    Marine toxins - saxitoxin (STX) and tetrodotoxin (TTX)

    Saxitoxin (STX) and tetrodotoxin (TTX) are two of the most poisonous marine biotoxins known. They have a very interesting history, both used as a means for suicide and murder STX has been announced publicly by the world`s governments as a chemical weapon, which made it attractive for many government agencies and no longer interesting for toxicologists and seafood producers. TTX is known as the toxin of `fugu` and also as the main agent in the zombification rituals in some Caribbean cultures. However, intentional intoxications by STX and TTX are not often as much as badly prepared fugu meals or saxitoxin-infested shellfish.A precise diagnosis can determine whether a victim has been intentionally intoxicated. The treatment of intoxication by STX and TTX is based on gastric evacuation, symptomatic relief and the body`s natural recovery mechanisms. There are some potential treatment options, but they need more examination and financing to advance

    Систематизирани и обобщени са демографските данни и медикаментозното лечение на 535 хоспитализирани пациенти с диагноза хронична обострена сърдечна недостатъчност, със запазена (СНзФИ) и редуцирана фракция на изтласкване (СНрФИ), NYHA клас III-IV. Те са проследени за период от пет години – от януари 2010 до декември 2014 година. Данните включват възраст, пол, фракция на изтласкване на лявата камера (ФИ), съпътстващи заболявания, прилагани медикаменти, противопоказания, нежелани ефекти и токсикологични рискове. Проучена е фармакологичната терапия на пациентите със СН и е разгледана и сравнена според пола и ФИ на пациентите. Установява се, че най-често предписваните лекарства след бримковите диуретици са бета-блокерите, АСЕ-инхибиторите/ангиотензин рецепторните блокери и минералкортикоид рецепторните антагонисти, а Ivabradine е предписван с най-малка честота. Установени са разлики между СНрФИ и СНзФИ в предписваната препоръчана терапия. Оценено е придържането към лечението на СН според европейските препоръки, като е извършена корекция според коморбидностите. При хоспитализираните пациентите със СН са установени редица коморбидности, водещи до непредписване на медикаменти, повишен риск от хиперкалиемия и други потенциални странични и токсични ефекти. Проследени са фармакологичната терапия и демографските показатели за пет годишен период и е установена динамика и в двете групи показатели. При проследяване на предписваната терапия в изследвания петгодишен период се установява все по-добро придържане към препоръките за лечение на СН. През последната година от проучването фармакотерапията се доближава в най-голяма степен до водещи европейски регистри.The thesis presents a systemization and summary of the demographic data and pharmacological therapy of 535 hospitalized patients with a diagnosis of chronic decompensated heart failure, with preserved (HFpEF) and reduced ejection fraction (HFrEF), NYHA class III-IV. They were followed-up for a period of 5 years – from January, 2010 to December, 2014. The data include age, gender, left ventricular ejection fraction (EF), concomitant diseases, medicaments used, contraindications, adverse effects and toxicological risks. The pharmacological therapy of HF patients was examined and compared by patients’ gender and EF. It was established that the most frequently prescribed medicaments, after loop diuretics, were beta blockers, ACE-inhibitors/angiotensin receptor blockers and mineralocorticoid receptor antagonists with Ivabradine being the least frequently prescribed one. We determined that there are differences in the prescribed therapy between HFrEF and HFpEF. We assessed the adherence to the HF treatment based on the European recommendations with a correction for comorbidities. A number of comorbidities were observed in HF patients leading to non-prescription of medicaments, increased risk of hyperkalaemia, and other potential side and toxic effects. The pharmacological therapy and demographic indicators for a 5-year period were examined and the dynamics of both groups of indicators were established. When examining the pharmacological therapy in the 5-year period a better adherence to the HF treatment recommendations was observed. During the last year of the study, the pharmacotherapy was closest to the leading European registries

    Biological toxins are one of the most serious public health threats. They are cheap, easy to disperse and can cause illness and death even in very small doses. The onset of clinical signs may be delayed - from a few hours to a couple of days. Thus, to evaluate the exposure as a cause for complains is sometimes difficult and indistinguishablefrom a common illness. Toxins and other biological threat agents are a perfect alert for those, who are trying to cause panic and to weaken the social and economic stability. The health care community at all levels, mainly those in hospital emergency departments and in private practice, needs to identify the occurrence of an intentional toxin release.The threat of bioterrorism requires economic, political and medical awareness. It is of importance, that governments (federal, state and local) ensure one efficient infrastructure for managing toxic attacks and spend the necessary funds to support the protection of the public`s health, in such a manner as to achieve the best possible results

