Evidence of executive dysfunction, disinhibition and apathy in cognitive decline and Alzheimer\'s dementia in people with Down syndrome
Although a neuropathological correlation has been established between Down syndrome (DS) and Alzheimer\'s disease (AD), the early symptoms of dementia present atypically in the DS population. There is evidence that frontal-subcortical circuits play an important role in the initial presentation of dementia in DS, including changes in behaviour and executive dysfunction. The present study aimed to investigate factors associated with frontal lobe functioning (executive dysfunction, disinhibition and apathy) during cognitive decline and AD in adults with DS. METHODS. 92 individuals with DS aged over 30 years were evaluated and divided into three groups of diagnosis (stable cognition, prodromal dementia and AD) using the Cambridge Examination for Mental Disorders in Adults with Down Syndrome and others with Intellectual Disability (CAMDEX-DS), previously validated as part of our methodology. Participants were assessed with an executive function protocol developed for people with intellectual disabilities by researchers from University of Cambridge, and were rated for executive dysfunction, disinhibition and apathy by an informant using the Frontal Systems Behavior Scale (FrSBe). In addition, data on characteristics of frontal behaviour, memory and orientation were analysed through CAMDEX-DS in conjunction with an English sample totalling 162 participants with DS over 30 years old and divided into four groups: stable cognition under 45 years, stable cognition above 45 years, prodromal dementia and AD. RESULTS. Reports of executive dysfunction, disinhibition and apathy through FrSBe were correlated with participants\' cognitive performance: the higher the behavioural dysfunction in these areas, the worse the cognitive performance in executive tasks. Disinhibition and executive dysfunction were associated with diagnoses. The odds of having AD increased in parallel with increases in FrSBe scores (p <= 0.5). In the CAMDEX-DS analysis, amnestic and non-amnestic symptoms were found to be present before there was evidence of a cognitive decline. During the progression to dementia, those symptoms tended to worsen. Memory and orientation were poorer in the prodromal dementia group than in the stable cognition group (odds ratio 35.07, P < 0.001) as was executive function (odds ratio 7.16, P < 0.001). Disinhibition was greater in the AD group than in the prodromal dementia group (odds ratio 3.54, P = 0.04), and apathy was more pronounced in the AD group than in the stable cognition group (odds ratio 34.18; P < 0.001). CONCLUSION. Executive dysfunction, disinhibition and apathy were present in individuals with DS and stable cognition. These measures hasten the initial cognitive decline of AD and are related with cognitive performance in executive function tasks. Frontally mediated behaviour should be taken into consideration during the clinical evaluation of adults with DS. Future studies considering the intersection of neuropathology, brain connectivity, and behaviour may aggregate knowledge about the basis and nature of these associations, leading to the development of effective preventive strategie
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Family characteristics as protective factors for mental disorders in children and adolescents
Estudos de intervenção devem ser realizados para identificar as melhores estratégias para melhorar o ambiente familiar e o impacto dessas intervenções no desenvolvimento de transtornos mentais em crianças e adolescentesIntroduction: According to the World Health Organization, approximately 8% of children/adolescents aged between 5 and 14 years and 14% aged between 10 and 19 years had mental disorders in 2019. The likelihood of developing a mental disorder result from the interaction of individual, family, and structural factors. The family context is important for the development of behavioral and emotional characteristics of children. Children and adolescents who live in families with high levels of cohesion, low levels of control and conflict are less likely to develop mental disorders. Parents with authoritative styles have children with fewer mental disorders. Several studies have investigated risk and protective factors, but few have addressed the impact of family characteristics as a protective factor for mental disorders in children and adolescents in a longitudinally monitored community sample. Objective: To investigate characteristics of the family environment and parenting styles as protective factors for mental disorders in children and adolescents. Method: Cross-sectional and longitudinal analysis of the Brazilian High Risk Cohort Study were carried out in 2511 participants aged 6 to 14 years. The Development and Well-Being Assessment Questionnaire (DAWBA) was used at baseline and after 3 years of follow-up to diagnose mental disorders and different questionnaires to determine protective and risk factors at baseline. Parenting style and family environment were investigated with the Parental Attachment Instrument and the Family Environment Scale, respectively. Principal component analysis was carried out with the variables of the Family Environment Scale to obtain a variable called Family Environment Factor. Odds ratios were obtained by multiple logistic regression at baseline and 3 years later. Outcome: At baseline, 652 (26%) of participants scored for at least one mental disorder. The frequency of mental disorders was higher in children with mothers with neglectful style compared to mothers with affectionate control (permissive) and great care (authoritative). In the multiple regression, the factors that increase the chance of having a mental disorder were: 1) low socioeconomic status; 2) mothers who drank during pregnancy; 3) mothers who had a medical condition during pregnancy; 4) children who suffered Bullying; 5) children who suffered some type of abuse (sexual, physical or negligence) and 6) children of parents with mental disorders. The Family Environment Factor showed a negative association with the presence of mental disorders (OR=0.87, 95% CI [0.84-0.91], p<0.001), that is, for each increase in the value of the Family Environment Factor, the chance of having a mental disorder decreases by 26%. Both parenting styles and the Family Environment Factor were not associated with maintaining the absence of mental disorders at the 3-year follow-up. Conclusion: The presence of mental disorders in children and adolescents is associated with numerous risk factors and a healthy family environment can minimize the effect of these factors. Intervention studies should be carried out to identify the best strategies to improve the family environment and the impact of these interventions on the development of mental disorders in children and adolescent
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Adverse experiences in childhood and adolescence, mediators and obsessive-compulsive disorder: a study with patients, siblings and controls
