Umbilical endometriosis associated with umbilical hernia. Management of a rare occurrence
Riassunto
Riportiamo un raro caso di endometriosi ombelicale primitiva associata a ernia in
cui è stata effettuata l’asportazione dell’ombelico e la sua ricostruzione immediata
con una tecnica di recente descrizione. L’ernia è stata riparata secondo la tecnica
di Mayo e dopo ampia resezione del sacco erniario per possibile presenza di
tessuto endometriale al suo interno. La ricostruzione dell’ombelico è avvenuta
mediante due lembi semicircolari creati all’interno di un’ellisse disegnata sulla cute
in senso verticale preoperatoriamente. Il tipo di incisione ha reso agevole la riparazione
dell’ernia, che è avvenuta senza l’ausilio di protesi sia per le piccole dimensioni
dell’ernia, sia per mancanza di dati in letteratura sull’uso di protesi in caso
di endometriosi. I risultati estetici sono stati giudicati soddisfacenti a 6 mesi
dall’operazione per l’assenza di cicatrice ipertrofica e per l’aspetto anatomico del
nuovo ombelico
La chirurgia conservativa nel cancro della mammella
La chirugia conservativa fa ormai parte dei protocolli utilizzati nel trattamento del carcinoma della mammella con T fino a 3 cm. La recidiva locale rappresenta la maggiore problematica sia per la individuazione clinica sia per il trattamento conseguenziale non sempre bel differenziale da una seconda neoplasia.. L'esperienza diretta dal 1987 al 2004 su un totale di 1504 tumori della mammella trattati, riguarda 803 tumori (53,4%) con T inferiore a 3 cm arruolati per un trattamento chirurgico conservativo. L'introduzione della tecnica del linfonodo sentinella ha modificato l'entita' della dissezione ascellare che prevede nelle pazienti T1a-b la sola asportazione del linfonodo sentinella se non metastatico all'esame estemporaneo. La percentuale di recidive locali è stata del 3.5% a distanza di 5 anni e del 6% a 10 anni
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Effectiveness of fibrin glue in conjunction with collagen patches to reduce seroma formation after axillary lymphadenectomy for breast cancer
BACKGROUND: Axillary lymphadenectomy remains an integral part of breast cancer treatment, yet seroma formation occurs in 15% to 85% of cases. Among methods employed to reduce seroma magnitude and duration, fibrin glue has been proposed in numerous studies, with controversial results.
METHODS: Fifty patients underwent quadrantectomy or mastectomy with level I/II axillary lymphadenectomy; a suction drain was fitted in all patients. Fibrin glue spray and a collagen patch were applied to the axillary fossa in 25 patients; the other 25 patients were treated conventionally.
RESULTS: Suction drainage was removed between postoperative days 3 and 4. Seroma magnitude and duration were significantly reduced (P * .004 and .02, respectively) and there were fewer evacuative punctures in patients receiving fibrin glue and collagen patches compared with the conventional treatment group.
CONCLUSIONS: Use of fibrin glue with collagen patches does not always prevent seroma formation, but it does reduce seroma magnitude and duration, as well as necessary evacuative punctures
SENTINEL LYMPH NODE BIOPSY IN THE TREATMENT OF BREAST CANCER. EXPERIENCE IN 527 CASES
Knowledge of axillary lymph node status is a key aid to staging and prognosis and it represents a guideline for adjuvant therapy in breast cancer. Despite the morbidity it causes, complete axillary dissection was long the mainstay of treatment. Sentinel lymph node biopsy has proved so reliable in the evaluation of node involvement that axillary node dissection is now generally performed when sentinel node biopsy tests negative. METHODS: In this 3-phase study, 50 patients were enrolled to evaluate the learning curve of sentinel node biopsy (phase 1, September 1997-January 1998); 256 patients (age range 27-81 years) with infiltrative breast cancer (T <3 cm, clinical N0) underwent level 1 lymph node dissection when the sentinel node tested negative at histopathology (phase 2, February 1998-March 2001); 221 patients with T <3 cm underwent dissection of the sentinel node when it tested negative for metastasis (phase 3, April 2001-March 2005). RESULTS: The sentinel node was preoperatively detected in 98.6% of cases after peritumoral and intradermic injection of the radionuclide tracer and intraoperatively in 99% (90% with radio-guided surgery, 10% with vital staining). The sentinel node was positive in 15% of patients with T1 and metastatic in 65%. CONCLUSIONS: Our results are in line with the published data; therefore, the study will go forward to examine the role of the micrometastasis in the sentinel node and of in-transit tumoral cells
Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more