    Concept of „death" in the lyrics of Nadezhda Teffi

    Nadezhda Teffi (former Buczynska, born as Lochwicka, 1872-1952) - poet and novelist, one of the most interesting characters in Russian literature of the first half of the twentieth century. In 1920 Teffi left Russia supplying the same community of thousands of Russian immigrants. In Paris, still accompanied by the opinion of eminent satirist, but a poetic works can trace many aspects of the Teffi creative attitude, and especially reflective and nostalgic nature of the emigration work. The experience of exile provoked to think about the country, the world and herself in this world. Similar sentiments reflect the poems included in the books of poetry: Шамрам. Песни Востока (Songs of the East, Berlin, 1923), Passiflora (Berlin 1923) and in the poems published in collective volumes and anthologies. The article analyses the concept of death in the lyrics of Teffi. Semantical structure of this concept includes both objective senses and individual imaginations of the author

    This article’s topic is the analysis of the theme of sleep and ways of his linguistic representation in Nadezhda Teffi’s poetry. The author distinguishes various updates of the selected motif, divided them according to their meaning and thematic scope and pointed out their importance in the poetic achievements of the Russian emigrant. The subject of the analysis is the context of direct nominations of the lexeme, its derivates, semantic words and antonyms, appearing in the debut volume of poems Seven Fires (1910) and in Poetry Poems published in Exile: Passiflora Shamram. Songs of the East (1923). The research concept adopted by the author remains in line with the communication style of the text, whose tasks cover various aspects of the analysis of the literary text perceived as a form of communication reflecting both the stylistic standard and the individual style of the author

    A. Brusilov's Wife Portrait: N. Brusilova at Her Motherland and in Emigration

    The author reveals the life and personality of an outstanding fignhting commander of World War I, Aleksei Brusilov’s wife. Her role in his life was shadowed during the Soviet period because of her anti-soviet position mostly due to her orthodox religious views. The paper is based on the unpublished correspondence between Nadezhda Brusilov) and her husband in 1914–1917 when A. A. Brusilov was at the front, and his wife was engaged in charity work in the rear, as well as her diary written during the years she lived in Czechoslovakia as an emigrant (5972 GARF F. A. A. and N. V. Brusilov). The letters show a great emotional unity of the couple although the relationship between them has not always been smooth. N. V. Brusilov had a strong character, tried to infl uence her husband, give recommendations how to conduct military operations, to avoid casualties, but she managed to brighten up the loneliness of the commander in the last period of his life, taking care of him, preserved and published his memoirs. Brusilov believed their marriage happy. After Aleksei Brusilov’s death in 1926, Nadezhda Brusilov went to Czechoslovakia (1930) under the guise of treatment and did not returned to Russia. She was marginalized in the emigrants’ milieu because of Alexei Brusilov’s position during the Soviet — Polish War in 1920 when he joined the Red Army. The position of her husband she unequivocally justified patriotic motives. She fixed her nostalgic feelings for the lost homeland and her loneliness in a foreign land in her diary. It was not intended to be published as N. V. Brusilov feared damage to people stay in Russia. Her diary is an important source on her life abroad as an emigrant, her thoughts about her lost homeland, her views on Orthodox Church, both in Russia and abroad

    The November Uprising in Nadezhda Golitsyna’s "The Memoirs"

    The article presents Nadezhda Ivanovna Golitsyna’s (1796—1868) memories of the Polish November Uprising. Golitsyna was a wife of a count Alexander Fyodorovitch Golitsyn(1796—1864), who was a high‑ranking Tsarist official. They lived in Warsaw, where the husband of the author of the Memoirs worked in the office of the Grand Prince Konstantin. Golitsyn was, most probably, a secret agent of the Third Division. After the outbreak of the November Uprising the Golitsynys, together with prince Konstantin’s troops, left Warsaw. The Poles’ struggle for independence is presented in Golitsyna’s Memoirs as a sudden and unexpected rebellion organised by a group of students against the Russian ruler. Golitsyna criticises the participants of the uprising, and describes their actions as imprudent and reckless. In her opinion, the November Uprising is mainly associated with necessity of marching off from the capital of the Kingdom of Poland, with difficult living conditions, suffering, arrests, and death of the loved ones.