This disorder is characterized by the occurrence of obsessions and/or compulsions. Some environmental factors related to OCD are adverse experiences in childhood and adolescence (AECA), which may have effects on cognitive, behavioral and emotional development. In addition, there are other processes involved such as resilience, experiential avoidance, and dissociative experiences, which may be potential mediators. Although this importance is recognized, there are few studies that have investigated jointly the AECA and these processes in patients with OCD, siblings without OCD and controls. The objective of this study was 1a) to compare the three groups with regard to AECA, resilience, experiential avoidance, dissociative experiences, and to investigate whether these variables are predictors of which group belongs to 1b) to characterize and compare the trajectory of AECA in all three groups. 2a) to verify if AECA, experiential avoidance, resilience and dissociative experiences predict clinical outcomes (number of psychiatric disorders, severity of obsessive-compulsive, depressive and anxious symptoms); 2b) to verify if the relationship between AECA and obsessivecompulsive symptoms, of anxiety and depression is mediated by processes of experiential avoidance, resilience and dissociative experiences. The sample consisted of 72 participants divided into 24 trios (patient, sibling without OCD and control). The statistical analyzes used were Kruskal-Wallis/Wilcoxon tests, multinomial regression models and generalized estimation equation models to compare the groups. Poisson and linear regression models and mediation models were used for the relationship outcomes between the variables. The instruments used were: maltreatment and abuse chronology of exposure; resilience scale for adults; acceptance and action questionnaire II; multidimensional experiential avoidance questionnaire; dissociative experiences scale; in addition to clinical data. In relation to AECA, a higher frequency of peer emotional abuse was observed in the group of patients compared to controls. Patients had lower resilience scores and higher experiential avoidance than the other groups. However, the siblings did not differ from controls. Experiential avoidance was the best variable to differentiate both patient and non-patient groups and siblings from controls. Predictors of clinical outcomes were dissociative and AECA in the patient sample. Part of the relationship between AECA (among which was peer emotional abuse) and depressive symptoms was mediated by resilience and experiential avoidance. The findings of the present study emphasized the importance of measuring AECA, especially, scales that contemplate emotional abuse between peers. The importance of the study of processes such as resilience, experiential avoidance and dissociative experiences as predictors and mediators of psychopathological outcomes was highlighted. The data from the present study are expected to motivate future studies that address the importance of AECA prevention and the development of social skills and protection factors related to resilience and its developmental impac
The Neurocircuitry of Goal-Directed Behavioral Control in Obsessive-Compulsive Disorder
Previous research hypothesizes that compulsivity, a hallmark of OCD, arises from an imbalance between goal-directed behaviors and habits. Neuroimaging and anatomic studies have shown that these behaviors are mediated by parallel frontostriatal circuits organized in a ventral-to-dorsal gradient. In compulsivity, abnormalities in this circuitry underlie a dysfunctional predominance of habits over goal-directed behavior. However, goal-directed behavioral control is complex, encompassing different cognitive sub-processes, such as reward processing, set-shifting, and planning. Differences in deficits in these subprocesses and their underlying circuit abnormalities may explain the phenotypic heterogeneity of OCD. This study aims to disentangle the different neuropsychological deficits leading to impaired goal-directed behavioral control in OCD and their underlying neurocircuitry at the subject level. Methods: 59 adults with OCD and 54 healthy controls (HC) underwent 3T resting-state fMRI. We used the CONN toolbox to preprocess images and perform ROI-to-ROI analyses, using the brain areas that comprise three different frontostriatal circuits as regions of interest. The three selected circuits were the Ventral Affective (VAC), Ventral Cognitive (VCC), and Dorsal Cognitive (DCC). Patients also underwent neuropsychological tests to assess the cognitive domains thought to be mediated by these circuits, respectively: the Cambridge Gambling Task (CGT) for decision-making, the Intra-extra Dimensional Set Shift (IED) for set-shifting and the One Touch Stockings of Cambridge (OTS) for planning. Connectivity scores, measuring the difference in circuit connectivity for each patient compared to the average connectivity observed in HC, were calculated using nonparametric statistics from Network Based Statistics analyses. Regression models were used to investigate the association between connectivity scores and performance on the respective subprocesses of goal-directed behavioral control for each patient. Results: Patients and HC were matched for age, sex, and IQ. The mean Y-BOCS score for patients was 29.81 (SD 5.69), indicating moderate-severe OCD. Most patients (76.3%) were using at least one psychotropic medication and had at least one psychiatric comorbidity at the time of the scan. As measured by connectivity scores, abnormal right VCC functional connectivity was associated with poorer overall performance on the IED, driven primarily by intradimensional shift errors. We found no association for the left VCC. Abnormal right DCC functional connectivity was associated with poorer performance on high-difficulty OTS problems but not on easy and moderate-difficulty ones. No significant association was found between left DCC functional connectivity and performance on the OTS. No association was found between VAC functional connectivity and performance on the CGT. Total Y-BOCS, depression, and anxiety scores did not affect the findings. Conclusions: the results align with previous evidence demonstrating specialized functions of distinct frontostriatal circuits in goal-directed behavioral control. The right-lateralization of findings is consistent with earlier studies. Negative findings for the VAC may be attributed to task characteristics. Subject-level analyses highlight that not all patients with OCD exhibit the same neuropsychological deficits and underlying brain mechanisms. Our findings pave the way for a circuit-based taxonomy of OCD and personalized neurotherapeutic
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Finally, we show by means of an example that our findings have a high practical relevance.information science;Pearson correlation;cosine;similarity measure;author cocitation analysis
Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more
sophisticated methods
