36, 48, and 60 months. The LSI group achieved a clinically significant WL% (−7.5%) before BS, and at the time of surgery, the two groups had similar body weights and demographic statuses. At all points, until the 24-month follow-up, the two groups displayed similar WLs%. With regard to the longer follow-ups, the LSI group maintained weight loss until the last timepoint (60 months), whereas the non-LSI group experienced weight regain at 36, 48, and 60 months. In a real-world context, a structured behavioral LSI prior to GBP seems to prevent longer-term weight regain
Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more
