    The impact of pathology, type of resection, and age at surgery on the postoperative seizure and cognitive outcome in patients with temporal lobe epilepsy

    In this thesis, epilepsy- and surgery-related factors were investigated for their effects on postoperative seizure and cognitive outcome in patients with medically intractable temporal lobe epilepsy (TLE). As a first step, Borger, Hamed, Taube, et al. (2021) evaluated various clinical factors contributing to an (un-)favorable surgical outcome in a large consecutive cohort. While seizure freedom rates and cognitive changes were comparable to those of the previously reported cohorts, this was the first study to identify the histopathological diagnosis of 'no hippocampal sclerosis with gliosis only' (GO) as a risk factor for seizure recurrence after surgery. This questions the current assumption that GO is a predecessor of the histopathological diagnosis of 'hip-pocampal sclerosis' (HS). Hence, Grote, Heiland, Taube, et al. (2022) and Taube, et al. (2022) investigated differences between GO and HS further. The later epilepsy onset, higher surgical failure rates, mild memory impairments before surgery, and greater postoperative memory losses favor the hypothesis that GO represents a distinct clinical entity. Moreover, we found evidence of transcriptomic dysregulation in GO suggestive of inflammatory processes through activation of the complement system. Our results question not only the current clinical practice in treating patients with TLE due to GO, but also emphasize the need for an investigation of inflammatory mediators at the onset of TLE. The involvement of spatiotemporal networks in the epileptic brain exceeding the lesional epileptogenic zone (EZ) is increasingly well recognized. This has significant implications for optimal resection extent because surgically targeting these epileptic circuits may result in improved seizure outcomes. Borger, Schneider, Taube, et al. (2021) retrospectively evaluated the impact of surgically removing the piriform cortex (PIC), considered a hub for generating and spreading seizures, on postoperative outcomes. Seizure outcomes improved with larger PIC resection extent, while the risk of complications and adverse neuropsychological events did not increase. Future studies need to assess the surgical amenability of the PIC and identify how residual involved cortical and subcortical circuits contribute to surgical failures in the treatment of focal epilepsies. The fifth study by Delev, Taube, et. al (2020) assessed the safety and efficacy of temporal lobe surgery (TLS) in late adulthood. While TLE has been linked to accelerated brain aging, the effects of TLS are inconclusive. Since seizures were controlled in the majority of our patients, TLS can counteract the contribution of epileptic dysfunction, injuries during seizures, and high drug load to cognitive impairments. The cognitive costs of TLS were comparable to those reported in younger cohorts despite evidence of more severe memory impairments already before surgery. Even though longterm outcome studies indicate no progressive memory decline beyond the immediate postoperative losses, TLS in late adulthood may nonetheless contribute to reaching a disabling level of extensive memory impairment much earlier. Especially in the light of accumulating evidence that epileptogenesis and neurodegeneration share common etiological mechanisms, patients most at risk of severe cognitive sequelae following epilepsy surgery must be identified via early investigation of cerebrospinal fluid and neuroimaging biomarkers. The present thesis contributes to our understanding of the impact of clinical and etiological factors on postoperative cognitive and seizure outcomes. The results emphasize the importance of pathology-dependent surgical counseling and decision-making to improve individual medical care

    Veränderte neuronale Aktivität auf soziale Berührung im Nucleus Accumbens und Nucleus Caudatus bei Patienten mit Major Depression

