Utilità dell'anamnesi nello studio dei lattanti con infezioni urinarie a rischio di uropatie malformative
The incidence of clinical symptoms and of anamnestic records were evaluated in 34 healthy controls and in 75 infants, under 18 months of age, consewcutively admitted to the pediatric clinic for symptomatic urinary tract infections (UTI); in 40 of them a urinary tract malformation (UTM) was demonstrated while in the other 35 the UTIs were uncomplicated.
Clinical symptoms were significantly more frequent in patients than in controls.
No differences in all parameters considered were found between the two groups of patients, except for the age at first radiological investigations which appeared significantly lower either in all children with UTM, or only in males.
Children with UTM also appear to have had more symptoms suggesting a compromised physical condition.
Our results, which should be confirmed by a prospective study, suggest that the use of detailed and specifically oriented chart in order to take a precise clinical history with a quantification of the more important symptoms could contribute, with the results of bioumoral indexes, to selct the children with symptomatic UTM at ahigher risk for UTM in whom early radiological investigations have to be made.

L'Ambulatorio ospedaliero, nuove prospettive di utilizzo
Gli Autori presentano i risultati di uno studio prospettico condotto sull'utenza dell'ambulatorio della Clinica Pediatrica di Padova.
L'analisi dei dati così ottenuti evidenzia che l'ambulatorio studiato è composto da 2 servizi dissimili tra loro per tipo di utenza e qualità di prestazioni erogate. Il primo, ambulatorio di pediatria generale, è utilizzato da un'utenza locale che si rivolge alla Clinica Pediatrica in prima istanza e spesso per quesiti di tipo consultoriale. Per le sue caratteristiche questo tipo di attività può essere considerata come indicatore inverso di una buona pediatria territoriale e di integrazione ospedale-territorio ed è destinata in prospettiva a ridursi vistosamente. Le funzioni di controllo dei dimessi e di consulenza per problema di media entità svolte anch'esse da questo servizio generale, sono invece proprie di ogni ambulatorio ospedaliero con piccolo o medio bacvino di utenza e dovrebbero essere incentivate, grazie anche alla dotazione di semplici presidi diagnostici.
Il secondo tipo di servizio, costituito dagli ambulatori specialistici, è caratterizzato da un'utenza ad ampio respiro territoriale, equamente distribuita in tutta la fascia d'età pediatrica, generalmente inviata da altre strutture e con patologie frequentemente complesse e a decorso cronico. Questi paramentri definiscono un servizio che deve essere accentrato in pochi poli a funzione regionale.
La proliferazione indiscriminata di simili servizi in molti ospedali, specie se associati ad altri tipo ospedale di giorno, non pare invece giustificata nè conveniente
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Diagnostic factors in pediatric primary headache
Primary headaches are frequent in children. They are difficult to diagnose because there is much disagrrement about the interpretation of the historical data and the use of signs and/or symptoms in diagnosis. It would be useful, therefore, to standardize this procedure.
We used linear discriminant analysis to determine a classification rule capable of diagnosing new cases of chronic and recurrent primary headache in children. We considered 23 symptoms in 118 patients. Through discriminant anlysis we chose five variables: frequency of the attacks, type of pain, neurologic deficits, nausea, and vomiting.
With this classification rule, we obtained a total correct classification of 84.7% for migraine, psychogenic headache, and non-defined headache in respect to the diagnoses formulated by a pediatrician and a child neuropsychiatrist after 3 months of follow-up. Our method for diagnosing migraine has a sensitivity of 95% and a specificity of 100%. The algorithm, validated on another 105 pediatric patients, produced a total correct result of 82.9%

Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