    Depressionen ist eine der häufigsten psychischen Erkrankungen unserer Gesellschaft und hat damit einen hohen Stellenwert für unser Gesundheitssystem. Die Verarbeitung sozialer Belohnungen hängt im Wesentlichen mit Sozialverhalten zusammen. Bislang ist jedoch nicht bekannt, welche Auswirkungen schwere depressive Störungen auf die Verarbeitung sozialer Belohnungen haben. Ziel der hier vorliegenden Studie ist es herauszufinden, ob die Verarbeitung sozialer Berührungen als Maß für soziale Belohnung bei Patienten mit Depressionen gestört ist und, inwieweit sich diese Verarbeitung von gesunden Kontrollprobanden unterscheidet. Zusätzlich wurde untersucht, ob es nach einem vierwöchigen stationären Aufenthalt, der eine antidepressive Therapie beinhaltete, zu einer Veränderung in der Berührungsverarbeitung kommt. Hierzu führten wir eine kontrollierte Prä/Post-Längsschnittstudie durch, für die insgesamt 51 Patienten und 40 gesunde Teilnehmer jeweils beider Geschlechter im Alter zwischen 20 und 60 Jahren rekrutiert wurden. Sämtliche Patienten wurden im Zeitraum zwischen Juni 2016 und April 2018 stationär in der Klinik für Psychiatrie und Psychotherapie des Universitätsklinikums Bonn aufgenommen. Zur Untersuchung der neuronalen Berührungsverarbeitung wurde ein soziales Berührungsparadigma im Rahmen eines fMRT-Scans durchgeführt. Anschließend führten wir einen Vergleich der neuronalen Reaktionen auf soziale Berührungen im Belohnungsnetzwerk zwischen den Patienten und der Kontrollgruppe durch. Außerdem verglichen wir die Verhaltensbewertungen des Berührungskomforts sowie die allgemeine Aversion gegenüber sozialen Berührungen in den beiden Gruppen. Insgesamt zeigte sich nach dem stationären Aufenthalt über alle Patienten gemittelt eine klinische Verbesserung von 40,4 %. Erwartungsgemäß bewerteten die Patienten die zwischenmenschlichen Berührungen als weniger beruhigend und lehnten sie im Vergleich zu der Kontrollgruppe vermehrt ab. Die fMRT-Analysen wiesen unabhängig von der Zeit bei den Patienten im bilateralen Nucleus Accumbens, sowie im Nucleus Caudatus beidseitig eine verringerte neuronale Reaktion nach. Diese Ergebnisse deuten darauf hin, dass die Verarbeitung von sozialen Berührungen, als Metrik für soziale Belohnungen bei Patienten mit Depressionen beeinträchtigt ist. Der beschriebene Effekt persistierte unabhängig von der Behandlung über die Zeit und blieb trotz einer klinischen Symptomverbesserung nachweisbar. Dies lässt darauf schließen, dass die pathologische Belohnungsverarbeitung trotz der klinischen Stabilisierung fortbesteht. Somit können auch bei Genesung anhaltende soziale Einschränkungen bei den Beteiligten bestehen bleiben und dies zur Entstehung oder Verstärkung sozialer Isolation führen. Angesichts dieser negativen Folgen sollten zukünftige Studien die aufgeführten Ergebnisse bestätigen und neue Behandlungsansätze entwickeln, die vermehrt auf die Verarbeitung von Belohnungen und Störungen der Körperwahrnehmung ausgerichtet sind

    Einsamkeit und Angstgedächtnis : Geschlechts- und Einsamkeitsspezifische Amygdala-Fehlregulation bei der Konditionierung und Extinktion von Angstreizen

    In der randomisierten, kontrollierten Querschnittsstudie wurde der Einfluss geschlechtsspezifischer Einsamkeit auf Angstkonditionierung und Extinktion sowie auf ein experimentelles Trauma untersucht. Methoden: Es wurden 82 gesunde ProbandInnen (44 männlich) eingeschlossen und mit der UCLA-Loneliness Scale in eine einsame und eine nicht-einsame Gruppe eingeteilt. Per funktioneller Magnetresonanztomographie erfolgte ein Furchtkonditionierungs- und Extinktions-Paradigma, daraufhin folgte ein experimentelles Trauma-Film-Paradigma. Während des experimentellen Trauma-Film-Paradigmas wurden physiologische Daten gemessen, sowie vor- und nachher mithilfe von Fragebögen psychologische Daten erfasst. Intrusive Gedanken und Kommunikationsverhalten wurden durch ein Onlinetagebuch über drei Tage beurteilt. Ergebnisse: Das Traumaparadigma führte unabhängig von Einsamkeit und Geschlecht zu einer experimentellen Traumatisierung im Sinne einer psychologischen und physiologischen Stressantwort aller ProbandInnen. Einsame Männer zeigten mehr Intrusionen als nicht-einsame Männer und der gegenteilige Effekt war bei Frauen zu beobachten. Einsame Männer konnten ihr Gesprächsbedürfnis nach Offenem Erzählen über das Trauma im Vergleich zu nicht-einsamen nicht ausreichend befriedigen. Bei der Furchtkonditionierung und Extinktion gab es geschlechtsspezifische Unterschiede in der Aktivierung des neuronalen Angstnetzwerks. Auffallend waren eine erhöhte Amygdala-Aktivität von einsamen Männern während der Konditionierung sowie eine stärkere Konnektivität einsamer Männer zwischen Arealen des neuronalen Angstnetzwerks und übergeordneten Zentren. Diskussion: Die Anfälligkeit einsamer Männer für Intrusionen könnte sich mit der Diskrepanz zwischen Gesprächsbedürfnis über das Trauma und tatsächlich geführten Gesprächen erklären lassen. Die Verarbeitung der traumatischen Erinnerungen konnte nicht durch offenes Erzählen vorangetrieben werden. Diese Diskrepanz zwischen gewünschter und tatsächlich erhaltener Traumabewältigung könnte in Verbindung mit der Definition von Einsamkeit als Diskrepanz zwischen erhaltener und gewünschter sozialer Interaktion stehen. Zudem besteht eine einsamkeits- und geschlechtsspezifische erhöhte Aktivität von angstassoziierten Hirnarealen, die die vermehrten Intrusionen bei einsamen Männern erklären könnte. Das Unvermögen einsamer Männer, ihr Bedürfnis nach offenem Erzählen zu befriedigen, zusammengenommen mit der erhöhten Aktivität angstassoziierter Hirnareale, zeigt, dass eine Vulnerabilität einsamer Männer für vermehrte intrusive Gedanken und demzufolge möglicherweise für eine Posttraumatische Belastungsstörung vorliegt

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
